3.Commonly Encountered Pathogens and Their Drug Sensitivity:Analysis of 233 Patients with Lung Infection

OBJECTIVE To study commonly encountered pathogenic bacteria leading to lung infection and their drug sensitivity. METHODS In the period of Jan 2003 to Feb 2005,the pathogenic species and drug sensitivity of 233 positive samples isolated from 496 phlegm samples were analyzed.RESULTS Totally 234 strains of pathogenic bacteria were(isolated) from 233 phlegm samples.It had been found that 132 strains of Gram-negative bacilli,(accounted) for(56.4%);57 strains of Gram-positive cocci,accounted for 24.4 %;41 strains of fungi,accounted for 17.5%;and 4 strains of Gram-positive bacilli,accounted for 1.7%.The pathogens of the highest infective rate were:(Pseudomonas) aeruginosa(15.4%),Candida albicans(11.6 %),Enterobacter cloacae(6.0%),Klebsiella(pneumoniae)(5.6%),Escherichia coli(5.1%),Staphylococcus aureus(4.3 %),Streptococus pyogenes((4.3%)),Acinetobacter baumannii(3.6%),and C.tropicalis(3.6 %).Results of drug sensitivity indicated that drugs selected sensitive to Gram-negative bacilli were:imipenem(75.4%),piperacillin/tazobactam(38.9%),(ciprofloxacin)(37.3%),gentamicin(37.3%),and amikacin(36.5%).Drugs sensitive to Gram-positive cocci were:vancomycin(75%),rifampin(55%),and erythromycin(35%).CONCLUSIONS The commonly encountered pathogens of lung infection in our hospital are: P.aeruginosia,C.albicans,E.cloacae,K.pneumoniae,E.coli,S.aureus,and S.pyogenes.Infective rate of Candida is increasing evidently.Antibiotic resistance of bacteria(becomes) severe,drugs applied became less and less.Clinic should(actively) develop phlegm cultivation and drug sensitivity test,applying antibiotics reasonably,reducing production of bacteria resistant to drugs.

Angiotensin-Ⅱ-induced apoptosis was widely involved in the pathogenesis of diabetes mellitus, cardiovascular diseases, etc. The mechanism of angiotensin-Ⅱ-induced apoptosis is complicated, including the receptor (AT1R and AT2R) level regulation, Fas/FasL pathway, and the regulation by p53 family, Bcl-2 family, Caspase family, Ang-(1-7), etc. Mitochondria damage and oxidative damage play important roles in the apoptosis process. This article reviews the previous researches on angiotensin-Ⅱ-induced apoptosis.
5.Effects of norepinephrine on brain edema of rats with severe burn

Objective To investigate the effects of norepinephrine on brain edema of rats in 24 h after severe burn.Methods A total of 48 healthy Wistar rats were randomly divided into 8 groups: normal control group,1,2,5 mg/kg norepinephrine,burn group,burn with 1,2,5 mg/kg norepinephrine pretreatment groups(n=6 in each group).The rats in all burn groups were scalded into 40%TBSAⅢ degree burn.Pathological features were observed,and blood brain barrier,brain water(%) were examined in postburn 24 h.Results Pathological evidence of brain edema exhibited in the burn group and burn group with norepinephrine pretreatment,and increased permeability of blood brain barrier and brain water were observed.The burn with norepinephrine pretreatment groups were more significantly severe in comparison with simple burn groups and normal control group.Conclusion Norepinephrine may play an important role in brain edema in postburn 24 h,suggesting that stress of postburn may induce brain edema.

AIM: To investigate the changes of gene expression of vascular endothelial growth factor(VEGF) in norepinephrine/ burn serum-induced astrocytes.METHODS: Immunofluorescence staining was used to show the distribution of VEGF in astrocytes after 24 h using norepinephrine/burn serum stimulation.Western blotting was used to detect protein expression of VEGF.Real time PCR was used to investigate expression of VEGF mRNA.RESULTS: ① Green fluorescence of protein expression of VEGF in astrocytes was increased when treated with high dose norepinephrine(50 ?mol/L).Green fluorescence of protein expression of VEGF in astrocytes was increased distinctness after burn serum stimulation.Green fluorescence protein expression of VEGF in astrocytes was increased significantly when high dose norepinephrine combined with burn serum stimulation was added.② VEGF protein expression in burn serum stimulating group was increased,and VEGF protein expression was significantly increased when burn serum was added during moderate(20 ?mol/L),high dose norepinephrine stimulation.③ Expression of VEGF mRNA was increased in burn serumtreated astrocytes.Expression of VEGF mRNA was increased significantly when norepinephrine-stimulated astrocytes exposed to barn serum,and as norepinephrine dose increases gradually.CONCLUSION: Norepinephrine and burn serum play an important role in inducing VEGF protein expression in astrocytes,suggesting that stress reaction of postburn is an important cause in inducing brain edema by excreting VEGF in astrocytes.
8.Norepinephrine induces brain tissue VEGF protein expression in severe burn rats

AIM: To investigate the effect of norepinephrine (NE) on brain tissue VEGF protein expression in 24 h after severe burns. METHODS: (1) 40% total body surface area (TBSA) Ⅲ? burn model was made in Wistar males rats. Brain water (%) was examined at 24 h after burn. (2) NE levels in the rat brain tissue were determined by high performence liquid chromatography. (3) VEGF protein levels in the rat brain tissue were determined by Western blotting at 24 h of postburn. RESULTS: (1) Brain edema exhibited at the burns and burn with norepinephrine groups. (2) A increase in NE levels in the rat brain tissue was observed at the burns. NE levels increased significantly in burn with norepinephrine groups. (3) VEGF protein expression in the rat brain tissue was gradually increased with increasing in norepinephrine stimulating dose. VEGF protein expression in the burn rats brain tissue was significantly increased in burn with high dose of norepinephrine stimulation. CONCLUSION: Norepinephrine induced VEGF protein expression in rat brain tissue at 24 h after severe burn, suggesting that increases in norepinephrine level postburn induce brain edema.

Blood GSHpx activities of populations living in Keshan disease affected and non-affected areas were examined. The enzyme activities of people in affected areas were lower than that in non-affected areas. That of agricultural population is lower than that of workers and staff members living in same affected areas. The enzyme activities of patient suffering from Keshan disease were a little lower than that of other children. After oral administration of Na2SeO3 for one year, the enzyme activities of children in affected areas increased to the same level of that in non-affected areas. Significant correlation was found between selenium concentration of blood and hair and GSHpx activities.
10.Clinical analysis of 87cases of male hypogonadotropic hypogonadism

Objective To analyse the clinical characteristics of male hypogonadotropic hypogonadism.Methods Clinical data of 87 cases of male hypogonadotropic hypogonadism were retrospectively analyzed.Results Mean age was(18.8?4)years old,and 25 patients were diagnosed as Kallman syndrome.The level of serum LH,FSH,and T was low,and LH responded poorly to GnRH stimulation.Most patients' bone age was delayed,and all patients' CT or MRI of pituitary was normal.Conclusion According to the above clinical features,and testosterone replacement or hCG and/or plus hMG are the choice of treatment.
