1.Cushing′s disease: a comparison between pituitary macroadenoma and microadenoma

Objective To compare the difference of clinical characteristics between patients with pituitary corticotroph macroadenoma and microadenoma. Methods From 1985 to 1998, 76 inpatients with Cushing′s disease, 12 cases of macroadenoma and 64 cases of microadenoma were reviewed. The clinical manifestations, the course before diagnosis, blood routine, biochemical and endocrinic examinations were retrospectively analyzed. Results Defect of vision field and galactorrhea were more frequent and the duration of disease before diagnosis were shorter in macroadenomas than those in microadenomas. Lymphopenia, hypokalaemia, lower creatine kinase level and hyperphosphoroemia were also more pronounced in macroadenomas. In macroadenomas, the plasma ACTH and cortisol (F) concentrations at 0:00 were obviously higher than those in microadenomas, but plasma ACTH and F at 8:00 and 24h urinary free cortisol were similar in these two groups. The relative nyctohemeral variation of ACTH was less in macroadenomas than that in microadenomas, the relative nyctohemeral variation of F was similar in these two groups. One month after operation, clinicl manifestations of macroadenomas significantly improved, but the number of these patients with significant blood ACTH decrease was significantly less than that in microadenomas. Conclusion Due to higher amount and automatism of ACTH secretion, increased blood F level induces variation of clinical characteristics and laboratorial findings in patients with macroadenoma compared to the patients with microadenoma.
2.Character of early tinnitus about sudden deafness.

OBJECTIVE:
To study the clinical characteristics of early tinnitus in patients with sudden deafness.
METHOD:
Forty-seven patients with sudden deafness and early tinnitus recieved subjective review and filled out classified questionnaire of tinnitus. By pure tone audiometry and tinnitus matching test, we obtain the distribution, quantity and matching loudness of tinnitus patients.
RESULT:
(1) Objective examination: the most common frequencies of tinnitus were 8000 Hz and 4000 Hz. The most common numbers were two frequencies. The matching loudness was (16.63 ± 8.37) dB. (2) Subjective examination: the tinnitus was monotone (74.0%), sustained (89.0%), third level loudness (45.0%), third level annoying (60.0%), high frequency (83.0%), sound of cicadas (66.7%). The most influencing factors were hearing (28.2%) and mood (29.5%). (3) Relationship between tinnitus and pure tone hearing threshold curve: there was a significant correlation between the frequency of pure tone audiometry and the matching loudness of tinnitus (r = -0. 370, P < 0.01). There was no significant correlation between the frequency of pure tone tinnitus and the matching loudness of tinnitus (r = -0.083, P > 0.05).
CONCLUSION
(1) The results of subjective examination were consistent with those of objective examination. (2) The tinnitus should be considered in the early treatment of sudden deafness.

3.Simultaneous Determination of 20 Anti-Obesity Drugs Illegally Added in Weight-loss Functional Foods by Matrix Solid Phase Dispersion-High Performance Liquid Chromatography Tandem Mass Spectrometry

An analytical method based on high performance liquid chromatography tandem mass spectrometry has been developed for the simultaneous determination of 20 anti-obesity drugs ( fenfluramine, phenylpropanolamine, sibutramine, sertraline, rimonabant, bupropion, citalopram, fluoxetine, benfluorex, topiramate, zonisamide, caffeine, phenolphthalein, emodin, indapamide, bumetanide, torasemide, triamterene, orlistat, phenformin). that were extracted from various weight-loss functional foods by ethanol-acetone(7:3, V/V)and purified by primary secondary amine (PSA) and octadecyltrimethoxysilane(ODS) under ultrasonication. The analysis was carried out on HPLC-MS /MS by electrospray ionization using multiple reaction monitoring after the chromatographic separation on Waters Atlantis T3 (3 μm, 150 mm × 2. 1 mm) column. Identification was achieved by the retention time and the ion ratio, quantification was done by the external standard method. The limits of detection for the appetite suppressants were 0. 05-3. 0 mg/kg. The mean recoveries at the three spiked levels were 67 . 1%-101 . 4%, with the intra-day precision less than 10%and the inter-day precision less than 15%. The method is reliable, accurate, reproducible and suitable for the determination of the anti-obesity drugs in different weight-loss functional foods.
4.Impact of colpocleisis on body image in women with severe pelvic organ prolapse

Objective To investigate the impact of colpocleisis on body image in women with severe pelvic organ prolapse (POP). Methods From Oct. 2005 to Feb. 2010,60 POP patients with stage Ⅲ and Ⅳ by POP quantitation system underwent total or partial colpocleisis. Patients received body image evaluation before and 1 year after operation. Results One year after operation, 52 (87% , 52/60) patients completed body image evaluation. Before and 1 year after operation, the ratio of answer Not at all of questions such as Have you felt less physically attractive as a result of your vaginal prolapse? , Have you been feeling less feminine as a result of your vaginal prolapse? , Did you find it difficult to look at yourself naked? , Have you been feeling less sexually attractive as a result of your vaginal prolapse?, Have you felt dissatisfied with your body? were 25% and 96% ( P < 0. 01 ) , 21% and 96% ( P < 0. 01) , 37% and 67% (P = 0.018), 29% and 96% (P<0.01), 12% and 83% (P<0.01), respectively, indicating significant improvement on body image after operation for patients treated by colpocleisis. Conclusion Women underwent colpocleisis for severe POP could not decrease their body image as a result of the disability of vaginal intercourse.
