1.Effect of monosialo gangliosides-1 and Radix salviae miltiorrhizae on hyposemic ischemic encephalopathy

ObjectiveTo observe the effect of monosialo gangliosides-1 (GM-1) and Radix salviae miltiorrhizae (RSM) on neonatal hyposemic ischemic encephalopathy (HIE).Methods62 cases of HIE were randomly divided into treatment group and control group, and both groups received conventional therapy. Injection of RSM and GM-1 were added into the treatment group in 7~10 days therapy. ResultsHigh curative effect, average curative effect and total curative effect in the treatment group of 31 cases were 80.6%, 16.1% and 96.8%, while that in control group of 31 cases were 32.3%, 51.6% and 83.9% respectively. There was significant difference in the high curative effect and total effect between the two groups (P<0.05). ConclusionThe treatment combined with RSM and GM-1 can enhance the curative effect on HIE without any side effect.
2.Investigation on the genotyping of Acinetobacter baumannii by DiversiLab system

Objective To evaluate the usefulness of DiversiLab system for genotyping of Acinetobacter baumannii.Methods Fifty-eight non-duplicated clinical Acinetobacter baumannii isolated from 15 cities in China in 2005 were typed by rep-PCR-based DiversiLab system.The results were compared with those of PFGE and multilocus sequence typing. Simpson's index of diversity was used to compare the discriminatory power among DiversiLab system, PFGE and MLST.Results Fifty-eight Acinetobacter baumannii isolates were differentiated into 5 clusters and 25 unique types by DiversiLab system. MLST identified 35 distinct sequence types, which fell into one clone complex of CC22 and 35 singletons, while PFGE resolved 5 pulsotypes and 34 unique types.Simpson's diversity indices for DiversiLab system, MLST and PFGE were O.876, O.944 and 0.961, respectively.Conclusions The discriminatory power of DiversiLab system is lower than that of PFGE and MLST.But as a simple, fast and reproducible typing method, it could be used as a first-line typing tool for the analysis of a large number of isolates.
3.Multilocus sequence typing of carbapenem resistant Acinetobacter baumannii from Zhejiang Province

Objective To investigate the molecular epidemiology of carbapenem resistant Acinetobacter baumannii from Zhejiang Province using multilocus sequence typing.Methods Three hundred and two imipenem or meropenem resistant A.baumannii isolates were collected from eleven tertiary hospitals of eleven different regions of Zhejiang Province in 2009 and 2010.Multilocus sequence typing was used for molecular typing.eBURST was used to analyze the results of multilocus sequence typing.PCR was used to detect the OXA-type carbapenemase genes.Results Seventeen sequence types (STs) were identified from three hundred and two A.baumannii isolates.eBURST analysis revealed eleven STs belonged to clonal complex 92 (CC92) and corresponded to European clone Ⅱ lineage.CC92 was the predominant carbapenem resistant A.baumannii clone of Zhejiang Province,which accounted for 94.4% (285/302) of all isolates and distributed in all 11 hospitals.blaOXA-23 gene was identified in 97.4% (294/302) of all isolates.Conclusions Carbapenem resistant A.baumannii CC92 were clonal disseminated among multiple hospitals of Zhejiang Province.blaOXA-23 was the most popular carbapenemase gene in carbapenem resistant A.baumannii.(Chin J Lab Med,2013,36:303-307)
4.Fetal ventricles and posterior cranial fossa development in second and third trimesters and their variations with gestational age: a quantitative MRI study

Objective:To investigate the normal range of fetal ventricles and posterior cranial fossa development in the second and third trimesters and their variations with gestational age using quantitative MRI analysis.Methods:This retrospective study enrolled 675 pregnant women who underwent prenatal MRI examination with an average gestational week of 29.0±8.5 in the Third Affiliated Hospital of Guangzhou Medical University from January 2016 to January 2020. MRI data of all the subjects were collected and analyzed, including left lateral ventricle trigonometric width (LLVTW) and right lateral ventricle trigonometric width (RLVTW), third ventricle width (TVW), fourth ventricle width (FVW), the anterior-posterior diameter of the fourth ventricle (APDFV), cavum septum pellucidum width (CSPW), cisterna magna width (CMW), etc. Spearman, Pearson correlation analysis, and t-test were used for the statistical analysis. Results:(1) Totally 675 fetuses were recruited, including 392 female and 283 male fetuses. No statistical difference of gestational weeks at MRI was found between male and female fetuses. (2)The mean value of TVW and CMW of the female fetuses were significantly higher than those of male fetuses [(0.60±0.05) vs (0.63±0.04) cm, t=-5.059; (0.57±0.14) vs (0.67±0.15) cm, t=-7.445; both P<0.001]. Spearman correlation analysis showed that TVW and CMW were negatively correlated with fetal gender ( r=-0.179 and-0.312, both P<0.001). (3)Pearson correlation analysis showed that LLVTW, RLVTW, TVW, FVW, APDFV, CSPW, and CMW were all positively correlated with gestational weeks ( r=0.310, 0.267, 0.205, 0.801, 0.829, 0.216 and 0.284, all P<0.001). FVW and APDFV were significantly linearly correlated with gestational weeks (r=0.801 and 0.829, both P<0.001). (4) There was no significant change in LLVTW and RLVTW in the second trimester, but a slight increase was found in the third trimester. TVW showed a scattered distribution in the second and third trimesters and increased slightly with the gestational week at 26 to 27 +6 gestational weeks. FVW and APDFV increased linearly while. CSPW increased slowly with gestational weeks in the second and third trimesters. CMW increased slightly with gestational weeks in the second and third trimesters but showed little change in the third trimester. Conclusions:The development of fetal ventricles and posterior cranial fossa in the second and third trimesters show a growth trend of varying degrees with the increase of gestational weeks. TVW and CMW are significantly negatively correlated with the fetal gender.
5.Therapeutic value of zoledronic acid in the prevention of osteoporotic hip sequential fracture

Objective:To investigate the clinical value of zoledronic acid in improving the prognosis of osteoporotic hip fracture and preventing its sequential contralateral hip fracture.Methods:A retrospective study of 206 patients diagnosed with hip fragile fractures in China Medical University Affiliated Shengjing Hospital and treated with anti-osteoporotic drugs after surgery was conducted. The prognosis of patients with hip fracture using survival rate analysis and the risk factors of acute febrile complications with multivariate analysis after zoledronic acid treatment were evaluated. Furthermore, chi-square test and multivariate analysis was used to explore whether zoledronic acid decreases the occurrence of the contralateral sequential fracture.Results:The 3-year survival rate of patients with overall hip fracture was higher in zoledronic acid treatment group compared with control group( P=0.026), with the incidence of fever at 53.3%. The age [ OR=0.786, P=0.027, the area under receiver operating characteristic (ROC) curve was 0.724] and cardiopulmonary complications ( OR=0.043, P=0.025, the area under ROC curve was 0.628) were significantly correlated with the occurrence of acute febrile response. The incidence of sequential contralateral fractures in zoledronic acid treatment group was significantly lower than that in control group ( χ2=4.356, P=0.037). The application of zoledronic acid ( OR=0.160, P=0.007, the area under ROC curve is 0.586) and the type of femoral neck fracture ( OR=0.196, P=0.001, the area under ROC curve is 0.607) were statistically associated with the occurrence of sequential fractures. Conclusion:Zoledronic acid treatment improves the outcome of patients with osteoporotic hip fracture and reduces the incidence of sequential hip fractures, especially femoral neck fracture. Senile age and cardiopulmonary complications might be associated with lower risk of acute febrile reactions after zoledronic acid treatment.
6.Analysis of influential factors for dose-corrected trough concentration of amisulpride based on therapeutic drug monitoring

OBJECTIVE To analyze the influential factors for dose-corrected trough concentration (C/D) of amisulpride, and to provide reference for rational use of it in clinic. METHODS The results of the last serum concentration monitoring and relevant medical data of schizophrenic inpatients who were monitored for the treatment of amisulpride in the Mental Health Center of the First Hospital of Hebei Medical University from March to November 2021 were collected retrospectively and analyzed the effects of age, sex, body mass index (BMI), serum creatinine level, and combined medication on the C/D value of amisulpride. RESULTS A total of 133 schizophrenic inpatients were included, and median dose of amisulpride was 600.00 mg/d, median serum concentration was 332.57 ng/mL, and median value of C/D was 0.61 ng·d/(mL·mg). The serum concentration of 49 patients was within the recommended range of relevant guideline (100-320 ng/mL), and 27 patients exceeded the laboratory warning concentration (640 ng/mL). There was no statistical significance in drug dose, serum concentration or C/D value among the patients of different genders (P>0.05). The serum concentration of minor patients was significantly lower than that of adult patients (P<0.05), but there was no statistical significance in drug dose or C/D value between minor patients and adult patients (P>0.05). The C/D value of amisulpride in obese patients (BMI>28 kg/m2) was significantly lower than that in normal patients (BMI 18.5- 23.9 kg/m2); the combination of olanzapine, sodium valproate and benzodiazepines did not affect the C/D value of amisulpride (P>0.05); the level of serum creatinine in patients was not related to the dose of amisulpride (r=0.081, P>0.05), but was positively related to the C/D value of the drug (r=0.285, P<0.05). CONCLUSIONS The age, BMI and serum creatinine level of patients are closely related to the C/D value of amisulpride. In the clinical treatment of schizophrenia with amisulpride, we should fully consider the patient’s age, body mass, serum creatinine and other factors, and develop a personalized drug regimen for patients to ensure the safety and effectiveness of treatment.
