1.Optimization of the Extraction Technology of Compound Epimedium Chewable Tablets by Orthogonal Test

OBJECTIVE:To optimize the extraction technology of Compound epimedium chewable tablets. METHODS:Using the extraction rate of icariin,the content of crude polysaccharide and yield of extractum as index,the effects of water amount,ex-traction time and extraction times on extraction effect were investigated by orthogonal test. The extraction technology was optimized and validation test was conducted. RESULTS:The optimized extraction technology was as follows as 6-fold water,extracting 3 times,2 h each time. The validation test showed that the average extraction rate of icariin was(83.80±0.02)%,the average con-tent of crude polysaccharide was (29.28 ± 0.55) mg/g,and the average yield of extractum was (28.47 ± 0.29)%(RSD≤0.55%, n=3). CONCLUSIONS:The optimized extraction technology is stable,and can be used for the extraction of Compound epimedi-um chewable tablets.
2.Effect of pumping normal saline after norepinephrine therapy on hemodynamics of patients with hypovolemic shock

Objective:To explore the effect of continuous pumping normal saline at same infusion speed before flushing or sealing the tube on the hemodynamics of patients with hypovolemic shock receiving norepinephrine.Methods:A total of 56 cases of hypovolemic shock patients receiving norepinephrine via micro-pump were randomly assigned to the observation group and the control group (28 cases in each group). Patients in the control group received conventional operation method to flush or seal the tube, while patients in the observation group continued pumping normal saline at the same infusion speed, followed by flushing or sealing the tube based on the control group. The changes of hemodynamics after flushing or sealing the tube was compared between two groups.Results:After 30 s, 1 min, 2 min, 3 min, 5 min of flush or seal the tube, the systolic blood pressure were (105.4±17.4) mmHg, (106.3±21.3) mmHg, (102.1±14.5) mmHg, (100.6±16.9) mmHg, (101.0±14.3) mmHg (1 mmHg=0.133 kPa), mean arterial blood pressure were (81.1±8.6) mmHg, (82.4±9.9) mmHg, (78.6±7.3) mmHg, (76.9±6.7) mmHg, (75.7±6.5) mmHg in the observation group, lower than those in the control group [(150.6±26.7) mmHg, (151.8±30.1) mmHg, (139.7±29.8) mmHg, (125.3±25.3) mmHg, (114.4±21.6) mmHg and (107.4±11.6) mmHg, (106.1±11.5) mmHg, (98.1±11.1) mmHg, (88.9±9.6) mmHg, (79.5±8.0) mmHg], the differences were statically significant ( P<0.05). After 30 s, 1 min, 2 min, 3 min of flush or seal the tube, the diastolic blood pressure were (69.0±10.50)mmHg, (70.5±10.7)mmHg, (66.9±8.3)mmHg, (65.1±6.2)mmHg in the observation group, lower than those in the control group [(85.9±11.6) mmHg, (83.3±8.7) mmHg, (77.3±7.4)mmHg, (70.7±7.2) mmHg], the differences were statically significant ( t value was 10.199-18.464, P<0.05). Conclusions:Before flushing or sealing the tube, continue pumping normal saline at same infusion speed can reduce the risk of abnormal hemodynamics for the hypovolemic shock patients receiving norepinephrine via micro-pump.
3.Effect of Acupuncture Combined with Language Training on Subcortical Aphasia

Objective To observe the clinical effect of acupuncture combined with language training on subcortical aphasia.Methods 32 cases in treatment group and 26 cases in control group were given routine medication,in addition,treatment goup accepted acupuncture and language training.Results After 4 weeks of treatment,patients' language function improved significantly in treatment group compared with that of control(P<0.05).The total effective rate was 87.5% in treatment group,while it was 61.5% in control group(P<0.05).Conclusion Acupuncture combined with language training is effective on subcortical aphasia.
4.Effect of acupuncture using Xingnao Kaiqiao needling method on patients with post-stroke depression

ObjectiveTo investigate the effect of acupuncture using Xingnao Kaiqiao needling method on patients with post-stroke depression (PSD).Methods172 PSD patients were randomly divided into the acupuncture group and control group with 86 cases in each group. The acupuncture group was treated with acupuncture using Xingnao Kaiqiao needling method, and different symptoms were also punctured various acupoints; the control group was given fluoxetine treatment, 20 mg/d. Two groups were given the same routine treatment and early rehabilitation, 8 weeks being a course.ResultsThe total effective rates of the acupuncture group and control group were individually 89.54% and 73.26%, and there was a significant difference between two groups (P< 0.05).ConclusionAcupuncture treatment using Xingnao Kaiqiao needling method can effectively treat post-stroke depression without obvious side effect.
5.CT/MRI features and pathological analysis of primary renal rare benign tumors

Objective To improve the diagnostic accuracy of primary renal rare benign tumors by exploring and analyzing the CT/MRI and clinical pathologic features.Methods 9 patients with primary renal rare benign tumors pathologically proven lesions after operation who had CT or MRI exams with contrast were enrolled in our hospital.The radiological and clinical pathological features of all tumors were analyzed respectively.Results The cases were the mixed epithelial and stromal tumor of kidney (MESTK),cystic nephroma(CN),renal leiomyoma(RL)and renal oncocytoma(RO)respectively.Location:6 cases were in left kidney and 3 in right kidney.Size:long diameter 2.5-8.9 cm,mean 5.7 cm;short diameter 2.5-8.4 cm,mean 4.9 cm.The tumor shape included oval(n=7) and irregular(n=2).9 cases protruded from the renal surface.MESTK showed polycystic lesions on preconstrast,various degrees of cystic wall and septum enhancement were detected on enhancement scans.CN showed cystic lesions and calcification on preconstrast, slight septum enhancement were detected on enhancement scans.RL showed heterogeneous density or signal with patchy necrosis and clear border on precontrast,the obvious enhancement was found on cortex and medullary phase and slight washout enhancement was detected on excretory phase.RO showed heterogeneous hypointense on T1 WI,hyperintense on T2 WI and mixed isodensity and hypodensity on CT precontrast.Slight delayed enhancement was found in central scar and washout enhancement was detected in parenchyma on three phases.Conclusion There are some special CT/MRI characteristics for primary renal rare benign tumors,which could improve their diagnosis and differential diagnosis combined with the clinical pathological features.
6.Correlation between CHI3L1 polymorphism and serum YKL-40 and bronchial asthma

Objective To investigate the association between CHI3L1 polymorphism and asthma in the Han population of Guangdong,and to explore the clinical values of chitinase-like protein YKL-40 in evaluating disease severity and monitoring the progression of the disease.Methods Total 251 objects were divided into asthma group (n=150)and control group(n=101).The single nucleotide polymorphism (SNP)at CHI3L1 gene was detected with Massarray method. The serum YKL-40 levels, total IgE levels, peripheral blood eosinophil percentage (Eos%)of the obj ects in asthma and control groups were detected. The asthma patients were divied into asthma exacergation group and stable group,and the serum YKL-40 levels were compared.Results The distribution of A/G at rs3806448 of CHI3L1 had significant difference between asthma and control groups (P<0.01 ), and the frequency of A allele at rs3806448 of the patients in asthma group was significantly higher than that in control group (OR= 1.93,95% CI=1.30-2.62,χ2=11.6,P<0.01).The serum YKL-40 levels,total IgE and Eos% of the patients with AA at rs3806448 were higher than those of the patients with GG or AG at rs3806448 (P<0.05);the FEV1% of the patients with AA was lower than those of the patients with GG or AG at rs3806448(P<0.01). The serum YKL-40 level of the patients in asthma group was significantly higher than that in normal control group (P<0.01),and the serum YKL-40 level of the patients in asthma exacergation group was higher than those in asthma stable group and normal control group(P<0.01).The serum YKL-40 level of the patients in asthma group was positively correlated with the peripheral blood Eos% and total IgE level(rEos=0.348,rIgE=0.437,P<0.01), and was negatively correlated with the lung function(r=-0.745,P<0.01). Conclusion The polymorphism of CHI3L1-rs3806448 may correlate with asthma of Han population in Guangdong area.The serum YKL-40 may be a new biomarker to evaluate asthma disease severity and helpful for monitoring illness stage.
7.Pathogenesis of influenza A (H5N1) virus-induced viral pneumonia in the rhesus macaque

At present ,the mechanism of highly pathogenic avian influenza H5N1 virus causing human infection or death is still not fully clear .In order to better understand the pathogenesis of the disease ,the rhesus macaques were infected with H5N1 virus (AF148678/ACGoose/Guangdong/11961H5N1) .We analyzed the clinical symptoms ,characteristics of the virus invades body ,pathological changes ,and immune response to discuss the pathogenesis of viral pneumonia induced by H 5N1 virus infection from the early time to the recovery time .The rhesus macaques were infected with H5N1 virus through nasal .Clinical signs were assessed daily ,and major organs and blood were collected for detection of blood routine analysis ,viruses were isola-ted and titrated from organs ,and pathologic and immunohistochemical were also conducted .As a result ,the rhesus macaques in-fected with H5N1 virus experienced fever ,dyspnea ,and anorexia .The respiratory tract was the major target of the virus and the virus could not replicate in organs outside the respiratory tract .Positive staining cells by immunohistochemistry were bronchial epithelial cells and alveolar macrophages .Rhesus macaques experienced temporary severe pneumonia after 1-3 days ,mainly be-cause of neutrophils infiltration ;gradual recovery 6 days later ,mainly with macrophage infiltration ;lung tissue presented recov-ery state after 14 days ,mainly with T lymphocytes infiltration .Finally ,we concluded that the predilection of the H 5N1 virus to infect the lower airway suggests that it may be a limiting factor in human-to-human transmissibility of the H5N1 virus .The pathogenesis may include virus invasion ,replication and immune injury .
8.Repeated Injection of Botulinum Toxin Type A for Lower Limb Spasticity after Stroke

Objective To observe the effects of repeated Botulinum toxin type A (BTX-A) injection on lower limb spasticity after stroke.Methods 180 cases with lower limb spasticity after stroke were divided into the treatment group (n=90) and the control group (n=90). The treatment group was treated with BTX-A injection twice in the spastic muscles at interval of 3~6 months, while both the treatment group and the control group accepted the rehabilitation based on the neurodevelopmental therapy. They were assessed with modified Ashworth Scale (MAS), Fugl-Meyer Lower Limb Assessment (FMAL), Berg Balance Scale (BBS), modified Barthel Index (MBI) before each injection, and 3 d, 7 d, 1 month, 3 months after each injection or the same time for the controls. Results There was significant difference in scores of MAS, FMAL, BBS, MBI for the treatment group among before and 3 d, 7 d, 1 month after each injection (P<0.05), but not significant between 2 injections (P<0.05). There was significant difference in scores of all the assessment between the treatment and control group at the same time (P<0.01). Conclusion Repeated intramuscular injection of BTX-A can reduce the spasticity of lower limb after stroke.
9.The effect of tanshinone on meibomian gland cell proliferation, differentiation and lipid synthesis

Objective To investigate the effects of cryptotanshinone and tanshinone Ⅱ A,two major monomer components of tanshinone,on the proliferation,differentiation and lipid synthesis of rat meibomian gland epithelial cells in vitro.Methods The eyelid meibomian gland tissue was isolated from 2-month-old SD rats and co-cultured with 3T3 trophoblasts for 5 days.Cryptotanshinone and tanshinone Ⅱ A were prepared with DMSO into different concentrations.The cells were grouped to 0.125 μmol/L,0.250 μmol/L,0.500 μmol/L,1.250 μmol/L and 2.500 μmol/L drug groups and treated for 48 hours,respectively.Only dimethyl sulfoxide (DMSO) was added in medium in the DMSO control group.The expressions of keratin 14 (K14) and p63 in frozen sections of meibomian gland tissue and clones of meibomian gland cells were detected by immunofluorescence.Real-time fluorescence quantitative PCR was used to assay the relative expression of K16,cell proliferation related antigen 67 (Ki67) and CCAAT enhancer binding protein α (C/EBPcα) in meibomian gland cell clones.Crystal violet staining and oil red staining were used to evaluate the colony formation rate and lipid synthesis of meibomian gland epithelial cells.Results Primary cultured meibomian gland cells were cloned on day 4-5 in vivo,and the cloning area was increased on day 7 after culture,p63 and K14 were positively expressed in clones.Compared with the DMSO control group,the relative expression levels of Ki67 mRNA were significantly elevated in the 0.500 μmol/L,1.250 μmol/L and 2.500 μmol/L cryptotanshinone groups (all at P < 0.05).The relative expressions of Ki67 mRNA in the 0.500 μmol/L and 1.250 μmol/L tanshinone Ⅱ A groups were significantly higher than those in the DMSO control group (all at P<0.05).No significant difference was found in the relative expression of K16 and C/EBPα mRNA among different concentrations of cryptotanshinone or tanshinone Ⅱ A group (all at P>0.05).No lipid drop was found in the tarsal gland cell clones;however,the accumulation of lipid was seen in the cell clusters at the margin of the clones by oil red O staining.The average clone formation rate of tarsal gland cells in the 1.250 μ mol/L cryptotanshinone group was (2.55±0.20)%,which was significantly higher than (2.05±0.13)% in the DMSO control group (t =4.379,P<0.05).The average clone formation rate of tarsal gland cells in the 1.250 μmol/L tanshinone Ⅱ A group was (2.25±0.20)%,there was no significant difference between 1.250 μmol/L tanshinone Ⅱ A group and DMSO control group (t=1.616,P>0.05).Conclusions Cryptotanshinone and tanshinone Ⅱ A promote the proliferation of meibomian gland epithelia cells,but play less impacts to lipid synthesis of meibomian gland epithelia cells in vitro.cryptotanshinone promote the clone tormation of meibomian gland epithelia cells.
10. Value of fibrinogen to albumin ratio on predicting spontaneous recanalization of infarct-related artery in patients with acute ST-segment elevation myocardial infarction

Objective:
To investigate the value of fibrinogen to albumin ratio (FAR) at admission on predicting spontaneous recanalization of infarct-related artery (IRA) in patients with acute ST-segment elevation myocardial infarction (STEMI).
Methods:
Clinical data from 255 acute STEMI patients ((61.1±11.2) years old, 189 males) who underwent emergency coronary angiography within 12 hours in our hospital from December 2015 to April 2018 were retrospectively analyzed. The acute STEMI patients were divided into non-spontaneous recanalization group (thrombolysis in myocardial infarction (TIMI) flow grade 0-1, 203 cases) and spontaneous recanalization group (TIMI flow grade 2-3, 52 cases). Multivariate logistic regression analysis was used to evaluate related factors of IRA spontaneous recanalization. The receiver operating characteristic (ROC) curve was used to evaluate the value of FAR in predicting spontaneous coronary recanalization.
Results:
There was no significant difference in age,gender, hypertension, diabetes, smoking,systolic blood pressure,diastolic blood pressure,heart rate, duration of chest pain, type of infarction, infarct-related artery, door-to-balloon time, and drug used before admission between non-spontaneous recanalization group and spontaneous recanalization group (all
