1.Clinical observation of oxcarbazepine suspension monotherapy for 2 to 4-year-old patients newly diagnosed as partial epilepsy

Objective To investigate the efficacy and safety of oxcarbazepine (OXC) suspension for treating the 2 to 4 years old patients newly diagnosed as partial epilepsy.Methods A total of 62 patients between 2 to 4 years old diagnosed as partial epilepsy,selected from the outpatients of the pediatric neurology in the hospital from October 2009 to March 2011,were randomly divided into the experimental group of 32 patients and the control group of 30 patients.Experimental group:OXC suspension,the initial dose was 8-10 mg· kg-1 d-1,orally taking 2 times every day,increasing the dose by 10 mg · kg-1 d 1 once every 7 days to the complete control of the seizure,the target dose was 20-40 mg· kg-1 · d-1 Control group:oral administration of carbamazepine (CBZ) group,the initial dose was 5 mg· kg 1 · d-1,increasing the dose once every 5-7 days up to 10-15 mg · kg-1 · d-1,if necessary,the dose could be 20 mg · kg 1 d-1 to maintain.The observation period was 26 weeks.Results The rate of valid cases in OXC group after 13 weeks and 26 weeks were both 78.1% (25/32),and the rate of non-attack cases were 53.l% (17/32) and 50.0% (16/32),respectively; the rate of valid cases in CBZ group after 13 weeks and 26 weeks were 76.7% (23/30) and 70.0% (21/30),respectively,and the rate of non-attack cases were 50.0% (15/30) and 40.0% (12/30) (x2 =0.022,0.004 ; P =0.883,0.947 respectively.).In the 26th week,the quit rate of OXC group was 6.2%,while the quit rate of CBZ group was 13.4%.The rates of adverse reactions of OXC and CBZ were 15.6% and 26.7%,respectively,with no significant difference.Conclusion OXC suspension monotherapy for 2 to 4 years old patients with partial epilepsy was significant effective,and no significant difference when compared with carbamazepine group.The rate of the adverse reactions in the OXC group was relatively fewer,and the extent was slight.
2.Detection of chemokines in patients with Kawasaki disease and Henoch-Schonlein purpura and its clinical significance

Objective To investigate the changes and clinical significances of the chemokines of interferon-?-inducible protein-10(IP-10),monocyte chemoattractant protein-1(MCP-1) and growth-related oncogene-?(Gro-?) involved in pathagenesis of Kawasaki disease(KD) and HenochSchonlein purpura(HSP).Methods The chemokines production of IP-10,MCP-1 and Gro-? were assayed by ELISA in 15 patients with KD,12 patients with HSP and 10 healthy children.Results The plasma levels of IP-10 and MCP-1 were markedly elevated in KD group [(394.2?176.4)and(420.5?163.4)ng?L-1]compared with HSP group[(94.8?66.4)and(109.2?76.6)ng?L-1] and the control group [(76.4?46.5)and(87.7?47.8)ng?L-1](all P0.05),as well as Gro-? between the three groups.Conclusion Monocyte may enhance the immune damage in KD pathogenesis,and the levels of IP-10,MCP-1 may be important indexes for KD.Neutrophil may be not involved in pathogenesis of HSP and KD.

4.Expressions of P53,topoisomerase Ⅱ and multi-drug resistance associated protein in tissues of colorectal cancer of patients combined with chronic schistosomiasis

Objective To investigate the expressions of P53,topoisomeraseⅡ(TopoⅡ)and multidrug resistance associated protein(MRP)in tissues of colorectal cancer of patients combined with chronic schistosomiasis.Method The retrospective case-control study was adopted.The clinicopathological data of 338 colorectal cancer patients who were admitted to the Zhejiang Cancer Hospital between January 2008 and December 2010 were collected.Cancer tissue specimens from surgical resection were collected.Among 338 patients,80 were combined with chronic schistosomiasis and 258 were combined with non-chronic schistosomiasis.The expressions of P53,TopoⅡand MRP were dectected using immunohistochemistry(IHC).Ranked data were presented as percentage and analyzed using the non-parametric test.Results The negative,weak positive,positive and strong positive expressions of P53 were respectively 5.00%(4/80),87.50%(70/80),3.75%(3/80),3.75%(3/80)in tissues of colorectal cancer of patients combined with chronic schistosomiasis and 28.68%(74/258),19.38%(50/258),16.67%(43/258),35.27%(91/258)in tissues of colorectal cancer of patients combined with non-chronic schistosomiasis,with a statistically significant difference(Z=-2.962,P<0.05).The negative,weak positive,positive and strong positive expressions of TopoⅡwere respectively 8.75%(7/80),51.25%(41/80),22.50%(18/80),17.50%(14/80)in tissues of colorectal cancer of patients combined with chronic schistosomiasis and 12.01%(31/258),55.43%(143/258),22.48%(58/258),10.08%(26/258)in tissues of colorectal cancer of patients combined with non-chronic schistosomiasis,with no statistically significant difference(Z=-1.551,P>0.05).The negative,weak positive,positive and strong positive expressions of MRP were respectively 7.50%(6/80),40.00%(32/80),28.75%(23/80),23.75%(19/80)in issues of colorectal cancer of patients combined with chronic schistosomiasis and 24.42%(63/258),38.37%(99/258),24.03%(62/258),13.18%(34/258)in tissues of colorectal cancer of patients combined with non-chronic schistosomiasis,with a statistically significant difference(Z=-3.408,P<0.05).Conclusion There are abnormal expressions of P53 and MRP in tissues of colorectal cancer of patients combined with chronic schistosomiasis,which may be involved in the hypothetical mechanism of chronic schistosomiasis inducing carcinogenesis of colorectal cancer.
5.Studies on the Methods of Cost-utility Analysis for Cholelithiasis Surgery Patients

OBJECTIVE: To explore a research path and statistical analytical method for cost-utility analysis suitable for gallstone surgery patients.METHODS: A decision-making model for the treatment of cholelithiasis surgery was established;the costs(C) and utility values at each nodal point(? U) were collected,with the results subjected to sensitivity analysis and threshold analysis.RESULTS: The cost-utility ratios(CUR) of laparoscopic operation and open surgery were 4 605.6(? 30%) yuan/QALY and 5 979.7(? 30%) yuan/QALY,respectively.One dimension sensitivity analysis of each( every) parameter reveals that the former CUR value is greater than the latter one.C1 and ? U1 showed the biggest impact on the CUR of laparoscopic operation,while C5 and ? U5 did on the CUR of open surgery.When C1=10 205.6 yuan,or ? U1=1.303 or C5=10 979.8 yuan or ? U5=3.095,CUR of laparoscopic operation was equal to CUR of open surgery,which means that the conversion of threshold was achieved.CONCLUSION: The designed research path and statistic analytic methods are suitable for the cost-utility analysis of cholelithiasis surgery patients.
6.Expression difference of DNA mismatch repair gene hMLH1 and hMSH2 between schistosomiasis-associated colorectal cancer and sporadic colorectal cancer.

OBJECTIVETo investigate the expression difference of DNA mismatch repair gene hMLH1 and hMSH2 between schistosomiasis-associated colorectal cancer and sporadic colorectal cancer.
METHODClinical and pathological data of colorectal cancer patients receiving operations in Zhejiang Cancer Hospital between January 2008 and December 2010 were retrospectively analyzed. Patients were divided into schistosomiasis group(n=80) and sporadic group (n=258) according to the preoperative history and pathologic results. Pathological specimens were collected and tissue chips were made to analyze the expression of hMLH1 and hMSH2 by immunohistochemistr.
RESULTSCompared with sporadic group, older age [(62.2 ± 9.6) year vs. (57.2 ± 11.7) year, P=0.000)], lower platelet level [(197.0 ± 59.6) × 10(9)/L vs. (217.0 ± 84.3) × 10(9)/L, P=0.02] and lower WBC level [(5.9 ± 1.9) × 10(9)/L vs. (6.6 ± 2.8) × 10(9)/L, P=0.02] were found in schistosomiasis group. Ratio of low differentiation-undifferentiation tumor was significantly higher in schistosomiasis group [44.2% (34/77) vs. 4.9% (12/247), P<0.05]. Lower positive rate of hMLH1 expression [77.5% (62/80) vs. 98.1% (253/258), P=0.000] and hMSH2 expression [75.0% (60/80) vs. 95.3% (246/258), P=0.000] was found in schistosomiasis group compared with sporadic group. Concurrent schistosomiasis was one of the risk factors of hMLH1/hMSH2 deficiency (RR: 0.913, 95% CI: 0.836-0.997, P=0.043), but not an independent factor (RR: 0.951, 95% CI: 0.867-1.043, P=0.286).
CONCLUSIONSchistosomiasis is associated with lower positive expression of hMLH1 and hMSH2, which indicates that hMLH1/hMSH2 deficiency may be a potential mechanism of schistosomiasis inducing carcinogenesis of colorectal cancer.

7.Antagonistic effects of tranilast on proliferation and collagen synthesis induced by TGF-beta2 in cultured human trabecular meshwork cells.

Whether tranilast had antagonistic effect on proliferation inhibition and collagen synthesis promotion induced by TGF-beta2 in cultured human trabecular meshwork cells was investigated. Suspension of 1 x 10(4) cultured human trabecular meshwork cells of 3-5 passage was distributed in each well of a 96-well disk and divided into control group and experimental group. After 24 h, 0 microg/ml (control), 12.5 microg/ml, 25 microg/ml, 50 microg/ml tranilast with 3.2 ng/ml TGF-beta2 were added into the incubation medium. Another 24 h later, proliferation and collagen synthesis in cultured human trabecular meshwork cells were examined respectively by using tetrazolium-based semiautomated colormetric (MTT) assay and 3H-proline incorporation with liquid scintillation technique. The results showed absorbance (A) values of the experimental groups were 0.9036 +/- 0.3017, 1.1361 +/-0.1352, 1.2457 +/- 0.1524 according to the different concentrations of tranilast, and 0.8956 +/-0.1903 of the control group. In comparison with the control group, 25 microg/ml (q'= 3.23, P< 0.05), 50 microg/ml (q'=4.70, P<0.01) tranilast significantly antagonized the decrease of the A values induced by TGF-beta2 in the cultured human trabecular meshwork cells. In comparison with the control group [817.37+/-124.21 cpm/10(4) cells], 12.5 microg/ml (620.33+/-80.46 cpm/10(4) cells, q'= 4.26, P<0.05), 25 microg/ml (594.58+/-88.13 cpm/10(4) cells, q'=4.81, P<0.01), 50 microg/ml (418.64+/-67.90 cpm/10(4) cells, q'=8.62, P<0.01) tranilast significantly inhibited the incorporation of 3H-proline into the cultured human trabecular meshwork cells promoted by TGF-beta2 in a dose-dependent manner. It was concluded that tranilast had the antagonistic effect on the proliferation inhibition and collagen synthesis promotion induced by TGF-alpha2 in the cultured human trabecular meshwork cells.

8.MRI findings of primary Rosa-i Dorfman disease in the central nervous system

Objective To investigate the imaging features of primary Rosa-i Dorfman disease (RDD)in the central nervous system and to increase the understanding of the disease.Methods The MRI findings of 5 cases with primary RDD of the central nervous system were analyzed retrospectively,and the related literatures were reviewed.Results Four cases located in the cranium,1 case located in the spinal cord,and 5 tumors were attached to the brain (ridge)membrane.The tumor was equal and low signal on T1 WI,and equal or slightly high signal on T2 WI.Obvious enhancement could be seen on enhance study,and obvious enhancementwas visible in adjacent brain (ridge)membrane,in which 1 case involved the skull,with thickening and strengthening of the skull.Tumors were attached to the meninges in a wide range,the local meninges of 3 cases were penetrated by tumor.Conclusion If young and middle-aged males have no obvious specific clinical manifestations,the images of MRI are single or multiple lesions that are closely related to the brain (ridge)membrane in larger extent and easily penetrate the brain (ridge)membrane,the possibility of RDD should be considered.
9.Preliminary study of stereotactic cardiac radioablation in radiotherapy of ventricular arrhythmia

Objective:To introduce the stereotactic cardiac radioablation (SCRA) based on the stereotactic body radiotherapy (SBRT), and comprehensively evaluate the new approach by short-term effectiveness and safety.Methods:Patients with ventricular arrhythmia (VA) were evaluated and included in this clinical trial, who were immobilized by vacuum bag and performed simulation with 4-dimensional computed tomography (4DCT). In this study, the planning target volume (PTV) was set as the target to design a SBRT plan using volumetric modulated arc therapy (VMAT), which was evaluated by dose parameters such as R 50%, homogeneity index and conformity index, etc. The results of Holter and echocardiography were monitored during the follow-up and compared with the data before treatment. Results:Three subjects with ventricular tachycardia (VT) and one with premature ventricular contraction (PVC) received the same prescription of 25 Gy in a single fraction. The average volume of PTV was 71.4 cm 3(60.3-89.4 cm 3). The average time of beam delivery was 12.0 min (4.5-21.0 min). And the short-term follow-up lasted for an average of 18 weeks (14-25 weeks), which showed significant decrease in both VT and PVC load without complications. Conclusion:This study reports the implementation method of SCRA and proves its short-term effectiveness and safety, but the effects and standards of the key radiotherapy techniques still need to be explored.
10.Thoughts on the strategy and mechanism for the regular prevention and control of COVID-19

Since the outbreak of the COVID-19 at the end of December 2019, Hubei province has actively adopted a series of prevention and control measures such as “quarantine, treatment, testing, and containment”, and have basically blocked the spread of COVID-19. However, with the development of overseas epidemics and the occurrence of case clusters in local areas, we not only face the threat of imported cases, but also face the urgent need to resume normal work and daily life. This puts forward higher requirements for regular prevention and control of COVID-19. Therefore, we should more deeply understand the significance of regular prevention and control as well as the epidemic situation in our province, summarize experience and lessons, and adhere to the prevention and control strategy of “government-led, group-specialist combination, and specialized-oriented”. Meanwhile, it is necessary to implement the working requirement that combines regular prevention and control surveillance with rapid emergency response to local COVID-19 outbreaks. Furthermore, we should establish a regular multi-point trigger early warning mechanism for COVID-19, strengthen the reserve of emergency supplies and carry out training and drills on epidemic prevention and control across the province to make full preparations for the coming autumn and winter epidemics. The most important is to reform the system of disease prevention and control and public health, comprehensively improve the ability of prevention and treatment, and promote the modernization of public health governance.
