1.The non-surgical treatment of adenomyosis of the uterus

The non-surgical treatment of adenomyosis of the uterus is to relieve dysmenorrhea,to reduce heavy menstrual flow,and to retain or enhance fertility.The main therapeutic methods include medical therapy,uterine artery embolization,and focal ablation therapy (ultrasound-guided percutaneous microwave as well as high intensity focused ultrasound coagulation).For women who are not seeking pregnancy,the levonorgestrel-releasing intrauterine system (LNG-IUS) is the first-line medical therapy for heavy menstrual bleeding,with combined hormonal contraceptives as second choice and gonadotropin-releasing hormone agonist (GnRH-a) serving as preoperative treatment as well as postoperative treatment.For the patients with infertility,prolonged GnRH-a protocol with in vitro fertilization-embryo transfer (IVF-ET) may be the best choice.In this article,non-surgical treatment of adenomyosis of the uterus is discussed through literature review as well as author's clinical experience.
2.Domestic vs imported vecuronium bromide on effects of muscular relaxation

AIM: To compare domestic and imported vecuronium bromide on the effects of muscular relaxation and cardiovascular system. METHODS: Thirty patients undergoing elective surgery were divided into domestic one group and exotic one group. The onset time, T1 recovery to 10% of control, clinical duration, T1 recovery to 75% of control and recovery index of each group were measured by train-of-four stimulation and recorded. The changes of arterial pressure and heart rate of each group were recorded before induction, before intubation, intubation, and 1,2,5 min after intubation. RESULTS: The changes of arterial pressure and heart rate were not obvious diffecance between two groups. The effect of muscular relaxation did not differ after administration either. CONCLUSION: The significant different changes between domestic and imported vecuronium bromide are not noticed. Both on the effects of muscular relaxation and cardiovascular system are similar.
3.Diagnostic Value of Video-EEG in Children's Non-epileptic Seizures

Objective To evaluate the value of Video-EEG for the diagnosis of children's non-epileptic seizures (NES). Methods A retrospective analysis was conducted in 105 cases of children with NES, which were diagnosed by 1 to 4 hours' video-EEG surveillance. Risk factors that interfered with the diagnosis were discussed. Results There were 91 cases (87%) of physiological NES and 14 cases (13%) of psychological NES among the total 105 NES. Misdiagnoses were made in 11 of 43 cases who received common EEG examination before. Conclusion Video-EEG was a reliable method to confirm the clinical diagnosis of non-epileptic seizures and differentiate it from epilepsy. EEG abnormality during seizure-free periods was the main risk factor for the misdiagnosis of NES.
4.Study on the expression of vascular endothelial growth factor in patients with adenomyosis of the uterus

Objective To investigate the role of angiogenesis and expression of vascular endothelial growth factor (VEGF) in the pathophysiology of adenomyosis of the uterus. Methods The study included 32 patients with histologically proven adenomyosis and 30 patients with asymptomatic leiomyoma of the uterus. Immunohistochemical staining was used to detect VEGF expression in different parts of the uterus. A computerized morphometric study on the VEGF expression was performed. Results The VEGF H-Score of glandular cells in the endometrium of adenomyosis uterus [proliferative phase (9.6?1.4), secretory phase (11.7?1.6)] was much higher than that of leiomyoma uterus [proliferative phase (8.3?1.7),secretory phase (10.2?1.5)] (P0.05). The VEGF H-Score of myometrium around the ectopic lesions [proliferative phase (9.5?1.3), secretory phase (8.7?1.3)] was higher than that of normal myometrium [proliferative phase (4.8?1.9), secretory phase (4.5?1.4)] (P
6.Clinical trial on the effectiveness and safety of triptorelin in treatment of uterine leiomyoma

Objective To evaluate the effectiveness and safety of triptorelin in the treatment of uterine leiomyoma. Methods A multi-center, prospective, randomly controlled clinical trial was carried out from Dec. 2002 to Mar. 2004 in three university hospitals. A total of 125 qualified patients with uterine leiomyoma were randomly divided into either triptorelin group (63 cases) treated with 3.75 mg triptorelin injected intramuscularly or leuprorelin group (62 cases) treated with 3.75 mg leuprorelin injected subcutaneously. Both drugs were injected every 28 days for a total of 3 months. Results All 125 patients finished the trial. The uterine volumes were similar before treatment between the triptorelin group and the leuprorelin group and were decreased significantly after drug therapy (P0.05).) The volumes of the largest leiomyoma decreased significantly after drug therapy (P0.05). Patients with serum level of 17?-estradiol 0.05). Dysmenorrhea, noncyclic pelvic pain and pressure-like symptoms were relieved quickly and remarkably in both groups after treatment. The rates of adverse event occurred in 71% of patients in both groups. The main side effects included flare-up effects and hypoestrogenic symptoms. Nine patients in the triptorelin group and 6 in the leuprorelin group received add-back therapy with tibolone 1.25-2.50 mg/d because of remarkable climacteric-like symptoms. Conclusion Treatment of uterine leiomyoma with triptorelin for 3 months is both effective and safe in Chinese women.
7.Influence of previous abdominopelvic surgery on gynecological laparoscopic operation

Objective To investigate the influence of previous abdominopelvic surgery on gynecological laparoscopic operation.Methods A retrospective analysis of 3 283 cases of gynecological diseases by laparoscopic operation patients in Peking University First Hospital from 2007 January to 2012 December,among them,719(21.90%) patients with previous abdominopelvic surgery history (study Group),2 564 (78.10%) patients have no history of abdominopelvic surgery (control group).Study group 719 patients,previous operation times:one time in 525 cases,194 cases were multiple; previous operation:185 cases of gynecological surgery,305 cases of obstetric surgery,108 cases of general surgery,and 121 complex surgery (include at least two kinds of surgery) ;previous operative approach:650 cases laparotomy and 69 cases laparoscopy.Compared two groups of patients with abdominopelvic adhesion and the gynecologic laparoscopic operation situation,analyzed the influence of previous abdominopelvic surgery on abdominopelvic adhesionon and gynecological laparoscopic operation.Results The incidence of abdominopelvic adhesion in the patients with previous abdominopelvic surgery was 51.2% (368/719),which was significantly higher than that of 8.2% (211/2 564) in patients without previous abdominopelvic surgery (P<0.01).But the study group score (median 3) and the degree of abdominopelvic adhesion [mild 49.7% (183/368),moderate 36.1% (133/368),severe 14.1% (52/368)] compared with the control group score (median 2) and degree [mild 55.0%(116/211),moderate 25.6%(54/211),and severe 19.4%(41/211)] were no statistical difference (P=0.930,P=0.684).Super-umbilical primary trocar site were chosen more common in patients with previous abdominopelvic surgery (23.1%,166/719) was significantly higher than that in the control group (3.3%,85/2 564; P<0.01).And the rate of conversion to laparotomy was 0.6% (4/719)significantly more than the control groups(0.l%,2/2 564; P=0.023).Compared with other groups,patients with gynecological or complex surgery or multiple operation history presented more severe abdominopelvic adhesion both in the score and degree (P<0.01).The rate of super-umbilical primary trocar site,hospitalization time,operation time and bleeding during operation in patients with multiple operation history were significantly higher than those with single operation history (P<0.05) ; the rate of blood transfusion,postoperative complication and conversion to laparotomy showed no statistical difference between the two groups (P>0.05).Conclusion The laparoscopic operation could be carried out successfully and safely in patients with a history of various abdominopelvic operations,but the conversion rate increases,for patients with a history of multiple operation because of pelvic adhesion increases the difficulty of the laparoscopic operatio
8.Sequence analysis of HA and NA genes of human infected H9N2 avian influenza virus in Yunnan province, 2019

Abstract: Objective To investigate the molecular characteristics of the H9N2 avian influenza virus (AIV) causing human infection in Yunnan Province in 2019, and to provide the scientific basis for the prevention and control of avian influenza in Yunnan Province. Methods Influenza virus typing was performed by real-time RT-PCR in two influenza-like illness samples, and the Illumina Miseq high-throughput sequencer was used to determine the viral genome sequence. HA and NA gene sequence alignment and phylogenetic tree construction were performed using Mega7.0 software. Results Real-time RT-PCR results showed that two influenza-like illness samples were positive for H9N2 subtype. The full length of HA and NA were obtained by genomic sequencing. Sequence system evolution analysis showed that the HA and NA of the two AIVs in Yunnan Province were in the same evolutionary clade as A/Chicken/Zhejiang/HJ/2007 and belonged to the G57 type. The HA nucleotide and amino acid homology of the two AIVs were 93.92% and 95.00%, respectively, and the NA nucleotide and amino acid homology was 93.31% and 82.03%, respectively. The nucleotide (amino acid) homology of HA was 92.29%-96.94% (93.77%-98.43%) and 92.84%-94.92% (94.18%-96.23%), respectively, and NA nucleotide homology (amino acid) were 91.81%-97.60% (77.82%-94.83%), 94.38%-97.22% (85.47%-94.55%), respectively, compared with that of human infected H9N2 epidemic strains obtained in China from 2015 to 2020. Both AIVs HA protein cleavage site sequences were PSRSSR↓GLF, which was in line with the characteristics of low pathogenic influenza. The analysis of HA protein receptor binding site showed that amino acids at positions 109, 161, 163, 191, 202, 203 and 234 were consistent with the reference strains, while amino acids at position 198 were mutated to T. N166D and 168N mutations were also found in HA protein, and both AIVs had 7 potential glycosylation sites. Analysis of the erythrocyte binding site of NA gene found that there were amino acid mutations at positions 369, 402, 403, and 432, and amino acid deletion at positions 63-65 was found in the NA genes. There were 4 and 5 potential glycosylation sites in the two AIVs, respectively, and no drug resistance site mutations were found. Conclusions The receptor binding sites, erythrocyte binding sites and glycosylation sites of HA and NA genes of H9N2 AIV in Yunnan Province have different degrees of variation, and monitoring and prevention and control should be strengthened.

We report the case of a patient with catecholaminergic polymorphic ventricular tachycardia resulting
from a RYR2 mutation who had not only typical electroencephalogram changes, but also epileptiform
discharges in electroencephalogram. We believe the changes were closely related to the RYR2 mutation.
10.Analysis of the complications of gynecological laparoscopic operation within 10 years

Objective To investigate on the incidence of gynecological laparoscopic operation complications within ten years.Methods From January 2003 to December 2012,clinical data and the complications of 4 897 cases undergoing gynecological laparoscopic operation in First Hospital of Peking University were studied retrospectively.Those surgeries included 876 cases with hysterectomy,662 cases with myoectomy,3 266 cases with adnexa surgery,93 cases of diagnostic laparoscopy operation.Results The complications occurred in 29 cases,the incidence rate was 0.59% (29/4 897).The Incidence rate in Hysterectomy group was 1.83% (16/876),which was significantly higher than 0.60% (4/662) in myoectomy group and 0.28% (9/3 266) in adnexa surgery group.Twenty nine cases of complications were 14 cases with organ injures (48%,14/29),5 cases with hemorrhage complications (17%,5/29),8 cases with infectious complications (28%,8/29),2 cases with incisional hernia(7%,2/29).Conclusion The major complication of gynecological laparoscopic operation complication was organs injuries,which was associated with difficulty and scope of the operation.
