1.Impact of Butylphthalide on Homocysteine, CRP and Nerve Function in Patients with Acute Progressive Cerebral Infarction

Objective:To investigate the impact of butylphthalide on homocysteine, CRP and nerve function in the patients with a-cute progressive cerebral infarction. Methods:Totally 122 cases of patients with acute cerebral infarction were randomly divided into the observation group (n=60) and the control group (n=62). The control group was treated with the routine treatment, and the ob-servation group was treated with butylphthalide additionally. The treatment course was two weeks. The total effective rate, blood Hcy, the level of plasma CRP and the changes of neurological function deficits scale in both groups before and after the treatment were ob-served and compared. Results:The overall response rate in the observation group (81. 67%) was significantly higher than that in the control group (64. 52%). After the treatment, the blood Hcy and CRP in both groups were significantly lower than that before the treatment(P<0. 05), and that of the observation group was significantly lower than that of the control group(P<0. 05). The neuro-logical function deficits scale in both groups were significantly lower than that before the treatment(P<0. 05), and that of the observa-tion group was significantly lower than that of the control group at the same period. The two groups during the treatment showed no sig-nificant adverse reactions. Conclusion: The effect of butylphthalide in the patients with acute cerebral infarction is effective, which can reduce blood Hcy, the level of plasma CRP and the neurological function deficits scale, and improve the neurological function with-out obvious adverse reactions during the treatment course.
2.Influence of Sijunzi Decoction on the pharmacokinetics of Levofloxacin in experimental spleen dificiency rat

AIM: To investigate the influence of Sijunzi Decoction(Radix et Rhizoma ginseng,Rhizoma atractylodis macrocephalae,poria,Radix et Rhizoma glycyrrhizae) on the pharmacokinetics of Levofloxacin(LVFX) in rats with deficiency of spleen. METHODS: Twenty-four rats were randomly divided into four groups: NS(normal rats given 0.9% saline solution),NS+SJZT(normal rats given 0.9% saline solution and Sijunzi Decoction),R(rats pretreated with Reserpine) and R+SJZT(rats pretreated with Reserpine and then cured with Sijunzi Decoction).After a single oral administration of LVFX 20 mg/kg,blood samples were collected at different intervals.The concentrations of LVFX plasma were determined by HPLC.Pharmacokinetic parameters were determined from the plasma concentration-time data. RESULTS: Reserpine led to the syndromes similar to the deficiency of spleen,a traditional Chinese medicine syndrome.The pharmacokinetic parameters of LVFX in NS,NS+SJZT,R and R+SJZT groups were as follows: AUC_((0-∞))=(8.55 ?0.99),(7.41?1.39),(4.68?0.95) and(7.89?1.41)mg/(L?h),respectively and C_(max)=(3.31?0.63),(2.38?1.15),(1.29?0.45) and(3.35?1.15) mg/L,respectively.Compared with the parameters of LVFX in NS group,Reserpine markedly decreased AUC_((0-∞)) and C_(max) of LVFX(P
3.Analysis of Pathogen Distribution and Drug Resistance of Cerebrospinal Fluid in Pyogenic Encephalitis Pa-tients in Our Hospital during 2011-2014

OBJECTIVE:To analyze the distribution and drug resistance of pyogenic encephalitis in our hospital,and to pro-vide reference for rational use of antibiotics. METHODS:The cerebrospinal fluid pathogen of 4 255 patients with pyogenic encepha-litis in our hospital during Jan. 1st,2011-Dec. 31st,2014 were cultured and identified,and drug sensitivity test was conducted. RE-SULTS:A total of 834 pathogens were isolated with positive detection rate of 19.6%,including 576 strains of gram-positive bacte-ria,accounting for 69.1%;255 strains of gram-negative bacteria,accounting for 30.6%;3 strains of fungi,accounting for 0.4%. Top 3 gram-positive bacteria were Coagulase-negative staphylococci(436 strains),Staphylococcus aureus(56 strains)and Entero-coccus (29 strains). Top 3 gram-negative bacteria were Klebsiella pneumoniae (46 strains),Acinetobacter baumannii (38 strains) and Escherichia coli(31 strains). Top 3 departments were neurosurgery department(506 strains),ICU(169 strains)and severe re-spiratory disease department (64 strains). Results of drug sensitivity test showed that no drug-resistant Staphylococcus and Entero-coccus strains to vancomycin,teicoplanin and miuocycline was found;nonfermenting Gram-negative bacilli showed low resistant to minocycline. CONCLUSIONS:Gram-positive bacteria dominates the detection rate of cerebrospinal fluid of pyogenic encephalitis patients;drug resistance of various pathogens is serious and clinicians should choose antibiotics based on drug sensitivity.
4.Establishment and elementary application of normative screening model in stroke risk group

Objective To establish the normative screening model and apply it to screen and manage the stroke risk group ,and to observe its application effect .Methods The screening management team in our department was set up for establishing the norma-tive stroke screening model .Then the stroke screening works were carried out in the outpatient department and the community pop-ulation .The screened groups of stroke high risk were implemented the 1-year control according to the guidelines of stroke screening and intervention .Results 9 631 individuals of stroke risk were screened ,among them 1 786 high-risk patients were screened out with a rate of 18 .55% .The interventions on the risk factors existing in the high-risk patients were implemented .The comparison of blood glucose ,blood pressure ,blood fat ,fat index ,smoking ,physical exercise and reasonable diet before and after interventions in the high-risk patients showed statistically significant differences (P<0 .05) .Conclusion Establishment of the normative manage-ment model of stroke screening is the important guarantee to do a good job of stroke screening and control for high risk group ,and significantly decreases the risk factors for high risk group .
5.The regulation and the mechanism of melatonin in calcium overload of pancreatic acinar cell

Objective To investigate the regulation and the mechanism of Melatonin in calcium overload of pancreatic acinar cell in acute necrotizing pancreatitis (ANP). Methods Fifty-four Sprague-Dawley (SD) rats were equally divided into three groups:sham-operation group (SO group),ANP group (created with retrograde cholangiopancreatography injection of sodium taurocholate) and MT group (ANP model made after intra-peritoneal injection MT for 30 mins).Rats were sacrificed at 1,4 and 8hours after operation and pancreas tissues were underwent pathological examination.The free calcium concentration of pancreas tissues was determined by fluorescence minitoring method; and the expression of CaMK Ⅱ in pancreas tissues at mRNA and protein level was tested by real-time PCR and Western Blot.Results Pancreatic pathological injury in ANP groups was progressively severe as time extended,which was obviously ameliorated in MT group compared with ANP group (the t value of compared pathological score at 1,4 and 8 hour was:-7.95,-9.72 and -7.69,all P=0.00).Compared at same time point,the free Ca2+ concentration of pancreas tissues in ANP group was significantly higher than that of SO group (the t value of 1,4 and 8 hour was 13.09,18.58 and 16.56,all P=0.00).It was a little bit higher in MT group compared with that of SO group,however was significantly lower than that of ANP group (the t value of 1,4 and 8 hour was -10.03,-11.75 and -11.02,all P =0.00).Compared with SO group,the expression of CaMK Ⅱ at mRNA and protein level significantly increased in ANP group; MT significantly inhibited its expression.Conclusions The expression of CaMK Ⅱ may be inhibited by MT interfere,and then lower the calcium overload in pancreatic acinar cell,which play a role in pancreas tissue protection.

Objective To study the effect of simvastatin on the radiosensitivity of esophageal squamous cell carcinoma .Methods Interrupted radiation was used to induce the radioresistance EC 9706 ,named R‐resistant cell . The effect of simvastatin on the proliferation and radiosensitivity of EC 9706 and R‐resistant cells were examined by Cell Counting Kit‐8 assay and colony formation assay ,respectively .Results The radioresistance EC9706 ,named R‐resistant ,was induced successfully .The results of Counting Kit‐8 assay and colony formation assay indicated that simvastatin could sensitize EC9706 and the radioresistance of R‐resistant to irradiation .Conclusion Simvastatin can sensitize esophageal squamous cell carcinoma to irradiation .
7.Investigation on the management status of breast cancer-related lymphedema prevention and treatment

Objective:To understand the management status of breast cancer-related lymphedema, and to provide a basis for the formulation of prevention and treatment plans for breast cancer-related lymphedema.Methods:Using accidental sampling, a questionnaire designed by the research team was used to survey nurses in 58 hospitals in Jiangsu Province.Results:This study surveyed a total of 58 hospitals, of which 53 were general hospitals and 5 specialized hospitals. Only 11 hospitals (18.97%) had treatment rooms dedicated to conservative treatment of lymphedema, 9 hospitals (15.52%) had opened lymphedema clinics, and 15 hospitals (25.86%) had professional international lymphedema therapists. All the 58 hospitals had different degrees of prevention and management of breast cancer-related lymphedema.Conclusions:The comprehensive management of lymphedema has not yet been achieved, and the management standards of lymphedema also need to be further improved. It is important to establish a unified and standardized management plan and practice standards.

9.Effects of regenerated silk fibroi film on cytokine expression in the Ad-VEGF165 transgenic fibroblast

BACKGROUND:Previous studies have demonstrated that regenerated silk fibroin film can promote pcDNA3.0-vascular endothelial growth factor 165(rEGF165) transfected L929 cells to express VEGF.OBJECTIVE:To investigate the effects of regenerated silk fibroin film on expression of cytokines related to angiogenesis in the fibroblasts transfected by adenovirus mediated-VEGF165(Ad-VEGF165).DESIGN,TIME AND SETTING:ontrolled observational cell gene engineering experiment performed by analysis of variance at the Laboratory of Cellular and Molecular Biology,Soochow University between November 2007 and ApriJ 2008.MATERIALS:Regenerated silk fibroin film was provided by Professor Li Ming-zhong,who was from Department of Material Science and Engineering,Soochow University.METHODS:The QBI-293A and WI-38 fibroblasts cultured on the regenerated silk fibroin film.polyvinyl chloride film and (Ad-GFP)and treated by phosphate buffered saline(PBS)for controls.MAlN OUTCOME MEASURES:VEGF mRNA was detected by real-time reverse transcription-polymerase chain reaction (RT-PCR);the expression levels of VEGF,angiogenin 1(Ang 1),fibroblast growth factor 2(FGF2),and platelet-derived growth factor(PDGF)were detected by enzyme-labeled immunosorbent assay(ELISA).RESULTS:The VEGF mRNA expression in the fibroblasts cultured on the regenerated silk fibroin film was increased but that in the fibroblasts cultured on the polyvinyl chloride film was signifcantly decreased(P<0.05).ELISA results demonstrated that not only VEGF gene expression in 293A and WI-38 cells transfected bv Ad-VEGF165 cultured on regenerated silk fibroin film was high,but also Ang 1 expression increased significantly(P<0.05).Meanwhile,the expression levels of FGF2 and PDGF were normalin the fibroblasts cultured on the regenerated silk fibroin film.CONCLUSION:Adenovirus vector can be effciently transfected into fibroblasts cultured on the regenerated silk fibroin film and can express VEGF and Ang 1 protein with highly biological activity,which accelerates angiogenesis.Regenerated silk fibroin film also can maintain the normal expression levels of FGF2 and PDGF,which ale related to wound healing.
10.Analysis of driver gene mutations in colorectal cancer by using next-generation sequencing

Objective:To investigate the characteristics of gene mutations in colorectal cancer(CRC)patients by using next-generation generation sequencing(NGS).Methods:Blood and tissue samples were collected from 90 CRC patients admitted to Beijing Hospital between August 5, 2016 and December 29, 2020.Analysis of driver gene mutations was performed by using a 1021-gene NGS panel.Results:There were 43 tissue samples and 83 blood samples.Also, 36 patients had both tissue and blood samples.The frequency rates of KRAS and BRAF mutations were 51.2%(22/43)and 20.9%(9/43)in tissue samples, and 3 rare concomitant KRAS/ BRAF mutations were detected.The frequency rates of KRAS and BRAF mutations were 26.5%(22/83)and 10.8%(9/83)in blood samples.In patients with tissue and blood samples, the rates of KRAS and BRAF mutations were 52.8%(19/36)and 10.8%(8/36). Conclusions:The rate of KRAS mutations in tissue samples from colorectal cancer patients is similar to rates reported in the literature, but the rate of BRAF mutation and the rate of rare KRAS and BRAF co-mutations are higher than those reported from other countries.
