1.Jiangtang Mixture Combined with Ear Acupuncture for Type 2 Diabetic Insulin Resistance:A Clinical Observation of 40 Cases

Objective To study the therapeutic effect and mechanism of Jiangtang Mixture combined with ear acupuncture for type 2 diabetic insulin resistance(IR).Methods Totally 120 cases of type 2 diabetic IR were randomly and equally divided into western medicine(Metformin) group,Chinese medicine (Jiangtang Mixture) group and Chinese medicine with ear acupuncture group.The treatment lasted for one month.The therapeutic effect,fasting plasma glucose(FPG),fasting insulin(FINS),postprandial 2 hours plasma glucose(P2hPG),hemoglobin A1c(HbA1c),blood fat,insulin Sensitivity index(ISI) and insulin resistance index(IRI) were observed before and after treatment.Results The total effective rate in Chinese medicine with ear acupuncture group was 92.5%,which was superior to those in Chinese medicine group (P
2.Effect of different filling materials on percutaneous vertebroplasty and the image characteristics

BACKGROUND:Now the filling material which is used for the treatment of osteoporotic compression fractures is mainly polymethacrylic acid (PMMA),but the PMMA as a permanent foreign body cannot promote bone regeneration,and has a downward extension of the possibility of mechanical properties with time prolonging,so a new type of filling material used to substitute PMMA is looking.OBJECTIVE:To investigate the effect and image characteristics of different filling materials on percutaneous vertebroplasty performing on a goat model of osteoporotic compression fracture.METHODS:Three vertebral fracture models were established on osteoporosis goats by using an appropriate mechanical stress.Under the G-arm X-ray machine,the fractured vertebrae was performed with percutaneous vertebroplasty and then filled with polymethyl methacrylate (PMMA),self-setting calcium phosphate cement (CPC) and recombinant human bone morphogenetic protein-2(rhBMP-2)/self-calcium phosphate cement.RESULTS AND CONCLUSION:① A total of 36 vertebral bodies of 12 goats were successfully performed with percutaneous vertebroplasty,and there was no significant difference in filling quantity among three materials (3.5±1.8) mL (P>0.05).Bolus resistance of PMMA was strong,but operative time was short,and the image was great;the bolus resistance of self-CPC and rhBMP-2/self-CPC was weak,operative time was long,but there was no leakage placeholde.②CT scanning results on the 2~(nd) day after surgery demonstrated that the three materials were disorderly distributed in the vertebral body,showing high-density patching shaping.Six out of 36 vertebral bodies were leakage,which was induced by PMMA (n=4),self-CPC (n=1),and rhBMP-2/self-CPC (n=1).③ Scanning electron microscope indicated that all materials in the PMMA group were not able to closely bind with host bone,but self-CPC and rhBMP-2/self-CPC were able to closely bind with host bone.The results suggested that rhBMP-2/self-CPC is characterized by long-term operation,short time of bone substitute,and stabilization of reconstructed spine.

Objective:To clarify the mechanism of crisis serum' mediated gametocyte infectivity to the mosquito vector Methods:Observing the effects of mouse serum , which was obtained 5 days after P yoelii infection (D5 serum) on gametocyte infectivity by IFA and mosquito live feeds, and the production of IFN ??TNF ??IL 4 and NO - 2 in the hosts in vivo and in vitro by ELISA and Griess reaction And to investigate the ability of malaria parasitized red blood cell extract (PRBC extract) to induce NO Results:The development of the gametocytes from mice 5 days postinfection into ookinetes were completely inhibited D5 serum was not immediate to inhibit gametocyte development, which was injected intravenously into the mice 3 days after P yoelii infection But 4 h later after injection D5 serum stimulated the increasing IFN ? and NO production and inhibited gametocyte infectivity Moreover, PRBC extract showed the ability to induce NO Conclusion:Infected host serum blocks transmission of P yoelii via a nitric oxide dependent mechanism
4.Studies on the expression of Pys25 and Pys21 of Plasmodium yoelii and transmission blocking immunity

Objective:To investigate the expression of antigens in Plasmodium yoelii sexual stage and transmission blocking effects of monoclonal antibodies(McAbs).Methods:Observing the effects of anti Pys25 McAb4 and anti Pys21 McAb10 on developmental course of parasites in mosquitoes by direct mosquito feeds on passively immunized P.yoelii infected mice and the expression of Pys21 and Pys25 from gametocytes to ookinetes in indicated culture times by IFA and Western blotting.Results:The transmission blocking activity of the anti Pys25 McAb4 was complete and more potent than that of the anti Pys21 McAb10.Both Pys25 and Pys21 were presented in whole developmental course from gametocytes to ookinetes.Furthermore,the expression of Pys25 appeared to be earlier than that of Pys21 on zygote surfact.Conclusion:Pys25 and Pys21 are target antigens of transmission blocking immunity and that anti Pys25 McAb4 has more significantly transmission blocking activity is related with the early stage expression of Pys25 on zygote surface.
5.Clinical analysis of 58 cases of amyloidosis

Fifty eight patients with amyloidosis diagnosed by biopsy were admitted to Beijing Friendship Hospital from January 1983 to September 2007 and their clinical data were analyzed retrospectively.Twenty of the 58 cases were systemic amyloidosis,and other 38 were local amyloidosis.In 20 cased of systemic amyloidosis,kidney was involved in 75% cases,liver was in 55%,nervous system Was in 55%and heart Was in 50%.The lesion of local amyloidosis was usually limited in one single organ.
6.One-stage total knee arthroplasty for knee osteoarthritis combined with ipsilateral femoral extra-articular deformity:individual design

BACKGROUND:Total knee arthroplasty for knee osteoarthritis accompanied with extra-articular deformity is stil chal enging. An individual replacement plan should be designed to obtain good mechanical axis and to recover suitable soft-tissue balance. OBJECTIVE:To observe the safety and feasibility of individual y designed one-stage total knee arthroplasty for knee osteoarthritis combined with ipsilateral femoral extra-articular deformity. METHODS:A total of eight patients with knee osteoarthritis combined with ipsilateral femoral extra-articular deformity were enrol ed in this study. According to preoperative plan, five patients underwent intra-articular compensatory osteotomy, and the remaining three received extra-articular corrective osteotomy. Multi-planar deformities as wel as soft-tissue conditions were recorded. Main outcome measures included the hospital for special surgery score, Western Ontario and McMaster Universities Arthritis Index score and alignments deviation. RESULTS AND CONCLUSION:The patients were averagely fol owed up for 29 months. The hospital for special surgery score changed from preoperative 32.5 points to postoperative 87.5 points. The Western Ontario and McMaster Universities Arthritis Index score was increased from preoperative 37.1 points to postoperative 88.8 points (P<0.05). Mean mechanical axis deviation (either varus or valgus) changed from preoperative 17.1° to postoperative 1.4° (P<0.05). Al cases with extra-articular corrective osteotomy healed within 3 months, and none had postoperative infection, loosening or venous thromboembolic events. One patient was with 5° residual knee anti-sheets. Results verified that one-stage total knee arthroplasty procedures are effective in the treatment of knee osteoarthritis accompanied with ipsilateral femoral extra-articular deformity in accordance with individual preoperative plan.
7.Treatment of lumbar disc herniation by microendoscopic tubular retractor system (METRx) with a postmedial approach: Report of 290 cases

Objective To evaluate the therapeutic efficacy of microendoscopic tubular retractor system (METRx) with a postmedial approach in the treatment of lumbar disc herniation. Methods A total of 290 patients with lumbar disc herniation were treated with METRx. The duration of follow-up ranged from 3 months to 33 months. Results According to the Nakai scale, 81% (235 cases) of the patients were evaluated as excellent, 13.4% (39 cases) as good, 4.8% (14cases) as fair and 0.7% (2 cases) as poor. Postoperative infection in intervertebral space was found in 2 cases. Conclusions METRx has a satisfactory short-term therapeutic efficacy in the treatment of lumbar disc herniation.
8.A comparison between arthroscopic and open retrograde intramedullary interlocking nails in the treatment of femoral supracondylar fracture

Objective To compare the curative effect between arthroscopic and open retrograde intramedullary interlocking nails in the treatment of femoral supracondylar fracture. Methods A total of 46 patients with femoral supracondylar fracture underwent the treatment by either arthroscopic (19 patients, Group A) or open (27 patients, Group B) retrograde intramedullary interlocking nails. Results The operation time, surgical blood loss and bone union time in the Group A were less than those in the Group B ( t =-4 193, -21 517 and -4 055, respectively; P =0 000). Follow-up for (10 3?3 6) months in the 46 patients revealed the results of therapy were, according to the Kolmert criteria, excellent in 15, good in 3, fair in 1 in the Group A while excellent in 7, good in 14, fair in 4 and poor in 2 in the Group B, without statistically significances between the two groups in the “good” or “excellent” rates (94 7% vs. 77 8%; ? 2 =1 345, P =0 246). Conclusions Use of arthroscopic retrograde intramedullary interlocking nails has advantages of shorter operating time, less blood loss and quicker bone union, benefiting the recovery of knee joint functions.
9.Cases Analysis of Pharmaceutical Care Practice Performed by Clinical Pharmacist in the Patients with Tumor

Objective: To explore the working methods of clinical pharmacists for improving the rationality of drug use through participating in individualized treatment practice for the patients with tumor.Methods: Clinical pharmacists participated in the individualized treatment of the patients with tumor, found out adverse drug interactions, adjusted dosage for the renal insufficiency patients, treated infection or pain and corrected inappropriate drug combination, etc.Results: Clinical pharmacists provided integrated pharmaceutical care and offered medical advice in accordance with the individualized information of patients, which improved the treatment effect and avoided potential adverse drug reactions or adverse events.Conclusion: Clinical pharmacists provide pharmaceutical care and participate in the performance of individualized treatment for cancer patients, which can effectively improve the level of drug treatment.
