1.Logistic regression analysis of the influence factors for health management demand by community residents

Objective To explore the factors influencing health management demand by community residents.Methods Using data from the survey of Health Management Demand by Community Residents in Hangzhou,this paper examined the influence factors with the logistic model from such four aspects as individual characteristics,economic characteristics,health level and health awareness.Results The health awareness poses a positive impact on health management demand;the level of health has a negative impact while income has an inflection point(5 000 yuan per month).However,individual characteristics have no significant effect. Conclusion Such measures as enhancing health management awareness of the residents,building a health management system centering on communities and supported by health management firms,and building health management information platform,can effectively satisfy such demand.
2.Therapeutic effect of medical and health care joint management on patients with diabetic foot ulcer

Objective To build new management model for diabetic foot disease,and to observe the therapeutic effect of diabetic foot ulcers under the new model.Methods 52 patients with newly diagnosed diabetic foot disease were recruited and randomly divided into the experimental group (n=26) and the control group (n=26).While the experimental group adopted the interdisciplinary collaboration model (nurse-doctor jointed diabetes care management model),the control group were treated by the single department pattern.The foot ulcer healing rate,average length of stay (ALOS),fasting plasma glucose (FPG),2 h postprandial blood glucose (2 h PBG),glycosylated hemoglobin (HbA1c),and the patient awareness of the knowledge about diabetes self-management were respectively compared at 3 and 6 months.Results After receiving a multidisciplinary joint diagnosis and treatment,FPG and 2 h PBG of the patients from the experimental group [(6.44±2.93)mmol/L,(8.52± 2.09)mmol/L] were significantly decreased than those of the patients from the control group [(8.17±3.19)mmol/L,(10.77±3.27)mmol/L].HbA1c at 3 and 6 months (7.2%,7.7%) and ulcer recurrence rate (3.8%) after 6 months of patients of the experimental group were significantly reduced than those of the control group(8.9%,9.1% and 15.4%).When compared with patients of the control group,patients of varying severity in the experimental group showed significantly shortened ulcer healing time and hospital stay.Moreover,patients from the experimental group showed statistically higher knowledge level and self management ability (P<0.05).Conclusion Through interdisciplinary collaboration,podiatric medical joint management model can significantly improve cure rate,shorten ALOS,improve blood sugar control and patient satisfaction.

7.Application of intraaortic balloon counterpulsation during primary percutaneous coronary intervention in patients with cardiogenic shock complicating with acute myocardial infarction

Objective To investigate the safety,efficacy and effects of emergent percutaneous coronary intervention (PCI) in patients from Second Affiliated Hospital,Medical College,Zhejiang University with cardiogenic shock (CS) complicating acute myocardial infarction (AMI).Method Twenty-seven patients with CS complicating with AMI were treated by PCI with intraaortic balloon counterpulsation (IABP) support.The change of hemodynamics before and after IABP and PCI,the characteristics of PCI,the mortality during hospitalization, the major adverse cardiac events (MACE) and left ventricular ejection fraction at 30-day follow-up were observed.Results The hemodynamics were significantly improved after IABP.No patients died during PCI.Two patients died after PCI and the total mortality was 7.4% in hospital.During the period of 30-day follow-up, one patient died of heart failure.The left ventricular ejection fraction greatly improved at 30 days after PCI. Conclusions The data suggested that the use of IABP during PCI in patients with CS complicating AMI was safe, decreased mortality and improved prognosis.
8. Correlation of powder particle size, cell wall-breaking, rate and in vitro dissolution rate of Liuwei Dihuang Pill

Objective: To investigate the relationship among powder particle size, cell wall-breaking rate, and dissolution of Liuwei Dihuang Pill (LDP) in vitro. Methods: Particle size and cell wall-breaking rate of four kinds of LDP were measured by laser particle size instrument and optical microscope. The loganin and paeonol were taken as indexes, the feature of dissolution for the above four kinds of LDP was examined by grouting method. Results: There were significant differences in the distribution of particle size, wall-breaking rate, and dissolution among different crushing degrees of LDP. With the decrease of the powder particle size, the wall-breaking rate and relative cumulative dissolution of loganin and paeonol increased. The rate of characteristic cell wall-breaking in the micro powder of Corni Fructus, Alismatis Rhizoma, and Moutan Cortex could reach 100%. Conclusion: Moderately changing degree of smash could cause cell wall-breaking rate changes, so as to control effective ingredient the dissolution rate and extent of Chinese medicine.
9. Study on analgesic effects of aqueous extract from Asari Radix et Rhizoma combined with Nimodipine and its mechanism

Objective: To study the analgesic effects of aqueous extract from Asari Radix et Rhizoma (ARR) combined with Nimodipine and its mechanism. Methods: Forty healthy male Wistar rats were r andomly divided into control, model, ARR, Nimodipine, and ARR+Nimodipine groups. Except the control group, the neuropathic pain models of rats were produced in the rest groups by the ligation of sciatic nerve. Rats in each group were ig administered for two weeks after operation. The aqueous extract (200 mg/kg) from ARR was given to rats in ARR group, Nimodipine (40 mg/kg) was given to rats in Nimodipine group, and the aqueous extract (200 mg/kg) from ARR was given to rats after 30 min administration of Nimodipine (40 mg/kg) in ARR+Nimodipine group, and physiological saline was given to rats in the control and model groups. Thermal paw withdrawal latency and mechanical paw withdrawal reflex threshold of rats in each group were measured on one day before operation and on the days 1, 3, 5, 9, and 14 after 30 min of administration. Results: The thermal paw withdrawal latency and mechanical paw withdrawal reflex threshold of rats in each group were not significantly different before and after the operation (P>0.05). The thermal paw withdrawal latency and mechanical paw withdrawal reflex threshold of rats in the model group were significantly lower than those in the control group at various time points after the operation (P<0.05 and 0.01). The thermal paw withdrawal latency and mechanical paw withdrawal reflex threshold of rats in ARR and Nimodipine groups were significantly higher than those in the model group from the day 5 after the operation (P<0.05 and 0.01). The thermal paw withdrawal latency and mechanical paw withdrawal reflex threshold of rats in ARR+Nimodipine group were significantly higher than those in the model group from the day 3 after the operation (P<0.05 and 0.01), and were significantly higher than those of both in ARR and Nimodipine groups from the day 5 after the operation (P<0.05 and 0.01). Analgesic effects of ARR+Nimodipine group were better than those of separate ARR and Nimodipine groups. Conclusion: The aqueous extract from ARR combined with Nimodipine has the ideal analgesic effects.
10. The construction and expression of Luffin-β-targeted fusion immunotoxin as well as its inhibitory effect on non-small cell lung cancer cells

Objective: To investigate the inhibitory effect of Luffin-β-targeted fusion immunotoxin, which contains endoplasmic reticulum-resident signal fragment KDEL (Lys-Asp-Glu-Leu-COOH) and the uPAcs (cleavage site of urokinase-type plasminogen activator), on NSCLC (non-small cell lung cancer) cells. Methods: Luffin-β gene was cloned by RT-PCR (reverse transcriptase-polymerase chain reaction). Fused gene Luffin-β-KDEL-uPAcs was constructed by primer extension method, and inserted in pET-32a(+) vector to form the recombinant vector pET-32a(+)/Luffin-β-KDEL-uPAcs. After pET-32a(+)/Luffin-β-KDEL-uPAcs imported into Escherichia coli, the exression of fusion protein TELKP (Trx-EK-Luffin-β-KDEL-uPAcs) was induced, and then the TELKP was purified. EK (enterokinase) was used to digest TELKP to produce the targeting fused immunotoxin LKP (Luffin-β-KDEL-uPAcs). CCK-8 (cell counting kit-8), RT-PCR and Western blotting were employed to test the cytotoxic effect of LKP cleavaged by uPA (urokinase-type plasminogen activator) for setting free immunotoxin Luffin-β on NSCLC cells in vitro. Results: The recombinant vector pET-32a(+)/Luffin-β-KDEL-uPAcs could be induced to express the fusion protein TELKP, which contained Trx tag of vector pET-32a(+) and its relative molecular mass was about 4.88×104. The immunotoxin protein LKP, which contained 290 amine acids and its relative molecular mass was about 3.1 8×1104, could be produced after the fusion protein TELKP was digested by EK. The immunotoxin Luffin-β, which possessed cytotoxic effect on tumor cells, could be released from LKP protein cleavaged by uPA in vitro, and its cytotoxic effect was dose-dependent. Conclusion: Fused gene Luffin-β-KDEL-uPAcs and its prokaryotic express vector are successfully constructed. Recombinant fusion immunotoxin LKP, whose relative molecular mass is about 3.1 8×104, is successfully prepared. The immunotoxin Luffin-β, which possesses cytotoxic effect on tumor cells, can be released from LKP protein cleavaged by uPA in vitro. Copyright © 2013 by TUMOR.
