1.Serological and genetic analysis of a novel 27delC variant in A subgroup: a case report

[Objective] To conduct serological identification and molecular mechanism study on a ambiguous ABO blood group. [Methods] Standard serological techniques were used for the forward and reverse typing of ABO blood type. ABO gene coding and regulatory regions were analyzed by PCR after DNA extraction. Monoclonal sequencing was used to detect the haplotypes of the DNA sequence, and bioinformatics analysis was applied to predict the possible translation outcomes of the mutated DNA sequence. [Results] The sample’s red blood cells showed mixed field agglutination with anti-A, and the serum agglutinated with B cells, exhibiting serological characteristics of subtype A. Direct sequencing and monoclonal sequencing analysis of the ABO gene confirmed one allele as O02, the other had a c.27delC mutation compared with A102, which could cause the translation sequence to terminate prematurely at the 19th amino acids. Analysis and prediction suggested that the mutation might affect the function of the transferase through mechanisms such as shifting the initiation codon, altering the reading frame and affecting the splice sites. [Conclusion] This case is a rare A subtype caused by the c.27delC variation, and the impact on the glycosyltransferase may involve multiple mechanisms, which require further research and exploration.
2.Evaluation of the quality of Alpinia katsumadai from different habitats based on HPLC fingerprint and determina-tion of active ingredient content

OBJECTIVE To evaluate the quality differences of Alpinia katsumadai from different habitats. METHODS High- performance liquid chromatography(HPLC) was used to establish the fingerprints of A. katsumadai from 18 batches of different habitats, and the quality of A. katsumadai from different habitats was comprehensively evaluated by similarity evaluation, cluster analysis (CA), principal component analysis (PCA), orthogonal partial least squares-discriminant analysis (OPLS-DA) and the content determination results of alpinetin, pinocembrin, cardamonin and alnustone in A. katsumadai. RESULTS The similarity of HPLC fingerprints for 18 batches of A. katsumadai was >0.9. Eleven common peaks were identified from the chromatogram, and four of them were specifically characterized. Both CA and PCA grouped 18 batches of A. katsumadai into 3 categories, extracting 2 principal components (the cumulative variance contribution rate reached 89.798%). OPLS-DA identified 9 quality difference markers, namely the components corresponding to peaks 4, 9, 3, 2, 7 (pinocembrin), 8 (cardamonin), 6 (alpinetin), 10 and 11 (alnustone). The content of alpinetin, pinocembrin, cardamonin, and alnustone ranged from 4.507 1-11.579 7, 5.154 4-14.183 3, 5.109 5-13.588 3 and 4.494 6-11.277 2 mg/g, respectively. CONCLUSIONS The quality of A. katsumadai from different habitats is quite different, and the quality of A. katsumadai from Hainan is the best.
3.A phase Ⅱ clinical study of the efficacy and safety of antaitasvir phosphate combined with yiqibuvir for the treatment of chronic hepatitis C in adults

Objective:To evaluate the efficacy and safety of antaitasvir phosphate 100 mg or 200 mg combined with yiqibuvir for 12 weeks in patients with various genotypes of chronic hepatitis C, without cirrhosis or compensated stage cirrhosis.Methods:Patients with chronic hepatitis C (without cirrhosis or compensated stage cirrhosis) were randomly assigned to the antaitasvir phosphate 100 mg+yiqibuvir 600 mg group (100 mg group) or the antaitasvir phosphate 200 mg+yiqibuvir 600 mg group (200 mg group) in a 1∶1 ratio. The drugs were continuously administered once a day for 12 weeks and observed for 24 weeks after drug withdrawal. The drug safety profile was assessed concurrently with the observation of the sustained virological response (SVR12) in the two patient groups 12 weeks following the drug cessation. The intention-to-treat concept was used to define as closely as possible a full analysis set, including all randomized cases who received the experimental drug at least once. The safety set was collected from all subjects who received the experimental drug at least once (regardless of whether they participated in the randomization group) in this study. All efficacy endpoints and safety profile data were summarized using descriptive statistics. The primary efficacy endpoint was SVR12. The primary analysis was performed on a full analysis set. 4.Causal Relationship Between Immune Cells and Risk of Inflammatory Joint Diseases:A Mendelian Randomization Study

Objective A two-sample Mendelian randomization approach was used to explore the causal relationship between im-mune cells and the risk of inflammatory joint diseases.Methods A dataset of 731 immune cells and osteoarthritis,ankylosing spondyli-tis,gout,and psoriatic arthritis was obtained from the Finngen database and the GWAS catalog database.The causal relationship between immune cells and osteoarthritis was explored using inverse variance weighting,weighted median and MR-Egger methods,and the stabili-ty and reliability of the research results were evaluated using Cochran's Q test,MR-Egger regression,MR-PRESSO,Leave one out method,and Meta-analysis.Results A total of 45 immune cells were obtained with osteoarthritis,35 immune cells with ankylosing spondylitis,29 immune cells with gout,and 36 immune cells with psoriatic arthritis in a potential causal relationship(P<0.05).There was a significant causal relationship between HLA DR on CD14-CD16+monocyte and ankylosing spondylitis(P<6.8 × 10-5),and the results were stable,but Meta-analysis showed that the causal relationship was not significant(OR=1.00,95%CI:1.00-1.00).Conclusion This study preliminarily reveals the causal relationship between immune cells and inflammatory joint diseases represented by ankylosing spondylitis from the perspective of genetics,and provides an important theoretical basis for future precision medicine and indi-vidualized treatment.
5.Association between neonatal birth weight and maternal type 2 diabetes mellitus: a bidirectional Mendelian randomization study

Objective:
To examine the association between neonatal birth weight and maternal type 2 diabetes (T2DM), so as to provide insights into the formulation of the early T2DM prevention and improvements of maternal and children health.
Methods:
Single-nucleotide polymorphisms (SNPs) were collected from two genome-wide association studies (GWAS) that examined the association between neonatal birth weight and maternal T2DM. Inverse variance weighted method was employed for forward Mendelian randomization with 26 birth weight-associated SNPs as instrumental variables and maternal T2DM as the study outcome and for reverse Mendelian randomization with 18 maternal T2DM-associated SNPs as instrumental variables and maternal effects of neonatal birth weight as the study outcome. The heterogeneity was examined using Cochran's Q test, and the pleiotropy was evaluated using MR-Egger regression, while the robustness of the results was evaluated using leave-one-out test.
Results:
Cochran's Q test detected heterogeneity across two studies (P=0.019, 0.038), and random effect models were employed. Mendelian randomization showed that an increase in neonatal birth weight by per standard error (approximately 488 g) resulted a 29.9% reduction in the risk of maternal T2DM (OR=0.701, 95%CI: 0.547-0.899), and maternal T2DM increased the neonatal birth weight by 0.074 standard errors (OR=1.074, 95%CI: 1.043-1.106). No horizontal pleiotropy was seen for instrumental variables (P=0.241, 0.188). With each SNP excluded in turn, the results of Mendelian randomization study were robust.
Conclusion
There are bidirectional associations between neonatal birth weight and risk of maternal T2DM.
6.Clinicopathological Significance of Poorly Differentiated Clusters in Liver Metastatic Lesions of Colorectal Carcinoma

Objective To investigate the clinicopathological significance of PDC in liver metastases and analyze the correlation of PDC between liver metastases and primary lesions. Methods Retrospective analysis of 72 matched cases of colorectal cancer with liver metastases was performed. The PDC in primary tumor and liver metastatic lesion was interpreted synchronously, and then the relationship between PDC in liver metastasis and clinicopathological parameters was analyzed based on the correlation of PDC between primary and metastatic lesions. In addition, PDC were interpreted in accordance with Uenos' standard. Results Among the 72 cases of liver metastasis of colorectal cancer, the number of G1, G2, and G3 graded by PDC was 28, 24, and 20, respectively. The PDC in liver metastatic lesion was correlated with tumor budding in liver metastatic lesion and PDC grade of primary lesion. No significant correlation with the size and number of liver metastatic lesion, the site, WHO grade, depth of invasion, lymph node metastasis, vascular invasion or tumor budding of the primary lesion was observed. Conclusion A positive correlation is found between liver metastasis of colorectal adenocarcinoma and PDC grade of primary tumor. Evaluating the PDC grade of primary tumor may provide a reference for the risk of liver metastasis.
7.Meta-analysis of coffee intake and all cancer mortality in East Asian population

Objective :
To explore the association between coffee intake and all cancer mortality in East Asian populations.
Methods :
We searched literatures which assessed the relationship between coffee intake and cancer mortality in Asian populations published by December 10th,2018 from China National Knowledge Infrastructure,Wanfang Database,VIP Database and PubMed. We conducted category and dose-response meta-analyses using Stata 15.0.
Results :
A total of 335 relevant articles were retrieved; five articles were finally included in the meta-analyses,of which four were carried out in Japanese population and one in Singaporean Chinese population. The total sample size of the five articles was 361 802,and the number of deaths from cancer was 17 664. The results showed that coffee intake reduced the risk of all cancer mortality in East Asian populations(RR=0.93,95%CI:0.87-0.99). There was no statistical significant association between coffee intake and all cancer mortality in East Asian men(RR=0.94,95%CI:0.77-1.15). Among East Asian women,coffee consumption reduced the risk of all cancer mortality by 12%(RR=0.88,95%CI:0.81-0.95). All cancer mortality risk decreased with the increase of coffee intake,and reached the lowest point at one and a half cups of coffee per day(RR=0.92,95%CI:0.86-0.98).
Conclusion
Coffee intake reduced the risk of all cancer mortality in East Asian populations,which was obviously found in East Asian women. Drinking one and a half cups of coffee a day had the lowest risk of all cancer mortality.
8.Efficacy and safety of tetrandrine tablets and didofenac sodium tablets in the treatment of osteoarthritis

Objective To explore the clinical efficacy and safety of tetrandrine tablets and diclofenac sodium tablets in the treatment of osteoarthritis,thus to provide reference for the formulation of scientific and effective treatment.Methods From March 2015 to March 2016,Seventy patients with osteoarthritis in the People's Hospital of Yuyao were selected as study objects and divided into research group and control group according to the random number table method,with 35 cases in each group.The two groups were given glucosamine hydrochloride tablets treatment,the control group received diclofenac sodium tablets on the basis of treatment,while the research group received tetrandrine tablets on the basis of treatment of the control group,4 weeks for a course of treatment.The VAS score,joint tenderness index(TJS),joint swelling index (SJS),duration of morning stiffness,adverse reactions and clinical efficacy in the two groups were observed before and after treatment.Results The total effective rate in the study group was 97.14% (34/35),which was higher than 80.00% (28/35) in the control group,the difference was statistically significant between the two groups(x2 =23.932,P =0.001).Compared with before treatment,the VAS scores,SJS,TJS of the two groups were significantly decreased(all P < 0.01).The study group compared with the control group,decreased more significantly,VAS score (t =6.78,P < 0.05),SJS (t =5.78,P < 0.05),TJS (t =6.65,P < 0.05),and the morning stiffness time (t =6.98,P < 0.05) were significantly reduced.The incidence rate of adverse reactions of the research group 5.71% (2/35) was significantly lower than 31.43% (11/35) of the control group(x2 =19.237,P < 0.01).Conclusion Tetrandrine tablets combined with diclofenac sodium tablets in the treatment of osteoarthritis has low incidence of adverse reactions,high safety and ideal clinical effect,it can be widely used in clinical.
9. Treatment of sebaceous gland hyperplasia with fractional Q-switched 1064 nm laser

Objective:
To investigate the therapeutic effect of fractional Q-switched 1 064 nm laser in treatment of sebaceous gland hyperplasia.
Methods:
36 patients with sebaceous gland hyperplasia were recruited and treated with fractional Q-switched 1 064 nm laser, once per 1-2 weeks, 6-8 times for a course, lesion clearance, post-treatment scaring and pigmentation were observed.
Results:
Skin lesions gradually shrank or came off after one course of treatment with fractional Q-switched 1 064 nm laser, leaving mild difference of texture compared with the surrounding skin, no obvious scaring or pigment anomaly occurred, no recurrence was found during 6-month follow-up.
Conclusions
Fractional Q-switched 1 064 nm laser is effective for clearance of sebaceous gland hyperplasia, furthermore, it presents more pleasant cosmetic outcome could be obtained by a single technique.
10.Effects of Dexmedetomidine Combined with Ulinastatin on Stress Reaction Indexes in One-lung Ventilation Lobectomia Pulmonalis

OBJECTIVE:To observe the effects of dexmedetomidine combined with ulinastatin on stress reaction indexes,ex-travascular lung water value(EVLW)and pulmonary vascular permeability indexes(PVPI)and other parameters in one-lung venti-lation(OLV)lobectomy patients. METHODS:A total of 80 patients underwent OLV lobectomy selected from the Affiliated Hospi-tal of Southwest Medical University during Nov. 2015-Nov. 2016 were divided into control group (group N),ulinastatin pretreat-ment group(group U),dexmedetomidine continuous pump group(group D),ulinastatin pretreatment+dexmedetomidine continu-ous pump group(group U+D),with 20 cases in each group. Thirty min before anesthesia induction,all patients were given atro-pine sulfate 0.5 mg intramuscularly and received catheterization of right internal jugular vein and ipsilateral femoral artery under lo-cal anesthesia. Based on that,group N was given 0.9% Sodium chloride injection 100 mL intravenously;group U was given sodi-um chloride mixed solution 100 mL containing Ulinastatin for injection 200000 U intravenously;group D was given continuous in-travenous pump of Dexmedetomidine hydrochloride injection 2 μg/mL mixed with sodium chloride mixed solution at 1 μg/(kg·h), and after induction intravenous pump at 0.5 μg/(kg·h)until the end of surgery;group U+D was given Sodium chloride mixed so-lution containing 200000 U ulinastatin 50 mL and 4 μg/mL dexmedetomidine sodium chloride mixed solution 50 mL intravenously (same dose as above during induction period and maintenance period). The levels of blood glucose,partial pressure of oxy-gen [p(O2)],adrenocorticotropic hormone (ACTH),norepi-nephrine (NE),heart rate (HR),cardiac output (CO),sys-temic vascular resistance(SVR),EVLW and PVPI were com-pared among 4 groups 30 min before anesthesia induction(T1),2 h after the beginning of surgery(T2),immediately after extuba-tion(T3)and 12 h after surgery(T4). Liquid intake and output volume were recorded during T2-T4. The occurrence of ADR among 4 groups was observed. RESULTS:There was no statistical significance in each index among 4 groups at T1 (P>0.05). During T2-T4,the levels of blood glucose,ACTH,NE,SVR,EVLW and PVPI in group N were significantly higher than other 3 groups, and group U and D were significantly higher than group U+D,with statistical significance(P<0.05);there was no statistical sig-nificance between U and D(P>0.05). The levels of p(O2)and CO in groups U+D were significantly higher than other 3 groups, with statistical significance (P<0.05);there was no statistical significance among other 3 groups (P>0.05). HR of group N and U were significantly higher than other 2 groups,with statistical significance (P<0.05). There was no statistical significance be-tween group N and U,between group D and U+D(P>0.05). There was no statistical significance in liquid intake or output vol-ume among 4 groups (P>0.05). No obvious ADR was found in 4 groups. CONCLUSIONS:Dexmedetomidine combined with ulinastatin can significantly relieve stress reaction,and reduce the levels of part blood flow and respiratory dynamics indexes as EV-LW,PVPI in patients with OLV lobectomy so as to play lung protective effect.
