1.The role of tracheotomy in children with severe hand foot and mouth disease

Objective To investigate the role of tracheotomy in children with severe hand foot and mouth disease.Methods Eight children with severe hand foot and mouth disease collected from July 2009 to August 2011 in PICU were retrospectively reviewed.Clinical manifestation and laboratory data were analyzed and followed up.Results Eight patients were enterovirus 71 (EV71) positive.Hormones,intravenous immune globulin,mechanical ventilation and vasodilators had been administered in patients with cardiopulmonary failure.They had severe neurologic sequelae,requiring prolonged intubation.MRI imaging of 7 cases showed abnormal brainstem,pons and medulla oblongata,1 case showed swelling of brain tissue.A total of 8 patients underwent a tracheotomy in mean (25.63 ± 8.86) d.The older was 22 months,the younger was 8 months and the mean age was (13.13 ±4.22) months.Four cases died,the other survived.Two cases were successfully decannulation,the other two were still in the tracheotomy.Conclusion Tracheostomy play an active role in reducing airway resistance,clearing airway secretions,providing the opportunity for subsequent rehabilitation.Eight patients were followed up for three years,neurological sequelae could not be restored.Four cases repeatedly suffer from pneumonia,atelectasis,leading to a serious decline in quality of life.
2.Treatment of chronic diabetic foot ulcer with the combination of an inorganic activity induced skin wound dressing and a nano-silver dressing

Objective To evaluate the effect of Dermlin,an inorganic activity induced skin wound dressing,in combination with a nano-silver dressing,on chronic diabetic foot ulcer.Methods A total of 93 elderly diabetic patients with chronic foot ulcer were recruited and randomly assigned into the treatment group and the control group.In additional to routine systemic and topical treatment for both groups,the treatment group was given a combination of Dermlin and nano silver dressngs,while for the control group,iodophor gause was used to cover the ulcer.The two groups were compared on ulcer grading,wound healing time and positive rate of bacterial culture.Results The wound healing time in the treatment group was significantly shorter than that in the control group [(41.6±2.7) d vs.(61.9±2.9) d,t=-35.09,P=0.000].Positive rates of bacterial culture for the treatment group at admission,14 d after admission and 28 d after admission were 88.46%,23.07% and 15.38%,respectively,in contrast to 88.89%,59.26% and 40.74,respectively,for the control group at the same time points (x2 =0.01,6.88,7.34,P=0.065,0.008,0.006).The rate of effectiveness for the control group was 63.83% (30/47),clearly lower than the 82.61% for the treatment group (38/46) (x2=4.17,P=0.035).The patients' foot ulcers were graded using the Wagner system.In the treatment group,27 patients with grade 2 ulcers showed an 88.89%effectiveness rate (24/27),considerably higher than the rate of 67.86% in the control group (18/28)(x2=4.60,P=0.032).For grade 3 ulcers,the effectiveness rate was 73.86% (14/19) for the treatment group and 63.15% for the control group (12/19),with no statistical difference between the two groups (x2=0.49,P=0.364).Conclusions The combination of Dermlin and Nanosilver can promote healing for grade 2 chronic diabetic foot ulcers,but has no obvious effect on grade 3 ulcers.
3.Advances on tick-borne encephalitis in children

Tick-borne encephalitis ( TBE) is a well-known central nervous system ( CNS) infection in children. The disease in children is generally milder,although severe illness may occur and even lead to perma-nent impairment of the quality of life due to neuropsychological sequelae. We review the epidemiological and clinical characteristics of tick-borne encephalitis,and summarise biological and virological aspects that are impor-tant for understanding the life-cycle and transmission of the virus. Tick-borne encephalitis virus is a flavivirus that is transmitted by ixodes persulcatus. Tick-borne encephalitis causes acute meningoencephalitis. The serologi-cal diagnosis is usually straightforward. No specific treatment for the disease exists,and immunisation is the main preventive measure.
4.Analysis on the Economic Operation of Township Public Hospitals in Jiangxi before and after the Medical Reform

Taking 175 township public hospitals in Jiangxi as the samples,descriptive statistics and comparative analysis were applied based on their financial yearly report data from 2012 to 2016 to analyze the operation situation of hospitals before and after the public reform,analyze the influences of medical reform policies on township public hospitals and propose relevant policies,so as to promote the soon coming urban public hospital reform in Jiangxi.
5.Analysis on the Liabilities of Urban Public Hospitals in Jiangxi

Objective:To understand the liability situation of urban public hospitals in Jiangxi.Methods:Qualitative and quantitative analysis,descriptive analysis and comparative analysis were used to analyze the causes and changes of urban public hospitals' debt increase.Results:To meet the increasing demand for health diagnosis and services,debt infrastructure and the purchase of equipment coupled with inadequate financial investment were performed in public hospitals,which resulted in the sustainably increasing of liabilities level in urban public hospitals.Conclusion:Governments at all levels should increase the capital expenditure of hospital construction and equipment investment,strictly control the public hospital debt behavior,the implementation of the reform of public hospitals canceled drug addition,establishing the characteristics of the medical industry personnel salary system and other measures to resolve the synchronous speed of public hospital debt and prompt public hospital debt risk control in the reasonable scope.

7.Observation of efficacy of quetiapine combined with paroxetine in the treatment of bipolar depression

Objective To explore the curative effect of quetiapine combined with paroxetine in the treatment of bipolar depression,and to provide reference for the choice of clinical treatment.Methods According to the digital table,96 patients with bipolar disorder were randomly divided into observation group and control group,48 cases in each group.The control group was treated with quetiapine orally,the observation group was treated with quetiapine combined with paroxetine.After 6 weeks of treatment,the clinical curative effect,the Montgomery Asberg Depression Rating Scale and the Beck pull fan Sen Mania Rating table score before and after treatment were compared between the two groups.At the same time,the adverse reactions of the two groups were compared.Results After 6 weeks of treatment,the total effective rate of the observation group was 70.83%,which was significantly higher than 50.00% in the control group,the difference was statistically significant(x2 =4.356,P =0.037).After 6 weeks of treatment,the Montgomery Asberg Depression Rating quantity score in the observation group was (14.01 ± 2.95) points,which was significantly lower than (18.07 ± 3.05)points in the control group,the difference was statistically significant (t =5.527,P =0.036).The Baker-pull fan Sen Mania Rating score before and after treatment between the two groups had no statistically significant differences.There were no serious adverse reactions in the two groups.Conclusion Quetiapine combined with paroxetine can effectively inhibit bipolar depressive episodes,avoid manic episodes,has good clinical efficacy and high safety,it is worthy of popularization and application.
8.Effects of Shuganjieyu capsule of plasma glucose and lipid metabolism and sleep and body weight in depression

Objective To study effects of shuganjieyu capsule of plasma glucose and lipid metabolism and sleep and body weight in patients with depression.Methods 90 patients of depression who received therapy from July 2013 to July 2016 in our hospital were selected.According to random number table, those patients were divided into the observation group ( n =45 ) and the control group ( n =45 ) .The control group was treated with citalopram, while the observation group was treated with shuganjieyu capsule, then glucose and lipid metabolism, sleep, weight and clinical curative effect were compared.Results After treatment, the fasting blood glucose (FBG), postprandial 2h blood glucose (2 hPG), triacylglycerol (TG), total cholesterol (TC) in the observation group were lower than the control group(all P<0.05), the sleep state self rating scale (SRSS) and the Pittsburgh sleep quality index ( PSQI) in the observation group were lower than the control group ( all P <0.05 ) .The body weight and body mass index in the observation group were lower than the control group (all P<0.05).The total effective rate in the observation group 95.56% (43/45)was higher than the control group 77.78% (35/45) (P<0.05); the incidence of adverse reactions in observation group 20.00% (9/45) was lower the control group 46.67% (21/45) ( P <0.05).Conclusion Shuganjieyu capsule is well for depression, which can conducive to maintaining glucose and lipid metabolism, weight stability, and improve sleep quality and clinical symptoms, and high security,it's worthy of application and promotion.
9.Clinical manifestations, neuroimaging features, treatment, and follow-up in four pediatric patients with glutaric aciduria type Ⅰ

Objective To investigate the clinical features,magnetic resonance imaging (MRI),treatment,and follow-up of patients with glutaric aciduria type Ⅰ (GA-1).Methods Four pediatric patients with GA-1 diagnosed in our hospital were included in this study.They were treated with special diets and carnitine supplements.MRI and tandem mass spectrometry (MS/MS) were performed,and the mental development indices were measured.Results GA-1 was confirmed 2 months,13 months,4 months,and 7 months after birth.Seizure had been observed before the disease diagnosis in three patients and disappeared after treatment.In all four patients,T2-weighted brain MRI showed frontotemporal atrophy or hypoplasia and enlarged subarachnoid space in the sylvian fissures and anterior to the temporal lobes.Diffusion weighted imaging revealed high-density lesions over both the putamen and globus pallidus.The patients were followed up for 4 to 5 years.Plasma amino acids and acylcamitine profile were monitored every 3-5 months.The mean C5DC level and C5DC/C8 were kept the higher limits of the normal ranges,especially in case 3.During the follow-up,the body weight was at-2 SD-0 and the height at-1 SD-0.Intellectual development test showed that case 1 and case 4 had mildly abnormal intelligence,whereas case 2 and case 3 had extremely severe intellectual disability.Gene test confirmed the presence of gene mutations in all four cases,including IVS10-2A > C homozygous mutation in cases 1,3,and 4 and [IVS10-2A > C] + [c.245G >c(p.Arg82Pro)] hybrid mutation in case 2.Two female children were smoothly enrolled by local kindergarten,while two male children were unable to walk alone due to delayed motor development and spastic paralysis.Conclusions The phenotype of GA-1 patients is not remarkably correlated with its genotype correlation.Newborn screening is essential for identifying GA-1 patients.
10.Evaluation of functional esophageal dismotility by high-frequency intraluminal ultrasonography in patients with gastroesophageal reflux

Objective To investigate the functional esophageal dismotility in patients with gastroesophageal reflux disease(GERD) by simultaneous high frequency intraluminal ultrasonography(HFIUS) and esophageal manometry,and evaluate the correlation between the peak pressure and the peak muscle thickness of the esophagus.Besides,to identify the potential efficacy and feasibility of the HFIUS for evaluating the functional gastrointestinal diseases(FGIDs).Methods Ten GERD patients and 5 healthy controls were enrolled into this study.High-frequency ultrasonic probe and esophageal manometry catheter were simultaneous administrated.The cross sectional image of esophageal motion was recorded by HFIUS and was analyzed by the contractility index (CI)of the longitudinal smooth muscle(LSM)and circular smooth muscle(CSM),the duration of the contraction (DC)and the maximum cross section area(CSA)during distension;In addition,esophageal peak pressure during wet-swaltow was measured by manometry.Results Tile contractility index of the LSM and CSM of esophagus at 5 cm,10 cm,1 5 cm and 20 em proximal to the LES was significantly lower in patients with GERD than that of controls(P<0.05).The duration of the contraction was longer in patients with GERD.But there was no siginificant difference of the maximum CSA during distension between GERD patients and normal controls(P>0.05).However,the simultaneous manometry only demonstrated nonspecific esophageal motility disorder on 6 of 10 GERD patients.And there was positive correlation between the peak pressure and the peak muscle thickness at all sites along the axis of the esophagus,revealed the pressure was increasing along with the thickening of the esophageal muscle layers(r=0.552-0.736).Conclusions Functional esophageal dismotility played an important role in the pathogenesis of GERD.Meanwhile,there was positive correlation between the esophageal pressure and the muscle layer thickness of the esophagus.HFIUS can detect the minor changes of the layer structure of the esophagus and is a potentially useful imaging modality for evaluating esophageal dismotility as well as FGIDs.
