BACKGROUND:Forceful fixation and enough blood supply in fracture site are the fundament for treating bone nonunion. OBJECTIVE:To investigate the outcome of autologous lilac bone-grafting and plate fixation in treatment of the ununited distal ulnar fracture with osseous defects. DESIGN,TIME AND SETTING:The own control study was performed at the Department of Orthopaedics,Sixth People's Hospital,Shanghai Jiao Tong University from August 2002 to May 2006. PARTICIPANTS:Of 21 cases of the ununited distal ulnar fracture with osseous defects,13 cases were induced by ulnar and radial fracture, and 8 cases were induced by ulnar fracture. There were 15 cases of open fracture and 6 cases of closed fracture of first onset. Bone defects were 1.5 cm-5.0 cm,averagely 3.1 cm. METHODS:Bone defects were filled with intercalary lilac grafts. Length and axial ray of ulna were controlled. Ulna was fixed utilizing 5-8 wells 1/3 tubular plate, reconstruction plate or locking compression plate. At least two screws were used at the distal end,and 3 or 4 screws were fixed in the proximal end,FoUow-up was conducted once per month to observe clinical appearances and radiograph in each patient. MAIN OUTCOME MEASURES:Fracture nonunion, dorsal extension and palmer flexion of wrist joint, pronation and supination of the forearm were measured. RESULTS:All 21 cases were followed up for a mean time of 14.7 months. All the patients obtained bony union, and a mean time of 4.6 months (ranged 3 to 7 months). The distal ulnar healed with < 10° of angular deformity in a coronal plane in one patient and with 15° of angulation in a sagittal plane in another one, while others (19 patients) obtained satisfactory alignment. The excellent and good rate was 90.5%. CONCLUSION:Intercalary bone-grafting with autologous lilac and secure fixation for treating the ununited distal ulnar fracture with osseous defects can reconstruct ulnar length, correct deformities, and obtain good bone healing and functional recovery.

Objective To summarize the clinical features and biological markers of critically ill children with pancreatic necrosis. Methods The clinical data of 5 ill children whose autopsy confirmed pancreatic necrosis in our hospital from 2003 to 2009 were collected and the pathological results were analyzed. Results Five cases appeared as acute onset and fever. Clinical manifestation of 2 cases included abdominal pain and 2 cases represented abdominal bulging, lots of flatulence and gastrointestinal bleeding. Four cases represented abnormal liver function and significant decrease of albumin,3 cases with increase of C-reactive protein. No case showed abnormal pancreas acoustic image of abdominal B-ultrasonography. Autopsy confirmed that 5 cases represented varying degrees necrosis of the pancreas, which complicated with the damage of adrenal gland, liver, lung, heart, spleen, kidney, intestine, thymus, mediastinal and mesenteric lymphnodes. All died in 36 hours after the children'conditions were changed. Two cases died of acute hemorrhagic necrosis of the pancreas,3 cases died of multiple organ failure based on pancreatic necrosis. Conclusion Pancreatic necrosis in critically ill children presents acute onset and short course,is prone to result in multiple organ damage or failure.

A method for the infrared spectrophotometric determination of total ph-ospholipids in seed of Juglans regia and acinus of Citrullus vulgaris was reported. The average recovery of total phospholipid in this test was 97.2%, the CV was 1.66%. The contents of total phospholipid in seed of Juglans regia and acinus of Citrullus vulgaris were 438.46-528.85mg% and 241.78 -247.47mg% respectively. The rapid quantitative analysis of the distribution of various phospholipid components in these two specimens was carried out by using thin layer chromatographic scanning and the corrective method of absorbance proportional coefficient. The results showed that the main phospholipid components in both specimens were PC and PE. The fatty acid composition of the total phospholipids of both specimens were analysed by GC-MS-DS (gas chromatography-mass spectrometry-data system).

Corticotropin-releasing factor(CRF)is one neuroendocrine peptide which is closely related with the stress. It has been proved that any stress can lead to the person and animal's blood CRF level increasing, and high-level CRF urges the hypothalamic neuron calcium ion inflow.Newborn pediatricians pay more attention to whether blood CRF level can act as one director of assessing severity of newborn hypoxic-schemic brain damage. This article reviews the CRF and it's acceptors,CRF secretion,CRF physiological action and it's adjustment, and hypoxia-ischemia stress and CRF,in order to provide the theory basis for reviewing the severity of hypoxic-schemic brain damage in neonates.
