Chinese Journal of Geriatrics 2016;35(1):8-12
Objective To investigate the clinical characteristics and the associated risk factors for short-term prognosis in elderly patients with acute decompensated heart failure, in order to provide the evidence for improving clinical survival. Methods Clinical data of 200 elderly patients with acute decompensated heart failure in our hospital from June 2010 to December 2014 were retrospectively analyzed. The associated risk factors for prognosis were analyzed by using multivariate logistic regression analysis. All patients were followed up for six months, and cardiovascular events during the same follow-up period were recorded. All patients were divided into non-event group (n=158, no readmission during the follow-up period) and event group (n=42, readmission or death during follow-up).Results All the 200 elderly patients with acute decompensated heart failure had completed the 6 months follow-up, 23 (11.5%) patients died from cardiovascular events, and 19 (9.5%) patients had readmission due to heart failure.New York Heart Association (NYHA) classification, age, heart rate, systolic blood pressure, left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD), and left ventricular end-diastolic dimension (LVEDD) had significant differences between the two groups (x2=5.791, t=2.514, 2.552, 2.500, 2.582, 3.870, 2.275, all P<0.05).Levels of NT-terminal pro-brain natriuretic peptide (NT-proBNP), high sensitivity C reactive protein (hs-CRP), red cell distribution width (RDW), creatinine, and uric acid were higher in the event group than in the non-event group (t=54.948, 9.932, 2.815, 8.888, 2.368, all P<0.05).The hemoglobin level was lower in the event group than in the non-event group (t-2.455, P<0.05).Multivariate unconditional logistic regression analysis showed that NYHA classification, LVEF, and NT-proBNP were the independent risk factors for the prognosis of acute decompensated heart failure in elderly patients (x2 =10.438, 61.943, 6.976, all P<0.05).The mortality rate was 11.5% within the six months follow-up.The area under the receiver operating characteristic (ROC) curve for NT-proBNP to predict short-term mortality was 0.860 (SE=0.034,P=0.000, 95% CI: 0.794-0.926).The mortality within 6 months was lower in NT-proBNP ≤205 ng/L group than in NT proBNP> 205 ng / L group (x2=5.385, P=0.020).Conclusions NYHA classification, LVEF, and NT proBNP are the risk factors for the prognosis of acutely decompensated heart failure in the elderly. The elderly patients with acute decompensated heart failure have a poor prognosis.

Objective To study the function of let-7 and miR-24 in UVB-induced apoptosis. Methods After NIH3T3 cells were irradiated with UVB.Hoechest33342/PI staining was used to study the cell apoptosis and RT-PCR was used to uralyzc the expression level of let-7 and miR-24.In addition, potential target genes of these miRNAs in PicTar were classified into different function categories through GOstat software. Results Compared to the control, the NIH3T3 cells exposure to UVB appeared typical apoptotic and necrotic ceils by fluorescence microscope. The exprossion level of let-7 and miR-24 in NIH3T3 cells after UVB irradiation was higher than that of the control.Among the target genes,casp3,bc1212,map3kl and cdk5 were also involved in UVB-induced apoptosis mechanism. Conclusion Let-7 and miR-24 play a role in UVB-induced apoptosis.

Codon is an important carrier of genetic information in organisms. In evolution, codon bias is formed in many organisms. In this study, the analysis of expressed genes in Lonicera japonica Thunb. is presented. The codon bias of genes with complete coding region is analyzed through CodonW. And we calculated the codon usage bias of hydroxycinnamoyl-CoA: quinate hydroxycinnamoyl transferase (HQT) genes. The results showed that L. japonica were bias toward the synonymous codons with A and T at the third codon position.

Objective To evaluate the efficacy of a new hybrid bioartificial liver support system in the treatment of severe viral hepatitis. Methods A hybrid liver support system consisting of plasma exchange device, plasma perfusion column (charcoal) and a hollow fiber reactor inoculated with 1?10 9 aggregated human hepatocytes or porcine hepatocytes was developed. Ten patients with severe viral hepatitis were treated using this hybrid system. Results The effective rate of the hybrid bioartificial liver support system was 70% (7/10). The mental status and signs of toxicity of patients were improved markedly. Two patients of coma Ⅲ woke up and grade Ⅱ encephalopathy in 4 patients disappeared after treatment. At the same time, a marked decrease in total serum bilirubin and prothrombin time was noted after treatment. Finally, Three out of 10 patients recovered completely. Two patients were bridged to liver transplantation. Five patients died from the complication of hepatic failure or discharge refused advice due to worsening of disease. Conclusions The evident clinical improvement shows that this artifical liver system has prominant liver support effects for the patients with severe viral hepatitis and liver failure.

Conclusion As2 O3 inhibits human renal carcinoma cell line 786-O proliferation through inhibiting the PI3K-Akt transduction path-way ,and has potential clinical value for the treatment of kidney cancer .

Objective To study the expression level of miR-21 in UVB irradiated HaCaT cells and A431 cells. Methods Real-time qPCR was used to examine the expression level of miR-21 in HaCaT cells and A431 cells after 50 J/m2UVB radiation. The possible target genes were predicted by PicTar and performed function categories with Gostat analysis. Results Compared with the HaCaT cells, miR-21 the expression level in A431 cells increased over 4 times. At 2h and 4h after UVB irradiation, the expression level of miR-21 in HaCaT cells were up regulated, and it lowered 2 times at 8 h compared with the control. There was no further change in the expression level of miR-21 after 12 h. While miR-21 expression levels in A431 cells were not changed signifcantly. The results of target prediction and Gostat analysis suggested that PIK3R1, BCL2 and E2F3 were involved in the cell differentiation and cell process. Conclusion miR-21 possibly involved in the pathogenesis of epidermal squamous cell carcinoma and the mechanism of UVB-induced injury.

After the intervention, the level of IgA(1.92±0.63) g/L, IgG(11.36±1.26) g/L, IgM(2.01±0.57) g/L in the observation group was higher than that in the control group (1.62±0.49), (9.58±1.23), (1.60±0.47) g/L, respectively, and the differences between the two groups were significant ( P=0.020, <0.001, =0.001, respectively). In the observation group, the hospital stay(12.1±1.7) d, postoperative ambulation time (1.9±0.6) d and exhaust time (3.1±0.4) d was less than that in the control group(17.2±2.4) d, (2.8±1.0) d, (4.2±0.8) d, respectively, and there were significant differences between two groups(all P<0.001). The postoperative complication rate of 5.0%(2/40) in the observation group was significantly lower than 25.0%(10/40) in the control group (χ 2=6.275, P=0.012). Conclusion:The patients with radiation enteritis and intestinal obstruction treated by conventional surgery were given eras intervention, which improved the nutritional level, immune function and inflammatory stress reaction of the patients, improved the treatment effect of the patients, and shortened the hospitalization time.
