1.The technique and development of total skin electron irradiation-Taiwan experiences

Objective The aim of this study is to present two novel technique?Lying on position and rotational board TSEI in which uniform dose to the whole body as well as the soles and vertex of scalp can be achieved in one electron beam treatment fraction. Methods Patients are treated with two groups in prone and supine position by leaning on inner rotational board in prone and supine position, each group can further be separated into two subgroup with tilting and rotation positions for treatment. By incorporating all these setting on the aswirl board with the optimal gantry angle, the patient will final accumulate compensated dosages on whole body surface. Results The dose uniformity for the rotational board?TSET or Lying on position could be control within ±10%. Conclusions The two TSEItechnique?Lying on and Rotational board TSEI techniques presented in this study is able to deliver a uniform dose to the patient’s skin surface as well as to the vertex and the soles all in one time, eliminating the troubles of having to further boost fields for these two regions when using the Stanford Six Field Technique.
2.Imageological and histological assessment of biodegradation properties of CPC/DBM/rhBMP-2 composites with optimal formula

Objective To analyze the radiographical and histological features of implanted composites of decalcified bone matrix(DBM) impregnated with calcium phosphate cement(CPC) and recombinant human bone morphogenetic protein-2(rhBMP-2) in bone defect,and evaluate its biodegradation properties.Methods By prefabricating the rabbit DBM,the CPC/DBM/rhBMP-2 composites were prepared in a DBM/CPC ratio of 2∶8 and the rhBMP-2 content of about 1.2mg/cm3.Twenty-four rabbits were divided into groups A and B(12 each).Animals in group A were implanted with the composites in the bone defects at femoral condyle and femoral muscle,and the rabbits in group B CPC was implunted in the same place.Animals were sacrificed at 6,12 and 24 weeks after operation(4 each),and the implants were examined radiographically and histologically.Results It was shown by radiography that,in group A,the margin of implants in femoral condyle defects became irregular 6 weeks after operation;the appearance of the implants appeared hazy and shrunken in size 12 weeks after operation;and most of the implant was absorbed and the medullary cavity became partly rediotransparent 24 weeks after operation.The density of the implants in muscles beoame less dense gradually during 6 to 24 weeks after operation.While in group B,the appearance and density of the implants in femoral condyle defects and muscles were not obviously changed during 6-24 weeks after operation.Histological observation showed that,in group A,the interface between the implants in the bone defects was filled with newly formed bone grown into the composites 6 weeks after operation.New bone formed inside the composites 12 weeks after operation,and the bone defect was repaired,and most of the implants were replaced by new bones,with a little portion of it remained 24 weeks after operation.Fibrous tissue was seen to grow into the composites in muscles during 6-12 weeks after operation,and only a little of the composites remained 24 weeks after operation.In group B,no fibrous tissues or new bones grew into composites implanted in femoral condyle defects or muscles 6-12 weeks after operation,and the bone defects was still filled with materials of CPC,and no new bone or fibrous tissue was found inside CPC 24 weeks after operation.Conclusion Compare with CPC,the DBM with an optimal formula is are easy biodegradable and can be replaced by autologous bone.
3.A new naphthaquinone derivative from Pyrola calliantha H. Andres.

To investigate the chemical constituents of hemostatic extract of Pyrola calliantha H. Andres, the extract was subjected to chromatographic separation and purification. Along with some known compounds, a new naphthaquinone derivative was isolated and identified as 2-(1, 4-dihydro-2, 6-dimethyl-1, 4-dioxo-3-naphthalenyl)-3, 4, 5-trihydroxylbenzoic acid by physicochemical and spectroscopic analysis.
4.The effect of medical ozone injection for the treatment of knee joint osteoarthritis on serology:a comparative study between intra-articular and intravenous injection in rabbit models

Objective To compare the variations of serum cytokines before and after ozone injection with different dosages and between intra-articular injection of knee joint and autohemotransfusion injection in experimental rabbits,to make a further understanding of the biologic mechanism of ozone therapy on osteoarthritis(OA).Methods Thirty-six New Zealand white rabbits were equally and randomly divided into six groups:(1) normal group,(2) OA model group,receiving no treatment,(3) group L10,receiving 10 ?g/ml ozone per intra-articular injection,(4) group L30,receiving 30 ?g/ml ozone per intra-articular injection,(5) group S10,receiving autohemotransfusion with 10 ?g / ml of ozonized blood per injection and(6) group S30,receiving autohemotransfusion with 30 ?g/ml of ozonized blood per injection.OA models were prepared by injecting 0.5 ml of collagenase II solution into the knee joint space two times with an interval of three days.Blood samples of all groups were collected 4 weeks after the last ozone treatment for further analysis.Serum levels of NO,T-SOD,TNF-? and IL-1? were estimated.Results The serum levels of NO,T-SOD,TNF-? and IL-1? in the model group and all treated groups were significantly higher than that in normal group(P0.05).Conclusion(1) An increase in the serum levels of NO,T-SOD,TNF-? and IL-1? exists in rabbit OA models.(2) Ozone has no inhibitary effect on the serum levels of NO,T-SOD,TNF-? and IL-1?,regardless of what route of administration of ozone is adopted.(3) The effect of "oxidative preconditioning" is not local,but rather systemic.
5.Imaging measurement of skin puncturing point and angle for unipedicular vertebroplasty and clinical application

ObjectiveTo explore puncture point and angle of skin for unipedicular vertebroplasty by imaging measurement and combining clinical practice in order to pr vide reference for clinical work.MethodsCT images of thoracic and lumbar vertebrae of imaged with multislice helical CT were random chosen.From high to low position vertebrae of easy fracture T6 to -L5,its angle between vertebraI pedfcle stalk line and 1/3 of vertebraI body stalk line fore and median of point of intersection was measured.The angle of sternocostal joints route was also measured.At the same time the distance between site of skin puncture and spinous process was measured.ResultsThe angle between the thoracic vertebraI pedicle and sagittal section was (28.2 ± 3.8) °,the lateral distance of the spinous process was ( 31.6 ± 5.4) mm.The angle of thoracic vertebraI pedicle lateral route was (35 ± 3.4) °,the distance was ( 55.2 ± 5.3 ) mm.In the lumbar vertebrae pedicle route,the angle of L1 -L4 was (29.3 ± 5.6 ) °,the distance was (38.4 ± 4.5 )mm.Ls was (39.2 ± 3.3) °,the distance was ( 71.6 ± 6.2) mm.Combining clinical practice,the lateral distance of the spinous process in thoracic vertebraIpedicle route,the best puncture point was 30 mm,in thoracic vertebraI pedicle lateral route it was 40 mm,in L1-L4 it was 45 mm,in L5 it was 60 mm.ConclusionsThe unipedicular vertebroplasty can be performed rapidly,accurately and safely,and it reduces complications,cost and time.
6.The correlation between nuclear factor kappa B expressed in peripheral blood mononuclear cells and the characteristics of coronary atherosclerosis plaque in patients with acute coronary syndrome

Objective To explore the correlation between expression of nuclear factor kappa B (NF-kappa B) in peripheral blood monouclear cells(PBMC) and the severity of coronary artery lesion in patients with acute coronary syndrome(ACS). Methods The 81 patients were diagnosed with coronary heart disease, and all of them underwent immediate or selective coronary angiography(CAG) The nuclear protein level of activated NF-B was detected by Western blot and semi-quantity image analysis was done. Coronary angiograms were scored according to Gensini integration. The relationship between NF-κB and Gensini score was analyzed. Results The protein expression of NF-κB in PBMC in ACS group was significantly higher than that in stable angina group and control group (0.85±0.18 vs. O.75±0.21 and 0.71±0.23, F=3.72, P<0.05). The protein expression of NF-κB was not correlated with Gensini score(r=0.07, P>0.05). Conclusions The ACS patients have the activation of NF-κB in PBMC. The protein expression of NF-κB is not correlated with Gensini score, which does not suggest an implication of the severity of coronary artery.
7.Clinical research progress of presepsin on sepsis

Sepsis is a frequent condition with high mortality,and its early diagnosis in the pediatrics department is one of the keys to improve survival. C-reactive protein and procalcitonin have been used as bio-markers in the diagnosis of sepsis,but they have limited susceptibility and specificity,and can be elevated in non inflammatory condition. Presepsin is the free fragment of a glycoprotein expressed on monocytes/macro-phages. Preliminiary reports suggest that levels of presepsin are significantly higher in septic patients com-pared to healthy individuals, and presepsin is useful for evaluating the severity of sepsis.
8.Clinical characteristics of severe pneumocystis pneumonia in children without human immunodeficiency virus ;infection

Objective To investigate the clinical parameters,risk factors,treatment and clinical outcomes of pneumocystis pneumonia(PCP) in children without human immunodeficiency virus(HIV). Methods Retrospective a-nalysis was made for the clinical features,risk factors,treatment and prognoses of the non-HIV infected severe PCP pa-tients hospitalized at Pediatric Intensive Care Unit( PICU) of Children′s Hospital Affiliated to Capital Institute of Pedi-atrics. Results During April of 2010 to April of 2014,there were 10 cases of non-HIV infected severe PCP in PICU of Children′s Hospital Affiliated to Capital Institute of Pediatrics. All of the patients had predisposing diseases,in which 3 cases had connective tissue diseases,2 cases had acute leukemia,3 cases had severe pneumonia and 2 cases had con-genital immunodeficiency. The main clinical manifestations of those 10 patients were fever, cough, tachypnea and obvious dyspnea. All patients developed respiratory failure. The median value of Pediatric Critical Illness Score was 79. The median arterial oxygen pressure was 58 mmHg(1 mmHg=0. 133 kPa). The median oxygenation index was 103 mmHg. The median alveolo-arterial oxygen partial pressure difference was 43. 8 mmHg. The median CD4+T-lympho-cytes counts was 169 ×106/L. Eight patients on admission had mixed infection. Acute respiratory distress syndrome (ARDS) occurred in all of the patients,and 7 cases of them had multiple organ dysfunctions. All of the patients re-quired ventilation support. The median day for invasive mechanical ventilation days was 11 and the median day for non-invasive ventilation days was 6. The pneumothorax occurred in 5 patients. All patients received trimethoprim-Sulfame-thoxazole as initial therapy and Caspofungin treatment in combination in 7 cases of the patients. Six patients had nosoco-mial infection. The median time of PICU stay was 15. 5 days. Six patients survived and the mortality was 40%(4/10 cases) . Conclusions PCP is a kind of fatal diseases which occurred in patients with immunocompromised conditions and concurrent ARDS or multiple organ dysfunctions. Diagnostic suspicion and mechanical ventilation therapy with lung protective ventilation strategies may improve the clinical outcomes of non-HIV-infected PCP in children.
9.Imaging study of non-bronchial systemic collateral supply in hemoptysis

Objective To investigate the significance of non-bronchial systemic collaterals (NBSCs) in supplying bronchial hemoptysic lesions,and to study the morphological features of bronchial artery (BA) when NBSCs become the predominant supplying vessels. Methods Multi-slice helical CT angiographic findings in 124 patients with bronchial hemoptysis were retrospectively analyzed. 3D reconstruction of thoracic systemic arteries,including BAs and NBSCs,was performed at the console work station with the help of real-time thin-slice enhanced helical CT scanning. The number of NBSCs and BAs was calculated,and the internal diameter of the arteries and the thickness of pleura in the vicinity of the pulmonary lesion were measured. According to the presence or absence of NBSCs,the patients were divided into NBSCs group and non-NBSCs group. The relevant data was statistically analyzed. Results NBSCs group included 36 cases,the mean internal diameter of BA was (1.850 ? 0.631)mm and the pleura adjacent to the pulmonary lesion was obviously thickened in 22 cases (61%) with a thickness of 2.7-16.0 mm [mean(7.71 ? 4.12) mm]. In the non-NBSCs group (n = 88),the mean internal diameter of BA was (2.200 ? 0.528) mm and the pleura adjacent to the pulmonary lesion was obviously thickened in 7 cases (8%) with a thickness of 1.1-2.4 mm [mean(1.7 ?0.53) mm]. 10.Effect of sevoflurane on expression of α7 nicotinic acetylcholine receptor in hippocampal neurons of rats

Objective To evaluate the effect of sevoflurane on the expression of α7 nicotinic acetylcholine receptor (α7nAChR) in hippocampal neurons of rats.Methods Forty-eight pathogen-free healthy adult Sprague-Dawley rats of both sexes,weighing 210-280 g,aged 3-4 months,were divided into 4 groups (n=12 each) using a random number table:control group (group C) and 1%,3% and 5% sevoflurane groups (S1,S2 and S3 groups).In S1,S2 and S3 groups,1%,3% and 5% sevoflurane were inhaled for 1 h,respectively.Morris water maze test was performed at 1 and 7 days after anesthesia to assess the cognitive function.Six rats in each group were sacrificed after the end of the test.The brains were then removed and the hippocampi were isolated for detection of the expression of α7nAChR (by Western blot) and contents of tumor necrosis factor-alpha and interleukin-1 (by enzyme-linked immunosorbent assay).Results Compared with group C,the number of crossing the platform was significantly decreased at 1 and 7 days after anesthesia in group S1,the time of staying at the target quadrant was significantly shortened,the number of crossing the platform was decreased,and the rate of time of staying at the original platform quadrant and rate of swimming distance at the original platform quadrant were decreased at 1 and 7 days after anesthesia in S2 and S3 groups,and the contents of tumor necrosis factor-alpha and interleukin-1 were significantly increased,and the expression of α7nAChR was down-regulated at 1 and 7 days after anesthesia in S1,S2 and S3 groups (P<0.05).Compared with group S1,the expression of α7nAChR was significantly down-regulated at 1 and 7 days after anesthesia in S2 and S3 groups (P<0.05).Compared with group S2,the expression of α7nAChR was significantly down-regulated at 1 day after anesthesia in group S3 (P<0.05).There was no significant difference in each parameter of Morris water maze test among group S1,group S2 and group S3 (P>0.05).Conclusion The mechanism by which sevoflurane induces inflammatory responses in hippocampi is related to down-regulation of α7nAChR expression in hippocampal neurons of rats.
