1.Analysis of the risk factors for postoperative residue, relapse following myomectomy

Objective To elaborate the risk factors for leiomyoma residue and relapse after different approaches of myomectomy.Methods From Jan.2005 to Dec.2010 and Jan.2011 to Jan.2013,769 patients underwent myomectomy were recruited in Beijing Obstetrics and Gynecology Hospital.The patients demographic,leiomyoma characteristics,preoperative gonadotropin-releasing hormone agonist (GnRH-a) therapy,surgical approach,pathological type,follow-up information were collected.Results Leiomyoma number was the risk factor of postoperative residue and relapse,with the leiomyoma number increased one,the risk of residue and relapse increase 1.085 times (OR=1.085,95%CI:1.019-1.154,P=-0.010),1.043 times (RR=1.043,95%CI:1.014-1.073,P=0.003) respectively.Leiomyoma type (intramural leiomyoma) was the risk factor of relapse (RR=1.665,95%CI:1.029-2.693,P=0.038).Age was not the risk factor for postoperative residue rate (P=0.828) and relapse rate (P=0.193).GnRH-a didn't increase the postoperative residue and relapse rate (P=0.542,0.133).The postoperative residue rate (P=0.764),relapse rate (P=0.279) between transabdominal and laparoscopic myomectomy groups had no significant difference.Bizarre leiomyoma (RR=5.678,95%CI:1.373-23.490,P=0.017) and celluar leiomyoma (RR=2.201,95%CI:1.466-3.303,P<0.01) were the risk factors for postoperative relapse rate.Conclusions Leiomyoma number,leiomyoma type (intramural leiomyoma) are the main risk factors for postoperative relapse.Pretreatment of GnRH-a and laparoscopic approach wouldn't increase the rate of residue and relapse.Bizarre leiomyoma and cellular leiomyoma have a higher relapse rate than common leiomyoma.
2.Pharmacokinetics of Metronidazole and Tinidazole in Human Dreast and the Effects on Dreast-fed Infants

Objective To investigate the effect of metronidazole and tinidaxole in breast milk on infants and their concentrations. Methods The concentration of metronidazole and tinidazole inbreast milk was assayed by high performance liquid chromatography(HPLC). Results The pharmacokineticparameters(mean?s) t max(h), t 1/2ke of Metronidazole(20 mg/kg, n=8 ) and Tinidazole (13 mg/kg,n=7) in lactation women infused with a single dose were(1.7?1.0)h、(20?5) mg/L and (1.3?0.6)h、(17?3) mg/L respectively. The total amount of metronidazole and tinidazole ingested by the infant via breast milk were 3.02 mg/kg/day and 2.58 mg/(kg?d) respectively. which were 1/5~1/10. and 1/6~1/8 of the routine therapeutic dose for infants.

Objective To improve scientific research capacity and disciplines development,we performed grading system to specialize and quantize the management in Beijing Obstetrics and Gynecology Hospital,Capital Medical University.Methods With assistance of forum survey related to 20 clinical and technical departments in hospital,we assessed the status of disciplines development,designed assessment criteria,and revised relevant regulations in grading system.Results The implementation of grading system promoted scientific achievements,academic communications and human resource development in hospital as relative to their pre-implemental controls.Conclusions The implementation of grading system facilitates scientific research and improves disciplines development.
4.Study on the current situation, problems and countermeasures of patent in Beijing Obstetrics and Gynecology Hospital from 2002 to 2017

Objective Objective To find out the existed problems and suggest for possible countermeasures for the patents management through keep track of the trend of patents in Beijing Obstetrics and Gynecology Hospital,Capital Medical University.Methods The data of patents from the department of science and technology in Beijing Obstetrics and Gynecology Hospital,Capital Medical University during 2002-2017 were extracted,to analyze the number of application and grants of patents,type,the authorized inventors'department,project and funding.The method of the stage of technological development was used to deduce the development stage of the technology innovation ability.Results The number of patents application and grants was small in Beijing Obstetrics and Gynecology Hospital from 2002 to 2017.The wave-like increasing trend was observed in the number of patent application,while the stable change in the number of grants of patents.The Compound Annual Growth Rate of the number of application and grants of patents were 14.9% and 8.2%,respectively.In terms of patent types,most patents (69.2 %) were utility models patents.The proportion of service invention was 50%.The grants of patents were not affected by research funding.The technology innovation ability of patent is developing,and there will be a rising trend in the future.Conclusions The hospital should strengthen the patent management,improve the management of patent policy,cultivate full-time professional staff,strengthen the cooperation with the patent agency and enterprise,and the construction of patent conversion platform to promote development of patent quality.
5.The situation of science and technology awards in Beijing Obstetrics and Gynecology Hospital from 1998 to 2017

Objective To develop the management system of science and technology awards through understanding dynamically the trend of awards in Beijing Obstetrics and Gynecology Hospital,Capital Medical University.Methods The data from the department of science and technology in Beijing Obstetrics and Gynecology Hospital,Capital Medical University were used to learn about some informations of science and technology awards including the winner's age,education,title and department,the proportion of rank and level,the trend of awards from 1998 to 2017,the number of social association awards from 2013 to 2017.Results The kinds of science and technology awards were up to 11.The 23 awards were obtained from 1998 to 2017,which were mainly composed of social association awards (65%) and gave priority to the second rank (48%).The age of the first winners were from 51 to 55 years.There were 9 departments obtained awards,accounting for 45% of all the hospital clinical medical department.The increasing trend in the number of awards were observed from 1998 to 2017,and in nearly five years,the number showed the fast rising trend.Conclusions The less number of awards were obtained in Beijing Obstetrics and Gynecology Hospital,Capital Medical University.Whereas the most of awards were supported by social association.We were lack of the high rank and high level awards.The higher age showed that there were not enough youth scientific research and talent reserves.The situation need to be further strengthened the management and promoted the development in science and technology awards.
6.The progress and prospect of assessment measures of uterine cavity function for infertility patients

Objective:To explore the progress and prospect of evaluation methods for infertility patients.Methods:From September 2017 to December 2020,199 cases of infertility patients who have accepted mini-hysteroscopy examination in the Department of Gynecological Endocrinology,Beijing Obstetrics and Gynecology Hospital,Capital Medical University.The patients who had primary ovary insufficiency,tubal infertility were excluded;patients with hysteroscopy contradiction,such as acute or sub-acute vaginal tract inflammation,en-dometrium lesion or endometrium carcinoma,severe cardiovascular,liver or kidney insufficiency were excluded.40 patients were excluded and 15 patients lost follow up,144 patients were eventually included in the analysis.The patient's age,body mass index,infertility etiology,preoperative and postoperative stimulate ovulation cycle,pregnancy state and pregnancy time were recorded.The effect of mini-hysteroscopy on ovulation induction cycle was analyzed in pregnant patients.Results:The average age of 144 patients was 32.10±4.67 years old.96 (66.67%) patients were found to have intrauterine abnormalities,while 48 (33.33%) patients were found to have normal intrauterine cavity.48 cases of pregnancy,accounting for 33.33%;the median and interquartile ranges[M (Q1,Q3)]of ovulation induction cycles before and after surgery were respectively[3 (0,5)],[2 (1,3)],the difference was statistically significant(P < 0.05).Among the 48 pregnant patients,32 of them had no structural lesions in hysteroscopy,the ovula-tion induction cycle of these patients before and after hysteroscopy were respectively[5 (0,9)],[1 (0,3)],the difference was statistically significant (P < 0.05).Conclusion:Uterine cavity evaluation is important for the assessments of fertility.As an advanced diagnosis instrument,mini-hysteroscopy can not only timely detect the intrauterine abnormalities of patients,but also with great application value in shorten the ovulation cycle in infertility patients and increase the pregnancy rate.
7.Thoughts on the procurement and management of research-use reagents and consumables in hospitals

Objective Along with the fast increasing of scientific research capacity,more and more financial funding were available at hospital level,at the same time,the kinds and quantities of the reagents and consumables for scientific research uses are also increased.As a result,procurement risks become a gig concern in hospitals.Thus,how to increase the cost-effectiveness of their financial funds while lowering the risks and hidden troubles are the key issues need addressing urgently.Methods This paper took Beijing Obstetrics and Gynecology Hospital,Capital Medical University as an example,summarized its service conditions of reagents and consumables for scientific research uses in past three years (2014-2016),according to the comparison and analysis of their actual prices with the guiding prices on the same as indicated on the informational procurement platform,to discuss the existing issues and risks.Results Under the current procurement management system of reagents and consumables for scientific research uses,the purchasing price for some items are relatively high.Conclusions This paper specifically proposed some measures and advices on risk prevention and control during the period of transition with the intention to reduce the purchasing cost,increasing the costeffectiveness of the financial funds,and lowering the risks and hidden troubles in procurements.
8.Project management of clinical research initiated by investigators in medical institutions

Objective:This paper aimed to explore the problems and solutions in the management of Investigator-Initiated Trail (IIT) in medical institutions, to provide suggestions particularly in the absence of relevant high-level policy support.Methods:By combining literature review with working practice experiences, this paper analyzed the current situation of IIT’s management and the rationale, put forward suggestions.Results:IIT related problems identified including as the responsible entities of the management, research topics selection, protocol design, continuity, research team construction, ethical review and the cultivation of research management personnel. The centralized management will help the standardized management of such projects, the training of investigators and team building, the review awareness strengthening of the ethics committee, improvement of management ability.Conclusions:IIT plays an important role in promoting the development of medical scientific research. Medical institutions should integrate resources, provide platforms, train investigators assure the quality of IITs.
9.Safety analysis of cesarean myomectomy in twin pregnancies with intramural myomas

Objective:To estimate the safety of myomectomy in twin pregnant women with intramural myomas during cesarean section.Methods:The clinical data of 145 cases of twin pregnancies with intramural myomas who were delivered by cesarean section in Beijing Obstetrics and Gynecology Hospital, Capital Medical University from June 2013 to December 2021 were collected. Maternal demographics, fibroids′ characteristics, maternal and fetal outcomes were compared between groups of cesarean section with myomectomy (myomectomy group, 49 cases) and cesarean section only (non-myomectomy group, 96 cases).Results:Compared with non-myomectomy group, myomectomy group had significantly prolonged operative time [50.0 minutes (37.5-57.5 minutes) vs 40.0 minutes (35.0-50.0 minutes), respectively; P=0.007] and significantly longer postoperative hospital stay [4.0 days (3.0-4.0 days) vs 3.0 days (3.0-4.0 days), respectively; P=0.047). Other maternal and fetal outcomes such as estimated blood loss, hemoglobin difference, postpartum hemorrhage, blood transfusion, B-Lynch structure, uterine artery ligation, postoperative fever and neonatal Apgar score showed no significant differences (all P>0.05). For intramural myomas <5 cm, there were no significant differences in maternal and fetal outcomes between myomectomy group and non-myomectomy group (all P>0.05). For intramural myomas ≥5 cm, operative time [55.0 minutes (40.0-60.0 minutes) vs 42.5 minutes (40.0-50.0 minutes), respectively; P=0.019] was significantly prolonged, postoperative hospital stay [4.0 days (4.0-5.0 days) vs 4.0 days (3.0-4.0 days), respectively; P=0.048] was significantly longer in myomectomy group than non-myomectomy group, but there were no significant differences in other maternal and fetal outcomes (all P>0.05). Conclusion:For twin pregnancies with intramural myomas, it is safe and feasible to remove intramural myomas during cesarean section by experienced obstetricians.
