Objective Retrospectively analyzed the data of patients with delayed bleeding after colorectal polypectomy, summarized the risk factors and treatment methods of bleeding patients, and provided the basis for further prevention and treatment of postoperative delayed bleeding. Methods Collected the clinical data of 1 243 patients who were admitted into the department of gastroenterology of third affiliated hospital of the third military medical university and accepted polypectomy with colonoscopy from January 2014 to December 2016.Divided these patients into the bleeding group and the non-bleeding group according to whether there was delayed bleeding after surgery. The age, size of polypus, location of polypus, postoperative pathology of the two groups were compared and the postoperative treatment of bleeding was evaluated. Results Among the 1 243 patients underwent colorectal polypectomy, there were 14 cases of delayed bleeding, and the incidence was 1.13%.In the bleeding group, there was 1 case of secondary delayed bleeding and 2 cases of bloody stool after hemostasis for the delayed bleeding.Delayed bleeding occurred at (4.73±2.49)days after surgery. The predilection site of of polypus was rectum in the bleeding group (7/14,50%), and the diameter of polypus was (16.65±4.91)mm in the bleeding group, which was lager than (8.07±4.23)mm in the non-bleeding group with statistical difference (P<0.05).The proportion of hypertensive and diabetic patients in the bleeding group was significantly higher than that in the non-bleeding group (P<0.05).Juvenile polyps and tubular adenoma with high grade intraepithelial neoplasia were more common in the bleeding group(P<0.05).The bleeding group achieved good hemostatic effect by purse suture, hemostatic clip, electrocautery or injection hemostasis. Conclusion Older age, hypertension and diabetes, lager size of polypus, rectum polypus, juvenile polyps and tubular adenoma with high grade intraepithelial neoplasia were risk factor for delayed bleeding. In the event of delayed bleeding, different choice of purse suture, hemostatic clip, electrocautery or injection hemostasis according to different wounds can achieve the desired effect.
2. Visualization Study on Literatures of Helicobacter pylori Therapy Based on CiteSpace From 2014 to 2018
Chinese Journal of Gastroenterology 2019;24(9):534-539
Background: Helicobacter pylori (Hp) is one of the pathogenic factors of many kinds of gastric diseases, and its eradication is the research hotspot and difficulty in this field. Aims: To analyze the related literatures in the field of Hp therapy in the past five years, and draw the research status, hot spots and development trends in this field, thus to provide a reference for the specialists in this field. Methods: The original articles from 2014 to 2018 were retrieved from Web of Science Core Collection database of Web of Science platform by searching 'Thesaurus (TS)= (Helicobacter pylori) and (treatment or therapy)'. CiteSpace5.4. R4 (64-bit) was applied to analyze authors, institutions, key words, references, etc., and visual map was drawn. Results: A total of 2 489 articles were included. Kim JH, Suzuki H, Malfertheiner P published most of the literatures in the field of Hp therapy. Seoul National University, Baylor College of Medicine were the main institutions. The most frequent key words in the past five years were Helicobacter pylori, infection, eradication, Helicobacter pylori infection. Literatures with the highest frequency of co-citation were mainly concentrated in consensus and guideline literature, which constituted the basic knowledge and development trend in this field. The knowledge structure and evolution of Hp therapy field reflected the past research hotspots in this field. Conclusions: CiteSpace measurement analysis indicates that Hp antibiotic resistance and eradication programs are the focus and difficulty of future research.

To investigate the expressions and changes of serum soluble intercellular adhesion molecule-1 (sICAM-1) in lung cancer patients before and after operation and the roles of sICAM-1 in differential diagnosis, metastatic potential, and prognosis. Methods From 2002 to 2003, a total of 17 samples of lung carcinoma tissues and sera were studied. The other 11 non-cancer patients were employed as the controls. Cell surface sICAM-1 levels were analyzed by immunohistochemistry method and ELISA. Results The serum sICAM-1 concentration in the cancer patients was (432.0?124.4) ng/ml. In the controls, however, the serum concentration of sICAM-1 was (262.3?77.7) ng/ml. In patients with lung cancer, the serum concentration of sICAM-1 was significantly higher than that of the controls (P0.05). Conclusion The serum sICAM-1 concentrations in lung cancer patients may play an important role in staging and may also serve as a useful indicator of advanced disease.

Objective To analyze the related literature of natural orifice translumenal endoscopic surgery(NOTES),discuss the development status and direction of the NOTES.Methods NOTES related documents were retrieved from the China National Knowledge Internet(CNKI),Wanfang Data,China Biomedical Literature Database(CBM) and Superstar Medalink. The related project were statistically analyzed.Status quo and development direction were analyzed. Results The journal of NOTES literature was scattered, involving numerous departments, especially the gastroenterology, urology, oncology, obstetrics and gynecology. The largest number of NOTES were reported in the vagina, followed by the stomach and the rectum. The post agencies and the area were mainly in the developed areas of strong scientific research colleges and universities and clinical teaching hospitals. The most published types of literature at present were the research and review of NOTES combined laparoscopy, and more basic research literature. The directions of collecting the reseach of NOTES by keywords were main from several aspects:NOTES combined with laparoscopy for colorectal cancer, urology, hepatobiliary surgery, cardiothoracic surgery, and so on;NOTES in animal experimental study;the application range included NOTES devices, work space, artificial interface for disease, and development of NOTES.Conclusion NOTES is the current research hotspot.Currently the domestic development is relative lag, which is in the primary stage of development. The basic research of NOTES need further study and clinical application need to strengthen, otherwise, application of NOTES instrument need to speed.

Journal of Clinical Hepatology 2020;36(8):1799-1804
ObjectiveTo analyze the articles on endoscopic retrograde cholangiopancreatography (ERCP), an important method for minimally invasive treatment of biliary and pancreatic diseases, published worldwide, and to investigate the status, hotspots, and development trends in this field. MethodsThe web of science core collection database in Web of Science platform was selected to search by the subject words “TS = (Cholangiopancreatography, Endoscopic Retrograde)”, for the articles published from January 1, 2015 to December 31, 2019, and the literature type was selected as “article”. CiteSpace 5.6.R2 (64-bit) was used to analyze the authors, key words, institutions, countries (regions), and references and plot visualized maps. ResultsA total of 1535 articles on ERCP were included. The analysis showed that Hiroyuki Isayama had the highest number of published articles, followed by Yousuke Nakai and Takeshi Ogura, and University of Tokyo had the highest number of published articles, followed by Shanghai Jiao Tong University and University of Ulsan College of Medicine. Moreover, the US, Japan, and China were the top three countries from the aspect of the number of published articles in the recent 5 years, and ERCP, complication, and risk factor were the key words with the highest frequency in the recent 5 years. The most frequently cited articles mainly focused on the complications of ERCP and related consensus or guidelines. ConclusionThe main research hotspots and trends in the field of ERCP are related factors and clinical prevention of ERCP-related complications.
