Silent brain infarction (SBI) is mainly diagnosed by brain imaging which is more common in the elderly population.It is considered as the early clinical stage of symptomatic cerebral infarction and cognitive impairment.Its risk factors include age,history of smoking,hypertension,diabetes,metabolic syndrome,carotid atherosclerosis,chronic kidney disease,atrial fibrillation,and obstructive sleep apnea.The related serum biological markers include high-sensitivity C reactive protein,homocysteine,cholesterol,uric acid,and soluble intercellular adhesion molecule-1.This article reviews the advances in research on the imaging characteristics,risk factors,serum biological markers and clinical significance of SBI.
2.The relationship among the D-dimer,C-reactive protein and left tatrial diameter in the patients with atrial fibrillation

Objective To investigate the relationship among the D-dimer,C-reactive protein and left atrial diameter in the patients with atrial fibrillation(AF). Methods 53 AF patients were selected,the control group consisted of 32 cases of sinus rhythm(SR). The levels of D-dimer, CRP and LAD in AF group were compared with each other, and with those in the control group. Correlation analysis between D-dimer, CRP and LAD were made.Results D-dimer,CUP and LAD in AF group were significantly different from those in SR group. D-dimer and CRP in persistent AF and permanent AF differed significantly from that in SR and paraxymal AF. LAD in the group of persistent AF differed from that in SR group. LAD in permanent AF was significantly different from that in paraxymal AF and SR. Positive linear correlations were found between D-dimer, CRP and LAD in AF (r=0.376; 0.33).Conclusion Hypercoagulable state existsed in AF; the elevation of CRP should be the result and related to persistence time of AF. Positive correlation existsed between LAD and hypercoagulable state and inflammatory reaction.
3.Comparative study on the characteristics of neuroendocrine dysfunction in children and adolescents with craniopharyngioma

Objective To compare the impact of the mass effects in situ ( MEIS ) of the sella tumor on neuroendocrine function in children and adolescents with craniopharyngioma before surgery. Methods A total of 227 inpatients with craniopharyngioma in Beijing Tiantan Hospital, Capital Medical University, from October 2009 to October 2014, were retrospectively analyzed. These patients were divided into children group ( n = 167 ) and adolescent group(n=60) according to the age at the time of the first diagnosis. The clinical characteristics and damage degrees of neuroendocrine function by MEIS of sella tumor were analyzed and compared between these two groups before surgery. Result (1) Clinical characteristics of neuroendocrine function:Among hypothalamic dysfunctional manifestations, central diabetes insipidus showed the highest percentage(children group 35. 93%vs adolescent group 31. 67%), followed by the abnormal appetite and obesity ( children group 19. 76% vs adolescent group 28. 33%). The incidences of abnormal body temperature regulation, sleeping disorder, personality abnormality, and cognitive abnormality all were less than 5%. There were no statistical significant differences in the aforementioned hypothalamic dysfunction parameters between two groups (P=0. 524). Among pituitary-target glands dysfunction parameters, growth hormonce ( GH )-insulin-insulin like growth factor Ⅰ( IGF-Ⅰ) axis dysfunction showed the highest percentage ( children group 64. 07% vs adolescent group 50. 0%), followed by pituitary-gonad axis dysfunction in adolescent group (53. 33%), hyperprolactinemia ( children group 31. 14% vs adolescent group 43. 33%), pituitary-thyroid dysfunction(children group 22. 16%vs adolescent group 28. 33%), pituitary-adrenal gland dysfunction(children group 20. 36%vs adolescent group 25%). There were no statistical significant differences intheseabnormalpituitary-targetglandaxes(exceptpituitary-gonadaxis)betweentwogroups(P=0.475). (2) Comparison of damage degrees of neuroendocrine dysfunction: The patients with normal neuroendocrine function accounted for 10. 2%in children group and 8. 3%in adolescent group. The patients with 1 to 4 items of neuroendocrine dysfunction accounted for 75. 6%in children group and 73. 3%in adolescent group. The patients with more than 5 items of neuroendocrine dysfunction accounted for 14. 4%in children group and 18. 4%in adolescent group. There were no significant differences between two groups(Z=-1. 63,P=0. 103). Conclusions There were no significant differences in characteristics and damage degrees of MEIS of the sella tumor on neuroendocrine dysfunction between children and adolescents with craniopharyngioma. It suggests that systematical evaluation on hypothalamus-pituitary-targets axis function is very important for reducing the risks of further neuroendocrine dysfunction in young patients with craniopharyngioma after surgery.
4.Analysis of different antibacterial effects in patients with chronic pelvic inflammatory disease infected by enema and the influence of it on endovaginal microecologics

Objective To analyze the effect of antimicrobial enema liquid to explore different bacterial infection patients with chronic pelvic inflammatory disease by clearing away heat and promoting diuresis, detumescence,promoting blood circulation and relieving pain, and to investigate the influence of antimicrobial enema liquid applicated in rectal on endovaginal microecologics.Methods Seventy-one cases of chronic pelvic inflammatorydisease patients were selected as the research objects in Obstetrics and Gynecology Hospital Affiliated to Fudan University from March 2011 to January 2012.The bacterial culture of cervical secretions in patients with chronic pelvic inflammatory disease(excluding the vulva and vaginitis) was swab by cotton swab, then bacterial culture and bacterial vaginal infection Wulian analysis were conducted.The minimal inhibitory concentration(MIC) and minimum bactericidal concentration(MBC) of the common pathogenic bacteria were determined by using the flat dish two times dilution method according to the different kinds of pathogenic bacteria.Wulian analysis bacterial vaginal infection was reviewed after patients received the same kind of enema rectal administration.Results Main pathogenic bacteria in 71 cases of patients with chronic pelvic inflammatory disease were enterococcus faecalis of 12.5% (4/71), beta hemolytic streptococcus of 31.25% (10/17), escherichia coli of 34.38% (11/71), Staphylococcus aureus of 28.13% (9/71), mlicrococcus scarlatinae of 6.25% (2/71), bacillus typhosus of 6.25% (2/71), Shigella of 3.13% (1/71), staphylococcus epidermidis of 3.13% (1/71), klebsiella pneumoniae of 3.13% (1/71) and pseudomonas aeruginosa of 3.13% (1/71) .Chronic pelvic inflammatory disease enema on enterococcus faecalis, beta hemolytic streptococcus, escherichia coli, staphylococcus aureus and antibacterial activity was obvious, the minimum inhibitory concentration (MIC) were the original drug concentration of 1/32,1/32,1/16, 1/16, the minimum bactericidal concentration(MBC) respectively as raw material concentrations of 1/16, 1/8, 1/8, 1/8;has certain antibacterial activity against streptococcus pyogenes, typhoid salmonella, shigella flexneri, the MIC were the original drug concentrations of 1/8,1/8,1/4,the MBC were the original drug concentrations of 1/8,1/4,1/4.In vitro antibacterial activity on pseudomonas aeruginosa, coagulase negative staphylococci was not obvious.There were statistically significant differences in PH activity abnormality rate (67.61% vs 36.62%), positive rate of catalase (66.20% vs 29.58%), leukocyte esterase (56.33% vs 29.58%), neuraminidase (53.52% vs 25.35%), proline aminopeptidase (57.75% vs 32.39%) and glucosaminidase(52.11% vs 22.55%) after enema liquid applicated in rectal, and the difference was significant(P<0.5).Conclusion The use of clearing away heat and promoting diuresis, detumescence, Huoxue Zhitong enema on chronic pelvic inflammatory disease pathogenic bacteria have certain inhibition, especially enterococcus faecalis infection.The drug has efficient on chronic pelvic inflammatory disease and endovaginal microecologics.
6.Expression of NF-κB and dendritic cells in renal tissue of lupus nephritis

Objective To explore expression of nuclear factor kappa B (NF-κB) and change of peripheral blood dendritic cells(DCs) surface. Methods The expression of NF-κB and DCs in kidney of 40 LN cases were measured,by ALP-anti-ALP mice immune complex law(APAAP) and density gradient separation centrifugal meth-ods. Results kidney tissue of LN patients, NF-κB in the glomeruli and tubules showed a high level of expression ; CD80+ ,CD86+, HLA-DR+ expression decreasedtacrolimus group than the control group Division before and after the shape of the DCs had no effect on tacrolimus after the DCs could inhibit the proliferation of T cells and T cells to Th2 cell transformation. Conclusinn In vitro Tacrolimus inhibits the DCs mature in LN patients, NF-κB in LN kidney LN local expression and the occurrence and development of related.
7.Selection of targeted glioblastoma tumor cell-binding and internalizing peptides through phage display vector

AIM: To isolate peptides targeted binding and internalizing into glioblastoma cell line SWO-38. METHODS: Tumor cells were screened five rounds of whole cell screen through the Ph.D.-12 phage display library. The monoclone specific binding efficiency to the tumor cell was analyzed, and the DNA of phages were extracted, sequenced and translated to the sequences of amino acid. RESULTS: In the phage library after five rounds of screen , 10 of 13 monoclones had highly selective binding to SWO-38 cells. We found two repeated peptide sequences. CONCLUSION: Whole cell screening against tumor cells through random phage peptide library can obtain phage peptides with highly specific binding and internalizing ability. The peptides could be used as a therapy vector for tumor targeted delivery.
8.Isolation and Identification of a Lipopeptide

A lipopeptide compound was isolated from the culture of Bacillus subtilis HSO121 by methods of acidic precipitation, solvent extract, fractional precipitation, adsorption and prepared thin-layer chromatography; and its molecular structure was determined by by ninhydrin assay and IR methods following the Amino analysis, MS-MS and ESI-MS. It shows that the isolated lipopeptide consists of two homologues with molecular mass 1,022D and 1,036D and bearing a cyclic structure with the amino acid sequence Leu-Leu-Asp-Val-Leu-Leu-Glu in the peptide chain, which indicates that the isolated lipopeptide falls into the analogs of surfactin.
9.Effect of point massage on swelling pain alleviation of breast during neonatal maternal separa-tion

Objective To investigate the effect of point massage on swelling pain alleviation of breast during neonatal maternal separation.Methods 100 cases of parturients with neonaml maternal separation were randomly divided into the experimental group and the control group with 50 patients in each,the control group received routine breast care,the experimental group was given breast point massage from the first day after delivery for consecutive 3 days.Subjective evaluation of breast swelling paill,breast hardness,breast duct smooth and breast filling degree were compared and analyzed on the third day postpartum.Results For the experimental group,the self evaluation of breast pain was significandy lower,while the breast hardness,breast filling degree and breast duct smooth were better on the first 3 days postpartum than the control group. Conclusions Breast point massage Can significantly reduce the level of swelling pain for parturients with neonatal and maternal separation.
