Objective To investigate cranial decompression under temporal muscle in very-low position with large bone flap combined with mild hypothermia for severe cranial trauma. Methods 88 severe brain injury patients (GCS ≤ 8 ) were randomly divided into two groups, mild hypothermia group( n =48) and non-mild hypothermia group ( n =40),both presented with cranial decompression under temporal muscle in very-low position with large bone flap.Data of patients,GCS increment, rate of brain cistern display, rate of pupil shrink and therapeutic effect were ananlyzed. Results The treatment group showed better effect compared with the oontrol group(P<0.05).Condusion The therapy of cranial decompression under teniporal muscle in very-low position with large bone flap combined with mild hypothennia showed the advantage of depressing intracranial pressure, reducing crerebral edema and improving the therapeutic effect of severe brain injury patients.

Objective:To observe the effect of Tongguan Liqiao(opening orifices)needling plus tongue pressure resistance feedback training in treating post-stroke deglutition disorders. Methods:A prospective randomized controlled trial was conducted.A total of 120 patients with post-stroke deglutition disorders were divided into a control group and an observation group using the random number table method, with 60 cases in each group. Both groups were treated with routine treatments. The control group was treated with additional tongue pressure resistance feedback training, and the observation group was treated with additional Tongguan Liqiao needling based on the same intervention in the control group. After 4 weeks of treatment, the clinical efficacy was evaluated, and the complications that occurred during treatment were summarized. The FUJISHIMA Ichiro food intake level scale(FILS)and the swallowing quality of life questionnaire(SWAL-QOL)were scored before treatment and after 4 weeks of treatment. Results:The total effective rate of the observation group was higher than that of the control group(P<0.05).After 4 weeks of treatment, the FILS and SWAL-QOL scores in both groups were higher than those before treatment, and the scores in the observation group were higher than those in the control group(P<0.05).There was no statistical difference in the complication occurrence between the two groups(P>0.05). Conclusion:On the basis of routine treatments,Tongguan Liqiao needling plus tongue pressure resistance feedback training has a better effect in the treatment of post-stroke deglutition disorders. This method can effectively improve the swallowing function of patients, improve the quality of life, and is safe.

Objective To explore pathological characterization of the enlarged contrast enhanced area of hepatocellular carcinoma ( HCC) on contrast enhanced ultrasonography ( CEUS). Methods Thirty-five HCC patients underwent conventional ultrasonography and CEUS before hepatectomy. The tumor sizes were measured on conventional ultrasonography,CEUS and postoperative resected specimen at the largest section. The infiltration depth of the HCC was observed under microscope in the peripheral area of the tumors. HCC with the size of contrast enhancement area on CEUS 0. 3 cm larger than that on conventional ultrasonography was defined as group A, and the others as group B. The pathological character of the tumor and tissues surrounding the tumor were analyzed and compared between the two groups. Results Among 35 HCC, 40% (14/35) HCC increased in size on CEUS compared with ultrasonography. Of them,9 tumors with increased area on CEUS were confirmed to be invasive HCC on pathology. Using the enlarged contrast enhanced area on CEUS as the diagnostic criteria of invasive HCC, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value were 90%, 80%, 82.9%, 64.3%, 95. 2%, respectively. Conclusions The contrast enhanced area on CEUS can reflect the infiltrative tumor size more accurately compared with conventional ultrasonography.

ObjectiveTo establish a new biomechanical test for evaluating biomechanical properties of femoral metaphysis in ovariectomized rats.MethodsTwenty-five-month-old female Sprague-Dawley rats were randomly divided into ovariectomized (OVX) group and the sham-operated (Sham) group (n=10).Twelve weeks after operation, femur BMD and histomorphometry of distal femur were detected.Furthermore, femur biomechanical parameters were determined by diaphysis three-point bending test and a new designed metaphysis bending test. The relationships between the biomechanical parameters and BMD or bone histomorphometry were assessed with Pearson correlations.ResultsThe femur BMD,bone histomorphometrical indexes(%Th.Ar,Tb.N,Tb.Th),and biomechanical parameters(maximum load, yield load and stiffness) of OVX group significantly decreased compared with Sham group. In metaphysis bending test, the mean difference of the maximum load(Fmax),yield load(Fy) between group OVX and Sham were significant higher than that in diaphysis three-point bending test.Positive correlations between biomechanical parameters and femur BMD or bone histomorphometry indexes were observed in both the diaphysis bending and metaphysis bending test. The biomechanical parameters in the metaphysis bending test showed stronger correlations with BMD and bone.ConclusionFemur metaphysis bending test could be used to evaluate the biomechanical properties of osteoporosis. When compared with diaphysis bending test, femur metaphysis bending test is more sensitive in evaluating the change of biomechanical properties of femur in osteoporotic rats.
8.Analysis of Risk Prevention and Control in Production for Medical X-ray CT System and Some Suggestions for Field Inspection.
Chinese Journal of Medical Instrumentation 2018;42(2):111-114
In recent years, the R&D and production level for Computed Tomography(short for CT) have been greatly improved and developed in China. In fact, it leads to higher requirements for medical device regulators. In order to clear key points in field inspection for regulators, this article analyzes the risk prevention and control points in the production process of CT based on the 5M1E analysis, and combined with medical device good manufacturing practice (short for GMP), presents exploratively some suggestions on the field inspection of CT manufacturers. This article also has important sense for regulators to clear the pertinence and improve the efficiency during the field inspection.

Objective To investigate the possibility of increasing R0 resection rate in esophageal carcinoma after neoadjuvant chemotherapy. Methods 30 patients underwent operation after neoadjuvant chemotherapy mainly by paclitaxel combined with cis-platinum, including 18 squamous carcinoma cases. 10 adenocarcinoma cases and 2 small cell carcinoma cases. Results In these 30 patients, the overall effective rate of neoadjuvant chemotherapy is 56.7%(17/30)(CR+PR),R0 resection rate is 100%,and the incidence rate of anastomotic leakage is 3.3%(1/30),without perioperative death. Conclusion The neoadjuvant chemotherapv is significant in improving the R0 resection rate in esophageal carcinoma, and the impact of long-term survival after surgery needs further follow-up.
