1.Comparison of clinical effect of MTA and Vitapex for young permanent teeth with periapical incomplete apical closure

Objective To compare clinical effect of MTA and Vitapex for young permanent teeth with periapical incomplete apical closuret . Method Teeth 90 were chosen from 80 patients at 7-25 years old young permanent teeth with periapical and randomly divided into MTA group and Vitapex group 45 in each group.Two groups were treated with MTA and Vitapex paste treatment respectively.6-month and 12-month after treatment followed up, observed clinical efficacy of two groups.Results The six-month recall, X-ray ( MTA group 93.33%, Vitapex group 88.89%) and clinical parameters (MTA group 95.56%, Vitapex group 86.67%) success rate of two group was no significant difference.When the 12-month followed up, X-ray examination of MTA group's success rate was 93.33%, significantly higher than 68.89%Vitapex group (P<0.05), clinical indicators of success rate of 95.56%MTA group,significantly higher than the 71.11%Vitapex group (P<0.05).Pain during root canal therapy (EIP) of the occurrence, MTA group than Vitapex group (P<0.05).Conclusion The effect of short-term treatment of MTA in treatment of young permanent teeth with periapical incomplete apical closure is definite,it is an ideal induce apical molding material.

Designing of natural product-like compounds using natural products as template structures is an important strategy for the discovery of new drugs. Gambogic acid (GA), which is a Garcinia natural product with a unique caged xanthone scaffold, inhibits potent antitumor activity both in vitro and in vivo. This review summarized the researches on the identification of the antitumor pharmacophore of GA, and the design, structural optimization and structure-activity relationship (SAR) of natural product-like caged xanthones based on it.

It is generally accepted that the permeability of the blood-brain barrier in the fetus is higher than in the adult, but its structural basis is still not well known. 5.Value of SWI in intravenous thrombolytic therapy of acute cerebral infarction

Objective To investigate the decisional value of susceptibility weighted imaging(SWI) in intravenous thrombolytic therapy to acute cerebral infarction.Methods According to cerebral microbleeds(CMBs) by SWI, 35 cases of acute cerebral infarction in window-time random were divided into experimental group(14 cases) with CMBs and control group(21 cases) without CMBs.After intravenous thrombolysis, the number of hemorrhagic transformation(HT) was counted by SWI, and statistic difference was compared between the experimental group and the control group(P<0.05).Results After intravenous thrombolytic therapy of acute infarction,12 cases with HT among experimental group, and only 2 cases with HT among control group, there was significant statistically difference between two groups(P<0.05).Conclusion SWI sequence has greater sensitivity in detecting CMBs,and can also assess the state of blood brain barrier(BBB) and vascular wall indirectly.It plays a very important role in giving a decision in the treatment of intravenous thrombolytic with window-time.
6.Study on the modeling method and pathological parameters of chronic atrophic gastritis

Objective:To explore an ideal method for establishing a mouse model of chronic atrophic gastritis (CAG).Methods:CAG mouse models were established with five different modeling methods ( N-methyl- N′-nitro- N-nitrosoguanide (MNNG), sodium salicylate, sodium deoxycholate, Helicobacter pylori infection, and combinations of them) in BALB/c and C57 mice. The effect of each modeling method was evaluated by histological observation of gastric mucosa, plasma biochemical parameters, inflammatory response score, and the expression of anti-inflammatory factors. Results:The results of histological observation of gastric mucosa showed that all of the 5 methods could successfully establish CAG mouse models. In BALB/c mice, compared with the healthy control group, significant features of CAG accompanied with intestinal metaplasia was found in the model group established by combination of MNNG-free drinking, 2% sodium salicylate and 20 mmol sodium deoxycholate. From the results of serological detection, compared with the normal control group, the mRNA expression levels of related anti-inflammatory factors interleukin-2, interleukin-10, interleukin-13 and growth differentiation factor-15 of each model group decreased, which indicated that the mice of each CAG model group had different degrees of inflammation. The results of plasma biochemical parameters indicated that plasma gastrin of each group decreased and the ratio of pepsinogen Ⅰ and pepsinogen Ⅱ significantly dropped. The above results demonstrated that in BLAB/c mice, MNNG-free drinking, 2% sodium salicylate and 20 mmol sodium deoxycholate was better than other four modeling methods. For C57 mice, it was also found that simple chemical drug mutagenesis and Helicobacter pylori replication method both could successfully establish CAG models. No matter from pathological observation, relative expression of anti-inflammatory factors and analysis of plasma biochemical parameters, the effects of combination of the two methods was better. Conclusion:The CAG mouse model established by MNNG-free drinking, 2% sodium salicylate and 20 mmol sodium deoxycholate can provide a certain reference for the establishment and application of mouse model in CAG experiments in the future for pharmacological research.

Pan-vascular medicine is an emerging discipline focusing on atherosclerotic diseases,with the concept of multidisciplinary integration,emphasizing on exploring the mechanism of disease development from the whole of the organism's structure and function.At present,the basic mechanism system of pan-vascular diseases has yet to be perfected.The pan-vascular concept is highly compatible with the idea of Chinese medicine that focuses on the overall view.Deficiency of all qi is the root cause of pan-vascular diseases,while phlegm,blood stasis,and water-dampness and other tangible evils stagnate in the veins and channels as the symptoms of the disease,therefore,the disease mechanism can be highly summarized as"stagnation due to qi deficiency".Deficiency leads to the stagnation,blocking the veins and channels,and the deficiency worsens due to the stagnation and then damages the veins and channels,thus,it develops into a disease.Based on the theory of"stagnation due to qi deficiency",this paper takes endothelial cell dysfunction as the entry point of pan-vascular diseases,and considers that low endothelial cell immunity is the initiating factor of pan-vascular diseases,and that the widespread persistence of microinflammatory state is the key pathology to pan-vascular diseases.
8.The value of 7-point peripheral blood glucose in predicting asymptomatic hypoglycemia at night in elderly patients with type 2 diabetes mellitus

Objective:To explore the value of blood glucose at different time points in the 7-point peripheral blood glucose spectrum in predicting asymptomatic hypoglycemia at night in elderly patients with type 2 diabetes mellitus (T2DM) based on continuous glucose monitoring system (CGMS) .Methods:From January to October 2019, convenience sampling was used to select 225 elderly T2DM patients who were hospitalized in Xinxiang Central Hospital. We monitored the peripheral blood glucose at 7 time points after admission, and performed 72-hour continuous glucose monitoring (CGM) simultaneously. We investigated the occurrence of night hypoglycemia of patients, and divided patients into night symptomatic hypoglycemia group and night asymptomatic hypoglycemia group according to the results. We compared the hypoglycemia related indicators of two groups, drew the receiver operating characteristic (ROC) curve, calculated the area under the curve (AUC) to determine the best cut-point value of each indicator and the occurrence of asymptomatic hypoglycemia at night.Results:In the CGM of 225 patients, 74 cases had hypoglycemia at night, and the incidence was 32.89%, and 34 cases of them had asymptomatic hypoglycemia at night, accounting for 45.95% (34/74) . The fasting blood glucose, blood glucose after dinner and before bedtime of patients in night asymptomatic hypoglycemia group were lower than those of night symptomatic hypoglycemia group. The best cut-off points of blood glucose before bedtime and fasting blood glucose for predicting asymptomatic hypoglycemia at night were 6.9 mmol/L and 6.0 mmol/L respectively, and the AUC were 0.822 and 0.651 respectively.Conclusions:We should be alert to the occurrence of asymptomatic hypoglycemia at night in elderly patients with T2DM. Blood glucose before bedtime <6.9 mmol/L or fasting blood glucose <6.0 mmol/L indicates an increased risk of asymptomatic hypoglycemia at night.

Objective:To retrieve the evidence of scraping operation in patients with neck, shoulder and low back pain, and evaluate and summarize the evidence.Methods:Identified evidence-based problems based on the population, intervention, professional, outcome, setting and type of evidence (PIPOST) model, and searched UpToDate, British Medical Journal (BMJ) Clinical Evidence, Guidelines International Network, Registered Nurses' Association of Ontario, Scottish Intercollegiate Guidelines Network, New Zealand Guidelines Group, World Health Organization, American College of Physicians Club, BMJ Best Practice, Agency for Healthcare Research and Quality, Joanna Briggs Institute Evidence-Based Health Care Center Database, Nursing Consult, China Guideline Clearinghouse, Cochrane Library, Embase, PubMed, CINAHL, Web of Science, Medlive, China National Knowledge Infrastructure and SinoMed. The retrieval time limit was from March 1, 2012 to April 30, 2022. Four researchers independently evaluated the quality of the article, and two researchers summarized the evidence of the included article.Results:A total of 9 articles were included, including 1 guideline, 2 systematic reviews, and 6 randomized controlled trials. A total of 20 pieces of scraping application evidence were extracted from patients with neck, shoulder and low back pain, including 6 aspects, involving preparation before scraping, scraping intervention methods, scraping degree, precautions, contraindications, and handling of abnormal conditions.Conclusions:The best evidence of scraping therapy to improve patients' neck, shoulder and low back pain summarized in this study can provide a reference for improving the operation practice of medical and nursing staffs and nursing outcomes.
10.Consistency of continuous glucose monitoring system and scanning blood glucose monitoring system in predicting dawn phenomenon in patients with type 2 diabetes mellitus

Objective:To investigate the effectiveness of scanning glucose monitoring system in dawn phenomenon of patients with type 2 diabetes mellitus (T2DM) based on continuous glucose monitoring system (CGMS) .Methods:A total of 45 patients with T2DM hospitalized in the Endocrine Department of Xinxiang Central Hospital from June 2019 to June 2020 were selected as the research subjects by convenience sampling method. All selected patients were asked to wear the scanning glucose monitoring system after admission, and then the CGMS for continuous glucose monitoring after 48 hours. The scanning glucose monitoring system and the CGMS were taken off at the same time after 72 hours. The blood glucose analysis report of the same period was checked to determine whether the dawn phenomenon occurred. The consistency to determine dawn phenomenon was analyzed between CGMS and scanning blood glucose monitoring by Kappa test. According to the CGMS judgment results, the receiver operating characteristic curve (ROC) of the scanning blood glucose monitoring system was drawn to evaluate the optimal tangent value of the night blood glucose for predicting dawn phenomenon and the next day fasting blood glucose for evaluating dawn phenomenon.Results:Among the 45 study subjects, there was no statistical difference in determining the incidence of dawn phenomena between CGMS and scanning glucose monitoring system [55.56% (25/45) vs. 46.67% (21/45) , χ 2=0.711, P>0.05]. Consistency analysis showed that Kappa value was 0.895, and the consistency of CGMS and scanning glucose monitoring system in judging dawn phenomenon was good (Kappa=0.895, 95% CI 1.130-4.883, P<0.01) . The optimal tangent value of night blood glucose for predicting dawn phenomenon was 8.31 mmol/L, and the optimal tangent value of the next day fasting blood glucose for evaluating dawn phenomenon was 7.25 mmol/L. The value of the area under the ROC curve were 0.729 and 0.803 respectively. Conclusions:Scanning blood glucose monitoring can accurately judge the dawn phenomenon. When the night blood glucose value is 8.31 mmol/L, it indicates that the dawn phenomenon may occur. When the next day's fasting blood glucose value is 7.25 mmol/L, it indicates that the dawn phenomenon may already occur.
