1.Investigation and analysis of the nursing knowledge of rural patients with rectal carcinoma after permanent colostomy

Objective To investigate the nursing knowledge of rural patients with rectal carcinoma after permanent colostomy.Methods Data were collected on 54 patients with rectal carcinoma after permanent colostomy using the self-designed questionnaire and analyzed by statistical methods.Results The rural patients with permanent colostomy showed low level of nursing knowledge.Scores of patients with different sexes and family income showed significant difference.Conclusions Medical security system and medical staff should provide with more welfare policy and specialty support for the patients in rural areas to promote their knowledge of self-care.
2.Clinical efficacy of sequential therapy for butylphthalide combined with alteplase in the treatment of acute cerebral infarction

Objective To observe the effect of butylphthalide sequential therapy combined with injection of alteplase(injection with recombinant human collagenous tissue plasminogen activator) in the treatment of acute cerebral infarction (ACI).Methods 61 patients with ACI and within 4.5 hours of onset were selected,and they were divided into treatment group(butylphthalide combined with alteplase group,31 cases) and the control group (alteplase only group,30 cases) by random number table.The two groups were given glucose-lowering,statins for maintaining the blood pressure,as well as aspirin enteric tablets or clopidogrel for those without bleeding on the 24 hour review head CT in the acute phase.The treatment group was given alteplase for injection with sequential butylphthalide therapy (butylphthalide injection for 2 weeks followed by an oral administration of butylphthalide soft capsules for 3 months),and the control group was given only alteplase for injection.The National Institute of Health stroke scale (NIHSS) and Barthel index (BI) were used for evaluating the final clinical efficacy.Results The NIHSS scores of the treatment group compared with the control group at 24 hour,14 days and 3.5 months after the treatment had statistically significant differences (t =-0.102,-1.964,-2.444,P =0.037,0.018 and 0.012).The NIHSS scores of the treatment group had decreased 2%,27% and 41% compared with the control group at 24 hour,14 days and 3.5 months after the treatment,respectively.The BI score of the treatment group and control group at 3.5 months after treatment had statistically significant difference (t =-1.956,P =0.029).Conclusion The sequential butylphthalide therapy combined with injection of alteplase has good synergistic effect for the patients with onset of ACI less than 4.5 hours,and it is a safe and effective treatment method for ACI within 4.5 h and it is worthy of clinical promotion.
3.Relationship and significance of HIF-1α and bcl-XL in gastric cancer

Objective To study the expression of HIF-1α and bcl-XL in gastric cancer and their relationship with tumor angiogenesis,clinicopathologic feature and prognosis.Methods Immunohistochemical technique was used to detect the expression of HIF-1α and bcl-XL in 54 cases of gastric canoer.SPSS 12.0 software was used to analyze the relationship between the expression of HIF-1α, bcl-XL and tumor angiogenesis,clinicopathologic feature of patients.Results The positive expression rate of HIF-1α in gastric cancer(74.07%) was significantly higher than that in normal gastric tissue(0),P<0.01;The expression of HIF-1α in gastric cancer was significantly associated with TNM stage,invasive depth and lymph-node metastasis,P<0.05 or P<0.01;The positive expression rate of bcl-XL in gastric cancer(53.7%) was significantly higher than that in normal gastric tissue(33.3%),P<0.05;The expression of bcl-XL in gastric cancer was significantly associated with histological types,TNM stage and lymph-node metastasis,P<0.05;There was a positive correlation between expression of HIF-1α and bcl-XL (r=0.41,P<0.05).Conclusion HIF-1α and bcl-XL play a very important role in the development in gastric cancer and could be a factor in diagnosis of gastric cancer and estimation of prognosis.
4.Preparation and Tumor Inhibition Effect of Transferrin Modified Paclitaxel-loaded Liposome

OBJECTIVE:To prepare transferrin modified paclitaxel-loaded liposome(TF-PTX-LP),and to study the tumor in-hibition effect. METHODS:TF-PTX-LP was prepared by thin-film method,and morphology of TF-PTX-LP was observed. Qualita-tive and quantitative investigation were used to value the uptake efficiency of TF-LP and LP by HepG2 cells. The proliferation inhi-bition rate of HepG2 cells was investigated after treated with PTX,PTX-LP and TF-PTX-LP for 24,48 and 72 h. Tumor spheres were prepared by using HepG2 cells. Effects of normal saline,PTX,PTX-LP and TF-PTX-LP on the volume of tumor spheres were investigated after 0,1,2,4,5,6 and 7 d treatment. HepG2 tumor-bearing nude mice model was induced. Inhibitory effects of normal saline,PTX,PTX-LP and TF-PTX-LP(8.5 mg/kg by PTX)on transplantable tumor of tumor-bearing nude mice were in-vestigated. RESULTS:TF-PTX-LP showed uniform spherical shape,with particle size of 100-120 nm. The fluorescence intensity of HepG2 cells treated with TF-LP was stronger than that treated with LP(P<0.01). Compared with PTX and PTX-LP,TF-PTX-LP showed higher proliferation inhibition rate(P<0.01). Compared with normal saline,PTX and PTX-LP,tumor spheres were small-er in volume after treated with TF-PTX-LP,and inhibition rate of tumor was higher in tumor-bearing nude mice;there were statisti-cal significance after treated for 6,7 d(P<0.01). The proliferation inhibition rate and tumor spheres volume changed in time-de-pendent manner. CONCLUSIONS:TF-PTX-LP which owns good tumor inhibition effect is prepared successfully.
5.Establishment of biology reference interval of NSE in cerebrospinal fluid

Objective To establish the reference intervals of neuron specific enolase (NSE) in tumor markers in cerebrospinal fluid .Methods According to National Committee for Clinical Laboratory Standards document C 28‐A2 ,120 samples were collected to establish reference intervals .Then ,the established intervals were evaluated according to China National Accreditation Service for Conformity Assessment document CL38 :2012 .Results The biological reference interval of NSE in cerebrospinal fluid was 0-3 .14 ng/mL .There was no significant correlation between cerebrospinal fluid NSE level and age ,sex (P> 0 .05) .Conclusion Method used in this study could be ensured reliable ,accurate ,scientific and practical ,desirable for clinical requirement and with great poten‐tial for clinical application .
6.Reserch on the Tlyroid Function Reference Range of Different Pregnancy Period of Normal Pregnant Women

Objective To establish the reference range of thyroid function from the normal pregnant women in different pregnant period in Hanzhong region.Methods According to the NACB inclusion criteria,collected local resident pregnant women 4 031 cases,from July 2012 to December 2015.With the combination of random sampling,stratified sampling and cluster sampling,thyroid hormone levels were measured by a fully automated chemiluminescence analyzer and its accompanying reagents.All patients were divided into three groups:998 cases in early pregnancy (T1),1 543 cases in mid-pregnancy (T2),1 490 cases in late pregnancy (T3),and 105 cases of non-pregnant women in childbearing age (T0) were selected as control group.Results The levels of thyroid hormones were different among three periods of pregnant women.TSH were 0.25~5.32,0.42~6.26 and 0.61~7.68 mIU/L respectively in the early,middle and late stages.FT3 were 3.54~6.04,3.57~5.94 and 2.93~5.40 pmol/L,respectively.FT4 was 7.11~16.88,6.78~16.94 and 6.03~16.87 pmol/L.Thyroid hormone levels in pregnant women compared with non-pregnant women,there were significant differences (TSH:x2=233.183,P<0.05,FT4:x2 =388.12,P<0.05 and FT3:x2 =558.795,P<0.05).TSH were lower in early pregnant women comparing to non-pregnancy women,and higher in middle-late pregnant women.The change of FT3 and FT4 were consistent,and reduced with the extension of pregnancy comparing to non-pregnancy women.Conclusion The level of thyroid function in pregnant women were different from non-pregnant women.and the normal reference range of local pregnant specific thyroid hormone should be established.

"Tinglizi", the ripe seed of Descurainia sophia and Lepidium apetalum, is a member of Brassicaceae (Cruciferae). Traditionally, the former is called "Nantinglizi" (Descurainiae Semen) while the latter is called "Beitinglizi" (Lepidii Semen). In the theory of traditional Chinese medicine, it has the power to purge lung-fire, relieve dyspnea, promote diuresis and reduce edema, and it is mainly indicated in a case with phlegm-fluid accumulation, cough with excessive sputum, dyspnea with being unable to lie, and general swelling. In view of its wide-spread application in clinic, a comprehensive review of Lepidii Semen and Descurainiae Semen was conducted from the following aspects: herbalogical study, variety identification, historical evolution of processing, chemical constituents, pharmacological effects, quantitative determination and toxicity which could provide reference for further research and development of "Tinglizi".

9.Discrimination of Descurainiae Semen and Pantagirus Semen by HPLC fingerprints.

OBJECTIVETo discriminate Descurainiae Semen and Pantagirus Semen.
METHODA high-performance liquid chromatographic method was developed to establish the fingerprint of Descurainiae Semen, and hierarchical cluster analysis (HCA) and partial least squares discriminant analysis (PLS-DA) were applied to study HPLC fingerprinting and chemical recognition.
RESULTThere exists large difference of chromatographic peaks and its relative peak area of HPLC fingerprints between Descurainiae Semen and Pantagirus Semen, and after conducting statistical analysis, the result demonstrated that all samples were classified into three categories: Descurainiae Semen, Pantagirus Semen and their mixtures.
CONCLUSIONThe developed HPLC fingerprint combined with chemometrics can be utilized to discriminate between Descurainiae Semen and Pantagirus Semen, which was quick, simple, accurate and reliable an can provide the basis for the characterization and quality assess of Descurainiae Semen and Pantagirus Semen.

10.Survey on willingness of clinician to carry out clinical research in a tertiary hospital

Objective:To analyze the willingness of clinicians to carry out clinical research and its influencing factors, and to provide a reference basis for scientifically and effectively promoting clinical research and improving the enthusiasm of clinicians.Methods:A grade A tertiary hospital in Tianjin was selected as a questionnaire unit, and clinicians were selected by simple random sampling. SPSS26.0 was utilized for the Wilcoxon rank sum test, Kruskal-Wallis H test, and generalized linear model analysis.Results:The score of willingness to carry out clinical research among 273 clinicians was 5.00. The results of multivariate analysis showed that, in terms of working time, compare with ″≥31years″, ″≤10years″, ″11~20years″, and ″21~30years″ had statistical significance in association with the scores of willingness to carry out clinical researchs, all of which were positively correlated; compared with the ″clear″ awareness of hospital medical ethics review process, ″knowing some″ had a statistical significance in association with the willingness scores with a negative correlation; compared with ″having free time″, the association between ″busy clinical work and no time″ and the willingness scores was statistically significant, and showed a negative correlation; compared with ″time devoting can get results″, the association between ″a lot of time and efforts can not achieve results in a short time″ and the score of willingness was statistically significant, and was negatively correlated; compared with ″willing″ to participate in scientific research and training, ″it does not matter″ and ″unwilling″ showed statistical significance in association with the willingness scores, and negatively correlated.Conclusions:The willingness of clinicians to conduct clinical research was affected by multiple factors. Hospitals should emphasize the cultivation of young talents, carry out research training, and build research support teams to improve the enthusiasm for research.
