1.Typing,Precaution and Treatment of the Bone Cement Leakage Caused by Percutaneous Vertebroplasty

Objective To explore the typing,precaution and treatment of the bone cement leakage caused by percutaneous vertebroplasty(PVP).Methods PVP was performed in 38 cases(11 men and 27 women) with vertebral angioma and vertebral body compression fracture with osteoporosis chosen from November 2002.Results All the patients were followed up for 8 to 16 months and the effects were evaluated by the standards carried out by World Health Organization(WHO).The rate of complete response(CR) plus partial response(PR) was 100 percent.The patients with bone cement leakage by PVP were divided into 3 groups which underwent different remedial methods respectively.Conclusion The bone cement leakage could be prevented through the preoperative preparation,choosing the anaesthesia and scouting the whole operation.The patients with TypeⅠ,Ⅱ and Ⅲ_a needn't treatment,but those with type Ⅲ_b must accept open-operation.Vertebral eggshell technology with open vertebroplasty was effective on the treatment of the bone cement leakage with type Ⅲ_b caused by PVP.
2.Treatment of lumbar disc herniation by microendoscopic tubular retractor system (METRx) with a postmedial approach: Report of 290 cases

Objective To evaluate the therapeutic efficacy of microendoscopic tubular retractor system (METRx) with a postmedial approach in the treatment of lumbar disc herniation. Methods A total of 290 patients with lumbar disc herniation were treated with METRx. The duration of follow-up ranged from 3 months to 33 months. Results According to the Nakai scale, 81% (235 cases) of the patients were evaluated as excellent, 13.4% (39 cases) as good, 4.8% (14cases) as fair and 0.7% (2 cases) as poor. Postoperative infection in intervertebral space was found in 2 cases. Conclusions METRx has a satisfactory short-term therapeutic efficacy in the treatment of lumbar disc herniation.
3.A comparison between arthroscopic and open retrograde intramedullary interlocking nails in the treatment of femoral supracondylar fracture

Objective To compare the curative effect between arthroscopic and open retrograde intramedullary interlocking nails in the treatment of femoral supracondylar fracture. Methods A total of 46 patients with femoral supracondylar fracture underwent the treatment by either arthroscopic (19 patients, Group A) or open (27 patients, Group B) retrograde intramedullary interlocking nails. Results The operation time, surgical blood loss and bone union time in the Group A were less than those in the Group B ( t =-4 193, -21 517 and -4 055, respectively; P =0 000). Follow-up for (10 3?3 6) months in the 46 patients revealed the results of therapy were, according to the Kolmert criteria, excellent in 15, good in 3, fair in 1 in the Group A while excellent in 7, good in 14, fair in 4 and poor in 2 in the Group B, without statistically significances between the two groups in the “good” or “excellent” rates (94 7% vs. 77 8%; ? 2 =1 345, P =0 246). Conclusions Use of arthroscopic retrograde intramedullary interlocking nails has advantages of shorter operating time, less blood loss and quicker bone union, benefiting the recovery of knee joint functions.
4.Expression and Immunocompetence Characterization of Plasmodium falciparum Lactate Dehydrogenase

Objective To express lactate dehydrogenase (LDH) gene of Plasmodium falciparum FCC1/HN in the E. coli TG1 and analyse its immunocompetence. Methods The LDH gene of the P. falciparum was specifically amplified by polymerase chain reaction, and the recovered gene fragment was cloned into pGEX 4T 1 vector for expression of fusion protein with glutathione S transferase(GST). The recombinant plasmid was transformed into the E. coli TG1. Four mice (Kunming strain) were immunized with purified expressed protein(antigen) and the polyclonal antibodies were collected. The immunocompetence of recombinant protein was analysed by ELISA and Western blot. Results The LDH gene of P. falciparum was successfully expressed in the E. coli TG1. The expressed protein exhibited a specific reaction with immune sera obtained from rabbits immunized with P. falciparum . The specific humoral responses were induced in mice and the titer of the specific antibody was 1∶16 by two dimensional diffusion assay. Conclusion The LDH gene of P. falciparum has been successfully expressed in the E. coli TG1 and the expressed protein has high antigencity.
5.Expression of collagen subtype I mRNA during autogenous tendon formation induced by grafted human hair keratin artificial tendon

BACKGROUND: Numerous experiments and clinical practice show that human hair keratin artificial tendon induces the organism to form autogenous tendon. The process of autogenous tendon formation mainly involves the synthesis, secretion and package of collagen subtype I.OBJECTIVE: To explore the role of collagen subtype I mRNA expression in autologous tendon formation after human hair keratin artificial tendon implantation.DESIGN: A completely randomized controlled experiment based on the experimental animals.SETTING: Department of Histology and Embryology and Department of Biochemistry and Molecular Biology of Southern Medical University.MATERIALS: The experiment was conducted in the Experimental Animal Center of the First Military Medical University of Chinese PLA from May 2003 to September 2004. Totally 33 New Zealand rabbits of either gender,weighing 2.0 to 2. 5 kg, were provided by the center. The animals were randomly divided into experiment 3, 6, 9, 12, 16 and 20 weeks groups, negative control 9 and 20 weeks groups and normal control group. Among them,experiment 3 and 6 weeks group and normal control group had 3 rabbits in each and the other groups had 4 rabbits. Human hair keratin artificial tendons were normal human hair treated by a series of biochemical methods and were supplied by the Department of Biochemistry & Molecular Biology of the university. The human hair keratin artificial tendons were divided into three groups with different degradation rates, namely, fast(F), medium(B) and slow(Z). The tendons were made up of the fast, medium and slow degradation groups mixed at the ratio of 4: 3: 3.off by 1.0- 2.0 cm, human hair keratin artificial tendon was grafted by end-to-end anastomosis with both ends of the broken tendon before sewing control group, no artificial tendon was implanted although the animals ungroup was normal rabbits' tendon. Sampling was carried out at 3, 6, 9, 12, 16and 20 weeks after human hair keratin artificial tendon implantation in experiment groups, and at 9 and 20 weeks after operation in negative control group, respectively. The expression of collagen subtype I mRNA was detected at weeks 3, 6, 9, 12, 16 and 20 after grafting using reverse transcription polymerase chain reaction technique.MAIN OUTCOME MEASURES: The ratio of collagen subtype I mRNA to Glyceraldehyde-3-phosphate dehydrogenase(GADPH) mRNA in normal tendon and autogenous tendon induced by human hair keratin artificial tendon at all time points was calculated, and significance test between all these paired groups were performed.collagen subtype I mRNA/GADPH mRNA expression was 0.96 ±0.02 in expression of collagen subtype I mRNA/GADPH mRNA in autogenous tendon induced by human hair keratin artificial tendon in experiment group appeared at week 3, increased rapidly at week 3 to 6, peaked at week 6, and remained stable at week 9 to 20. The expression at week 6 was significantly higher in experiment group than in normal control group( F = 6. 254, P < 0.05); the expression at other weeks was also significantly higher in experiment group than in normal control group( F= 1. 258 - 1. 987, P > 0.05).CONCLUSION: The activation, proliferation and secretion of collagen protein as well as the synthesis of collagen subtype I by tenocytes may be responsible for autologous tendon formation after human hair keratin artificial tendon implantation.
6.Study on the Influence Factors for the Preparation of Reverse Microemulsions and the Optimization of Its Formula

OBJECTIVE:To study the effect of oil phase,emulsifier and drugs on reverse microemulsion.METHODS:Pseudo-ternary phase diagram was adopted to study the effect of Span-80/Tween-80(emulsifier),thymopentin(model drug), long-chain glyceride,medium-chain glyceride,non-glyceride(oil phase).on the preparation of reverse microemulsions.Optimal formula of reverse microemulsions was selected.RESULTS:The largest water-in-oil(W/O)area was obtained in reverse microemulsion which was prepared using medium-chain glyceride as oil phase.Final formula was confirmed and contained distilled water/ Span-80/Tween-80/caprylic/capric triglyceride(2∶3∶6∶9).In addition,thymopentin was incorporated into the aqueous phase.CONCLUSIONS:The preparation of reverse microemulsions will be influenced by emulsifier,constitution of oil phase and drug.The influence factors for the actual preparation of reverse microemulsions should be optimized.

Multidrug resistance proteins (MRP2, ABCC2) may play a role in drug resistance in epilepsy by
limiting gastrointestinal absorption and brain access of antiepileptic drugs (AEDs). We sought to
investigate the effects of ABCC2 polymorphisms on plasma carbamazepine (CBZ) concentrations
and pharmacoresistance in Chinese patients with epilepsy. ABCC2 rs717620, rs2273697, rs3740066
polymorphisms were genotyped by polymerase chain reaction amplification followed by restriction
fragment length polymorphism analysis or direct automated DNA sequencing in 80 patients treated
with CBZ monotherapy. There were no differences in CBZ maintenance doses or adjusted plasma
CBZ concentrations among the ABCC2 rs717620, rs2273697 and rs3740066 genotypic groups.
No associations between all the studied genotypes and haplotypes involving the three SNPs of
ABCC2 and CBZ resistance were observed in this patient cohort. These results suggest that ABCC2
polymorphisms may not contribute to interindividual variabilities in CBZ daily maintenance doses,
plasma concentrations, and treatment efficacy.

9.Recombinant human erythropoietin as a novel agent with pleiotropic effects against sepsis-induced acute kidney injury

Objective To investigate the protective effects of recombinant human erythropoietin (rHuEPO) on caecal ligation and puncture (CLP)-induced acute kidney injury (AKI).Methods A total of 260 healthy male Sprague-Dawley rats (250-300 g) were randomly divided into 6 groups:normal control group,sham group,CLP model group,the large dose rHuEPO (5000 U/kg)group,the middle dose rHuEPO (1000 U/kg) group,and the small dose rHuEPO (500 U/kg) group.The rat models of sepsis were established by CLP.In treatment groups,rats were treated with rHuEPO through caudalis injection after CLP surgery.Each group was divided into 2-,6-,12-,24-,36-hour subgroups with 10 rats.Rats were sacrificed and the tissue samples including kidney and blood samples were collected.The kidney function,plasma cytokines [interleukin 6 (IL-6) and tumor necrosis factor alpha (TNF-α)],kidney injury moleclue 1 (KIM-1) and inducible nitric oxide synthase (iNOS)were measured.Cytokines were determined by ELISA method.The expression of nuclear factor-kappaB (NF-κB) protein in kidneys were detected by immnunohistochemistry method.Pathological changes of kidney tissues were observed by light and transmission electron microscopy for cytokine content and apoptosis.Results Compared with CLP model group,renal function,the levels of TNF-α,IL-6,KIM-1 and iNOS in serum,the expression of NF-κB,significantly decresed in large dose rHuEPO group (all P < 0.05).rHuEPO also lessened the histological changes in large dose group.rHuEPO did not lessen the histological changes in others.Conclusion rHuEPO can inhibit the levels of TNF-α,IL-6 and iNOS in serum,thus modify the inflammatory response and provide protective effects against acute kidney injury induced by sepsis.
10.Predictive factors predicting inadequate ST-segment resolution in patients with acute ST-segment elevation myocardial infarction after percutaneous coronary intervention

Objective To survey ST-segment resolution in STEMI patients undergoing emergency percutaneous coronary intervention (PCI) and to find the specific clinical features of patients with inadequate ST-segment resolution.Methods A total of 198 patients were divided into two groups according to the ratio of ST-segment resolution:relatively adequate ST-segment resolution group (> 50%) and inadequate STsegment resolution group (< 50%).The clinical features,infarct-related artery and PCI-related evants were evaluated,and major adverse cardiovascular events (MACE including target vessel revascularization,recurrent myocardial infarction,or death) were recorded during hospitalization and follow-up period.Multivariate logistic analysis was used to identify relevant factors influencing ST-segment resolution of STEMI patients after treatment with PCI.The Statistical analyses of data were carried out using SPSS 10.0 software.Results (1) There were 156 patients with relativey adequate ST-segment resolution and 42 patients with inadequate ST-segment resolution.Of them,there were higher percentage of patients aged over 75years in the inadequate ST-segment resolution group than those in the relatively adequate ST-segment resolution group (9 cases,21.4% vs.14 cases,9.0% ; P <0.05).(2) In inadequate ST-segment resolution group,thetotal ischemic time was significant longer [(5.2 ±2.2) h vs.(3.0 ± 1.6) h,P <0.01].The infarctrelated artery (IRA) was more common at left anterior descending coronary artery (LAD) (27 cases,64.3% vs.69 cases,44.2%; P < 0.05) and there were fewer patients with TIM grade 3 of IRA in inadequate ST-segment resolution group after primary PCI than that in relative adequate ST-segment resolution group (32 cases,76.2% vs.140 cases,89.7% ; P < 0.05).There was a lower rate of using GP Ⅱ b/Ⅲ a receptor antagonist and a higher rate of prescribing IABP in inadequate ST-segment resolution group.(3) There is a higher incidence of MACE during hospitalization and follow-up period in patients with inadequate ST-segment resolution.(4) Multivariate logistic analysis indicated that age over 75 years,LAD occlusion,the total ischemic time were related to ST-segment resolution.Conclusions The patients with age over 75 years,LAD occlusion,longer ischemia time,and unemployment GP Ⅱ b/Ⅲ a receptor antagonist before PCI were prone to get inadequate ST-segment resolution and poor prognosis.Age over 75 years,LAD occlusion,and longer ischemic time were independent risk factors of the inadequate ST-segment resolution in STEMI patients after emergency PCI.
