Heart failure is becoming an increasingly common and significant problem in the field of pediatric cardiology,β-blockers as one of the most important cardiovascular drags in the pharmacopoeia,can not only obviously relieve symptoms of severe heart failure,but also reduce cardiovascular events and allcause mortality significantly,the efficacy is effective and high security.This article reviewed β-blockers in the treatment of children heart failure mechanism,the clinical application evaluation,the basic principles and relevant issues in use process,the appropriate choice and usage according to the clinical indications.

To explore the impact of acceleration load on the temporomandibularjoint in aviators and its clinical significance.617 aviators flying aircraft of different types without previous history of temporomandibularjoint disease were studied. Abnormal manifestations of the temporomandibularjointswere analysed. The incidence of temporomandibular joint symptoms was markedly higher in the fighter plane pilots than that in the transportation planegroup. Load induced by acceleration could be one of the main causative factors of temporomandibularjoint disorder.

[Objective]To study the clinical results of methods of improvement of pedicle screw stability in patients of osteoporosis. [Method]Thirty-four cases of patients with osteoporosis were operated using pedicle screw and methods of improvement of pedicle screw stability were used during the operations.Of which,male 20,female 14;the mean age of 57(range 40～71).According to Jikei grading scale for osteoporosis,8 were of early stage,11 of stageⅠ,8 of stageⅡ and 7 of stageⅢ.There were 14 cases of fractures and 20 cases of osteopathy.For patients of early stage and stageⅠ(19 cases),long and large size of pedicle screws were used,meanwhile stiff connection rod system,two cross-link devices,placement of pedicle screw with large angles in horizontal and sagital planes were applied.For patients of stageⅡ and stageⅢ(15 cases) bone cement was used to fill the pedicle hole to ensure the screw stabily.[Result]There were no neurologic and vessel injuries or aggravated as well as no breakage of screw except two cases with loosening of screw during the follow-up with the mean period of 14 months(range 9～26).The loss of correction of reduction in fractures was 5%,the rate of fusion was 100% in grafting.[Conclusion]Different methods of improvement of pedicle screw stability used in osteoporosis may avoid loosening of screw and loss of correction.

Objective To investigate the effects of transcription factor achaete scute-like 2(Ascl2)on epithelial-mesenchymal transition (EM T ) associated microRNAs .Methods Colon cancer LS174T cells were transfected with shRNA-Ascl2 vector and shRNA-control vector respectively ,then the transfected cells were selected with G 418 and stably transfected cell lines were estab-lished .Real-time PCR and Western-blot analysis were used to determine the interference effect .Transwell invasion experiment were used to observe the effects of Ascl2 RNA interference on cell invasion capability in vitro .MicroRNA chip analysis was used to de-tect the change of EMT-associated microRNA expression ,and real-time PCR experiment was used to validate the microarray re-sults .Results The mRNA and protein expressions were significantly reduced after Ascl2 interference (P<0 .01) .The numbers of invading cells were significantly decreased after Ascl2 interference (P<0 .01) .MicroRNA chip analysis found microRNA-200 fami-ly (including microRNA-200b ,microRNA-200a ,microRNA-429 ,microRNA-200c ,microRNA-141) was more than 2-fold upregula-tion after Ascl2 interference (P< 0 .01) .Conclusion Ascl2 regulates the invasion and metastasis of colon cancer cell ,possibly through transcription regulation of microRNA-200 family ,and then regulation of EM T .

BACKGROUND: Xiongbitong capsule (capsule) increases blood flow in myocardial ischemic area in patients with coronary heart disease, lessens the size of myocardial infarction and improves ischemic S-T alternation in electrocardiogram (ECG).OBJECTIVE: To observe the effect of the capsule on NO metabolism on arterial wall in rabbits and explore the improvement of such drug on myocardial blood flow and protection on myocardium.DESIGN: Randomized group division and control experiment were designed.SETTING: Department of First Internal Medicine, Hospital of Traditional Chinese Medicine, Weifang, Shandong.MATERIALS: The experiment was performed in Teaching-Research Room of Physiology of Weifang Medical College from April 2003 to February 2004. Totally 30 healthy adult rabbits were employed, of either sex and randomized into 3 groups, named normal control, model group and treatment group, 3 rabbits in each one.METHODS: ① Preparation of hyperlipemia (HP) model: In normal control, common granular forage was used. In model group, modeling forage was applied (common granular forage + 3% pig oil + 1% cholesterol powder). In treatment group, besides modeling forage, at the end of the 6th week, the capsule [composed of taizishen (Radix Pseudstellariae), yunling (Poria), gualou (Frnctus Trichosanthis), qingxia (Rhizoma Pinelliae), juhong (Exocarpium Citri Grandis), shichangpu (Rhizoma Acori Graminei), yujin (Radix Curcumaea), zhifuhua (Flos Inulae), jiangxiang (Lignum Dalbergiae Odoriferae), danggui (Radix Angelicae Sinensis), maidong (Radix Liriopis Platyphyllae) and wuweizi (Frnctus Schisandrae).] was administrated every day for gastric perfusion at 1 pill/kg with 10 mL physiological saline for 3 weeks. The intervention was applied totally for 8 weeks. ② Nitrieoxide synthase (NOS) and NO metabolic product kits were provided from Radiation Medical Institute of Military Academy of Medical Sciences. The operation was based on the instrnction. NOS activity was calculated and expressed by NO amount (nmol) produced in one-gram tissue per minutes.Spectrophotometer was used to measure the optic density of sample at 545nm. Sodium nitrite solution was used to prepare standard curve. The concentration of serum NO metabolic products, NO2- and NO3- was expressed with μmol/L. ③ T test was applied for difference comparison among measurement data.MAIN OUTCOME MEASURES: Comparison of NOS activity and concentration of serum NO metabolic products in vessel-wall tissue in eachgroup.RESULTS: Totally 30 rabbits all entered result analysis. For NOS activity and concentration of serum NO metabolic products in arterial-wall tissue,the results in model group and treatment group were lower remarkably than the control (P ＜ 0.05-0.01) and the results in treatment group were higher remarkably than model group (P ＜ 0.05).CONCLUSION: ① NO system of arterial wall is damaged in HP rabbits and NO metabolic level is decreased in serum. ② Xiongbitong capsule improves the above situation and brings the resistance of myocardial ischemia and protection of myocardium into play.

Objective To clarify the distribution characteristics of serum pepsinogen (PG ) and gastric cancer related tumor markers in the healthy population ,and analyse their value in health examination .Methods The levels of serum tumor markers ,pep‐sinogenⅠ(PGⅠ ) and pepsinogenⅡ(PGⅡ) were detected in 958 subjects who underwent physical examination by using the Lumi‐nex 200 instrument ,and the ratio of PGⅠ /PGⅡ(PGR) was calculated .Meanwhile ,the C13 breath test was carried out ,and relation‐ships among these clinical indicators were analysed .Results In this population ,The serum level of PGⅠ in male subjects was high‐er than that in female subjects ,there was statistically significant difference(P<0 .05) .The levels of serum PGⅠ ,PGⅡ ,carcinoem‐bryonic antigen(CEA) ,carbohydrate antigen 199(CA199) and carbohydrate antigen 242(CA242) were gradually increased with age ,while the PGR was slowly decreased with age .The positive rate of C13 breath test in male subjects was higher than that in fe‐male subjects ,there was statistically significant difference(P<0 .05) .Compared with subjects whose C13 breath test results were negative ,the serum levels of PGⅠ and PGⅡ were higher ,while the PGR was lower in subjects whose C13 breath test results were positive(P<0 .05) .Conclusion The distribution of serum PG might be correlated with age ,gender and the infection of Helicobact‐er pylori in healthy population .It is shown that the effects of tumor markers on gastric cancer screening are limited .

Objective To investigate the effects of pyrrolidine dithiocarbamate (PDTC) on plasma concentrations of proinflammatory cytokines and myocardial energy metabolism induced by cardiopulmonary bypass (CPB) .Methods Twelve healthy mongrel dogs of both sexes weighing 13.5-17.5 kg were randomly divided into 2 groups (n = 6 in each group) : PDTC group and control group. The animals were anesthetized with intraperitoneal 2.5% pentobarbital sodium 25 mg?kg-1, intubated and mechanically ventilated. In PDTC group PDTC 30 mg?kg-1 was given i.v. after tracheal intubation while in control group normal saline was given instead of PDTC. Aorta was clamped for 60 min and then undamped for 60 min reperfusion during CPB. Blood samples were taken before (baseline), 30 and 60 min after aortic clamping and 30 and 60 min after aortic unclamping for determination of plasma concentrations of TNF-? and IL-1?. Myocardial tissue was obtained before and 60 min after aortic clamping and 60 min after aortic unclamping for determination of myocardial content of adenine nucleotide (ATP, ADP, AMP, TAN, EC) and expression of ICAM-1 protein.Results Plasma TNF-? concentration was increased after aortic cross-clamping as compared to the baseline value before clamping in both groups but the TNF-? concentration was significantly lower in PDTC group than in control group (P

Objective To investigate the therapeutic effects of embryonic dopaminergic neurons,mesencephalon-derived neural stem cells with or without pre-differentiation as grafts in the treatment of Parkinson's disease(PD) in SD rats.Methods Forty SD Parkinsonian rats were randomly assigned to four groups as follows: normal saline grafted group(A),mesencephalon-derived neural stem cells without pre-differentiation grafted group(B),embryonic dopaminergic neurons grafted group(C) and mesencephalon-derived neural stem cells with pre-differentiation grafted group(D).Normal saline and different cells were transplanted into the rat's striatum by stereotactic technique.Therapeutic effects were evaluated based on the improvement of rat's rotation behavior at 2nd,4th and 8th week after operation and survival of tyrosine hydroxylase(TH)-immunoreactive(TH-ir) cells at 8th week after operation.Results No obvious amelioration in number of rotation was found in group A(F=0.294,P=0.830).In group B,the number of rotation was decreased after operation,but the difference was not significant compared with that before operation(F=0.335,P=0.800).The numbers of rotation were obviously decreased in group C(F=26.838,P=0.000) and group D(F=14.571,P=0.000) at 8th week after operation,but no significant difference was found between the two groups(P=0.764).TH immunohistochemical staining showed that the number of TH positive cells was 665?264 in group D,679?286 in group C,80?36 in group B,and 0 in group A,and no significant difference existed between group C and D(P=0.548).Conclusion As grafts,both embryonic dopaminergic neurons and mesencephalon-derived neural stem cells with pre-differentiation can improve the rotational behavior and cellular morphology of Parkinsonian rats after neural cells transplantation.
