1.Isaacs syndrome:a case report

ObjectiveTo analyze the clinical features and pathogenesis mechanism of Isaacs syndrome.MethodsA case with Isaacs syndrome was reporttedResults and ConclusionIsaacs syndrome is characterized by the occurrence of spontaneous and continuous muscle fiber activity, associated with muscle cramps, pseudomyotonia and myokymia, stiffness and delayed relaxation of the muscle. The stiffness and myokymia are present at rest and during sleep. Isaacs syndrome has been recently suggested to be produced through an immune-mediated mechanism in which voltage-gated potassium channels may be targeted by auto-antibodies.
2.Bariatric surgery for the treatment of metabolic diseases

Bariatric surgery began in 1950s.The rapid increase of obese patients and development of laparoseopic techniques lead to popularity of bariatric surgery all over the world in 1990s.Current mainstream of bariatric surgeries include laparoscopic adjustable gastric banding,sleeve gastrectomy,Roux-en-Y gastric bypass and biliopancreatic diversion.In this review,the efficacy of different surgical procedures was compared in the aspects of metabolic disorder remission and incidence of operation complications and mortality.Efficacy of bariatric surgery on the alleviation of metabolic disorders (including hypergycemia,hyperlipidemia,hypercholesterolemia),individual treatment and interdisciplinary cooperation should be emphasized.
5.The application of transcranial Doppler in predicting prognosis of patient with spontaneous intracerebral hemorrhage

Objective To investigate the value of transcranial Doppler(TCD)in the prediction of the outcome and determination of brain death in patient with spontaneous intracerebral hemorrhage. Methods The waveforms and the cerebral blood flow velocities of the bilateral middle cerebral arteries(MCA)were monitored and analyzed using EME TCD in 68 comatose patients with Glasgow scores ≤7. Results The results showed that persistence of retrograde diastolic flow or small systolic forward flow in bilateral MCA lasted for more than 1 hours were observed in 28 cases of comatose patients, which was a reliable indicator for predicting or confirming brain death. Asymmetrical TCD waveforms were characteristically associated with the localized brain damage. Conclusion TCD monitoring is a useful technique for predicting outcome and guiding treatment of patient with spontaneous intracerebral hemorrhage.
6.Clinical Significance of Oligoclonal Bands and IgG Index for Multiple Sclerosis

Objective To study the positive rates of oligoclonal bands(OCB)and the IgG index in patients with multiple sclerosis(MS)and in other nervous system disorders(OND).Methods Cerebrospinal fluid(CSF)and serum were collected from 54 patients with MS,271 patients with OND including 62 patients with infections of central nervous system and 209 patients with non-infections.The OCB and IgG index of CSF and serum were detected.Results The positive rate of OCB was 35.2% in MS,8.9% in OND(P<0.05),in which 24.2% in infections of central nervous system(P>0.05),4.5% in non-infections of central nervous system.The positive rate of IgG index was 83.3% in MS,78.2% in OND(P>0.05).Conclusion OCB is helpful to diagnose MS if infection diseases have be excluded,while IgG index can be referenced.
7.The Clinical significance of urine transferrin in the type 2 diabetic nephropathy and primary chronic glomerular disease

Objective To investigate the clinical significance of urine transferrin in the type 2 diabetic nephropathy and primary chronic glomerular disease. Methods The level of TRF and UAER in the urine of 114 patients with type 2 diabetes mellitus (T2DM), 108 patients with CGD and 30 healthy patients was measured by rate nephelometry and analyzed. Results Urine TRF significantly increased earlier than ALB at the early stage of DN; Linear correlation was found by correlation analysis between the level of TRF in urine level and the quantity of protein in urine. Conclusion TRF in urine obviously increased at the early stage of DN. It was more sensitive than UAER to reflect the early renal injury of DM. A statistically significant correlation was found between the TRF level and the quantity of protein in the urine.

Objective:To investigate the role of human telomerase reverse transcriptase(hTERT) in carcinogenesis of oral submucous fibrosis(OSF).Methods:The expression of hTERT was examined by immunohistochemical SP method in 10 cases of normal oral mucosa, 46 cases of OSF,14 of carcinogenesis of OSF and 10 of adjacent non-cancerous OSF tissue. 9.Identification and dynamic observation of fungous infections in two patients of ~(60)Co ? accident

Objective To identify the fungi isolated from two acute radiation sickness(ARS)patients as a result of an accidental 60Co irradiation,and to observe the sensitivity of the fungi to antifungal agents.Methods The pathogenic fungi were morphologically examined and identified with the VITEK 2 automatic microorganism analyzer and API 20C AUX yeast identifying card.The susceptibility of fungi to antifungal agents was tested with broth microdilution method.Results Candida parapsilosis and Sporothrix schenckii were identified from case A in the samples of blood,bone marrow,urine and stool etc.Most of pathogenic fungi were sensitive to the antifungal drugs in vitro.In case B,Candida parapsilosis,Candida tropicalis,Trichosporon asahii and Aspergillus terreus were identified in the samples of sputum,urine or stool etc.Accompanying with the prolongation of antifungal treatment,the sensitivity of fungi to the antifungal drugs were decreased remarkably.Conclusion Multiple infections in different organs could be caused by pathogenic fungi,such as Trichosporon asahii and Sporothrix schenckii.Although most of pathogenic fungi were sensitive to the antifungal drugs in vitro,the effects of antifungal treatment were not satisfactory owing to poor general conditions of 2 acute radiation sickness(ARS)patients and marked compromise of the immune system.Because of antibiotic and antifungal drugs were used early for preventive purpose,the clinical samples should be specially treated in order to raise the positive rate of fungal identification.
10.Effects of arecoline on the expression of human telomerase reverse transcriptase in oral mucosa keratinocytes

Objective:To investigate the effects of arecoline on the expression of human telomerase reverse transcriptase(hTERT)mRNA and protein in cultured normal human oral mucosa keratinocytes(KCs),and then to investigate the role of hTERT in carcinogenesis of oral submucous fibrosis(OSF).Methods:Normal human oral mucosa KCs were cultured in vitro,and in experiment group,KCs was divided into 0.03,0.06,0.09 g/L arecoline group,and 0.0 g/L arecoline group served as the control.The hTERT mRNA and protein expression of KC was examined by reverse transcription polymerase chain reaction(RT-PCR)and Western blotting.Results:Arecoline induced the hTERT mRNA and protein expression of KC in a dose dependent manner.hTERT mRNA and protein expression of KC were increased by 0.03,0.06 and 0.09 g/L arecoline,when compared with the control group(P
