Objective To analyze the characteristics of allopurinol hypersensitivity syndrome(AH),and pro-vide reference for clinical rational drug use.Methods AH cases induced by oral allopurinol administration were col-lected by dermatologists from January 1995 to January 2015 in our hospital,and 6 cases of the AH were statistically analyzed.Results All cases of AH were male and characterized by skin lesions,fever and damage of liver and kidney function.Although promptly managed,1 case of AH was mainly systemic damaged and died of multiple organ failure and the rest of those cases recuperated.Conclusion AH is an severe complication induced by allopurinol.The clini-cal medication should be strictly monitored in accordance with the instructions to reduce the incidence of AH and ensure medication safety.

Objective To prepare a harmine hydrochloride liposome formulation with high encapsulation efficiency. Methods Preformulation investigation was carried out to obtain the drug physicochemical properties such as solubility and lgD in buffers of different pH value. Harmine hydrochloride was encapsulated into liposomes by active loading method. Encapsulation efficiency of liposomes was determined after the free drug was separated from liposome by ultrafiltration. The influence factors on the encapsulation efficiency including drug-lipid weight ratio, incubation temperature, pH value of external water phase were investigated. Results As the pH value increasing, the solubility of harmine hydrochloride was decreased, while the apparent oil-water distribution coefficient was increased. By active loading method, the encapsulation efficiency could be over 80% when the drug to lipid weight ratio was under 1∶5. The pH gradient between intervesicle and intravesicle obviously influenced the encapsulation efficiency, while incubation temperature had little effect on encapsulation efficiency. Conclusion Active loading is suitable for preparing harmine hydrochloride liposome with high encapsulation efficiency.

In spite of much progress on its mechanism, diagnosis and treatment, diabetes mellitus remains a public health challenge. The harm of diabetes is not significantly reduced, instead shows an increasing tendency year by year. To achieve an in?depth and comprehensive understanding of the underlying mechanism, and to develop efficacious, stable and hypoglycemia?risk free drugs, it is crucial to gain more knowledge about diabetes from animal models. In this review, the types of diabetes animal models, modeling methods, the advantages and disadvantages, their applicable scope are discussed aiming to provide a reference for researchers to choose appropriate animal models.

Objective To explore the effect of skin expansion by B- mode ultrasound with modified Seldinger technique (MST) on preventing puncture site bleeding after the catheterization in breast cancer patients. Methods One hundred chemotherapy breast cancer patients who need to be catheterized PICC were recruited to participate in a randomised controlled trial in Tianjin Medical University Cancer Institute& Hospital, China. Fifty were assigned to the experimental group (using Intra- thecal intubation sheath), and 50 were assigned to the control group (expansion skin by scalpel). The two groups used modified Seldinger technique (MST) during the procedure of ultrasound guided PICC insertion, Patients errhysis and average frequency of dressing- change were collected for 1 week since catheterization to compare the effects of the two methods. Results Patients′errhysis and average frequency of dressing- change were (0.491±0.241) ml, (1.060±0.239) times in the experimental group, significantly lower than (2.903±1.046) ml, (3.500±0.646) times of the control group (t=15.889, 25.049, P < 0.01). Conclusions This study showed a lower incidence errhysis of skin expansion by intra- thecal intubation sheath used for PICC insertion. This technique deserved clinic application in PICC.

Objective To observe the clinical efficacy of Wenshen Xiaochuan Paste Recipe (WXPR) in preventing bronchial asthma, and to explore the mechanism. Methods Eighty patients were randomly divided into treatment group and control group. Each group had 40 patients. The control group was treated by conventional western medicine such as inhalation of steroid, and the treatment group was given oral use of WXPR based on the treatment for the control group. Treatment for both groups covered 60 days, and the follow-up lasted 10 months. The clinical outcomes included the clinical efficacy and immune indexes of peripheral blood T lymphocyte subsets CD3+, CD4+, CD8+and CD4+/CD8+ratio, and the serum IgE level. Results (1) After treatment and 12 months later, the scores of asthma control test (ACT ) in the two groups were increased obviously as compared with those before treatment, and there were statistically significant differences (P<0.05 or P<0.01) . The differences of ACT scores between the two groups were insignificant after treatment ( P>0.05) , but were significant ( P<0.05) 12 months later. (2) Within one year after treatment, the frequency of acute asthma attack was decreased in both groups (P<0.05 compared with that before treatment), and the treatment group had better effect on decreasing the attack frequency than the control group (P<0.05). (3) After treatment, the serum IgE, CD4+level, and CD4+/CD8+ ratio were decreased significantly, while CD8+ level was increased in the treatment group, and there were statistically significant differences ( P<0.05 compared with those before treatment). However, the differences of immune indexes in the control group were insignificant before and after treatment ( P>0.05). The improvement of immune indexes in the treatment group was superior to that in the control group ( P<0.05). Conclusion WXPR has good clinical efficacy in preventing and treating bronchial asthma. The mechanism is related with the inhibition of airway inflammation and improvement of immune function probably through regulating T lymphocyte subsets and lowering serum IgE level in asthma patients.

Objective To do a longitudinal study of nursing undergraduates′ humanistic care ability in different period of clinical practice, and analyze its trend, aiming to provide reference about how to intervene to enhance the nursing undergraduates' humanistic care ability. Methods A total of 67 nursing undergraduates were investigated about their humanistic care ability with the Care Ability Inventory (Caring Ability Inventory, CAI) at the beginning, middle and end of the internship. Results The scores of seniors′ humanistic care ability at the beginning, middle and end of the internship were 194.97±22.93, 192.18±26.87, 187.24±22.15, and there was a statistically significant difference among the three time points (F=3.234, P<0.05), there was significant difference of the dimension of courage in different periods (F=7.511, P<0.01). Conclusions There is a downtrend of nursing undergraduates′humanistic care ability in different stages of clinical practice, and we suggest that nursing educators and administrators should consider the characteristics of different periods in the clinical practice, and then do a targeted training to foster nursing students′humanistic care ability.

Codon usage bias is an important characteristic of genetic information transfer in organisms. Analysis of codon usage bias of different species is important for understanding the rules on genetic information transfer. The previous method for analysis of codon usage bias is mainly based on genomic data. However, this method is greatly limited, because the genome sequences of higher organisms are still not available up to now. In this study, we found that we could obtain the same optimal codons of Ganoderma lucidum (Curtis: Fr.) P. Karst based on its whole genomic data or large-scale transcriptomic data from its liquid-cultured hyphae, primordium and fruiting body, separately. This result indicated the feasibility to understand the codon usage bias based on the large-scale transcriptomic data. By calculating the proportion of rare codons of Escherichia coli and Saccharomyces cerevisiae in 26 terpene synthases (TS) of G. lucidum, we found that the rare codons of S. cerevisiae have a higher proportion in TS genes, while the rare codons of E. coli have relatively lower, suggesting that the TS genes of G. lucidum are possibly more difficult to be expressed in S. cerevisiae than in E. coli. Chemical synthesis of TS genes according to the yeast optimal codons will be an effective way to solve the problem on the mismatch of gene codon bias between the foreign genes and the host strain.

Objective To investigate the clinical effect of the chimeric flap based on the perforator of the posterior tibial artery for reconstruction of bone and skin defect in extremities.Methods From March,2007 to June,2013,the legs of the patients with bone nonunion,bone defect and skin soft tissue defect in extremities accepted digital subtraction angiography to find the larger perforators in the upper middle section of posterior tibia1 artery.Color doppler flow imaging (CDFI) was used to track branches and directions of the larger perforators and to look for the perforators which gave off secondary branches respectively to the skin and periosteum.According to the length of bone defect and the area of skin defect,the composite flap with the bone flap and skin flap was designed and cut out.It was based on the right single perforator of the posterior tibial artery which was chosen.The free composite flap was called the chimeric flap based on the perforator of the posterior tibial artery and repaired bone and skin defect in extremities.The size of osseous flaps ranged from 1.5 cm × 0.6 cm × 0.4 cm to 4.0 cm × 2.0 cm × 1.0 cm,and the size of cutaneous flaps of total 17 cases ranged from 2.0 cm × 2.0 cm to 7.0 cm × 6.0 cm.Results Followed-up for 3 to 18 months,all osseous flaps and cutaneous flaps survived,and all donor sites healed well.Nine cases of all cutaneous flaps were flat with the surrounding tissue and others bloated,but they were flat with the surrounding tissue after second phase to fat plastic.The fracture line disappeared and bone healed well at the time of 3-6 months after operation of bone graft to repair bone defect.According to the related evaluation criteria made by the Chinese Medical Association,the results of 10 cases of hand trauma patients were excellent in 7 cases,good in 2 cases,may in 1 case.Seven cases of lower leg and foot trauma patients could walk with load without pain.Conclusion Application of the chimeric flap based on the perforator of the posterior tibia1 artery is an ideal method to repair the small pieces of bone defect with skin soft tissue defect caused by various reasons in extremities.Because this kind of surgery doesn't sacrifice known vessels with minimal donor site morbidity,and healing time of the bone graft is short and the bone is not easy to absorb.

Objective To evaluate the effect of sevoflurane on the expression of aquaporin 8 (AQP8) in the intestinal mucosa in a pig model of hemorrhagic shock.Methods Twenty-four Bama miniature pigs of both sexes, weighing 22-25 kg, were randomly divided into 3 equal groups using a random number table: sham operation group (group S), hemorrhagic shock group (group HS) and sevoflurane group (group PS).The femoral artery and jugular vein were cannulated for blood pressure monitoring, blood-letting, and blood sampling in anesthetized pigs.Hemorrhagic shock was induced by withdrawing blood from the right femoral artery.Hemorrhagic shock was induced after cannulation in group HS.In group PS, 2％ sevoflurane was inhaled for 30 min after the model of hemorrhagic shock was successfully established.Before anesthesia, and at 0.5, 1, 1.5, 2, 3 and 4 h after hemorrhagic shock, blood samples were collected from the jugular vein for determination of serum D-lactic acid and intestinal fatty acid-binding protein (I-FABP) concentrations.The animals were sacrificed at 4 h after hemorrhagic shock, and the intestinal specimens were obtained for microscopic examination and for determination of AQP8 expression in the intestinal mucosa (by enzyme-linked immunosorbent assay).The intestinal water content was calculated.Results Compared with group S, the serum D-lactic acid and I-FABP concentrations, AQP8 expression, and intestinal water content were significantly increased in HS and PS groups (P＜0.05).Compared with group HS, the serum D-lactic acid and I-FABP concentrations, AQP8 expression, and intestinal water content were significantly decreased in group PS (P＜0.05).The pathological changes of intestinal tissues were significantly reduced in group PS as compared with group HS.Conclusion Sevoflurane can decrease the intestinal mucosal edema through inhibiting AQP8 expression, thus reducing hemorrhagic shockinduced damage to the intestinal mucosa in pigs.
