1.Stress reaction of inpatients injured, in the earthquake and its therapeutic interventions

Objective To observe the psychological stress reactions of 238 patients injured in earthquake in our hospital and explore their interventions to such reaction. Methods A total of 238 patients wounded in earthquake were recruited. Along with the active treatment of primary physical trauma, wide range of preventative psychological interventions for crisis was carried out individually or in group. The psychological emergency measures included communication and exchange of ideas, mental health education, and psychological support and a relaxing environment to the patients. All the patients were evaluated by using depression and anxiety assessment scales before and after the intervention. Results Various psychological stress responses were found in 238 patients after injury, in which sleep disorders, memory decline, tension and fear, wabble and emotional instability ranked the top 5. Some patients' psychological status recovered after one-week's hospitalization, while most recovered after two-week's hospitalization. Three patients were transferred to other professional psychiatric hospitals for professional psychological treatment and intervention because of serious psychological obstacle. Conclusion The wounded caused by earthquake manifested various psychological stress reactions. Immediate psychological intervention can effectively help the patients to recover psychologically, promote their physical rehabilitation, and prevent posttraumatic stress disorder.
2.Intraventricular dyssynchrony analysis by tissue synchronization imaging before and after revascularazation

Objective To investigate the global and regional dyssynchrony of left ventricle in patients with coronary artery disease before and after revascularization by tissue synchronization imaging(TSI).Methods Subjects with the left anterior descending coronary artery(LAD)stenosis>75% who underwent revascularization(n=26)and normal controls(n=30)were studied with TSI.Echocardiography was performed one day before revascularization,then repeated one month and three month after the operation,respectively.The sample volumes were located at the mid-myocardium of base and middle segments in apical four-,two- and three-chamber view of left ventricle.The time-to-peak velocity(Tp)of all myocardial segments were examined to access the regional dyssynchrony,and the standard deviations of Tp of 12 segments(TSD)were calculated to evaluate the global dyssynchrony.Results Before revascularization,Tp of segments in the anterior wall and the interventricutar septum in patients were more delayed than those of control group(P<0.01),and the color coding of ischemic segments were red and yellow.After operation,Tp of delayed segments were significant improved,and the color coding turn to yellow or even green,but there was still significant difference of Tp between disease group and control group(P<0.01 or P<0.05).TSD decreased gradually on the preoperatative,early postoperative,and later postoperative echocardiogram,and it on each time point of the disease group were all longer than that of control group.An improvement of TSD was observed after revascularization,especially for the three month examination(P<0.01),but the difference between early and later postoperation was not significant(P>0.05).Conclusions The asynchronous motion of LV is very obvious in CAD patients.After revascularization,both global and regional dyssynchrony were improved gradually.Moreover,TSI is a convenient and non-invasive way to quantitate left ventricular asynchrony with the parameters.
3.Assessment of ventricular synchrony in patients with acute myocardial infarction before and after percutaneous coronary intervention by speckle tracking imaging

Objective To evaluate the changes of ventricular synchrony in patients with acute myocardial infarction(AMI)before and after percutaneous coronary intervention(PCI)by speckle tracking imaging(STI).Methods Thirty patients with AMI were examined by traditional echocardiography and STI before and 5 d,1 month after PCI.Myocardial imaging of left ventricular apical long views and parasternal left ventricular short axis views were recorded and stored.The time to peak longitudinal systolic and diastolic strain rate(TS-SRL,TE-SRL)and to peak radial systolic and diastolic strain rate(TS-SRR,TESRR)were measured respectively.The differences of the maximum and minimum systolic and early diastolic peak time(TSL-diff,TER-diff)and the standard deviation(TSL-SD,TER-SD)in 18 longitudinal segments were calculated.And the differences and standard deviation of TS-SRR and TE-SRR(TSR-diff,TSR-SD,TER-diff,TER-SD)were calculated also.These parameters were compared before and after PCI.Results Compared with before and 5 d after PCI,the values of TSL-diff,TSL-SD,TEL-diff and TEL-SD in 1 month after PCI were decreased significantly(P<0.01).TSR-diff,TSR-SD,TER-diff and TER-SD were decreased also and the differences had statistical significance(P<0.01).The cardiac synchronized index and LVEF values showed good negative correlation.Conclusions The systolic and diastolic synchrony of left ventricle in AMI patients was abnormal.STI can evaluate left ventricular synchronized function and the changes before and after PCI to assess the effect of PCI.
4.δ-opioid receptors protect neurons against neuronal injury induced by oxygen-glucose deprivation

Objective To investigate the effect of cortical 8-opioid receptor (DOR) on oxygen-glucose deprivation-induced (OGD-induced) neuronal injury. Methods Primary cultured cortical neurons incubated with selective DOR agonist (TAN-67) and antagonist (naltrindole) or PKC inhibitor (chelerythrine, CHE) were exposed to OGD. Lactate dehydrogenase (LDH) release was detected after 24 h reperfusion. The expression levels of DOR were measured by Western blot. Results Compared with OGD group, TAN-67 significantly decreased OGD-indueed LDH release, and increased the expression levels of DOR, while nahrindole aggravated neuronal injury and decreased the DOR protein expression. CHE could abolish the LDH down-regulation induced by TAN-67 plus OGD (P< 0.05, compared with TAN-67 treated group). Conclusions DOR activation protects neurons against OGD injury. PKC might take part in the neuroprotection pathways of DOR.
5.Experimental study of myocardial perfusion by myocardial contrast echocardiography with computer-aided technique

Objective To introduce a new computer-aided technique applicable for myocardial contrast echocardiography(MCE)to quantitate automatically calibrated myocardial contrast intensity(CI)and to test the value of color-coded images of calibrated CI in assessing myocardial perfusion.Methods There were two experimental groups of anesthetized rabbits,which underwent 30 min(group I)and 120 min(groupⅡ)coronary occlusion followed by 60 min reperfusion.MCE was performed on all rabbits during occlusion and after reperfusion,and its images were analyzed by a new computer-aided technique.Myocardial calibrated CI of each segment was measured and a color-coded map was produced automatically by software.The risk areas and infarct sizes obtained by myocardial perfusional defect(MPD)and color-coded map were compared with those by fluorescent microsphere and triphenyl-tetrazolium chloride(TTC)staining.Results Compared with non-risk segments,myocardial CI values were significantly decreased in risk segments in group Ⅰ and Ⅱ before calibration (P<0.01),however,myocardial calibrated CI values were significantly decreased in risk segments in group Ⅱ(P<0.01)and not different in group Ⅰ after calibration.Calibrated CI in-70 pix was an optimal cutoff point to identify infarcted segments and to yield the sensitivity of 95% and specificity 87%.The correlation between the risk area by MPD and fluorescent staining was 0.84(P=0.003)whereas color-coded map and staining was 0.91 (P<0.001).The correlation between the infarct size by MPD and TTC was 0.75(P<0.001),and between color-coded image and TTC was 0.89(P<0.001).Conclusions MCE with a new computer-aided technique canassess quantitatively myocardial perfusion and identify automatically risk area and infarct region.
6.The up-regulation of p-p38 MAPK during the induction of brain ischemic tolerance induced by intermittent hypobaric hypoxia preconditioning in rats.

OBJECTIVETo explore the expression of p-p38 MAPK protein and the number of astrocytes expressing p-p38 MAPK in CA1 hippocampus in rats during the induction of brain ischemic tolerance induced by intermittent hypobaric hypoxia (IH) preconditioning.
METHODSThirty healthy adult male Wistar rats were randomly divided into 6 groups (n = 5 in each group): sham 0 min group, IH + sham 0 min group, sham 7 d group, IH + sham 7 d group, Ischemia (Is) 7 d group, and IH + Is 7 d group. Neuropathological evaluation was performed by thionine staining in CA1 hippocampus in rats. The expression of p-p38 MAPK in CA1 hippocampus was observed by immunohistochemical staining. And the number of astrocytes expressing p-p38 MAPK was observed by immunofluorescent double labeling.
RESULTSThe results showed that IH preconditioning induced brain ischemic tolerance successfully. At the same time, IH preconditioning obviously up-regulated the expression of p-p38 MAPK protein in CA1 hippocampus, and also increased the number of astrocytes expressing p-p38 MAPK.
CONCLUSIONIt might be concluded that IH preconditioning induced brain ischemic tolerance by up-regulating the expression of p-p38 MAPK protein in pyramidal neurones and astrocytes.

7.Effect of Compound Qingre Granule on the Expression of Peripheral Blood Lymphocyte Subsets of Acute Virus Infection Patients.

OBJECTIVETo observe interventional effects of anti-viral therapy and Compound Qin-gre Granule (CQG) on host cellular immune functions of acute virus infection patients.
METHODSThirty acute virus infection patients were recruited to detect peripheral lymphocyte subsets. They were randomly assigned to two groups, the Western medicine treatment group (treated with anti-virus Western medicine) and the integrative medicine treatment group (treated with anti-virus Western medicine plus CQG). T-cell subsets were re-examined 7 days later. Changes between before and after treatment were observed. Effect on host cellular immune functions and efficacy were compared between the Western medicine treatment and the integrative medicine treatment.
RESULTSCompared with the normal control group, the percentage of peripheral T cells increased, and the percentage of B/NK cells decreased in acute virus infection patients (P < 0.01). Meanwhile, in T cell subsets, the percentage of CD8+ T cells and CD8+ CD38+ T cells increased (P < 0.05, P < 0.01); and percentages of CD4+ T cells, CD4+ CD28 + T cells, and CD8+ CD28+ T cells decreased (P < 0.05, P < 0.01). After one-week treatment, percentages of CD4+ T cells, CD4+ CD28+ T cells, and CD8+ CD28+ T cells increased (P < 0.05, P < 0.01), while the percentage of CD8+ CD38+ T cells decreased (P < 0.01). More significantly, these changes were greater in the integrative medicine treatment group than in the Western medicine treatment group (P < 0.05).
CONCLUSIONSDisarranged cellular immune functions existed in acute virus infection patients. CQG could significantly improve viral infection induced immunologic derangement and immunologic injury.

8.Relationship Between Body Weight Changes From Early Adulthood to Middle Age and the Prevalence of Hypertriglyceridemia by Large-scale Cardiovascular Risk Factor Investigation

Objective: To explore the relationship between body weight changes from early adulthood to middle age and the prevalence of hypertriglyceridemia by large-scale cardiovascular risk factor investigation.
Methods: A total of 15 population groups from China multi-center collaborative study of cardiovascular epidemiology in 1998 were enrolled. There were approximately 1000 participants in each group including 50% of each male and female at the age of (35-59) years which was deifned as middle age. The participants were surveyed for cardiovascular risk factors including the body weight and body mass index (BMI) at the age of 25 years which was deifned as early adulthood. The participants were divided into 4 groups based on BMI at early adulthood: Low body weight group, BMI < 18.5 kg/m2, Normal body weight group, BMI (18.5-23.9) kg/m2, Overweight group, BMI (24-27.9) kg/m2 and Obese group, BMI ≥ 28 kg/m2. The changes of body weight from early adulthood to middle age were calculated by body weight at present minus body weight at 25 years; according to the differences, participants were divided into another set of 6 groups: differences < -7.5 kg, (-7.5 to -2.6) kg, (-2.5 to 2.5) kg, (2.6 to7.5) kg, (7.6 to 12.5) kg and > 12.5 kg. The relationship between body weight status at 25 years of age with subsequent changes and the prevalence of hypertriglyceridemia were investigated.
Results: A total of 13883 participants finished the investigation.①The prevalence of hypertriglyceridemia by 4 BMI groups were at 22.8%, 26.0%, 27.4% and 30.8% respectively (the trend ofP<0.01).②The prevalence of hypertriglyceridemia by 6 age differences groups were at 12.5 %, 14.0 %, 17.8 %, 24.2 %, 31.5 % and 40.9 % respectively (the trend ofP<0.01). Multivariate Logistic regression analysis indicated that overweight and obesity at the age of 25 years with subsequent weight gain were positively related to the risk of hypertriglyceridemia at the middle age (the trend ofP<0.01).
Conclusion: Overweight and obesity in early adulthood with subsequent weight gain were independently related to the risk of hypertriglyceridemia at the middle age in our survey.

The thermostability of the inulinase was studied in this resea rc h. Some alcoholic materials and thickening agent could enhance the thermostabli lity of the inulinase. Using glycerol、xanthic pastern and though orthogonal ex periments of three elements and three levels, a satisfying protective agent, whi ch included glycerin(6%), xanthan gum(0.6%) and CaCl_2 (100mmol/mL) and ha d a significant effect on the enhancement of the inulinase thermostability, was acquired.
10.Niacin accelerates LDL-C uptake in HepG2 cells via downregulation of PCSK9

Aim To explore the effects of niacin on LDL-C uptake and metabolism in HepG2 cells,and to clarify the functions of niacin in lipid-lowering and slo-wing the atherosclerosis process,thus to provide a sci-entific basis for niacin as a lipid-lowering drug in clini-cal development.Methods Oil red O staining was used to observe HepG2 cells after lipid uptake.Enzy-matic method was used to determine the content of in-tracellular free cholesterol (FC)and total cholesterol (TC).The LDLR levels on the surface of cell mem-brane were detected by immunofluorescence flow cy-tometer.The mRNA and protein expressions of LDLR, SREBP2 and PCSK9 were analyzed by qPCR and Western blot.Results The results of oil red O staining showed that the rate of oil red O-positive cells and the number of red lipid droplets were significantly in-creased in niacin group than control group.Niacin sig-nificantly increased the levels of TC and FC in HepG2 cells(P <0.05 ).What’s more,niacin significantly upregulated the expression of LDLR and significantly downregulated the protein expression of PCSK9,while it had no effect on the expression of SREBP2.Conclu-sion Niacin accelerates LDL-C uptake probably via downregulating the expression of PCSK9 and reducing the degradation of LDLR protein in HepG2 cells.
