1.VDD Pacemaker in an Unipolar Lead for Clinical Practice

Objective To inquire into the forward clinical results of ventricular pacing, dual-chamber sensing, atrial-triggered, and ventricular-inhibited (VDD) Pacemaker in an unipolar lead.Methods 16 patients with the pacemaker were studied from 1993 to 2002, including 12 male patients and 4 female patients at 49 to 75 years old (average age 64 4 7 8 years old). They were in normal sinus rhythmia with complete or high degree atrial-ventricular block. Lead electrode was inserted through subclavian venous access, and the pacemaker was implanted in the same side as the electrode.Results All patients were followed up from 1 to 112 months (average 68 8 6 3 months) after pacemaker implantation. Their heart functions were improved, symptoms disappeared, life quality remarkably improved. Atrial sensing of VDD was good. One patient was died from coronary heart disease with heart failure by following up for 42 months, but the rest were healthy. Conclusions VDD pacemaker implantation was simple, easy to perform if its indication was appropriate. It could alleviate patients symptom and its therapy was effective. We should think highly of using VDD pacemaker, especially at poor region.
2.THE EFFECT OF TADPOLE EXTRACT ON HELA CELLS AND ITS POSSIBLE MECHANISM

The tadpole extract without large molecular protein(T-871) was prepared from dry tadpoles. This extract and RPMI1640 medium were mixed in the ratio of 1:50(V/V) to form T-871 medium. HeLa cells were cultured in the T-871 medium in order to study the possible mechanism of HeLa cell differentiation induced by tadpole extract. We found that there was a decreased acitivity of LDH which may be released through HeLa cell membrane in the 2 day's cultured T-871 medium. After 3 day's culture in the T-871 medium we found that on the HeLa cell membranes, Na-K-ATPase activity reduced and the content of Con A receptors increased. When these cells were analyzed by flow cytometry(FACS-420), the results indicated that HeLa cells accumulated in G_2 and M phases of the cell cycle and the influence on DNA and RNA content of the HeLa cells was insignificant. HeLa cells cultured by RPMI 1640 medium or T-871 medium for 3 days were inoculated into the dorsal hypodermis of 12 nude mice, respectively. Then after 24 hours this extract was injected to each nude mouse at dose of 0.3 ml each day. Our study showed that T-871 could result in decrease of tumor formation such as delay of tumor nodule appearance and reduce of tumor weight. This study suggested that the changes of the cell membrane may be caused by the tadpole extract. It may also reduce malignancy of the HeLa cells.
3.Clinical efficacy of plasma exchange in 47 patients with severe bullous dermatoses or drug eruption

Objective To estimate the therapeutic value of plasma exchange (PE) in severe bullous dermatoses and drug eruption. Methods Plasma exchange was carried out to treat 47 patients with severe dermatoses including 15 cases of pemphigus, 17 cases of bullous pemphigoid and 15 cases of drug eruption who were intolerant or unresponsive to glucocorticosteroids and or immunosuppressants. Cobe Spectra blood cell separator was utilized to collect plasma and cell components from patients' blood, and the replacement fluid and cell components were infused back into patients. Patients received 1 to 3 sessions of plasma exchange. Results Of the 47 patients, 44 (93.62%) achieved satisfactory efficacy with relief of clinical symptoms and improvement of laboratory parameters 2 to 3 days after the plasma exchange. Side effects occurred in 11 (23.4%) patients, which included fever, shivering, numbness of limbs, pruritus and convulsion. Conclusion Plasma exchange is beneficial for the control of severe drug eruption and bullous dermatoses.
4.Analysis and countermeasures of current situation of moist-associated skin damage of medical staff caused by the second and third levels of personal protection equipment

Objective:To analyze characteristics, related factors and prevention status of moist-associated skin damage of medical staff caused by the second and third levels of personal protection equipment, and provide basis for formulating prevention and treatment countermeasures.Methods:From February 8 to 22, 2020, the convenient sampling method was adopted to select 1 814 medical staff from 161 hospitals in China as the research objects.The questionnaire was uploaded to Questionnaire Star website, and the questionnaire link was sent to the WeChat working group of medical staff. Medical staff voluntarily used mobile phones to fill in and submit demographic data, moist-associated skin damage data, preventive measures data and other data online.Data were exported from the website and the database was established after double check. SPSS software was used to analyze the occurrence characteristics, related factors and prevention status of moist-associated skin damage and put forward countermeasures.Results:A total of 1 761 medical staff from 161 hospitals in China submitted the questionnaire, including 290 males (16.47%) and 1 471 females (83.53%) .The overall prevalence rate of moisture-related skin damage was 18.85% (332/1 761) . The prevalence rate of third-level personal protective equipment was higher than that of second-level personal protective equipment, and the difference was statistically significant ( P=0.001) .The incidence of wearing time greater than 4 hours group was higher than that of less than or equal to 4 hours group, and the difference was statistically significant ( P<0.05) .The prevalence rate of multiple sites was higher than that of single site ( P< 0.001) . Binary Logistic regression analysis showed that the main associated factors that increased the risk were sweating and dampness ( OR=168.52, P<0.001) and wearing third-level personal protective equipment ( OR=1.65, P<0.05) , and only 12.72% (224/1 761) of them took preventive measures before damage. Conclusions:The incidence of moist-associated skin damage of medical staff caused by the second and third levels of personal protective equipment is relatively high. Among them, sweating and wetness and the third level equipment are the main risk factors, and the prevention is insufficient. It is necessary to strengthen assessment, cleansing, moisturizing, skin care, moisture absorption and other preventive strategies. After damage, local anti-inflammatory and protective treatments should be done on the basis of prevention.

Objective To evaluate the diagnostic value of contrast-enhanced ultrasound in breast precancerous lesions . Methods Retrospectively analyzed the contrast-enhanced ultrasound model and angiographic predictive model of 465 cases of the A prospective multicenter study of breast nodules contrast-enhanced ultrasound" that led the Sichuan Provincial People′s Hospital from January 2016 to April 2017 ,which included 69 cases of breast precancerous lesions and 396 other types benign lesions ,and the sensitivity ,specificity and accuracy of the diagnosis of breast precancerous lesions were calculated . Results The sensitivity of ultrasound predictive model for the diagnosis of precancerous lesions was 60 .9% and AUC was 0 .681 . Precancerous lesions mainly showed non-concentricity , increased homogeneity , and increased lesions;other types of benign lesions mainly showed non-centripetal ,high uniformity enhancement and lesion size unchanged . Conclusions Contrast-enhanced ultrasound shows a potential value in the differential diagnosis of precancerous lesions and other types of benign lesions ,that can help clinicians to take early intervention measures for breast precancerous lesions ,but there are still many problems to be solved .
6. Recombinant human interleukin-11 for chemotherapy-induced thrombocytopenia of acute leukemia: a multi-center study

Objective:
To explore the efficacy and safety of recombinant human interleukin-11 (rhIL-11) in treatment of chemotherapy-induced thrombocytopenia of acute leukemia.
Methods:
Acute leukemia patients with chemotherapy-induced thrombocytopenia [Platelets (Plt) < 50×109/L] in 6 centers nationwide from February 2016 to July 2016 were treated with rhIL-11 (2 mg/time, twice per day) by subcutaneous injection. Treatment lasted 7 days or at least until Plt≥ 50×109/L. The Plt recovery was observed during treatment.
Results:
A total of 112 patients were enrolled, and 2 patients decided to drop out of study. The efficacy population consisted of 110 patients, and the total response rate reached 74.5% (82/110). The average variation of Plt during treatment was (70±54)×109/L, and recovery average time of Plt for the patients with favorable efficacy was (8.7±3.0) days. In treatment with severe thrombocytopenia, rhIL-11 alone could shorten the recovery time compared with rhIL-11 combined with Plt transfusion [(8.0±2.6) d vs. (9.6±3.5) d,

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can cause acute respiratory distress syndrome, hypercoagulability, hypertension, and multiorgan dysfunction. Effective antivirals with safe clinical profile are urgently needed to improve the overall prognosis. In an analysis of a randomly collected cohort of 124 patients with COVID-19, we found that hypercoagulability as indicated by elevated concentrations of D-dimers was associated with disease severity. By virtual screening of a U.S. FDA approved drug library, we identified an anticoagulation agent dipyridamole (DIP) , which suppressed SARS-CoV-2 replication . In a proof-of-concept trial involving 31 patients with COVID-19, DIP supplementation was associated with significantly decreased concentrations of D-dimers ( < 0.05), increased lymphocyte and platelet recovery in the circulation, and markedly improved clinical outcomes in comparison to the control patients. In particular, all 8 of the DIP-treated severely ill patients showed remarkable improvement: 7 patients (87.5%) achieved clinical cure and were discharged from the hospitals while the remaining 1 patient (12.5%) was in clinical remission.
