1.Effect of ischemic stroke secondary prevention knowledge training on community doctors in Haidian

Objective We determined the proportion of doctors in Haidian community participating in the training of secondary prevention of ischemic cerebrovascular diseases related knowledge and the knowledge level of doctors so as to make a new targeted training plan,and assessed the effect of the training.Methods We enrolled 80 doctors,who filled out the questionnaire on secondary prevention of ischemic cerebrovascular diseases related knowledge,from 22 Haidian community center from March to July in 2014 in Beijing.The primary outcome was the proportion of doctors with the training of secondary prevention of ischemic cerebrovascular diseases related knowledge and the improvement rate of new training.Results Seventy (87.5%) community doctors didn't accept ischemic stroke secondary prevention system training.The understanding of Chinese Ischemic Stroke Subclassification and three cornerstones of secondary prevention of ischemic stroke(anti-hypertention,anti-lipidemia,antithrombotic) after the training was significant improvement (20% vs.82.5%,P < 0.05;31.25%vs.90%,P < 0.05),while according to different causes classification subtype the doctors could make corresponding strategies of secondary prevention of ischemic cerebrovascular diseases.The rates of all community risk factors identification,community health education,lifestyle intervention,antihypertension,aspirin use,statin use,hypoglycemic treatment were both above 60% no matter before the training or after the trainig.Conclusion Our results suggest that the community doctors in Haidian District lack corresponding knowledge of ischemic stroke secondary prevention.Further training of ischemic stroke secondary prevention is needed.
2.Analysis phylogenetic relationship of Gynostemma (Cucurbitaceae).

The sequences of ITS, matK, rbcL and psbA-trnH of 9 Gynostemma species or variety including 38 samples were compared and analyzed by molecular phylogeny method. Hemsleya macrosperma was designated as outgroup. The MP and NJ phylogenetic tree of Gynostemma was built based on ITS sequence, the results of PAUP phylogenetic analysis showed the following results: (1) The eight individuals of G. pentaphyllum var. pentaphyllum were not supported as monophyletic in the strict consensus trees and NJ trees. (2) It is suspected whether G. longipes and G. laxum should be classified as the independent species. (3)The classification of subgenus units of Gynostemma plants is supported.

3.Change of Endothelial Progenitor Cells in Hemodialysis Patients and its Relation with Framingham Risk Score

Objective To investigate the number and activity changes of endothelial progenitor cells(EPCs) in hemodialysis patients,and explore its correlation with the risk factors of coronary heart disease.Methords Total mononuclear ceils(MNCs) were isolated from peripheral blood of patients with chronic renal failure in long-term hemodialysis and from a normal control group by Ficoll density gradient centrifugation and then were plated on humanfibronectin-coated dishes.Mter 7 days of culture,EPCs were characterized as adherent cells by double staining with FITC-UEA-I and DI-LDL,and were further identified by the expression of CD34,CD133 and KDR with flow cytometry.EPCs migration was determined with modified Boyden chamber assay.EPCs adhesive assay was performed by replanting EPCs on humanfibronectin-coated plates and then counting the adherent cells.The relationship of the EPCs' number and activity with Framingham Risk Score of ten years was also be assessed.Results Number of EPCs and the migratory & adhesive capacity were significantly lower in patients than in the control(P
4.Transplantation of mesencephalon progenitor cells in treatment of Parkinsonian rats

Objective:To explore the feasibility of mesencephalon progenitor cells as dopamine neuron's graft in treatment of Parkinson disease(PD) in rats. Methods: Animal model of PD were established in SD rats by 6 hydroxydopamine. Mesencephalon progenitor cells were obtained from 13 day old fetal rats. After in vitro proliferation and differentiation, the cells were planted into PD rat striatum. Results: Progenitor cells were expanded in an undifferentiated state with endothelia growth factor and basic fibroblast growth factor as mitogens. IL 1 and striatal culture media further increased the number of TH immunoreactive cells. Transplantation of these neurons relieved the rotation symptom of PD rats. Conclusion: Transplantation of mesencephalon progenitor cells has certain effects on PD rats.
5.A Rapid Method by Type-specific Multiplex PCR for Clostridium perfringens on Surface of Traumatic Tissues

OBJECTIVE To establish a rapid detecting method for Clostridium perfringens on traumatic tissues by type-specific multiplex PCR.METHODS Established a simple and rapid method(TLS method) for purifying the DNA of genomes and plasmids in standard strains of C.perfringens wild strains on surface of open traumatic tissues and detected DNAs by type-specific multiplex PCR.RESULTS All types of C.perfringens could be detected by type-specific multiplex PCR.The sensitivity by PCR for type A of C.perfringens arrived at 7.5?103/ml and a whole run could finish within 5 h;the results by PCR entirely corresponded with those by cultureing.CONCLUSIONS New methods for purifying DNA of genomes and plasmids of C.perfringens are simple and rapid;there are high specificity and sensitivity for detecting DNA by multiplex PCR within short time,which can be practiced in clinical laboratory.
6.The analysis of measurement results of adult myopia biological optical parameters

Objective To investigate the correlations between the ocular biometric optical parame-ters and refractive dioptres in adults, assess the influence of the biometric optical parameters on the develop-ment of myopia, and provide a reference of prevention and treatment for myopia.Methods A total of 188 adults (376 eyes) aged 18 to 40 years old was enrolled in this study.Dioptres were measured with phorop-ter, A-mode ultrasound was used to measure the vitreous length and lens thickness, the axis length, anterior corneal surface semidiameter anterior chamber depth were measured with intraocular len-master ( IOL-mas-ter) .SPSS 17.0 software was used to analyze the date.Results ⑴Of the 376 eyes, the results showed a statistically significant difference between myopia and emetropia in the axis length, vitreous depth, anterior corneal surface semidiameter, anterior chamber depth and lens thickness( P <0.01).⑵The partial corre-lation analysis indicated a significant positive correlation between the refraction dioptres and axial length, vitreous length ( r =-0.90, -0.88, respectively, P <0.01);and significant strong correlation was also found between dioptres and anterior corneal surface horizontal and vertical semidiameter ( r =0.81, 0.84, respectively, P <0.01);But correlations between dioptres and lens thickness and anterior chamber depth were very weak ( r =-0.11, 0.12, respectively, P <0.01);the correlation between axial length and vit-reous depth was very high ( r =0.95, P <0.01).and the data indicated that the anterior corneal surface horizontal and vertical semidiameter were high positive correlation( r =0.91, P <0.01);the same high positive correlation between the axial length and the anterior corneal surface semidiameter ( horizontal/verti-cal) was also found ( r =0.78,0.81, P <0.01).Conclusions Two chief factors (corneal surface semi-diameter and ocular axial length) influencing the level of dioptres were high positive correlation, seeking the mutual coordination factors which promote corneal curvature change and axial growth and finding out the feedback mechanism of promoting the cornea sphericity might be conducive to further explore how to control the occurrence and development of myopia.
7.Relationship between abnormal circadian rhythm of blood pressure and early progress of minor stroke in the elderly

Objective To investigate the relationship between early progression in elderly patients with minor stroke and abnormal circadian rhythm of blood pressure.Methods From June 2013 to December 2016,180 patients with mild stroke (age >65 years,the National Institutes of Health Stroke Scale Score ≤3) at the Department of Neurology,Beijing Zhongguancun Hospital were recruited retrospectively.They were divided into either a progress group (n=48;the National Institute of Health Stroke Scale score increased≥2) or a non-progress group (n=132) according to whether they developed an early neurological deterioration (within 5 days after onset).The baseline data,risk factors for cerebrovascular disease (hypertension,diabetes mellitus,stroke,chronic smoking,alcohol consumption,hyperlipidemia,and homocysteine) in patients of both groups were compared.The normal (dipper-type blood pressure)/abnormal circadian rhythm of blood pressure (non-dipper-type blood pressure,super dipper-type blood pressure,and inverse dipper-type blood pressure) were monitored by ambulatory blood pressure monitoring.The guilty arteries were divided into severe stenosis or occlusion,non-severe stenosis or without stenosis.Multivariate logistic regression analysis and multi-class dummy variable analysis were further performed.Results The proportions of past diabetes history,stroke history,abnormal circadian rhythm of blood pressure,severe stenosis and occlusion of guilty artery in the patients of the progress group were higher than those of the non-progress group (70.8% [n=34] vs.49.2% [n=65],64.6% [n=31] vs.47.7% [n=63],89.6% [n=43] vs.26.5% [n=35],and 77.1% [n=37] vs.39.4% [n=52] respectively).The differences were statistically significant (all P<0.05).The factors of having statistical differences were substituted into multivariate logistic regression analysis,which showed that abnormal circadian rhythm of blood pressure (OR,7.072,95%CI 3.004-16.431;P<0.01) and severe stenosis or occlusion of guilty arteries (OR,6.217,95%CI 2.034-14.212,P=0.003) were the independent risk factors for early progression of minor stroke in the elderly.The dummy variable analysis of abnormal circadian rhythm of blood pressure showed that the super dipper-type blood pressure (OR,13.429,95%CI 4.175-111.668;P<0.01) in abnormal circadian rhythm of blood pressure was an independent risk factor for early progression of minor stroke in the elderly.Conclusion The abnormal circadian rhythm of blood pressure,especially super dipper-type blood pressure,may be the independent risk factor for early progression of minor stroke in the elderly,which needs to pay close attention to early intervention.
8.Nitric oxide and prostaglandin E2 secretion in osteocytes induced by intermittent cyclic compressive force.

This paper is aimed to investigate the effect of rest-inserted loading on the mechanosensitivity of osteocytes. In the investigation, cultured MLO-Y4 osteocyte-like cells were strained on cyclic compressive force (CCF) by the self-made compressive loading device. Then we observed the effect of different rest periods-inserted loading (5 s, 15 s, 30 s, respectively) on the mechanosensitivity of osteocytes. We then determined the levels of secreted nitric oxide (NO) and prostaglandin E2 (PGE2) by Griess method and enzyme linked immunosorbent assay (ELISA), respectively. We then stained the cytoskeleton F-actin using immunofluorescence. We found that the expressions of NO and PGE2 in rest-inserted strained groups (> 15 s) were significantly increased compared to those in the continuous strained group. And rest-inserted loading promoted the parallel alignment of stress fibers. It indicates that rest-inserted loading could promote the mechanosensitivity of osteocytes, and this might be related to the parallel alignment of stress fibers.

9.Preparation and evaluation of intra-articular injectable sinomenine hydrochloride-loaded in situ liquid crystals.

Phytantriol (PT), ethanol (ET) and water were used to prepare in situ cubic liquid crystal (ISV2). The pseudo-ternary phase diagram of PT-ET-water was constructed and isotropic solution formulations were chosen for further optimization. The physicochemical properties of isotropic solution formulations were evaluated to optimize the composition of ISV2. In situ hexagonal liquid crystals (ISH2) were prepared based on the composition of ISV2 with the addition of vitamin E acetate (VitEA) and the amount of VitEA was optimized by in vitro release behavior. The phase structures of liquid crystalline gels formed by ISV2 and ISH2 in excess water were confirmed by crossed polarized light microscopy and small angle X-ray scattering, respectively. Rheological properties of ISV2 and ISH2 were studied by a DHR-2 rheometer. In vitro drug release studies were conducted by using a dialysis membrane diffusion method. Pharmacokinetics was investigated by determination of sinomenine hydrochloride (SMH) concentration in synovial membrane after intra-articular injection of SMH-loaded ISH2 in adjuvant-induced arthritis rats. The optimal ISV2 (PT/ET/water, 64 : 16 : 20, w/w/w) loaded with 6 mg x g(-1) of SMH showed a suitable pH, injectable and formed a cubic liquid crystalline gel in situ with minimum water absorption in the shortest time. The optimal ISV2 was able to sustain the drug release for 144 h. The optimal ISH2 system was prepared by addition of 5% VitEA into PT in the optimal ISV2 system. This ISH2 (PT/VitEA/ET/water, 60.8 : 3.2 : 16 : 20, w/w/w/w) was an injectable isotropic solution with suitable pH. The new ISH2 was able to sustain the drug release for more than 240 h. Local pharmacokinetics study indicated that the retention time and AUC(0-∞) of ISH2 group were increased significantly compared with that of SMH solution group and the AUC(0-∞) of ISH2 group was 6.01 times higher than that of SMH solution group. The developed ISH2 was suitable for intra-articular injection that may apply to patients in the treatment of rheumatoid arthritis.
