2.Clinical study of modifiedXiaochengqi decoction in the treatment of gastroparesis syndrome

Objective To evaluate the efficacy of modifiedXiaochengqi decoction in treatment of gastroparesis syndrome (GS).Methods A total of 81 patients with GS from 2010 July to 2013 July in HuiZhou Municipal Central Hospital were selected and randomly recruited into a control group and a treatment group. The control group was treated with domperidone, 15-30 min before the meal, while the treatment group was additionally treated with modifiedXiaochengqi decoction on the basis of the control group. Both groups were treated for 4 weeks. The clinical curative effect, and the symptoms in both groups were observed. The serum motilin and gastrin concentration were measured by radio-immunity, the preprandial and postprandial electrogastrography was performed.Results The total effective rate in the treatment group was significantly higher than that in the control group (72.5%vs. 31.7%;χ2=11.911,P=0.000). The serum levels of motilin (treatment group: 532.8±16.7 ng/Lvs. 355.2±14.3 ng/L,t=51.089,P=0.000; control group: 505.1 ± 21.3 ng/Lvs. 372.9 ± 18.4 ng/L,t=30.074,P=0.000) and gastrin (treatment group: 69.8 ± 15.3 ng/Lvs. 54.3 ± 13.8 ng/L, t=4.758,P=0.000; control group: 62.6 ± 14.2 ng/Lvs. 53.4 ± 12.3 ng/L,t=3.136,P=0.002) were significantly reduced, and their decrease in the treatment group were greater than those in the control group (t values were 6.503, 2.196, allP<0.05). After the treatment, the dominant power (preprandial period: 84.5 ± 14.8μVvs. 72.3 ± 15.0μV, t=3.684,P<0.01; postprandial period: 100.5 ± 17.3μVvs.89.3 ± 15.9μV,t=5.713,P=0.000) and dominant frequency (preprandial period: 4.3 ± 0.4 cpmvs. 3.9 ± 0.2 cpm,t=3.035,P=0.000; postprandial period: 4.7 ± 0.5 cpmvs. 4.4 ± 0.5 cpm,t=2.700,P=0.000) were significantly higher, while the percentage of bradygastria (preprandial period: 50.6% ± 16.6%vs. 60.1% ± 16.3%,t=2.599,P=0.000;postprandial period: 36.6% ± 14.8%vs. 57.4% ± 15.3%,t=6.217, P=0.000) was significantly lower in the treatment group compared with the control group at preprandial and postprandial periods.ConclusionXiaochengqi decoction can obviously promote the gastric motility, and reduce the serum levels of motilin and gastrin in patients with GS.
3.Comparative study on values of blood physiological and biochemical for middle and old-aged cynomolgus monkey

Objective To provide reference of the general physiological index, blood physiology and biochemistry for middle and old-aged cynomolgus monkey.Methods 119 cynomolgus monkey aged from 11~16 (80 were males and 39 were females) were involved in the study.We detected the general physiological index( body weight, the crown-rump length and waist circumference ) , hematology and blood biochemistry parameters respectively and compared these parameters between males and females.Results Between males and females, there were significant differences in Body weight, the crown-rump length and Waist circumference of the general physiological index(P <0.01),NEUT#、LYMPH#、EO#、BASO#、NEUT%、LYMPH%、EO%、BASO%、MCV、MCH、RDW-CV、PCT、MONO%、HGB、HCT、MCHC、RDW-SD、MPV of hematology(P <0.01 or P <0.05)and TBIL、ALB、GLO、A/G、ALP、GLU、UREA、CREA、TCH、TG、CK、ALT、GGT of blood biochemistry ( P <0.01 or P <0.05 ) .There were no significant differences in other parameters ( P >0.05).Conclusion It provides reference of general physiological index, blood physiology and biochemistry parameters of cynomolgus monkey and basis for their application in biomedical domain.
4.Survey on cognitive status quo of nursing staffs on patients safety culture in second-class and tertiary hospitals of Nanchang City

Objective To investigate the current situation of nursing staffs in second-class and tertiary hospitals of Nanchang City and influencing factors in order to provide a basis for the management and research of patients safety.Methods The convenience cluster sampling method was adopted to extract the whole nursing staffs from 7 hospitals(4 second-class hospitals and 3 tertiary hospitals)in Nanchang City as the research subjects.Then the multiple regression method was adopted to analyze the influencing factors of patients safety culture in the second-class and tertiary hospitals of Nanchang City.Results The advantage fields of cognition of the nursing staffs in the second-class hospitals on the patients safety culture were the intra-department teamwork,expectation actions of managers for promoting safety,organizational learning and continuous improvement,feedback and communication of mistakes,while the fields needing to be improved were the communication openness,personnel allocation,non-penalty reactions on mistakes;the advantage fields in the tertiary hospitals were same to those in the second-class hospitals,and the fields needing to be improved included the inter-department cooperation,personnel allocation,non-penalty reactions on mistakes.Conclusion The cognition of nursing staffs in 7 hospitals of Nanchang City on the patients safety culture is in a higher level,but insufficiency still exists,partial fields need to be continuously improved.
5.Comprehensive Quality Evaluation of Gansu Baitiao Codonopsis Radix with Different Commercial Grades

ObjectiveTo compare the quality of Baitiao Codonopsis Radix with different commercial grades; To provide the basis for the rational utilization of Codonopsis Radix.MethodsTotally 54 batchs of Baitiao Codonopsis Radix with different commercial grades from main producing areas in Gansu were collected. The contents of lobetyolin and atractylenoideⅢ in Codonopsis Radix were detected by HPLC. Colorimetric method was used to detect the content of polysaccharide in the Codonopsis Radix. Chinese Pharmacopoeia 2010 Edition was referred to detect the content of extractum. One way ANOVA and factor analysis method were used to comprehensively evaluate the data.ResultsLobetyolin and atractylenoideⅢ were all negatively correlated with commercial grades. The results of the principal component factor ranking showed that the samples were not sorted by the order of commercial grades, with commercial grade two ranking in the forefront.Conclusion The quality of Baitiao Codonopsis Radix with different commercial grades is different, and the quality of commercial grade two is better than other commercial grades.
6.Influence of photoselective vaporization of prostate on erectile function in patients with benign prostatic hyperplasia.

OBJECTIVETo assess the influence of photoselective vaporization of the prostate (PVP) on the erectile function of the patient with benign prostatic hyperplasia (BPH).
METHODSUsing IIEF-5, we conducted a questionnaire investigation among 210 BPH patients before and after treated by PVP (n = 80) and transurethral resection of the prostate (TURP, n = 130). We also reviewed the clinical data and compared the pre- and post-operative penile erectile function between the two groups of patients.
RESULTSFollow-up was completed in 76 cases of PVP and 123 of TURP. The baseline data showed no statistically significant differences between the two groups in age, prostate volume, IPSS, QOL, Qmax, post void urine residual volume and IIEF-5 scores (P>0.05). Compared with the IEFF-5 score at the baseline (21.88 +/- 2.46), those at 3, 6 and 12 months after PVP were 16.72 +/- 3.17, 19.34 +/- 2.46 and 19.29 +/- 2. 18, respectively, significantly decreased at 3 months (P = 0.042), but with no remarkable difference at 6 and 12 months (P >0.05). Nor were there significant differences in the IIEF-5 score between the PVP and TURP groups at any time points (P>0.05). At 6 months after surgery, the incidence rates of erectile dysfunction were 11.7% and 13.7% in the TURP and PVP groups, respectively (P>0.05).
CONCLUSIONPVP may reduce erectile function in some cases in the early stage after surgery, but this adverse effect does not last long and is basically similar to that of TURP.

7.Investigation on the status and influencing factors of health promoting lifestyles in nursing undergraduates

Objective To investigate the status of health promoting lifestyles in nursing students , and to analyze its influencing factors .Methods Three hundred and seventeen nursing students were chosen according to cluster sampling method , and the cross-sectional study was carried out through the health promoting lifestyle profileⅡ( HPLPⅡ) and self-esteem scale .Results The average score of all items of health promoting lifestyle was (2.33 ±0.32), and the highest score was (2.63 ±0.29) (self-realization), and the lowest score was (2.04 ±0.33) (exercise).The difference was found in the health promoting lifestyles in different nursing students including different age , grade, living conditions and school satisfaction (P<0.05).The average score of self-esteem was (22.91 ±4.20), and the correlation analysis showed that the score of health promoting lifestyle was positively correlated with self-esteem (r =0.517, P<0.05).The major influencing factors of health promoting lifestyle were respectively self-esteem, school satisfaction and age , which can explain 19.7%of the variance in health promoting lifestyles .Conclusions Self-esteem may be an important factor of health promoting lifestyles in nursing students , and the improvement of self-esteem can help them to take positive health promoting lifestyle .
8.Effects of dexmedetomidine assisted postoperative analgesia on sleep quality in elderly patients after abdominal surgery

Objective To investigate the effects of dexmedetomidine used to supplement analgesia on sleep quality in elderly patients after abdominal surgery.Methods Ninety-eight elderly patients,56 males and 42 females,aged 65-80 years,BMI 18.5-25.0 kg/m2,ASA physical status Ⅱ or Ⅲ,who un-derwent elective abdominal surgery under general anesthesia were selected and divided into two groups using the random number table method:dexmedetomidine group(group D)and control group(group C),49 pa-tients in each group.The two groups were used the same drugs for induction and maintenance of anesthesia,with different analgesic formulas after surgery.Analgesia formula in group C was sufentanil 2 μg/kg,tropi-setron 4 mg,and normal saline to 100 ml,and in group D was sufentanil 2 μg/kg,dexmedetomidine 5μg/kg,tropisetron 4 mg,and normal saline to 100 ml.In the form of questionnaire survey,consensus sleep diary(CSD)was used to record the patient's sleep latency,number of awakenings and duration of waking after sleep 1 day before surgery and 1 day,3,and 7 days after surgery,and then the actual sleep time and sleep efficiency were calculated.The patient's venous blood was collected at 5:00 to 7:00 in the morning to detect serum melatonin,interleukin-6(IL-6),interleukin-1β(IL-1β)and tumor necrosis factor(TNF-α)concentration.Extubation time,the incidence of cough,agitation,respiratory depression,postoperative bradycardia,nausea and vomiting,lethargy,urinary retention,and other adverse reactions were recorded.Results Compared with group C,the sleep latency was significantly reduced,the actual sleep time and the sleep efficiency were significantly increased 1 day,3,and 7 days after surgery(P<0.05).Compared with group C,the concentration of postoperative melatonin in group D were significantly increased 1 day,3,and 7 days after surgery(P<0.05),the concentration of IL-6,IL-1β and TNF-α in group D were significantly reduced 1 and 3 days after surgery(P<0.05).There was no significant difference in the incidence of ad-verse reactions between the two groups.Conclusion Patient-controlled intravenous analgesia with dexme-detomidine assisted in elderly patients after abdominal surgery can improve sleep efficiency,increase sleep time and postoperative sleep quality,which may be related to the changes of melatonin and inflammatory factors after surgery.
9.The clinical effect of two thrombolytic regimens in patients with arteriovenous fistula embolism

Objective To observe the clinical effect of single urokinase and urokinase pump combined with low-molecular-weight Heparin in the treatment of autogenous arteriovenous fistula thrombolysis,and the influence on inflammatory factors [interleukin (IL)-1,IL-6,tumor necrosis factor-α (TNF-α)] and CD62p.Methods 20 hemodialysis patients hospitalized in our hospital for the treatment of thrombosis in fistula were selected.They were randomly divided into group A (n =10) and group B (n =10).The group A was treated by urokinase infusion,and the group B was treated with urokinase pump combined with low-molecular heparin respectively.Results Compared with that before thrombolysis,the blood flow rate was increased significantly while the IL-1,TNF-oα and CD62p decreased significantly in the two groups after thrombolytic treatment,with statistically significant difference (P < 0.05).Compared with the group A,the IL-1,IL-6 and CD62p in group B were decreased after thrombolytic therapy,with statistically significant difference (P < 0.05).Conclusions Urokinase combined with low-molecular-weight heparin is better than single urokinase in the treatment of arteriovenous fistula thrombolysis,providing a theoretical basis for clinical fistula thrombolysis treatment.
10.Clinical features of Parkinson disease with constipation

Objective To investigate the clinical features of Parkinson disease (PD)with constipation.Methods From October 2013 to August 2015,a total of 204 PD patients were consecutively recruited from the Departments of Geriatrics and Neurology,Beijing Tiantan Hospital,Capital Medical University,and were evaluated by Rome Ⅲ diagnostic criteria for functional constipation and other related scales of motor symptoms(MS)and non-motor symptoms(NMS),the activity of daily living(ADL)and quality of life.Results Overall,131 of 204(64.2%)PD patients with constipation were assigned to the PD-C group and 73 of 204 (35.8%)PD patients without constipation were assigned to the PD-NC group.In the PD-C group,38 of 131(29%)PD patients had constipation before motor symptoms occurred.The mean age and age of onset in the PD-C group were significantly older than those in the PD-NC group (64.13 ± 9.67 vs.58.35 ± 11.37;60.07 ± 10.46 vs.55.10±12.97;F=2.287,4.948;t=3.827,-2.788;P<0.01 for both).Meanwhile,compared with the PD-NC group,the PD-C group was associated with dramatically longer disease duration (2.25,range:1.00-5.00 vs.2.00,range:1.00-3.13;Z =-2.254;P < 0.05),increased scores of the Unified Parkinson Disease Rating Scale (UPDRS) Ⅲ (26.00 scores,range:18.00 37.50 vs.19.00,range:12.50-31.00;Z =-2.349,P < 0.05),more advanced stages on the Hoehn-Yahr (H-Y) scale (2.00 stage,range:1.50-2.50 vs.1.50,range:1.00-2.50;Z=-2.334,P<0.05),higher total numbers of NMS(11.00,range:6.00-15.00 vs.6.00,range:3.00-11.00;Z=-3.715,P<0.05),and higher numbers of NMS occurring before and after MS(before,2.00,range:0.00-4.00 vs.1.00,range:0.00-2.00;after,8.00,range:3.00-14.00 vs.5.00,range:2.00-9.50;Z =-2.612,-2.630,respectively;P<0.05 for both).Additionally,there were significant differences between the groups in the scores of the Hamilton depression scale (HAMD),the Hamilton anxiety scale (HAMA),the Pittsburgh sleep quality index (PSQI),the scales for outcomes in Parkinson disease-autonomic (SCOPA-AUT),the Fatigue severity scale (FSS),the Apathy scale,the Montreal Cognitive Assessment(MoCA)scale,the UPDRS Ⅱ and ADL Scale,and the PDQL-39 (all P<0.05).Binary Logistic regression analysis showed that age,SCOPA-AUT,HAMA and HAMD were risk factors for PD-C(OR=1.091,1.107,1.10 and 1.080;P<0.05 for all).Conclusions PD patients have a high incidence of constipation,and more than a quarter of patients have constipation before MS occurs.Meanwhile,PD patients with constipation are usually associated with old age and late age of onset,long disease duration,severe MS,frequent and severe NMS,bad cognition,emotional state and sleep,severe fatigue,and apathy.Moreover,advanced age,autonomic dysfunction,anxiety and depression increase the risk of PD with constipation.Constipation has a serious negative impact on the activity of daily living and quality of life in PD patients.
