1.Assessing tumor treatment response and prognosis in non-small cell lung cancer with perfusion CT

ObjectiveTo prospectively investigate whether any of the perfusion parameters would predict early tumor response to chemotherapy and/or radiotherapy and prognosis in non-small cell lung cancer (NSCLC). MethodsIn a prospective series, Perfusion CT were performed in 152 patients suspected lung cancer with 16-slice or 8-slice multislice CT. Contrast medium (50 ml) was injected at a rate of 4 ml/s with a power injector.The scanning delay was 10 seconds and the scanning time was 50 seconds.Among 152 patients,123 patients were proved lung cancer by pathology.With the perfusion 3.0 software, the parameters including blood flow (BF), blood volume (BV), mean transit time (MTT) and capillary permeability surface area product (PS) were calculated. The perfusion image quality was evaluated on a 4-1eveal scale. The treatment response after chemotherapy and(or) radiotherapy was assessed with Response Evaluation Criteria in Solid Tumors (RECIST) ,and then the relationship between perfusion parameters with early tumor response to chemotherapy and(or) radiotherapy was evaluated. Student t test and Kaplan-Meier estimates were used for data analysis. ResultsIn 84 patients (68.3%), the perfusion image quality was staged level 2 (moderate) and level 3 (good). Among them, 35 patients with NSCLC were assessed with RECIST after chemotherapy and (or) radiotherapy.In these 35 patients, The BF of responders and nonresponders was (81.0±33.6)and (56.3±23.1) ml·min~(-1)·100 g~(-1),respectively, which was significantly different(t=2.393, P=0.023). The median PFS of low-BF group (BF≤80 ml·min~(-1)· 100 g~(-1)) and high-BF group (BF>80 ml·min~(-1)·100 g~(-1)) was 11.8 and 8.0 months respectively (P>0.05), and the median PFS of low-BV group (BF≤6 ml/100 g~(-1)) and high-BV group (BF>6 ml/100 g~(-1)) was 9.2 and 8.0 months respectively(P>0.05), both of them were not significantly different.ConclusionNSCLC in high perfusion are relatively sensitive to chemotherapy and/or radiotherapy, and the response rate is relatively higher, but the progress time is relatively shorter.

Survivin may play adjuvant roles and Bcl-2 may play main roles in the inhibition of cell apoptosis in SCLC while the case is just reversed in NSCLC,it suggests that there were different apoptosis inducing factor and different apoptosis pathway due to different cell type in NSCLC and SCLC.
3.The effect of patient participation in humanistic nursing on quality of life and psychology of patients with gastric cancer

Objective To explore the effect of participatory humanistic nursing on the quality of life and psychology of patients with gastric cancer. Methods From May 2015 to May 2016, 68 patients with gastric cancer were enrolled in the study. All patients were divided into the experimental group and the control group with 34 patients in each group according to the principle of double-blind randomization. Patients in the control group were given routine clinical nursing, and patients in the experimental group were given the participatory humanistic nursing care. The quality of life and psychological state were evaluated by European Organization for Research and Treatment of Cancer Quality of Life Questionnaire and Symptom Checklist 90 edited by Derogatis and compared before nursing and after 6 months of nursing. Results There was no significant difference in the quality of life before nursing between two groups (P >0.05). After nursing, the scores of physical function, role function, emotional function, cognitive function, social function of the quality of life were 57.85±4.64, 68.43±5.23, 61.43±4.97, 58.47± 6.12, 53.57±5.54 in the experimental group and 43.56±4.56, 54.67±4.67, 56.34±4.74, 51.34±4.13, 50.46± 4.27 in the control group,and there were significant differences (t=2.593-12.808,P<0.01 or 0.05). There was no significant difference in the psychological state before nursing between two groups (P>0.05). After nursing, the scores of interpersonal sensitivity, depression, anxiety, hostility, terror, paranoia, psychosis of the psychological state were 1.65 ± 0.17, 1.40 ± 0.15, 1.45 ± 0.14, 1.46 ± 0.12, 1.52 ± 0.16, 1.54 ± 0.10, 1.43 ± 0.11 in the experimental group and 1.75±0.21, 1.72±0.24, 1.63±0.21, 1.75±0.23, 1.71±0.24, 1.65±0.10, 1.56 ± 0.12 in the control group, and there were significant differences (t=2.158-6.593, P<0.05 or 0.01). Conclusions Participating humanistic care can improve the quality of life and improve the psychological state, and the clinical effect is significant, it is worthy of being popularized and applied.
4.Study on Processing Method of Codonopsis Pilozula Prepared Product by Factorial Design and Response Surface Methodology

Objective To optimize the processing method of Codonopsis pilozula prepared product by the factorial design and response surface methodology. Method In the two different optimizing processing, methods were optimized respectively on basis of preplanned experiment design and the percentage of extracts were also determined respectively. Drying temperature and time, bran parching temperature and time respectively as independent variable, their OD value as dependent variable, multiple regression equation was obtained and binomial equation was simulated as well. Results The r2 of Codonopsis pilozula dried product and Codonopsis pilozula parched product were respectively 0.976 4 and 0.987 9, meanwhile their optimized processing methods were drying temperature 80 ℃, drying time 2 hours and parching temperature 250 ℃, parching time 1 minute. Conclusion The effect of independent variable to dependent variable can be analyzed on three-dimensional chart with the factorial design and response surface methodology, consequently it will help to optimize processing method of prepared product.
5.Evaluation of emergency transcatheter arterial embolization in intractable postpartum hemorrhage

Objective To assess the efficacy and safety of emergency transcatheter arterial embolization in the management of intractable postpartum hemorrhage. Methods Twenty-five patients with intractable postpartum hemorrhage were undertaken superselective catheterization into the bilateral internal iliac arteries or uterial arteries to find the causes and sites of bleeding through DSA and then followed by arterial embolization with gelfoam particles. Purpose:To study the effectiveness of inserting central venous catheter was inserted into pericardial cavity for the drainage and treatment of lung cancer hydropericardium. Methods:A central venous catheter into pericardial cavity by pericardial puncture and continuous drainage, then chemotherapeutic agents were administered. Results:Among twelve patients, complete response was seen in 4 cases and partial response in 7, no cardiostenosis and sinus appeared. Conclusions:This method is convenient, effective, safe for the treatment of lung cancer hydropericardium and without apparent complications.

Objective To identify the efficacy small interfering RNA against Pseudomonas aeruginosa expressing MexA-MexB-OprM efflux pumps.Methods Four siRNA ( siRNA1,siRNA2,siRNA3 and siRNA4) against mexB gene were designed and prepared by electricity transference in vitro.MICs of antibiotic combined with efflux pump inhibitors against multiple resistant strain PAO1 and PAO3 were determined by E-test method.The mRNA expression levels of efflux pump gene (mexB) were quantified by real time fluorescent quantitative PCR.Results siRNA expression vectors were constructed success by enzyme cut method.48 after PAO1 and multiple drug resistant PAO3 transfected with siRNA4,the sensibilities to antibiotic were enhanced.48 after PAO1 and multiple drug resistant PAO3 transfected with siRNAl,siRNA2 and siRNA3,the sensibilities to antibiotic didn't change obviously.48 after PAO1 and multiple drug resistant PAO3 ttransfected with siRNA4,the expression level of mexB was decreased obviously (P < 0.05 ).48 after PAO1 and multiple drug resistant PAO3 transfected with siRNA1,siRNA2 and siRNA3,the expression level of mexB didn't change obviously.Conclusion siRNA against Pseudomonas aeruginosa expressing MexA-MexB-OprM efflux pumps enhanced the sensibility to antibiotic and inhibited the expression of mexB gene.Our results demonstrate the using RNAi may be potential targets for novel therapies directed against treatment of antibiotic-resistant infections.
