1.Geriatric Hospital Pathogens of Lower Respiratory Tract Infection among Patients in ICU of a and Their Drug Resistance Analysis

OBJECTIVE To explore the pathogen distribution and their resistance in ICU of Geriatric hospital.METHODS 111 strains isolated from lower respiratory tract were collected from Sep 2006 to August 2007.The antibiotic resistance were tested.RESULTS Among 111 strains,G-strains accounted for 70.27%,in which Pseudomonas aeruginosa was the most common(20.72%).The G+bacteria were multi-resistant to most of the antibiotics except impenem.CONCLUSIONS The most common pathogens in ICU of geriatric hospital is G-bacteria with multi-resistance.The Antibiotics should be selected reasonably.
2.The Effect of Bronchial Arterial Embolization in Treating Massive Hemoptysis

Objective To analyse the clinical value of bronchial artery embolization in treatment of massive hemoptysis.Methods 87 patients with hemoptysis included bronchiectasis in 46 cases,pulmonary tuberculaosis in 18 cases,bronchial carcinoma in 15 cases,bronchial arteriovenous malformation in 2 cases and unknown hemoptysis in 6 cases underwent embolized treatment of bronchial arteriography or intercostal arteriography.Of them,bronchial artery embolization were performed in 78 cases,intercostal artery embolization were done in 6 cases,both embolization of bronchial artery and intercostal artery were in 3 cases,2 cases underwent superselective embolization with coaxial microcatheter.In the total 87 cases,gelatin sponge particles(GSP) alone was used in 85 cases,GSP and polyvinyl alcohol(PVA) were used in 2 cases.All the patients were followed up for 12~18 months.Results The hemoptysis was immediately stopped in 58 cases, remarkable improvement were 19 cases after operation. Recurrence of hemoptysis was found in five, three and two cases at one, two and four weeks respectively after embolization. All of the ten recurred cases accepted re-embolization and hemoptysis had been well controlled. The effective rate was 89%(77/87). There was not any severe complication in all the patients.Conclusion Bronchial artery embolization is a safe, effective and less invasive method in treatment of massive hemoptysis. It can be recommended to the non-surgical treatment patients.
3.Correlation of inositol level and glycolipid metabolism in pregnant women with gestational diabetes mellitus

Objective:To investigate the correlation between inositol level and glycolipid metabolism in gravidas with gestational diabetes mellitus (GDM).Methods:A cross-sectional study was conducted on 80 GDM cases undergoing routine examination at Fujian Provincial Maternity and Children's Hospital from November 2018 to August 2019, who were selected as GDM groups. Another 50 women with uncomplicated pregnancies during the same period were selected as the control group. Blood and urine inositol level and serum glycolipid profiles were compared between the two groups, and their association was analyzed. Independent or paired-sample t test, Mann-Whitney test, Chi-square (or Fisher's exact) test, and Pearson correlation test were performed for statistical analysis. Results:The serum inositol concentration and high-density lipoprotein (HDL) cholesterol in the GDM group were significantly lower [322.1 ng/ml (279.1-364.1 ng/ml) vs 403.8 ng/ml (391.8-425.3 ng/ml), Z=-7.879; 1.8 mmol/L (1.5-2.0 mmol/L) vs 2.0 mmol/L (1.7-2.2 mmol/L), Z=-2.419; both P<0.05], while the concentration of urine inositol, lipoprotein-a (lipo-a), 0 h-, 1 h-, 2 h-oral glucose tolerance test (OGTT) glucose, fasting insulin, and glycosylated hemoglobin (HbA1c) were significantly higher when comparing to the control group [192.2 ng/ml (171.0-219.9 ng/ml) vs 143.8 ng/ml (121.1-158.6 ng/ml), Z=-6.834; 253.2 mg/L (65.0-349.0 mg/L) vs 148.5 mg/L(46.5-159.3 mg/L), Z=-0.187; 5.0 mmol/L (5.1-5.6 mmol/L) vs 4.4 mmol/L (4.2-4.6 mmol/L), Z=-5.547; 10.0 mmol/L (9.1-11.3 mmol/L) vs 7.8 mmol/L (7.0-8.4 mmol/L), Z=-6.987; 8.6 mmol/L(7.6-9.4 mmol/L) and 6.6 mmol/L (5.7-7.1 mmol/L), Z=-7.100; 18.2 mU/L(10.6-25.9 mU/L) vs 11.0 mU/L (6.3-12.7 mU/L), Z=-4.537; 5.4%(4.5%-5.5%) vs 5.1%(4.9%-5.4%), Z=-3.468; all P<0.05]. (2) Serum inositol concentration was negatively correlated with fasting insulin and 0 h-, 1 h-, 2 h- OGTT glucose level ( r=-0.386, -0.416, -0.350 and -0.407, respectively); urinary inositol concentration was positively correlated with 0 h-, 1 h-, 2 h-OGTT glucose levels ( r=0.402, 0.389 and 0.429, respectively) (all P<0.05). Conclusions:Serum inositol concentration was decreased, and urinary inositol excretion was increased in women with GDM. Measurement of changes in inositol levels during the second trimester may be helpful to assess the metabolic status of pregnant women.
4.Transcript levels and functional analysis of DNA methyltransferases in CD4+ T cells of MRL/lpr mice

Objective To investigate the expression levels of DNA methyltransferases 1, 3A and 3B in CD4+ T cells of MRL/lpr mice, and explore their relationship with the expression levels of methylation-sensitive genes (ITGAL, CD70). Methods CD4+ T ceils were isolated from spleens of 16-week-old MRL/lpr and BALB/c control mice by anti-CIM antibody labeled magnetic beads. Transcription levels of DNA methyhransferases 1, 3A and 3B and methylation-sensitive genes(ITGAL, CD70) were measured by real-time mice when compared with BALB/c control mice and the difference was significant (P<0.05), while the expression of DNMTI and DNMT3A showed a tendency of decrease (P>0.05). The mRNA expression of CD70 was significantly higher (P<0.01), but the expression of ITGAL had no significant difference between the two P<0.01 ). Conclusion Decreased expression of DNMT3B may attribute to the elevated expression of methylation-seusitive gene CD70, thus lead to the dysfunction of CD4+ T cell and play an important role in the pathogenesis of SLE.
5.Multiple linear regression analysis of hip function and vitamin D levels before and after hip arthroplasty

BACKGROUND:The incidence of low serum level of vitamin D in patients undergoing hip arthroplasty and its impact has not been reported in China, indicating that it has not been brought to the forefront. OBJECTIVE:To determine the prevalence of low serum level of vitamin D in patients before total hip arthroplasty and its relationship with the hip function scores. METHODS:Forty-eight hips from 48 patients undergoing primary hip arthroplasty from July 2013 to August 2014 in the First Affiliated Hospital of Suzhou University were enrol ed. According to the serum level of vitamin D, patients were assigned to low-level (<20μg/L) and high-level (20≥μg/L) groups. The general information of patients, the hip function scores before and after replacement at the last fol ow-up in the two groups were observed and compared. The relationship between the serum level of vitamin D and the hip function scores before and after replacement was analyzed by multiple linear regression analysis. And the average fol ow-up was 12 months (11-14 months). RESULTS AND CONCLUSION:(1) The incidence of low vitamin D level was 82%(20 ng/mL serving as standard). (2) Compared with patients with high vitamin D level, patients with low level of vitamin D had lower preoperative Harris scores and Merle D′Aubigne-Postel score (P<0.05), and also at the last fol ow-up (P<0.05. (3) Based on the preoperative and postoperative Harris, the multiple linear regression analysis showed that there was a positive correlation between the level of vitamin D and Harris score both preoperatively and postoperatively (P<0.05). (4) These results suggest that there is a higher incidence of low level of vitamin D in patients undergoing arthroplasty, and hip function scores before and after replacement in patients with low level of vitamin D are lower than the high level patients. Moreover, there is a positive correlation between the level of vitamin D and the hip joint function scores. Therefore, it is advisable to supplement vitamin D and calcium preoperatively, and the level of vitamin D wil be helpful for disease assessment and prognosis.
6.An Experimental Study on the Early Changes in ~(18)F-FLT and ~(18)F-FDG Uptake of Tumor Cells after Chemotherapy

Purpose:The efficacy of evaluation of changes of tumoral uptake of 3'-deoxy-3'-[~(18)F] fluorothymidine (FLT) was comparatively analyzed with that of ~(18)F-FDG at early stage after anticancer chemotherapy.Materials and Methods:Cells derived from human lung adenocarcinoma were incubated with cisplatin (CDDP),5-fluorouracil(5-FU),doxorubicin (Dox),for 1,4,24 and 72h.The doses(CD-DP: 67 μM; 5-FU 1,540 μM;MTX: 440 μM;) were determined corresponding to a estimated 10% - 95% proliferation inhibition.The cells were allowed to recover before FLT or FDG being added into the culture media for 60 min.Cell counts,viability,estimated by MTT method,were used to evaluate the cytotoxic effects of chemotherapy.Results: FLT uptake was increased significantly at 1 and 4 h after treatment with 5-FU( 145 ± 12%,150 ± 14%,P <0.01).decreased at 24 h and 72 h.In contrast,FLT accumulation was significantly reduced at cytostatic concentrations of CDDP at different time.The uptake of FDG did not change significantly at early time points after treatment,but decreased at 72 h.Conclusion: The tumor cell uptake of FLT revealed specific changes depending on the auti-cancer drug used at much earlier time than FDG after chemotherpay.
7.Twenty-six clinical case analysis of defect repairing after the resection of basal cell carcinoma on external nose.

OBJECTIVE:
To study a surgical and repairing method for defects after the resection of basal cell carcinoma of external nose.
METHOD:
There are 26 cases of basal cell carcinoma that tumors have been resected completely after operation. For defect repairing in those 26 cases, 2 cases adopt direct suture method; 2 cases use skin graft repairing methods; 18 cases employ naselabial skin flap repairing method; 4 cases choose forehead pedicle skin flap repairing methods.
RESULT:
the wound of all the 26 cases was primary healed. Additionally, skin flap and skin graft were all survived. Follow-up studied in patients 1 to 3 years after the surgery showed that the local scar was not obvious and no tumor recurred or transferred.
CONCLUSION
Different surgical and repairing methods are performed to obtain a satisfactory results based on the area of defect and its location in nose. Naselabial skin flap is especially an ideal method to repair defects.

Objective Using polysulfon fibers, a new bioartificial liver was developed. This study was to evaluate the efficacy of this bioartificial liver in the support of a disfunctioned liver. Methods Hepatocytes were procured from swine using Seglen′s methods. OBJECTIVE:
To determine the role of fragile histidine triad (FHIT) and MDM2 in carcinogenesis of oral submucous fibrosis (OSF).
METHODS:
The expression of FHIT and MDM2 was examined by immunohistochemical S-P method in 44 OSF cases, 15 canceration tissues of OSF, and 10 normal oral mucosa tissues.
RESULTS:
The expression of FHIT was positive in the normal oral mucosa epithelium. The positive expression of FHIT decreased in the OSF and canceration tissues of the OSF.The rate of FHIT positive expression was significantly lower in canceration tissues of OSF than that of the OSF (P < 0.05). The expression of MDM2 was negative in normal oral mucosa epithelium. The positive expression of MDM2 increased in the OSF and canceration tissues of the OSF, and the rate of MDM2 positive expression was significantly higher in the canceration tissues of OSF than that of the OSF (P < 0.05).
CONCLUSION
The loss of FHIT and over-expression of MDM2 may play an important role in the carcinogenesis of OSF.
