1.POSTOPERATIVE RADIATION THERAPY (RT) FOR BREAST CANCER CLINICAL ANALYSIS OF 314 CASES

We analyzed retrospectively the result of postoperative RT by electron beam in 314 patients with breast cancer after a radical or modified mastectomy between 1976~1992. The radiation portals covered the supraclavicular, the axillary and the internal mammary lymph node chains and chest wall. D T40~50Gy/4~5w was delivered. The local regional failure and factors affecting the survival rates, such as the clinical stage, the status of axillary lymph nodes and radiation dose were analyzed. The symptomatic radiation pneumonitis (9.4%) was discussed.
2.The clinical significance of dynamic changes of serum D-dimers in esophageal carcinoma pre-and post-operation

Objective To investigate the relationship between dynamic changes of plasma D-dimer and survival rate of the esophageal carcinoma patients pre-and post-operation.Methods 30 cases of normal control group,160 cases of esophageal cancer group( including operation cases n =112),with the gold standard method for the determination of plasma D-dimer.Results There was a link between the level of D-dimer,TNM staging,lymph node metastasis and tumor size in esophageal carcinoma patients.Compared with the preoperation,the plasma D-dimer is significantly elevated 2 years later( t =7.35,P > 0.05 ).Conclusion Before or after the operation,dynamic changes of plasma D-dimer had a relationship with short-term survival rate.
3.Porous Se@SiO2 nanocomposites for treatment of steroid-induced osteonecrosis of the femoral head

BACKGROUND:Steroid-induced osteonecrosis of the femoral head (SONFH) is a common bone disease characterized as high morbidity and poor prognosis,but the pathogenesis is unclear.Oxidative stress treatment is closely related to the occurrence and development of SONFH,and has tremendous potential in the treatment of SONFH,which can be realized by Nano-Se.OBJECTIVE:To observe the protective effect of porous Se@SiO2 nanocomposite on chondrocytes by antioxidant stress,and to further explore its mechanism of protection and treatment of SONFH.METHODS:(1) In vitro experiment:The rat chondrocytes were isolated,cultured and identified.Then,the chondrocytes were cultured with porous Se@SiO2 nanocomposite to suppress the production of reactive oxygen species (ROS).(2) In vivo experiment:A total of 36 rats were randomly divided into three groups.Steroid-induced group and experimental group were treated with intraperitoneal injection of lipopolysaccharide and intramuscular injection of methylprednisolone to induce SON FH models.Seven days after modeling,the experimental group was intraperitoneally injected with porous Se@SiO2 nanocomposite.No intervention was done in control group (blank control).At 8 weeks after modeling,rat bilateral femoral heads were taken for hematoxylin-eosin staining and Micro-CT scanning.RESULTS AND CONCLUSION:Results from the ROS detection and TUNEL apoptosis tests showed that the level of ROS in the chondrocytes was significantly reduced after intervention with Se@SiO2 (P < 0.05).Micro-CT scanning findings showed that the bone mineral density,bone volume,bone area/bone volume,trabecular number,trabecular thickness,and trabecular separation in the steroid-induced and experimental groups were significantly different from those in the control group (P < 0.05).Hematoxylin-eosin staining results showed smooth femoral head,normal bone cells,chondrocytes and trabecular bone,as well as few empty bone lacunae and fat cells in the control group,while in the steroid-induced group,there was bone trabecular fracture,fat cell hypertrophy fusion,a large number of empty bone lacunae and obvious osteonecrosis.These manifestations were significantly improved in the experimental group.To conclude,the porous Se@SiO2 nanocomposite has good antioxidative stress ability,suppresses the ROS production and exerts therapeutic effects on SONFH.
4.MuItipIe xenobiotic metaboIism-associated cytochrome P450 isoforms might be invoIved in earIy activation of rat hepatic steIIate ceIIs in vitro

OBJECTIVE To investigate the role of several xenobiotic metabolism-associated CYP450 isoforms in the auto-activation of hepatic stellate cells(HSCs) invitro. METHODS HSCs were isolated from adult Wistar rats and cultured on plastic as an in vitroauto-activation model. Positive expression of α-smooth muscle actin(α-SMA)was used as an activated marker of HSCs and detected by immunocytochemical staining in HSCs cultured for 1,2,5 or 11 d. The expressions of CYP450 isoforms were analyzed by real-time RT-PCR in the HSCs. RESULTS The immunocytochemical stai-ning showed no α-SMA expression in HSCs cultured for 1 d,while its expression gradually increased during culture since day 2. ln terms of α-SMA expression,HSCs cultured for 1,2,5,and 11 d were classified as the quiescent,early,middle and later stages of activation,respectively. The RT-PCR results revealed that CYP1B1,CYP2B1/ 2,and CYP2E1 mRNA were expressed at high levels in the early stage of HSCs activation(at day 2),which were 2.1-,1.6-,and 23.9-fold those in the quiescent HSCs(day 1),respectively. Further study revealed that mRNA expressions of these up-regulated CYPs in the early stage of activation were diminished at the subsequent two stages. The levels of CYP1A1 and CYP1A2 mRNA decreased constantly throughtout the activation process. CONCLUSION Multiple xenobi-otic metabolism-associated CYP450 isoforms might be involved in the auto-activation of rat HSCs invitro.
5.Special operative techniques for intertrochanteric fractures without lesser trochanter fracture

Objective To explore the operative techniques for AO/OTA type 31-A3.1 and 31-A3.2 intertrochanteric fractures fixed with intrameduallary nail.Methods Seventy-four cases of unstable intertrochanteric fracture (AO/OTA type 31-A3.1 and 31-A3.2) from January 2007 to December 2010 were analyzed retrospectively.There were 33 males and 41 females,aged from 36-87 years (mean,71 years).The right hip was involved in 34 patients and the left in 40 patients.The mechanism of injury was traffic injuries in 27 cases,fall damage in 43 cases,injury by falling in 3 case and crush injury in 6 cases.Percutaneous joystick technique,Homann retractor technique,clamp technique and mini-incision was applied to aid reduction during the surgery.All cases were fixed with proximal intrameduallary nail.Clinical and radiographic outcomes were recorded.The postoperative hip function was evaluated using Harris score.Results With the aid of C-arm,closed reduction was performed in 73 cases.Only one patient experienced open reduction because of failure of closed reduction.Sixty-five cases were followed up for 8 to 23 months (mean,14.5 months).Bone union was observed in all cases with the average time of 7.4 months (range,8-23).The operation time,blood loss,the frequency of X-ray exposure was 75±4 minutes,135±5 ml and 24±3 times,respectively.At final follow-up,29 cases were classified as excellent,31 as good,and 5 as fair.The mean Harris hip score was 91.4 points (range,87-95).The overall rate of excellent or good result was 92.3%.Conclusion It is difficult to perform close reduction for unstable intertrochanteric hip fracture (AO/OTA type 31-A3.1and 31-A3.2),especially in relatively young patients.Special operative skills are necessary in some cases.
6.Meta-analysis of risk factors for rheumatoid arthritis complicated with interstitial pulmonary disease

Objective:A meta-analysis was conducted to investigate the risk factors of interstitial lung disease in patients with RA to provide a reliable reference for clinical practice.Methods:Publications of the China Knowledge Network, Vipshop, Wanfang, China Biomedical Literature Database, EMbase, PubMed, and Cochrane Library databases were searched to obtain literature on cohort studies and case-control studies for risk factors for RA complicated with interstitial lung disease from the time of database creation until December 31, 2022. Literature screening, information extraction, and final quality evaluation were performed independently by two investigators. The extracted data were analyzed using RevMan 5.3 software.Results:A total of 31 studies with a total of 26 747 patients were included in this study, of which 4 799 patients(17.94%) were patients with RA complicated with interstitial lung disease. A total of 12 risk factors were obtained included: advanced age [ OR (95% CI) =1.07(1.04, 1.09), P<0.001], high age at onset [ OR (95% CI) =2.11(1.17, 3.82), P=0.010], male [ OR (95% CI) =1.95(1.34, 2.84), P<0.001], long duration of disease [ OR (95% CI) =1.10(1.08, 1.13), P<0.001], smoking [ OR (95% CI) =2.47(1.57, 4.04), P<0.001], high disease activity [ OR (95% CI) =(1.21, 2.54), P=0.003), cough [ OR (95% CI) =6.10 (2.40, 15.52), P<0.001], decreased pulmonary carbon monoxide diffusing capacity [ OR (95% CI) =0.93(0.89, 0.96), P<0.001], and high titer anti-CCP antibody [ OR (95% CI) =1.14(1.03, 1.26), P=0.010], high titer RF [ OR (95% CI) =2.37(1.70, 3.29), P<0.001], high level of immunoglobulin A [ OR (95% CI) =1.26 (1.09, 1.46), P=0.002] and high level of glycoprotein antigen 125 [ OR (95% CI) =3.66 (1.81, 7.41), P<0.001]. Conclusion:The existing evidence have shown that advanced age, high age at onset, male, long disease duration, smoking, high disease activity, cough, decreased pulmonary carbon monoxide diffusing capacity, high titer anti-CCP antibody and RF, high level of immunoglobulin A and high level of glycoprotein antigen 125 are the main risk factors for interstitial lung disease in patients with rheumatoid arthritis. However, the evidence of Raynaud's phenomenon and tumor marker CA153 as risk factors for RA complicated with interstitial lung disease is not strong, and further high-quality prospective cohort studies are needed.

Periarticular fracture of the shoulder is a common type of fractures in the elderly. Postoperative adverse events such as internal fixation failure, humeral head ischemic necrosis and upper limb dysfunction occur frequently, which seriously endangers the exercise and health of the elderly. Compared with the fracture with normal bone mass, the osteoporotic periarticular fracture of the shoulder is complicated with slow healing and poor rehabilitation, so the clinical management becomes more difficult. At present, there is no targeted guideline or consensus for this type of fracture in China. In such context, experts from Youth Osteoporosis Group of Chinese Orthopedic Association, Orthopedic Expert Committee of Geriatrics Branch of Chinese Association of Gerontology and Geriatrics, Osteoporosis Group of Youth Committee of Chinese Association of Orthopedic Surgeons and Osteoporosis Committee of Shanghai Association of Chinese Integrative Medicine developed the Chinese expert consensus on the diagnosis and treatment of osteoporotic periarticular fracture of the shoulder in the elderly ( version 2023). Nine recommendations were put forward from the aspects of diagnosis, treatment strategies and rehabilitation of osteoporotic periarticular fracture of the shoulder, hoping to promote the standardized, systematic and personalized diagnosis and treatment concept and improve functional outcomes and quality of life in elderly patients with osteoporotic periarticular fracture of the shoulder.
