1.Influence on cerebral oxygen balance and metabolism of propofol target controlled infusion induced tracheal intubation in patients with laparoscopic surgery

Objective To discuss the influence on cerebral oxygen balance and metabolism of propofol target controlled infusion induced tracheal intubation in patients with laparoscopic surgery.Methods A total of 96 patients with gynecologic laparoscopic surgery were selected from January 2012 to October 2013.All patients were in propofol target controlled infusion anesthesia induction downward internal jugular vein retrograde through jugular vein ball tube,radial artery catheter.According to propofol target controlled infusion target concentrations,96 patients were divided into A group (32 cases,3 μ g/ml),B group (33 cases,5 μμg/ml) and C group (31 cases,7 μg/ml).Hemodynamic and Nacotrend index (NI) change were monitored in the process of surgery patients,with internal jugular vein and radial artery blood at pre-induction (To),intubation (T1),30 min after intubation (T2) and the end of the operation (T3) were detected and arterial blood oxygen saturation (PaO2),arterial oxygen content (CaO2),internal jugular vein ball blood oxygen saturation (SjvO2),internal jugular vein ball blood oxygen partial pressure (PjvO2) at different time points were compared among three groups.Artery-internal jugular vein ball low blood oxygen of components (Da-jvO2),brain oxygen uptake rate (CEO2),poor lactic acid content (Da-jvL) were calculated.Results The systolic blood pressure,heart rate and NI at T1,T2 was lower than that at To in three groups,and there was significant difference (P < 0.05),but there was significant difference in NI at T1,T2 among three groups (P < 0.05).Compared with T0,PaO2,PjvO2,SjvO2 and CaO2 at T1-T3 in three groups were increased significantly (P < 0.05),and the highest was C group,and then B group and A group.In the process of propofol target controlled infusion,Da-jvO2,CEO2 and Da-jvL showed a trend of gradual decline,Da-jvL decline highest significantly (P < 0.05).Conclusions Propofol target controlled infusion induced endotracheal intubation can decrease the laparoscopic surgery in patients with cerebral oxygen metabolism and the brain tissue of ischemia hypoxia tolerance,but will not lead to cerebral oxygen supply and demand imbalance,and 7 μ g/ml concentration of propofol anesthesia effect is the best.
2.Relationship Between Slow Coronary Flow and Characteristics of Coronary Lumen Condition in Relevant Patients

Objective: To explore the relationship between slow coronary lfow (SCF) and characteristics of coronary lumen condition in relevant patients.
Methods: We retrospectively summarized 2117 patients who received coronary angiography (CAG) in Weifang people’s hospital from 2012-08 to 2014-04 and 2 groups of patients were enrolled in our study. SCF group, according to TIMI frame count, the patients had coronary stenosis<50%, without coronary thrombosis, expansion and dissection,n=110. Control group, the patients had normal CAG at meanwhile,n=49. The diameters of left main (dLM), left anterior descending proximal section (dLADp) and middle section (dLADm), left circumlfex (dLCXp) and dLCXm, right coronary artery (dRCAp) and dRCAm were measured; the ratios of dLADp/dLADm, dLCXp/dLCXm, dRCAp/dRCAm were compared between 2 groups.
Results: The ratios of dLADp/dLADm, dLCXp/dLCXm, dRCAp/dRCAm were similar between 2 groups,P>0.05. Compared with Control group, SCF group had increased dLM, increased dLADp and dLADm, dLCXp and dLCXm, dRCAp and dRCAm, allP<0.05. Pearson correlation analysis showed that coronary TIMI frame counts were positively related to the diameters of distal and middle sections of each vessel, the correlation coefifcients in LAD were 0.036 and 0.204, in LCX were 0.368 and 0.183, in RCA were 0.253 and 0.209, allP<0.05. Left coronary TIMI frame count was positively related to the diameter of LM, the correlation coefifcient was 0.501,P<0.05.
Conclusion: Elevated coronary TIMI frame count was related to increased vessel diameter, the expended diameter of left and right coronary arteries may promote SCF development in relevant patients.
3.A comparative study of p1 6 gene methylation status in primary tongue squamous cell carcinoma and cervical lymph node metastasis

DNA from 32 cases of tongue squamous cell carcinoma(TSCC)specimens and the neck lymph node metastasis specimens were processed by bisulfite treatment.The methylation of the specimens was examined by Q-MSP amplification.The consistency of p1 6 methyla-tion of primary TSCC with that of lymph node metastasis tumor was 90.62%(P <0.05).We assumed that p1 6 gene promoter CpG island methylation rates in primary TSCC and metastatic lymph nodes are consistent,p1 6 gene methylation status can be a methylation predictor be-tween premary and lymph node metstasized TSCC.
4.Impact of different doses of dexmedetomidine on the anesthetic sedative and analgesic effect of anesthesia agents in hysterectomy of patients with uterine fibroids under laparoscopy

Objective To explore the impact of different doses of dexmedetomidine on the anesthetic sedative and analgesic effect of anesthesia agents in hysterectomy of patients with uterine fibroids under laparoscopy.Methods 90 patients with uterine fibroids with ASA class Ⅰ or Ⅱ who were going to undergoing general intravenous anesthesia laparoscopic hysterectomy were selected and they were divided into group A (dexmedetomidine 0.4μg·kg-1·h-1,n=30) and group B (dexmedetomidine 0.2μg·kg-1·h-1,n=30) and group C (no dexmedetomidine,n=30) by digital random method.The analgesia effect (VAS score) of group A,group B and group C preoperation and postoperative 1h,12h and 24h,and the sedation (Ramsay sedation score) were compared.General information,postoperative blood pressure,heart rate,pulse oxygen saturation,postoperative patients'' satisfaction on analgesic effect and the sedation effect and adverse reactions of the three groups were also compared.Results Age,body mass index and other basic information of three groups had no statistically significant differences (all P>0.05).The preoperative VAS score of the three groups also had no statistically significant difference (F=0.625,P>0.05).Postoperative 12h,24h,the VAS scores of group A and group B were lower than those of C group,which of group A were lower than group B (F=11.250,P<0.05,F=21.181,P<0.05).The Ramsay sedation score was higher than that of C group[(3.67±0.18) vs.(2.13±0.11)].The satisfaction of group A was significantly higher than group B and group C (96.67% vs.73.33%,46.67%,χ2=25.953).The Ramsay sedation score and satisfaction were higher than those of group B (all P<0.05).There were no significant differences in blood pressure (F=0.362,P>0.05),pulse oxygen saturation (F=0.978,P>0.05) and heart rate among the three groups before and after operation (F=1.268,P>0.05).There was no obvious adverse reaction in the three groups.Conclusion Dexmedetomidine can promote anesthetic sedative and analgesic effect of anesthesia agents in hysterectomy of patients with uterine fibroids under laparoscopy,and the dose 0.4μg·kg-1·h-1 of dexmedetomidine has better effect,feasibility and safety,it is worthy of clinical promotion.
6.Neural stem cells transplantation combined with monosialotetrahexosyl ganglioside for treatment of spinal cord injury in rats

ObjectiveTo investigate the curative effect of neural stem cells (NSCs) transplantation combined with monosialotetrahexosyl ganglioside (GMi) in treatment of acute spinal cord injury in rats.MethodsCompressive spinal cord injury model at T8 segment was established in the adult SD rats that were then randomly divided into three groups, ie, control group, NSCs transplantation group and NSCs + GM1 group.Continuous observation was performed at 1,2, 4 and 8 weeks.Functional neurological recovery of the injured spinal cord was evaluated with motor function scale, pathology, transmission electron microscopy and somatosensory evoked potential (SEP).ResultsThe motor function of the lower extremities was recovered at different degrees in three groups.While the motor function recovery level of the animals and the positive staining cells of the calcitonin gene-related peptide (CGRP) in the NSCs + GM1 group were higher than those in the other two groups at 4 and 8 weeks (P < 0.01).Compared with control group and NSCs group, focal necrosis and small vessel regeneration were observed only in the center of the injured segment in the NSCs + GM1 group at 8 weeks.Electron microscope scan showed edema under the membrane of the large myelin sheath in the control group, much intact myelin sheath, well-differentiated neurons and many kinds of synapse vesicles in the NSCs + GM1 group.The latent period of SEP was shortened markedly in the NSCs + GM1 group two weeks after transplantation (P <0.05).The latent period shortening was apparent in the NSCs group at 4 and 8 weeks after transplantation but was still longer than that in the control group.ConclusionsTransplantation of neural stem cells combined with use of GM1 can protect the nervous tissues after spinal cord injury, when GM1 reconstructs the spinal cord through promoting differentiation of the transplanted stem cells and linking with the host cells.
7.A systematic review on the correlation between selective dipeptidyl peptidase inhibitor vildagliptin and risk of arthralgia/osteoarthritis

Objective To systematically evaluate the correlation between selective dipeptidyl peptidase (DPP)-4 inhibi-tor ,vildagliptin and risk of arthralgia/osteoarthritis .Methods The following databases of PubMed (1978 to February 2016) , the Cochrane Library (Issue 4 ,2015) ,EMbase (1974 to February 2016) ,CBM(1978 to February 2016) ,CNKI(1978 to Feb-ruary 2016) ,VIP(1989 to February 2016) were searched .The randomized controlled trials (RCTs) for vildagliptin related ar-thralgia/osteoarthritis were assessed using the Cochrane Collaboration system .The meta-analysis was performed with RevMan 5 .3 software .Results Ten RCTs were included .Meta-analysis showed that vildagliptin had higher risk of arthralgia/osteoar-thritis than other oral hypoglycemic agents or placebo (RR=1 .24 ,95% CI 1 .08 to 1 .44 ,P=0 .003) .Further analysis indica-ted that patients received different doses of vildagliptin had higher risk of arthralgia/osteoarthritis than the placebo group (RR=1 .35 ,95% CI 1 .02 to 1 .78 ,P=0 .04) .In particular ,the group who took vildagliptin 50mg once daily had significantly higher risk of arthralgia/osteoarthritis than the placebo group (RR=3 .04 ,95% CI 1 .44 to 6 .44 ,P=0 .004) .In comparison of other oral hypoglycemic agents ,the vildagliptin group had higher risk of arthralgia /osteoarthritis (RR=1 .19 ,95% CI 1 .01 to 1 .41 ,P=0 .04) .Conclusion Based on the Meta-analysis results ,vildagliptin increases the risk of arthralgia/osteoarthri-tis .Especially vildagliptin 50mg once daily had two times higher risk than the placebo group .However the long-term safety of vidagliptin still needs to be confirmed by RCTs with larger samples and long term follow-up .
8.Prevalence of daily smoking among residents at ages of 15 years and older in Wuhan City

Objective:
To investigate the prevalence of daily smoking among residents at ages of 15 years and older in Wuhan City, so as to provide insights into tobacco control.
Methods:
A total of 2 700 permanent residents at ages of 15 years and older were sampled using a multistage random sampling method in three districts of Wuhan City in July, 2019. Residents' demographics, tobacco use, secondhand smoke exposure and awareness of harmful effects of tobacco were collected using the core questionnaire of the Global Adult Tobacco Survey, and the factors affecting daily smoking were identified among residents at ages of 15 years and older using a multivariable logistic regression model.
Results:
A total of 2 700 questionnaires were allocated, and 2 551 valid questionnaires were recovered, with an effective recovery rate of 94.48%. The respondents included 1 272 men (49.86%) and 1 279 women (50.14%), and the proportion of daily smoking was 19.84% among the respondents, with a mean daily tobacco use of 16.6 cigarettes. Multivariable logistic regression analysis identified men (OR=66.191, 95%CI: 25.344-172.873), ages of 45 to 64 years (OR=2.586, 95%CI: 1.158-5.775), farmers (OR =8.477, 95%CI: 1.126-63.786), secondhand smoke exposure at home (OR=9.395, 95%CI: 5.401-16.340), secondhand smoke exposure in workplaces (OR=1.923, 95%CI: 1.081-3.423) and no smoking at home (OR=0.459, 95%CI: 0.277-0.763) as factors affecting daily smoking among residents at ages of 15 years and older in Wuhan City.
Conclusions
There is a high likelihood of daily smoking among men, the middle-aged and elderly, farmers and individuals with exposure to secondhand smoke in workplaces at ages of 15 years and older in Wuhan City. Health education pertaining to tobacco control requires to be reinforced among high-risk populations, and creation of a tobacco-free home is recommended in public places and workplaces based on the complete smoking ban.
9.E-cigarette use among high school students in Wuhan City

Objective:
To investigate the use of e-cigarette and analyze its influencing factors among high school students in Wuhan City, so as to provide insights into developing control measures for adolescents.
Methods:
From September to November 2019, high school students in Wuhan City were selected by the multi-stage stratified cluster random sampling method, and basic information, e-cigarette use and second-hand smoke exposure were collected through questionnaire surveys. Proportions of e-cigarette current use and attempt to use were analyzed, and factors affecting the current use of e-cigarettes were identified using a multivariable logistic regression model.
Results:
Totally 5 700 questionnaires were allocated, and 5 602 valid questionnaires were recovered, with an effective rate of 98.28%. The respondents included 2 925 males (52.21%) and 2 677 females (47.79%); 4 033 high school students (71.99%) and 1 569 vocational high school students (28.01%). The proportion of attempt to use e-cigarettes was 9.23%, and the proportion of current e-cigarette use was 2.03%. Multivariable logistic regression analysis identified male (OR=2.112, 95%CI: 1.285-3.471), vocational high school (OR=1.967, 95%CI: 1.214-3.186), private high school (OR=9.684, 95%CI: 5.648-16.605), family second-hand smoke exposure (OR=3.064, 95%CI: 1.741-5.392), second-hand smoke exposure in public places (OR=4.402, 95%CI: 1.687-11.484) and having close friends who were smokers (OR=6.432, 95%CI: 3.219-12.852) as factors affecting current e-cigarette use among high school students.
Conclusions
The proportion of current e-cigarette use among high school students in Wuhan City was 2.03%. Male, vocational high school, private high school, second-hand smoke exposure and having close friends who were smokers may be promoting factors for e-cigarette use.
10.Gas Chromatographic Fingerprint of Oleic Acid of Whitmania pigra Whitman

Objective To establish the gas chromatographic(GC) fingerprint of oleic acid of Whitmania pigra Whitman for its quality control. Methods Ten batches of Whitmania pigra from different sources and processed by different methods were analyzed with Agilent 6890N gas chromatography detector on DB-WAX(30 mm × 0.32 mm × 0.25μm)column at the vaporizing temperature of 270℃, column temperature of 130℃and flame ionization detector (FID) temperature of 280℃. We used a software of Similarity Evaluation System for Chromatographic Fingerprint of TCM(Version of 2004A) published by Chinese Pharmacopoeia Commission to calculate GC similarity. Results Oleic acid content of Whitmania pigra processed by different methods had significant differences (F2,7 = 7.350, P = 0.019). The oleic acid content of samples dried after washing with clean water significantly differed from that of the samples processed by alumen or the slices dried naturally(P = 0.021, P= 0.009). The similarity of the fingerprints was in the range of 0.458 - 0.998. The similarity of samples from Lipu of Guangxi Province was the lowest. Conclusion The fingerprints of most of the samples have very high similarity. The established GC fingerprints can be used to effectively identify the qualified or inferior Whitmania pigra products, which will provide some reference for the quality control of Whitmania pigra.
