BACKGROUND:Erythromycin spreads widely in the body with a short period of effective concentrations and has a lot of adverse effects. Therefore,it is necessary to make erythromycin as targeted medicine.OBJECTIVE:To optimize the preparation conditions of erythromycin gelatin microspheres.DESIGN,TIME AND SETTING:An orthogonal controlled test was performed in the Department of Pharmacy,Guangdong Pharmaceutical University from June to December in 2005.MATERIALS:Erythromycin and gelatin.METHODS:According to the emulsion principle, erythromycin dispersed in the gelatin solution. In the process of preparing microspheres, the gelatin solution and oil should form W/O emulsion and then it turned into spheres by solidification. The formation and quality of microspheres were influenced by four factors, namely the concentration of gelatin, dosage of emulsifier, the solidification time and the speed of mixing. The arithmetic mean diameter of microspheres, the drug loading efficiency and the encapsulation efficiency were targets for the survey in this study on the basis of pretests. The best preparation conditions were optimized in accordance with the results of L9 (34) orthogonal tests. The optimized preparation conditions were obtained according to the results of orthogonal tests.MAIN OUTCOME MEASURES:The mean diameter of microspheres, the drug loading efficiency, the encapsulation efficiency, and the orthogonal tests were examined.RESULTS:The optimized preparation conditions of erythromycin gelatin microspheres included 15% gelatin,3.0 mL emulsifier,0.5 hour solidification and mixing at 1 000 r/rain.The erythromycin gelatin microspheres were regular in their morphology.Drug was enveloped in microspheres. The average particle size was (14.15±0.20) μm;the drug loading efficiency and the encapsulation efficiency were (5.83±0.38)% and (65.70±0.56)%,respectively.Over 90.16% of the microspheres was in the range of 7-25 μm;The reappearance of pharmaceutical technology was good.CONCLUSION:The optimized preparation conditions of erythromycin gelatin microspheres are obtained using L9 (34)orthogonal tests. The microspheres prepared meet the requirement of the size for lung targeting.

Objective To investigate the preventive effect of continuous quality improvement(CQI) on malnutrition, inflammation, peritoneal dialysis adequacy and cardiovascular events in elderly patients undergoing peritoneal dialysis. Methods A single-center prospective self-controlled study was performed.32 stable elderly patients to undergo continuous ambulatory peritoneal dialysis (CAPD) were included. The continuous quality improvement program was conducted by using the 4-step problem-solving framework called the PDCA cycle (plan,do,check and act). The dialysis adequacy, nutritional status, inflammation and cardiovascular events were analyzed before and after PDCA.Results Compared with before PDCA,the proportion of patients with Kt/V≥1.7 and urea kinetics (Kt/V) level were increased after PDCA [71.8% vs.93.75%,(1.97±0.36) vs.(2.08±0.33),both P< 0.05].Serum levels of albumin (ALB),prealbumin (PAB) and serum carbon dioxide combining power (CO2 CP) were increased after PDCA (all P<0.05).The proportion of patients with ALB≥40 g/L was increased after PDCA as compared with before PDCA (9.4% vs.31.3%,P<0.05).The nutritional variables including mid-arm circumference (MAC),triceps skinfold thickness (TSF),mid arm muscle circumference (MAMC),and lean body mass (LBM),lean body mass percentage (LBM%),normalized protein equivalent of total nitrogen appearance (nPNA),handgrip strength were improved after PDCA (P< 0.05).High sensitivity C-reactive protein (hs-CRP) level and the proportion of patients with hs-CRP>3 mg/L were decreased after PDCA as compared with before PDCA [(8.65±6.22) mg/L vs.(5.37±4.33) mg/L,53.1% vs.25%,both P<0.05].The incidence of peritoneal dialysis related peritonitis was reduced obviously from 1 case every 25.4 months to 1 case every 78.1 months after PDCA (P < 0.05).The hospitalization rate due to cardiovascular events was decreased after PDCA as compared with before PDCA (25% vs.3.13%,P<0.05).Conclusions CQI may significantly improve the malnutrition, inflammation and dialysis adequacy, and reduce the hospitalization rate due to cardiovascular events in elderly patients undergoing peritoneal dialysis.
10.Role of Kupffer cells in the development and progression of primary biliary cholangitis
Journal of Clinical Hepatology 2021;37(4):959-961
The incidence rate of primary biliary cholangitis (PBC) is increasing year by year, but there is still no specific medicine at present and PBC has a complex pathogenesis. Kupffer cells, as the key cells involved in immunoregulation, play an important role in PBC. When hepatocytes are damaged, Kupffer cells will be activated and release a large amount of inflammatory cytokines and chemokines, which participate in the development and progression of PBC. This article briefly reviews the role of Kupffer cells in PBC, so as to provide a theoretical basis for Kupffer cells as a potential target for the treatment of PBC.
