2.P-glycoprotein involved in regulation of penetration of carbamazepine and phenytoin through blood-brain barrier

Objective To study the effect of P-glycoprotein (PGP) in regulation of penetration of carbamazepine and phenytoin through blood-brain barrier, as to indicating the entry of epileptic drugs into the brain restricted by PGP and deducing the multidrug resistance mechanisms of refractory epilepsy. Methods The microdialysis probe was placed into the cortex of rats, and after systemic injection of carbamazepine and phenytoin, dialysate was collected and the antiepiletic drug concentration in the extracellular fluid of the cerebral cortex was determined by high-performance liquid chromatography (HPLC). Then observing whether the concentration of drugs in extracellular fluid can be enhanced by PGP’s inhibitor verapamil. Results Verapamil significantly enhanced the concentration of carbamazepine in extracellular fluid of the cerebral cortex during 60—90 minutes after injection ((1.74?0.28)?g/ml in 60 minutes and (1.87?0.31)?g/ml in 90 minutes,P
3.Moxibustion on Governor Vessel for ankylosing spondylitis due to kidney yang deficiency:a clinical controlled study

Objective:To observe the clinical effect of moxibustion on Governor Vessel for ankylosing spondylitis (AS) due to kidney yang deficiency.
Methods:A total of 60 patients of AS due to kidney yang deficiency were randomly divided into a treatment group and a control group by random digital table, 30 cases in each group. Both of the two groups were treated by oral administration of Sulfasalazine tablets. Additionally, the treatment group was given moxibustion on Governor Vessel, and the control group was given sham moxibustion. The treatment was given once every other day, 4 weeks constituted one course, 2 courses in total. Before treatment, after 1-course and 2-course treatments, the quantified scores of traditional Chinese medicine (TCM) syndromes and the symptoms scores recommended by AS International Society [including Bath ankylosing spondylitis disease activity index (BASDAI), visual analogue scale (VAS), Bath ankylosing spondylitis functional index (BASFI) and Bath ankylosing spondylitis metrology index (BASMI)] were respectively observed and recorded, and the change of safety item was monitored.
Results:After 1-course and 2-course treatments, the quantified scores of TCM syndromes were obviously improved than those before treatment in the two groups (P<0.05), and the score was improved better in the treatment group than that in the control group (P<0.05). In comparison within the group, there were no statistical differences between the scores after 1-course treatment and 2-course treatment in the two groups (P>0.05). After 1-course treatment, the items of the symptoms scores were improved than those before treatment in the two groups (P<0.05). After 2-course treatment, the therapeutic effect was more remarkable in the treatment group, with a statistical difference between the two groups (P<0.05). During treatment, no obvious abnormality in all safety items was noticed in the two groups.
Conclusion:Moxibustion on Governor Vessel can effectively relieve the clinical symptoms of AS patients, without obvious adverse reaction.
4.Relationship of optic materials with anterior and posterior capsule opacification as well as capculorhexis contraction of lenses

BACKGROUND: Capsule opacification is a common complication following implantation of intraocular lenses for a long period. Scholars are looking for an ideal intraocular lens so as to reduce the incidence of capsule opacification. OBJECTIVE: To study the effects of three intraocular optic materials with sharp optic edge on anterior and posterior capsule opacification as well as capsulorrhexis contraction. DESIGN, TIME AND SETTING: A randomized controlled study was performed at the Department of Ophthalmology, Affiliated Hospital of Chengde Medical College from May 2005 to December 2006. PARTICIPANTS: A total of 135 patients (148 eyes) with age-related cataract, including 73 males (80 eyes) and 62 females (68 eyes) and aging 52-81 years with the mean age of (71.44 ?6.83) years, were collected in this study. METHODS: All patients were performed with phacoemulsification combining with implantation of foldable intraocular lenses which were characterized by sharp optic edges. Thereafter, they were randomly divided into three groups: lyophobic material group (n=43, 49 eyes), who were implanted with lyophobic acrylic acid intraocular lens; hydrophilic material group (n=42, 46 eyes), who were implanted with hydrophilic acrylic acid intraocular lens; lyophobic/hydrophilic material group (n=50, 53 eyes), who were implanted with lyophobic/hydrophilic acrylic acid intraocular lens. MAIN OUTCOME MEASURES: Anterior and posterior capsule opacification as well as capculorhexis contraction were quantitatively evaluated 1 year postoperatively. RESULTS: At one year postoperatively, incidences of posterior capsule opacification were 8.3% in the lyophobic material group, 26.7% in the hydrophilic material group, and 15.3% in the lyophobic/hydrophilic material group, respectively, and there was significant difference (P 0.05). CONCLUSION: The optic intraocular lens of sharp optic edge does not have any effects on anterior capsule opacification or capsulorrhexis contraction, but has effects on formation of posterior capsule opacification; in particular, lyophobic acrylic acid can reduce the incidence to posterior capsule opacification.
5.Clinical Controlled Trial of Du Meridian Moxibustion for Ankylosing Spondylitis of Kidney Yang Deficiency Type

Objective To evaluate the therapeutic effect of Du meridian moxibustion on ankylosing spondylitis (AS) of kidney yang deficiency type. Method Sixty patients with active AS of kidney yang deficiency type were randomly allocated to treatment and control groups, 30 cases each. The treatment group received Du meridian moxibustion plus oral administration of sulfasalazine tablets and the control group, sham moxibustion plus oral administration of sulfasalazine tablets. Treatment was given once every other day, four weeks as a course, for a total of two courses. The Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) score, the Pain Visual Analogue Scale (VAS) score, the Bath Ankylosing Spondylitis Functional Index (BASFI) score and the Bath Ankylosing Spondilitis Metrology Index (BASMI) score were recorded and laboratory safety indicators were observed in the patients. Result The TCM syndrome score decreased in both treatment and control groups of AS patients at four and eight weeks of treatment (P<0.05). The therapeutic effect was significantly better in the treatment group than in the control group (P<0.05). There was no statistically significant difference in therapeutic effect between different courses (P>0.05). The BASDAI, VAS, BASFI and BASMI scores decreased in the two groups of AS patients at four weeks of treatment (P<0.05). They decreased more in the treatment group at eight weeks of treatment; there were statistically significant differences compared with the control group (P<0.05). The laboratory safety indicators were not markedly abnormal. Conclusion Du meridian moxibustion can effectively relieve the clinical symptoms and does not cause obvious adverse reactions in AS patients. It is a safe and effective way to treat AS.
6.Effect of resveratrol on the expressions of FoxO1 in kidneys of diabetic rats

Streptozotocin-induced diabetic rats were treated with resveratrol for 12 weeks.Compared with group of diabetic rats without treatment,the levels of urine albumin,serum creatinine,and blood urea nitrogen were significantly decreased and pathological changes in kidney were improved in treatment group ( P<0.05 ).The mRNA expressions of FoxO1 and catalase were higher and FoxO1 phosphorylation level in diabetic rats treated with resveratrol was lower than that in diabetic rats without treatment (P<0.05),suggesting that resveratrol may protect the kidneys of diabetic rats via regulating expression of FoxO1.

Ligusticum chuanxiong is a Common Chinese medicinal herb which is widely used in clinical treatment. Modern research has shown that the ingredients of Ligusticum chuanxiong such as tetramethylpyrazine and ferulic acid have multiple effects of scavenging oxygen free radicals,calcium-antagonism,vasodilating,inhibiting platelet aggregation and thrombosis and so on. This article reviews the research progress in pharmacological activities of the available compositions in ligusticum chuanxiong in the past several years.
8.Feasibility of dose reduction in lumbar spine multi-slice CT examination with automatic tube current modulation

Objective To investigate the feasibility of achieving consistent image quality with dose reduction technology in lumber spine MSCT examination with Z-axis automatic tube current modulation (ATCM). Methods Forty-eight patients diagnosed as lumber intervertebral disc protrution scanned twice by MSCT before and after interventional operations with the same coverage from third lumbar vertebra to first sacral vertebra. The first scan (FM) was with fixed tube current of 320 mAs. The follow-up scan was with ATCM with noise index (NI) of 12.0 HU. At the levels of L3-4, L4-5 and 15-S1, image quality, image noise and radiation dose were measured and analyzed. Image quality and radiation dose were compared by paired t-test and the image noise was compared by ANOVA test. Results The dosage of the ATCM had a 31.3% reduction compared with FM, the average DLP was(187.9±66.4)mGy·cm and(273.4±45.4) mGy·cm respectively, where t = 8.205, P < 0.05. The average noise and their deviations for the FM group were (9.8±2.4) HU,(9.9±2.4) HU, and (11.5±3.2) HU at level of L3-4, L4-5, 15-S1, respectively. With ATCM, the average noise was(12.0±0.8) HU, (11.7±0.6) HU, and (11.7±1.4) HU, respectively. There was statistical difference between the two groups (F = 23.31, P < 0.05). The image quality scores for the FM group were (4.7±0.3), (4.5±0.2), (4.5±0.2) and showed no statistical difference to ATCM group (4.6±0.3), (4.5±0.2), (4.5±0.2) at level of L3-4, L4-5, L5-S1, respectively, where t = 1.000, P > 0.050 Conclusion ATCM technique with the noise index setting at 12.0 HU can achieved a 31.3% dose reduction while keep the consistent image quality for lumbar spine MSCT study.
9.Effects of Yiqi Huayu Recipe on neural cell adhesion molecule in rats with lumbar nerve root compression

OBJECTIVE: To study the effects of Yiqi Huayu Recipe on neural cell adhesion molecule (N-CAM) in neuromuscular junctions during nerve regeneration in rats with lumbar nerve root compression. METHODS: The rats with lumbar nerve root compression were given Yiqi Huayu Recipe for 10, 20 and 30 days respectively. The distribution of N-CAM in neuromuscular junctions of soleus muscle in rats was examined with immunohistochemical method and confocal laser scanning microscopy technique. The acetylcholine receptor (AChR) was visualized with fluorescein-conjugated alpha-bungarotoxin (alpha-BTX). The overlap areas of N-CAM and AChR sites were measured with NIH image technique. RESULTS: The aggregates, sprouts and extensions of N-CAM in the neuromuscular junctions and the overlap areas of N-CAM and AChR sites in the Yiqi Huayu Recipe-treated group were all better improved than those in the untreated group. CONCLUSION: The expression of N-CAM is regulated according to the state of innervation for muscles. Yiqi Huayu Recipe may accelerate this nerve regeneration process.
10.Anatomical Landmarks for Lateral Ankle Ligament Reconstruction—a Cadaveric Study

Purpose To clarify the morphological parameter and describe the distance from the insertion of the lateral ankle ligaments to the adjacent bony landmarks through precisely anatomical explore of human cadaveric ankles,so as to provide anatomical evidences for the reconstruction of lateral ankle ligaments.Methods Nineteen ankle specimens were dissected to isolate the lateral ankle ligaments and measure the morphological parameters such as length,width,thickness and the distance from the insertion of the lateral ankle ligaments to the adjacent bony landmarks.Results The average length of anterior talofibular ligaments (ATFL) was 23.1 ± 2.98 mm,among which 8 were single-banded(42.1%)and 11 were double-banded(57.9%).The average distance from the fibular origination of ATFL to the anterior tubercle of fibula(AA)was 17.1 ± 3.00 mm,to the fibular obscure tubercle(AO)was 5.1 ± 1.69 mm,to the tip of the fibula(AT)was 14.1 ± 2.86 mm.The distances from the talus insertion of ATFL to the superior and inferior talus articular surface were 11.4 ± 2.25 mm and 18.4 ± 2.30 mm respectively,to the anterior lateral talus chondral surface was 4.8 ± 1.42 mm.The average length of calcaneofibular ligament(CFL)was 31.4 ± 3.55 mm.The average distance of the fibular origination from ATFL to CFL was 6.4 ± 2.55 mm.The average angle between ATFL and CFL was 116.6 ± 12.69°.The distance from the calcaneus insertion of CFL to the peroneal tubercle(CP)was 15.4 ± 2.86 mm,to the posterior superior border of calcaneus(CC)was 13.9 ± 2.46 mm,to the subtalar joint surface was 15.2 ± 3.21 mm.The coefficient variation assessing the anatomical reliability of different bony landmarks were as follows:ATFL fibular origination AA(17.54%) <AT(20.28%) < AO(33.14%),CFL calcaneus insertion CC(17.70%)<CP(18.57%)<CS(21.1%).Conclusion Certain variations exist in the morphological parameters and the distances from the insertion of the lateral ankle ligaments to the adjacent bony landmarks.It provides anatomical evidence for lateral ankle ligament reconstruction in treating chronic ankle instability.
