1.The Clinical Epidemiological Analysis of 1433 Cases of Type 2 Diabetes and Their Diabetic Complications

Objective To investigate the frequency of type 2 diabetes and diabetic complications. Methods The clinical and laboratory data of 1433 cases of patients with type 2 diabetes mellitus were retrospectively analyzed. Results The frequency of diabetic complications of patients with type 2 diabetes mellitus were 88.28%(1626/1433). The rates of complicated hypertension, coronary heart disease, peripheral neuritis, diabetic renopathy, diabetic retinopathy, hyperlipoidemia, urethritis, diabetic acidosis, pneumonia, fatty liver, diabetic foot gangrene, cholecystitis, skin infection, brain vessel disease and vegetative nerve disease were 46.9%, 33.2%, 29.7%, 25.4%,22.0%,13.0%,7.5%,4.2%,4.0%,4.0%,3.5%,3.5%,3.3%,2.9%,and 2.0%, respectively. Conclusion The frequency of diabetic complications of patients with type 2 diabetes mellitus were much high, and the distrubution of diabetic complications was different in different ages, course of disease and syndromes.

3.Psoralen reverses glutathione-S-transferaseπ-mediated multidrug resistance in breast cancer stem cells

BACKGROUND:Breast cancer stem cells not only lead to theoccurrence of breast cancer, but also may cause breast cancer metastasis and recurrence. The relationship between stem cells and cell resistance is also gaining increasing attentions, and the focus on the stem cell treatment may result in unexpected results.OBJECTIVE:To explore the reversal effect of psoralen on glutathione-S-transferase π (GST-π) in human breast cancer MCF-7/ADR cells and its mechanism.METHODS:MCF-7/ADR cells were cultured and enriched in serum-free medium to obtain breast cancer stem cells.RT-PCR and western blot were used to detect the expression of GST-π at the levels of gene and protein in the MCF-7/ADR cells after treatment with 0, 4, 8, 12, 16 mg/L psoralen. To observe the activation of nuclear factor-κB,western blot was used. The expression of GST-π was detected by RT-PCR in 18 μmol/L SN50 group and 8 mg/L psoralen group. Cell counting kit-8 assay was used to detect the effect of doxorubicin on cell proliferation.RESULTS AND CONCLUSION:Compared with the control group, psoralen reduced the expression of GST-π at the mRNA and protein levels, and significantly inhibited the activation of nuclear factor-κB. It was suggested that psoralen could reverse the multidrug resistance of human breast cancer MCF-7/ADR stem cells by decreasing the expression level of GST-π. The mechanism may be achieved by inhibiting the nuclear factor-κB signal pathway.
4.Effect of psoralen on TopoIIα expression in breast cancer stem cells

Objective To explore the effect of psoralen on topoisomerase IIα(TopoIIα) expression of breast cancer stem cells.Methods CD44+CD24-/low breast cancer stem cells were sorted from MCF-7/ADR by magnetic-activated cell sorting(MACS).We observed the growth characteristics of these stem cells through optical microscope and detected the growth-inhibitory effects of psoralen on breast cancer stem cells by CCK-8 assay and IC50 of adriamycin and adriamycin combined with psoralen to calculate the reversal index.The mRNA and protein expressions of Topo IIα were detected using RT-PCR and Western blot, respectively.Results Under the optical microscope, breast cancer stem cells presented spheres.IC10 and IC20 of psoralen on breast cancer stem cells were (6.77±0.23)μg/mL and (10.36±0.21)μg/mL.IC50 of adriamycin and adriamycin combined with psoralen on breast cancer stem cells was (90.03±3.56)μg/mL and (21.47±0.82)μg/mL, the reversal index was 4.19.Psoralen significantly raised the expressions of Topo Ⅱα at mRNA and protein levels.Conclusion Psoralen reversed the resistance of adriamycin by increasing the gene and protein expressions of breast cancer stem cells Topo Ⅱα and the drug targets.
5.Efficacy and Safety of Shenmai Injection in Adjuvant Therapy of Angina Pectoris:A Systematic Review

OBJECTIVE:To evaluate the therapeutic efficacy and safety of Shenmai injection in adjuvant therapy of angina pectoris,and to provide evidence-based reference in clinic.METHODS:Retrieved from CJFD,Wanfang database,VIP and PubMed,randomized control trials (RCTs) about Shenmai injection combined with routine plan (trial group) vs.routine plan sole (control group) in the treatment of angina pectoris were collected.Meta-analysis was conducted by using Rev Man 5.3 statistical software after data extraction and quality evaluation with Cochrane systematic evaluator manual 5.1.0.RESULTS:A total of 25 studies were included,involving 2 093 patients.Meta-analysis showed that clinical efficacy rate [RR=I.31,95% CI(1.25,1.37),P<0.001] and ECG total response rate [RR=1.42,95%CI(1.31,1.54),P<0.001] of trial group were significantly higher than those of control group,with statistical significance.Results of subgroup analysis showed that among high-dose subgroup(>40 mL/d) and low-dose subgroup(≤40 mL/d),clinical efficacy rate [high dose:RR=1.26,95% CI (1.19,1.34),P<0.001;low dose:RR=1.36,95% CI (1.27,1.45),P<0.001] and ECG total response rate [high dose:RR=1.39,95%CI(1.22,1.60),P<0.001;low dose:RR=1.43,95% CI (1.30,1.58),P<0.001] of trial group were significantly higher than those of control group,with statistical significance.There was no statistical significance in the incidence of ADR between 2 groups [RR=0.66,95%CI(0.32,1.40),P=0.28].CONCLUSIONS:Shenmai injection in adjuvant therapy of angina pectoris shows good therapeutic efficacy and safety in the treatment of angina pectoris.It is suggested to its clinical application start from the low dose.
6.Abdominal electroacupuncture for post-stroke constipation:a randomized controlled trial

Objective:To observe the effect of abdominal electroacupuncture(EA)on intestinal function in patients with constipation after stroke. Methods:A total of 100 patients with post-stroke constipation were divided into an observation group and a control group by the random number table method,with 50 cases in each group.The control group was treated with oral lactulose,10 mL each time,3 times a day,for 2 consecutive weeks.The observation group was treated with additional abdominal EA at Tianshu(ST25),Fushe(SP13),and Daheng(SP15)on the basis of oral lactulose,30 min for needle retention each time,once a day,for 2 consecutive weeks.The total effective rate after treatment,constipation symptom score,the score of patient assessment of constipation quality of life scale(PAC-QOL),serum motilin(MTL)level,and the score of mini-mental state examination(MMSE)were compared between the two groups. Results:After 2 weeks of treatment,the total effective rate was 93.5%in the observation group and 76.1%in the control group,and the difference between the two groups was statistically significant(P<0.05).The constipation symptom score,PAC-QOL score,MMSE score,and MTL level changed significantly after treatment in both groups(P<0.05).After treatment,the constipation symptom score and PAC-QOL score in the observation group were lower than those in the control group,while the MMSE score and MLT level were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05). Conclusion:On the basis of oral lactulose,EA at abdominal points can significantly improve the intestinal and cognitive function of stroke patients simultaneously.
7.Traditional Chinese Medicine Resource Census in Jilin Baishan Region

Taking the fourth census of resources of traditional Chinese medicine (TCM) as the background,this paper was aimed to study the local wild Chinese herbal medicine resources,to promote the cultivation development of local TCM industry.Related data was acquired through the survey of sample area,quadrat fixed-point investigation,and in combination with the field survey Ta-Zha way.The results showed that through the survey of Baishan city of medicinal plants,and visiting survey of TCM materials planting technology.This paper recorded 158 species of Chinese medicinal herbs which are common in Baishan area,belonging to 62 families.And many wild Chinese medicinal resources are widely exploited.It was concluded that this study provided important theoretical basis for the development and utilization of medicinal resources of Baishan and practice cultivation experience for reference.
8.Correlation between single nucleotide polymorphism of EIF2AK3 gene and the efficacy of recombinant human growth hormone in children with growth hormone deficiency

Objective:To investigate the association between single nucleotide polymorphism (SNP) of eukaryotic translation initiation factor 2-α kinase 3 (EIF2AK3) gene in growth hormone deficiency (GHD), and to determine whether the polymorphisms in the EIF2AK3 of children with GHD associate with the efficacy of recombinant human growth hormone (rhGH) therapy.Methods:Five SNPs of EIF2AK3 gene, including rs1805165 (G>T), rs13045 (A>G), rs867529 (C>G), rs11684404 (T>C), rs6547787(T>G) were selected and genotyped by a TaqMan probe method in 104 children with GHD and 269 normal height control children. Among them, 55 children with GHD were treated with rhGH. The height and weight of each patient with GHD after rhGH treatment were collected. Finally, to investigate whether there were differences between the efficacy of rhGH in children with GHD and different genotypes of EIF2AK3 gene polymorphism.Results:(1) The polymorphisms rs13045 and rs867529 of EIF2AK3 gene were associated with the occurrence of GHD ( P<0.05). (2) The haplotypes GACTG and GAGTT composed of SNPs rs1805165, rs13045, rs867529, rs11684404, and rs6547787 of EIF2AK3 gene increased the risk of GHD with OR (95% CI) of 2.05 (1.33-3.17) and 2.62 (1.48-4.65), respectively. The haplotypes GACTT and TGCCG reduced the risk of GHD, with OR (95% CI) of 0.68 (0.48-0.97) and 0.36 (0.23-0.57), respectively. (3) After determination the relationship between different genotypes and efficacy of rhGH with rs13045 and rs867529, it was found that there was no significant difference in height gain between rs13045 genotypes after rhGH treatment ( P>0.05). Compared with CC genotype, there was a less height gain of CG genotype at rs867529 by 0.099 cm for every 30 d( β=-0.099, 95% CI -0.162--0.018, P=0.016). Conclusions:The EIF2AK3 gene polymorphism (rs13045, rs867529) was associated with the occurrence of GHD. The height gain of CG genotype of rs867529 was lower than that of CC genotype in children with GHD treated with rhGH. The EIF2AK3 locus rs867529 genotype was associated with rhGH efficacy.
9.Postoperative complications in young adults with femoral neck fracture after internal fixation with compression buttress screws versus partially threaded cannulated screws: a prospective cohort study

Objective:To compare the complications in young adults with femoral neck fracture after internal fixation with compression buttress screws (CBS) versus 3 parallel partially threaded cannulated screws (PTS).Methods:A prospective study was conducted of the 120 young adults with femoral neck fracture who had been treated from July 2016 to December 2017 at Department of Orthopaedic Surgery, Shanghai Sixth People's Hospital. Of them, 60 were subjected to PTS fixation (control group) and 60 to CBS fixation (observation group) according to their will. In the control group, there were 38 males and 22 females with an age of (46.1±7.6) years, and 17 cases of types Ⅰ-Ⅱ and 43 cases of type Ⅲ by the modified Pauwels classification; in the observation group, there were 42 males and 18 females with an age of (44.8±8.1) years, and 11 cases of types Ⅰ-Ⅱ and 49 cases of type Ⅲ. The 2 groups were compared in postoperative complications. A stratified analysis was performed according to the modified Pauwels classification.Results:Comparability was indicated between the 2 groups because there was no significant difference in their baseline demographic information ( P>0.05). The incidences of fixation failure (8.3%, 5/60), nonunion (5.0%, 3/60), femoral neck shortening<10 mm(10.0%, 6/60) and lateral withdrawal (11.7%, 7/60) in the observation group were significantly lower than those in the control group [38.3% (23/60), 28.3% (17/60), 41.7% (25/60) and 71.7% (43/60), respectively] ( P<0.05). There was no significant difference in the incidence of avascular necrosis of the femoral head or of medial migration between the 2 groups ( P>0.05). The stratified analysis showed that the incidences of fixation failure and nonunion in the observation group were significantly lower than in the control group for fractures of the modified Pauwels type Ⅲ ( P<0.05). The incidences of femoral neck shortening<10 mm and fixation loosening in the control group were significantly higher than in the observation group for fractures of all the modified Pauwels types ( P<0.05). Conclusion:Compared with conventional PTS fixation, CBS fixation can significantly reduce postoperative complications in young adults with femoral neck fracture, especially those with high energy fracture of the modified Pauwels type Ⅲ.
10.Biomechanical properties of four internal fixations for femoral neck fracture: a finite element comparison

Objective:To compare the biomechanical properties of dynamic hip screw (DHS), traditional cannulated compression screw (CCS) configuration, traditional CCS configuration+medial locking plate and compression buttress screw (CBS) in the treatment of femoral neck fracture by finite element analyses.Methods:A simulation model of Pauwels type Ⅲ femoral neck fracture with discontinuous medial cortex was established by the finite element method. The maximum displacement, maximum principal stress, normal form equivalent stress, hip varus angle and fracture end stress were compared between DHS (group A), traditional CCS configuration (group B), traditional CCS configuration+medial locking plate (group C) and CBS (group D) in the simulation model.Results:In the internal fixation model in groups A, B, C and D, respectively, the maximum displacement of the femur was 0.41 mm, 2.04 mm, 0.94 mm and 0.30 mm; the maximum displacement of internal fixation 0.34 mm, 1.18 mm, 0.84 mm and 0.22 mm; the peak normal form stress of internal fixation 83.6 MPa, 231.4 MPa, 259.8 MPa and 194.8 MPa; the maximum principal stress of internal fixation 52.3 MPa, 216.3 MPa, 151.7 MPa and 74.6 MPa; the maximum normal form stress of the femur 101.1 MPa, 282.3 MPa, 100.5 MPa and 181.2 MPa; the maximum principal stress 99.7 MPa, 201.0 MPa, 60.9 MPa and 56.1 MPa; the axis angle of the femoral neck after loading 179.55°, 176.97°, 179.66° and 179.64°; the normal form equivalent stress at the fracture end ranged from 42.0 to 50.0 MPa, from 258.7 to 282.3 MPa, from 50.8 to 58.1 MPa, and from 45.3 to 60.4 MPa.Conclusion:Considering stability, stress distribution and prevention of hip varus and femoral neck shortening, CBS may be a choice treatment for femoral neck fracture because it is comparable to DHS in mechanical stability.
