1.A retrospective study of 548 cases of erectile dysfunction treated by penile prosthesis implantation

Objective To evaluate the different penile prosthesis for the management of erectile dysfunction. Methods Of the 548 cases,melleable prosthesis has been implanted in 228(41.6%),one piece inflatable prosthesis(Dynaflex) in 45 (8.2%),two piece inflatable prosthesis (Mentor) in 2 and three piece inflatable prosthesis in (AMS 700 CXM) 273 (50.2%). Results The overall complication rate has been 5.1%(28/548),nonmechanical in 8 (1.5%) and mechanical complication in 20 (3.6%). The mechanical complication rate of AMS malleable 600 and AMS 700 CXM were 1.3% (3/228) and 5.5% (15/273) respectively.The cylinder leakage of AMS 700 CXM was 3.7% (10/273). Conclusions The mechanical function and concealment have been significantly improved with AMS 700 CXM whereas improvement of the cylinder quality is yet needed.
2.ESWL for ureteral stone with acute colic and impaired renal function

Objective To study the application of ESWL for ureteral stones in acute colic and with impairment of renal function. Methods From Mar.1987 to Sep.2000,emergent ESWL was performed for 168 cases of ureteral stone in acute colic and with impaired renal function on IVU (nonvisualization on IVU). Results All the patients got well after ESWL.104 patients less than 60 with a history of renal colic within 1 month have been followed up for 1 year.The renal function has reco vered and the urinary tract became visualized on IVU. Conclusions Ureteral stone may induce acute injury to the kidney but the harm is temporary and reversible when the ureteral stone is cured by ESWL on time.
3.Impaction of different prostate biopsy strategies on tumor detection

Objective To investigate the impaction of different prostate biopsy strategies on the tumor detection.Methods 214 consecutive men who were highly suspected as prostate cancer were included in this study.TRUS guided prostate biopsy of 13 cores was conducted in each patient.Among these individuals,PSA(total prostate specific antigen) level were among 0.8 ng/ml and 112.3 ng/ml,mean 18.7 ng/ml,of which 203 cases were over 4.0 ng/ml.The prostate volume was between 12.3 ml and 182.5 ml with 61.3 ml of mean.DRE(digital rectal examination) was normal in 173 cases and abnormal in 41 cases.The tumor detection rate for each biopsy core was calculated.The sensitivity of different combinations of biopsy cores(eg.prostate biopsy strategies) was compared with a 13 core biopsy protocol,in which besides the sextant biopsies,the far lateral and mid regions of the gland were also taken,as 8 core and 10 core biopsy.Results Prostate cancers were detected in 77 of 214(36.0%) patients with 13 core biopsy protocol.Of the possible combinations of biopsy cores,10-core biopsy protocol that included cores at the base,mid gland and apex of the prostate with lateral base and mid cores detected 97.4% of cancers.The detection rate of this 10-core biopsy protocol was equivalent to that of the 13 core regional biopsy(P=0.5).Conclusions 10-core biopsy protocol that combines cores at the base,mid gland and apex of the prostate with lateral base and mid cores should be the preferred strategy in early detection of prostate cancer.
4.The crosstalk between Wnt and TGF-? signaling pathway in prostate cancer cell PC-3

Objective To investigate the crosstalk between Wnt signaling pathway and TGF-? signaling pathway in prostate cancer cell line PC-3. Methods Luciferase report assay and RT-PCR methods were used to explore the crosstalk between two pathways. Results Wnt signaling pathway activated TGF-? responsive CAGA-luciferase reporter activity. ?-catenin(WT), TCF-4(WT), GSK3? (KM) activated while TCF-4(DN) inhibited CAGA reporter activity. ?-catenin(WT), TCF-4(WT) synergized to activate TGF-? signaling while TCF-4(DN) had converse effect.TGF-? signaling pathway activated LEF-1 luciferase activity through a Smad3-dependent manner.Smad7 activated LEF-1 luciferase activity,Smad3 and Smad4 also synergically activated LEF-1 reporter activity.?-catenin(WT),TCF-4(WT) cooperated with each other to the activity of cyclinD1 promoter luciferase while Smad3 inhibited cyclinD1 promoter activity.The inhibitory effect of Smad3 on cyclinD1 promoter could be partially reversed by co-transfecting ?-catenin(WT),TCF-4(WT).TGF-? induced the expression of VEGF in PC-3 cells and this effect was enchanced by LiCl addition. Conclusions Wnt signaling pathway and TGF-? signaling pathway can activate each other in prostate cancer cell line PC-3.Two pathways have crosstalk and this might be important for the development and progression of prostate carcinoma.

Results Relative concentrations of urinary osteopontin in normal, pre and postoperative stone former group were 29.30 ,26.17 and 10.84 respectively. Endourology has developed very quickly during the past decades. For transurethral therapy of diseases of lower urinary tract, it has been gradually accepted by both urologists and patients for plasmatic electrocautery resection, holmium laser and electrochemical enucleation of benign prostatic hyperplasia. Flexible ureteroscope has been widely used for examination of upper urinary tract for unknown urinary bleeding and micronephrostomy has achieved great success for percutaneous nephrolithotripsy. Laparoscope has become very popular and can be used to perform most of the operations in the field of urology.
8.Efficacy of shock wave lithotripsy in treating patients with double J stent extubation difficulties due to the stone scales surface

Objective To evaluate clinical efficacy of extracorporeal shock wave lithotripsy (ESWL) for treating patients with double J (DJ) stent extubation difficulties.Methods A retrospective analysis were conducted from July 1999 to December 2013,total 30 cases (18 male and 12 female) were accepted the ESWL therapy for treating the DJ extubation difficult due to the stone scales surface.The average age in those patients was 46 years (range 24 to 83 years).The average duration of DJ stents indwelling was 8.3 months (range 1.5 to 36.0 months).There were 10 cases with single stone and 20 cases with multiple stones.The distribution of stone location included left kidney in 14 cases,right kidney in 6 cases,left ureter in 3 cases and right ureter in 7 cases.The ureteral stone scales around the DJ stent showed tubular distribution.The distribution of imaging characteristic included kidney stones around the DJ stent joint in 20 cases and surface coarse in 10 cases.The diameter of the stone ranged from 0.7 to 3.2 cm (mean 1.8 cm).All the 30 cases were treated by ESWL with the 2 000-3 300 times frequency (mean 2 800 times).Treatment voltage was 4-8 KV.Results Gravel time ranged from 40 to 70 min (mean 45 min).No intraoperative and postoperative complications occurred.Their DJ stents were easily extubated immediately after ESWL.One week after ESWL,all the patients reported the flushing of the stone.Conclusion ESWL is a simple,convenient and efficacy way to solve the DJ stent extubation difficulties due to the scales surface.
9.Clinical effect of extracorporeal shock wave lithotripsy in the treatment of horseshoe kidney stones

Objective To explore the clinical effect of extracorporeal shock wave lithotripsy (ESWL) for the treatment of horseshoe kidney stones.Methods A total of 42 cases of horseshoe kidney stones from March 1987 to January 2015 were enrolled.Among them,33 cases were male and 9 cases were female.Their age range was 19 to 71 years,average age was 42 years old.25 cases got left renal pelvis stone,17 cases were on the right renal pelvis side.The diameter range of stone was 0.4-3.7 cm,the average was 1.8cm.ESWL was applied to treat whole group.The prone position was used to removing calculus.The frequency of treatment was 2000-2700 times,with an average of 2500 times;the treatment voltage was 4-7KV.The gravel time of the treatment was 30-45min,the average was 35min.Results One time for ESWL was 29 cases,two times for ESWL was 8 cases,three times for ESWL was 3 cases,four times for ESWL was 2 cases.The first discharge stones time was 3 to 10 days.There was no special discomfort after operation.40 cases got hematuria after operation,which occurred in 1 to 3 days after ESWL Renal colic was in 7 cases which happened on 1-3 days after ESWL.Condusions Calculi in horseshoe kidney may be treated by ESWL.It is a safe and simple operation.The prone position may bring satisfactory effect.
10.Study of effect and mechanism of cyclooxygenase-2 on the prostatic hyperplasia in rats

Objective To investigate the effect and molecular mechanism of cyclooxygenase-2 (COX-2) on the benign prostatic hyperplasia (BPH) in rat model. Methods Thirty-six SD rats were randomly divided into 3 groups:normal control (group A,n=12),BPH model (group B,n=12) and BPH+selective COX-2 inhibitor celecoxib (group C,n=12). At the 5th week after treatment, the weight of the prostates was measured, and the morphological changes were examined under light microscope.Detection of ki-67 and TUNEL in prostatic epithelial and stromal cells was undertaken to assess the proliferation and apoptosis status.The protein and mRNA expression of COX-2,epidermal growth factor (EGF),basic fibroblast growth factor (bFGF) and transforming growth factor-?1 (TGF-?1) were analyzed by means of immunohistochemisty and RT-PCR. Results The prostate index [prostate wet weight (mg)/rat body weight (g)] of group B was significantly higher compared with those in groups A and C (1.88?0.17 vs 1.70?0.09 and 1.74?0.16,P0.05). Conclusions The increased expression of COX-2 may play an important role in the pathogenesis of BPH by modulating the expression of growth factors and affecting the proliferation and apoptosis of prostate cells.
