Objective We discussed the optimal method for intestinal preparation before colonoscopy examination in order to increase the success rate of intestinal examination and reduce the incidence of adverse effect. Methods We divided 120 patients who were to undergo colonoscopy examination into group A, B and C with 40 cases in each group. Group A received oral magensium sulfate 100ml and 6000ml water after that on the morning of the examination. Group B received oral magensium sulfate 50ml and 2000ml in the evening before the examination, oral magensium sulfate 50ml and 400ml water on the morning of the examination. Group C was given oral magensium sulfate 100ml and 4000～5000 water at the same time. Group A and B took part in appropriate activity and were given health education. The effect of intestinal preparation was compared between the three groups. Results The cleaning degree of group B was better than those of the other two groups (P<0.05). The adverse effect of group B was less than that of group C (P<0.01). Conclusions Oral intake of magensium sulfate in the evening before and on the morning of the examination for intestinal preparation could increase the cleaning degree of intestine, facilitate the observation of disease part and lessen adverse effect. It gave satisfying examination results and made patients satisfied.

Objective To assess the efficacy and safety of two kinds of arterial suture-mediated closure devices (Angioseal and Perclose) in patients after coronary arteriography(CAG) and percutaneous coronary intervention (PCI). Methods 1?020 patients (672 male, 348 female, mean age 57.8?2.4 yr) undergoing CAG or PCI were randomized into either the Angioseal group (CAG 380, PCI 120) or the Perclose group (CAG 392, PCI 128). The procedural success rate and occurance of local complications were compared between the two groups. Results Both kinds of devices had a high success rate (Angioseal 94% vs Perclose 96%) and there was no statistical difference between them. There were five cases of hematoma and no pseudoaneurysm in the Angioseal group. On the other hand, there were four cases of hematoma and three cases of pseudoaneurysm in preclose group. There was no statistal difference in terms of local complications between the 2 groups. Conclusion Both kinds of arterial suture-mediated closure devices (Angioseal and Perclose) are feasible and safe for patients undergoing CAG and PCI.

20/min,PaCO2 12?109 or 0.10.26 patients with sepsis infected by Gram negative bacilli were categorized as G-group,15 infected by Gram positive coccus as G+ group and 15 healthy persons as control group.All the cases were proved to have infection by body fluid examination under microscope or bacterial culture.The level of flagellin was determined by ELISA.The peripheral blood mononuclear cells(PBMC)were isolated by the density gradient centrifugation,and total RNA of PBMCs was extracted by RNA BIOzol extract.The level of TLR5 mRNA in the peripheral blood was detected by semi-quantitative RT-PCR.The statistic significance among groups was examined by Students' t test.Results Plasma flagellin levels were below the detection limit of the ELISA kit in both G+ and control groups,while the mean level of plasma flagellin amounted to 6.636?5.147ng/ml in G-group.TLR5 mRNA expression in G-group was significantly higher than that in G+ and control groups,and no significant difference was found between G+ group and control group.Conclusion Flagellin may be involved in the development of ALI through interacts with TLR5.

Objective To investigate the current condition of essential hypertension in the elderly aged over 60 yrs in rural and urban areas in Jinan. Methods 2 626 subjects aged over 60 years in rural and urban of Jinan were involved and were examined. The investigation was based on the unified questionnaire. The contents of questionnaire mainly included the history, treatment and control of hypertension. Results Prevalence of hypertension and rate of treatment in the male elderly in urban were 64.1% and 61.5% , respectively, and were higher than those in rural (56.9% and 38.3%, respectively). Objective To summarize the experiences in clinical diagnosis and surgical treatment of chest cancers invading the superior vena cava (SVC). Method Retrospective study of 12 cases with chest cancers invading the SVC, SVC and innominate vein were reconstructed with prosthesis in 3 cases, the side wall of the SVC was excised and repaired in 9 cases. Results Complete resection was performed in 10cases,2 cases had incomplete resection. There was no death associated with operation in these patients. All patients were followed up for 1 - 7 years after operation,and 3 cases were lost to follow up,5 of remaining 9cases were dead because of relapse and metastasis, their median survival time was 8 months,other 4 cases lived, their median survival time was 42 months. Conclusions Patients with chest cancers invading the SVC should have surgical treatment of not only tumor resection but also the vessel reconstruction,the prognosis would be better than without surgical treatment. These patients have a high quality of life and live with long-term survival.

Objective To evaluate the combined effects of photodynamic therapy (PDT) with PSD-007 and cytarabine (Ara-c) on human acute promyelocyte leukemia cell HL-60.Methods The experiments were divided into four groups:control group,PDT-only groups (PDT 1-4 groups:the combination of PSD-007 concentrations (5 μg/ml and 7.5 μg/ml) and the energy density of laser (EDL) (1.2 J/cm2 and 2.4 J/cm2),Ara-c-only groups (Ara-c A group:0.3μg/ml,Ara-c B group:1.2μg/ml) and combination groups (the pair-wise combinations of the PDT doses and Ara-c doses above).All combination groups were treated with three treating methods,including P24A (the cells were treated with PDT for 24 h,and then cocultured with Ara-c for another 24 h),A24P (treated with Ara-c for 24 h before PDT,and then cocultured with PDT for 24 h),and PA24 (treated with the Ara-c and PDT for 24 h).CCK-8 method was used to test the cell viability,and the combined effect was analyzed using King formula.The changes of cell cycles were analyzed using flow cytometry.Results In the case of low-dose PDT,the combination groups showed coordinated effect with all three methods,whereas in the case of high-dose PDT,they showed additive or coordinated effect in P24A and A24P,but it showed antagonistic effect in the schedule of PA24.Cell cycles were inhibited to G0/G1 phase by both PDT and Ara-c.Conclusions Coordinated effects could be found when HL-60 cells were treated with the combination of Ara-c and PDT.The effects depended on the dose of Ara-c and PDT and the operation schedules.The effects at low dose were more obvious than that of high dose,and allowing a 24 h period in between the addition of PDT and Ara-c also promoted the effects.

Objective To investigate the effect of endoplasmic reticulum stress (ERS)-associated apoptosis on renal interstitial fibrosis induced by unilateral ureteral obstruction (UUO) in rats. Methods Eighteen healthy male Wistar rats undergoing UUO were sacrificed at 3,7,14 days after operation. Additional seven rats underwent sham operation. Histological changes were observed by HE and Masson staining. Immunohistochemistry was performed on renal tissue for α-smooth muscle actin (α-SMA). Chromatometry was used to detect the content of hydroxyproline. Apoptosis cells were determined by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) and the electrophoresis analysis of genome DNA. The mRNA expression of glucose-regulated protein 78 (GRP78), which are important markers of ERS, were detected by RT-PCR. Western blotting was used to assess the protein level of GRP78 and cysteinyl aspartate specific proteinase-3 (caspase-3). Results Compared with sham operation group, the degree of renal interstitial and the level of hydroxyproline content of UUO group increased significantly (P<0.05). Immunohistochemistry staining indicated that a-SMA extensively expressed in renal tubular and interstitial cells. The apoptotic cells in the renal tubular and interstitium were continuously increased from day 3 to the end of experiment of UUO group. As early as 3 days after surgery, the mRNA level of GRP78 in UUO group increased compared with sham operation group (P<0.01), while the protein expression increased on day 7 after surgery (P<0.01). Prolonged ERS triggered apoptosis, the protein expression of caspase-3 increased significantly on day 3 after surgery (P< 0.05), and the expression sustained high level during the experiment afterwards. There was a positive correlation between GRP78 protein expression and hydroxyproline content (r =0.657, P< 0.01) as well as caspase-3 protein expression (r=0.714, P<0.01). Conclusions UUO induces a significant up-regulation in endoplasmic reticulum molecular chaperones at early stage, indicating that ERS response is activated in the rat kidney. Prolonged ERS can lead to renal tubular and interstitial cell apoptosis, and caspase-3-mediated ERS associated apoptosis may contribute to the fibrosis.

Objective To study the chemical constituents from the root of Rumex gmelini.MethodsThe compounds were isolated and purified by chromatography,polyamide column chromatography,and preparation HPLC etc.Their structures were elucidated by physicochemical and spectroscopic evidences.Results Ten compounds were identified as:nepodin (Ⅰ),emodin (Ⅱ),citreorosein (Ⅲ),chrysophanol 8-O-?-(6'-acetyl) glucopyranoside (Ⅳ),chrysophanol 8-O-?-D-glucopyranoside (Ⅴ),resveratrol (Ⅵ),9,9'-dianthranone-2,2'-dimethyl-5,5'-bis (?-D-glucopyranose)-9,9',10,10'-tetrahydro-4,4'-dihydroxy-10,10'-dioxo (trivial name:rumoside A) (Ⅶ),emodin-8-O-?-D-glucopyranoside (Ⅷ),resveratrol-3-O-?-D-glucoside (Ⅸ),and rutin (Ⅹ).Conclusion Compound Ⅶ is a new compound,named rumoside A.Compounds Ⅴ,Ⅷ,and Ⅹ are separated from R.gmelini for the first time.Compounds Ⅲ and Ⅳ are the compounds which have been found in the plants of Rumex L.for the first time.

Objective To evaluate the value of CEUS in dynamicly assessing the blood perfusion of hepatic alveolar echinococcosis (HAE) in rats.Methods The experimentally induced secondary HAE in totally 70 rats were studied at different time periods (gth week,28th week,and 50th week) with CEUS,and compared to pathology results.Results At the 9th week,55 HAE lesions (55/70,78.57％) presented hyperechoic and the rest of 15 HAE lesions (15/70,21.43％)presented mixed echogenicity.The single or multiple vesicular structures were found as pathological feature.At later stages (28th week and 50th week),the size of lesions increased compared to 9th week with more solid structures and the calcifications found in HAE lesions.Microscopically,the fibrous tissues surrounding the lesions gradually thickened and the microvascular accumulation were visible around the lesions.The HAE lesions at the 9th week showed the ring enhancement and central septa enhancement in CEUS.The HAE lesions at the 28th week and 50th week showed combinations of no enhancement,ring enhancement,and central septa enhancement in CEUS.The ratio of edge enhancement to maximum diameter of lesions decreased with the progression (P=0.02).Conclusion The ultrasonographic features in HAE lesions become more complicated with increasing calcifications and enlarged necrosis areas with the progression.The CEUS can reveal the dynamics of blood perfusion of HAE lesions at different stages.
