Objective To verify the effect of activin A neutralizing antibody (AANA) on inhibition of hepatic fibrosis and mRNA expression of various cytokines. Methods Mouse hepatic fibrosis model was induced by CCl_4. Male Kun-ming mice were divided into 5 groups: normal control group,olive oil control group, CCl_4 model group, AANA group and antibody control group. The mRNA expressions of activin A, transfer growth factor (TGF)-?_1, platelet derived growth factor (PDGF) and tissue inhibitors of metalloproteinas (TIMP)-1 in liver tissue were determined by semiquantitative reverse transcription-polymerase chain reaction(RT-PCR) analysis. Objective To discuss the clinical features and risk factors of death in children with severe pneumonia treated with invasive mechanical ventilation.Methods Through a retrospective analysis of children with severe pneumonia treated with invasive mechanical ventilation,who were hospitalized in PICU of Tianjin Children′s Hospital from Jan 2011 to Dec 2014,we analyzed the distribution of age,etiologic characteristics,mechanical ventilation,complications and background diseases.The single factor analysis and multiple factors Logistic regression analysis were performed to evaluate the risk factors of death.Results A total of 214 pediatric patients were included,134 were male,and 80 were female.The case fatality rate was 6.17%,the relevance ratio of pathogenic microorganism was 16.36%.The median age of death group was older than that of the survival group(4 mouths vs 2 mouths,P=0.039).The pediatric patients who were more than 1 year old in death group were more than the survival group(21.43% vs 15.8%,P<0.001).The common complications included dencephalopathy(11.68%) and electrolyte imbalance(8.41%).Anemia,cardiopathy and alloplasia of respiratory system were the top background diseases.The results of Logistic multivariate regression analysis showed that there were significant differences in the age above 1 year old(OR:1.019,95%CI:1.003-1.030,P=0.019),secondary acute respiratory distress syndrome(OR:7.254,95%CI:1.581-33.277,P=0.011) and accompanying cardiopathy(OR:0.47,95%CI:0.273-0.81,P=0.007).Conclusion The risk factors of death in children with severe pneumonia treated with invasive mechanical ventilation are the following:the age above 1 year old,secondary acute respiratory distress syndrome or accompanying cardiopathy.

Objective:To understand the similarities and differences of medical ethics education between Chinese and American medical colleges and universities,grasp the advantages and disadvantages,and analyze the enlightenment of American medical ethics education to our country.Methods:We used individual interview method of qualitative research methods and conducted semi-open interview.Results:China can adjust the curriculum provision appropriately,enrich the content of medical ethics education,improve the medical ethics practice education,enrich teaching approaches and diversify examination form in medical students moral education cultivation.Conclusion:Both China and the United States attach great importance to medical professionalism education,medical ethics training and the doctor-patient relationship education.However,there are significant differences in their history,culture,religion and morality,as well as the medical systems,education on medical students and medical ethics education.Views formed in the interview provide certain reference and guidance for Chinese medical ethics education at a certain level.

OBJECTIVETo investigate the gene expression of osteoprotegerin (OPG) and osteoclast differentiation factor (ODF/TRANCE/RANKL), two new members of the TNF-receptor superfamily, in giant cell tumor (GCT); to discuss the molecular mechanism of extensive bone resorption caused by GCT.

METHODSUsing TRIzol reagent to prepare total RNA from GCT sample and normal bone tissue. By a first-strand complementary DNA (cDNA) synthesis kit, cDNA was synthesized from 2.0 micro g RNA according to the manufacturer's instructions. cDNA was then amplified by PCR. Amplification products were resolved by electrophoresis on a 1.5% agarose gel and stained with EB. The relative quantity of the PCR products were determined and the mRNA levels of OPG, ODF, M-CSF (cofactor of ODF), and RANK (receptor of ODF) were compared with that of the normal bone.

RESULTSGCT contained highly expressed mRNA of ODF, OPG, M-CSF and RANK. There was mRNA expression of OPG, M-CSF and RANK and less expression of ODF in normal bone. The ODF mRNA and RANK mRNA in GCT were more abundant than that in normal bone. In GCT, the ratio of ODF mRNA exceeded OPG expression. But in normal bone, the OPG mRNA exceeded ODF expression.

CONCLUSIONSThe results suggest that GCT contains all signals including OPG, ODF, M-CSF and RANK that are essential for inducing osteoclastogenesis and promoting bone resorption.

Objective To investigate the effect of rosiglitazone (RGTZ) on anti-oxidation in aging rat kidney. Methods Twenty-four-month-old male Sprague-Dawley rats were randomly divided into three groups (n=10): control group (CON), rosiglitazone group (RGTZ) and caloric restriction group (CR). The CON rats were allowed ad libitum access to feed and tap water.The RGTZ rats received intragastric administration of RGTZ (4 mg·kg-1·d-1),and the CR rats were provided with a vitamin and mineral fortified version of the same diet at a level of 40% less food (by weight) than the CON rats. After 12 weeks all the animals were sacrificed by decapitation, and both the body weight and the percentage of kidney and heart in each group were measured.Western blot was performed to analyze the expression of PPARγ protein. The content of MDA and the activity of SOD and GSH-PX in kidney tissue were detected. Besides, frozen sections of kidney tissue were stained for senescence-associated-13 galactosidase (SA-β-Gal). Results The body weight of CR rats decreased obviously, in contrast, which did not change in CON and RGTZ group. Percentage of kidney and heart to body weight was normal in CR or RGTZ group after intervention. Western blot result showed that PPARγ protein expression in rat kidney was significantly higher in RGTZ and CR group as compared to CON group (P<0.05). Compared with RGTZ and CR rats, obviously lower activities of SOD and GSH-Px were noted in CON rats, however, the content of MDA was higher in CON rats. Additionally, the positive staining area of [3-Gal in CR and RGTZ group was significantly smaller than that in CON rats (P<0.05, P<0.01 ). Conclusion RGTZ can defer the kidney aging in senescence SD rat, and the mechanism may be related to amelioration of oxidative damage and enhancement of antioxidation.

Objective To observe the month age distribution of peroxisome proliferators activated receptor gamma (PPARγ) expression in rat kedney. Methods Wistar rats aged 3 months,12 months and 24 months were made as models who represented young, middle-aged and old group respectively. Western blotting, immunohistochemical (IHC) and in-situ hybridization (ISH) were used to detect the expression and location of protein and mRNA of PPARγ in rat kidney. Results Western blotting results showed that the expression of PPARγ protein was higher in 3 months group than in 24 months group (0.94±0.05 vs. 0.78±0.02, P＜0.01) and 12 months group (0.87±0.04, P＞0.05), and it reduced in 24 months group than in 12 months group (P＞0.05). By IHC,the PPARγ protein was localized predominantly in the nuclear of tubular epithelia and collecting duct cells in each group. In old age group, PPARγ protein was also detected little in the mesangial and Bowman's capsule epithelial cells. Meanwhile, the distribution of PPARγ mRNA with ISH was consistent with above findings. Additional, semi-quantitative analysis of ISH results verified that the level of PPARγ mRNA decreased with ageing. Conclusions As a nuclear transcription factor,PPARγ participates in the regulation of rat kidney aging.

Objective:This thesis aims to construct the evaluation index system ethics management in Beijing medical institutions for application research.Method:The author applied the evaluation system which was agreed and adjusted by expert groups in four medical and health institutions.Result:After three rounds of expert groups have reached the agreement,the Evaluation Index System on Human Research Ethics Management in Beijing Medi-cal Institutions is established,which contains 6 first-class indicators,16 second-class indicators and 39 third-class indicators.The application on four medical and health institutions show that the evaluation index system is practical,operable and is of high degree of distinction.Conclusion:The evaluation index system is scientific,rea-sonable,practical and is of high degree of validity,credibility and distinction.

Tumor cells expression of programmed death ligand-1 (PD-L1) is a major mechanism of immune escape and a predictor of therapeutic efficacy of immune checkpoint inhibitors. The expression of PD-L1 is regulated by a variety of mechanisms, among which epigenetic modifications such as DNA methylation, histone modification and non-coding RNA can promote the occurrence, development and drug resistance of tumors by regulating the expression of PD-L1. To clarify its regulation mechanism can bring new ideas for clinical immunotherapy of tumors.

Objective To evaluate the effect of early functional training guidance prevention of knee stiffness after frac-tures around the knee. Methods Choose From January 2012 to December 2012 in our hospital around knee fracture surgery 132 patients, were randomly divided into experimental group and the control group , 66 cases in the control group after the implementation of routine care, patients in the experimental group after the early stage used continuous passive motion of the knee ( CPM ) machine auxiliary functional exercise . Results Early postoperative rehabilitation experimental group and the control group were significantly increased knee ROM (P<0.05), but the experimental group after 2-8 weeks of knee ROM were significantly greater than in the control group (P<0.05). Postoperative rehabilitation and long-term experimental and control groups LKSS knee function scores were significantly improved recovery (P<0.05), the experimental group in three months, six months, 12 months LKSS scores were significantly higher (P<0.05). Postop-erative recovery and long-term experimental group and the control group standing balance BBS scores were significantly increased (P<0.05), the experimental group at 3 months, 6 months, 12 months of BBS scores were significantly higher (P<0.05).
