1.Clinical and MRI findings in children with transverse myelitis

Objective To explore the clinical, MRI features and relation of them in children with transverse myelitis. Methods We analyse retrospectively the clinical and MRI features of 10 patients with transverse myelitis. Results 6 patients with prodromal period had lower limbs and quadriplegia. Most of them had dysfunction of superficial sense and sphincter, and the former was light and recovered quickly. The T2WI in all 10 patients shows high intensity, with blurry border. Only 4 patients of them had light augmentation on spinal cord. We found no relationship of the traumatic range or augmentation of spinal cord and the prognosis (P0.05). Conclusion In children with transverse myelitis, the T2WI is the most sensitive, credible and differential non-traumatic examinational procedure. It seems that children patients have different features compared to adults. For children patients, they have slight sensory disturbance and recover quickly. Not all patients show augmentation on spinal cord. It may has the South North regional difference.
2.Reconstruction local defect after subtotal or extended resection of temporal bone

Objective To explore the best way for reconstruction of local defect after subtotal or extended resection of the temporal bone. Method The reconstruction of 33 patients who had undergone subtotal or extended resection of temporal bone between July 1993 to June 2003 in our hospital were retrospectively analysed. Results Thirty-three patients, aged 14-69 years old (average 45.0) were enrolled. Among them, 26 were suffering from malignancy (12 with recurrences, undergoing several operations). Subtotal resection of temporal bone was performed in 22 cases and extended resection in 11. Total survival rate of reconstructive tissues was 81.8%(27/33), in which pectoralis major island musculocutaneous flap accounted for 77.8%(7/9), temporalis musculocutaneous flap survived 100% (9/9), pedicled temporalis musculocutaneous flap associated with other local musculocutaneous flap survived 100%(4/4), 60.0% (3/5) of upper sternocleidomastoid musculocutaneous flap survived, and 66.7% (4/6) of fat transplantation survived. Nature and extent of the diseases, preoperative radiotherapy, relapse and postoperative leakage of cerebrospinal fluid were important influential factors for postoperative healing. Conclusions Reconstructive measures depended on the nature and extent of diseases, necessary size of tissue, local circulation, and influential factors of healing. The optimum way for the repair of a small defect after resection of non-malignant tumor was fat transplantation with dermis, and pedicled temporalis musculocutaneous flap for malignant tumors. Pectoralis major island musculocutaneous flap was employed in large defect and that with lack of blood supply. Skin grafting met several failures in these patients. Theoretically, free scapular skin flap is the optimum way for defects after subtotal or extended resection of temporal bone.

To investigate the role of fibrinolysis in adhesion formation in the middle ear,40 patients(45 ears) with otitis media with exudation (OME) were randomly divided into 3 groups:Dexamethasone(G1),Batroxobin1BU/ml(G2),and Batroxobin 2BU/ml(G3).The results were assessed by tympanotomy and irigation to compare the efficacy of treatment of batroxobin and dexamethasone.31 patients (33 ears) were analyzed for statistical significance.The efficacy of the treatment was evaluated by qualitative analysis of symptom and the Hearing Threshold of Air conduction of pure tone audiometry (AHT) at frequencies 0 25~2kHz,and by quantitative analysis of AHT.Cure rates of G1,G2,G3 were 41 7%(5/12),54 5%(6/11),70%(7/10) respectively;G3 was higher than G1 and G2( P 0 05). In either type 1 or type 2 diabetes,there is significant loss of β cell mass.Understanding the changing of β cell mass in the course of diabetes would provide important information for diagnosis and treatment.Functional imaging like magnetic resonance imaging or positron emission tomography (PET) can provide safe and noninvasive detection of loss of β cell mass in the course of diabetes.Among them,radio-labelled imaging is the most sensitive imaging procedure of the β cell ; For dihydrotetrabenazine PET imaging aiming β cell mass,there has been some primary outcome.For the key factor causing type 2 diabetes,18 F-6-deoxy-6-fluoro-D-glucose (18F-6-FDG) PET imaging is an effective tracer to study the glucose transporting condition in vivo.Further development of functional imaging will be of great value in diagnosis and treatment of diabetes.
7.Treatment of tracheobronchial tuberculosis by bronchoscopic intervention

Objective To discuss the clinical value of electronic bronchoscopic intervention in treatment of tracheobronchial tuberculosis. Methods Clinical features of 45 patients with tracheal and bronchial tuberculosis which were confirmed by electronic bronchoscope and treated by bronchoscopic intervention were retrospectively analyzed from January 2007 to December 2013 in our hospital. Results The efficiency of bronchoscopic intervention is 88.9%, of which 28 cases achieved a significant effect (accounting for 62.2%). Conclusion Electronic bronchoscopic intervention is a preferred way in treatment of tracheobronchial tuberculosis.
8.Study on Composition of Drug Property of Hot-natured Herb Euodiae Fructus

Objective To study the material basis of hot-natured herb Euodiae Fructus. Methods Euodiae Fructus extract was separated into the chloroform fraction, ethyl acetate fraction, n-butanol fraction and water fraction. With cold syndrome rat model by reserpine, the study confirmed that n-butanol fraction of Euodiae Fructus has influence on energy metabolism in rats. With the modified magnetic polymer microspheres, Euodiae Fructus n-butanol was divided into alkaloids, phenolic acid, fat soluble and water-soluble components, and the component of Euodiae Fructus influencing energy metabolism of model rats was further confirmed.Results The Euodiae Fructus extracts and alkaloids elevated rats temperature, reduced the Na+-K+-ATP activity, increased SDH activity, the content of ATP in liver tissue, the energy charge (EC) value and body heat production, and improve the level of energy metabolism.Conclusion The alkaloid is the material basis of hot-natured herb Euodiae Fructus.
9.Recent advances in the study of accelerated blood clearance phenomenon of PEGylated liposomes

It is generally believed that liposomes modified with polyethylene glycol (PEG) have no or lower immunogenicity. However, based on many recent literatures, when the PEGylated liposomes were repeatedly applied to the same animal, the immune responses occurred. The first injection of PEGylated liposomes resulted in a reduction in the circulation time and an increase in hepatic and splenic accumulation of the second dose of PEGylated liposomes in a time-interval, which was called "accelerated blood clearance (ABC)" phenomenon. Such immunogenicity of PEGylated liposomes presents a barrier in the research of liposomal formulations and their use in the clinics. This review focused on the definition, the method of verification, the development of the reason for ABC phenomenon, influencing factors of ABC phenomenon, and discussed if other PEGylated nanocarriers also induce ABC phenomenon.
10.Analysis on the Influencing Factors of Overdrinking

Objective To provide useful information on reducing risky drinking for the Dept.of Public Health taking a county in Yunnan province as an example to explore the risk factors related to risky drinking.Methods A total of 574 participants identified as Miao,Yi and Han People at or older than 12 years old,who had been living there for 6 months or more were selected and surveyed in 5 townships in the county through stratified sampling.Alcohol consumption was collected using the beverage-specific quantity frequency (BSQF) method and analyzed using binary logistic regression and chi-square test.Results The number of drinkers in the family,undesirable drinking environment among friends and enculturation (β =0.073) could increase the likelihood of risky drinking.Risky drinking pattern was positively associated with age (β =1.006),and negatively associated with gender (β =-2.947) and awareness of risky drinking among rural residents.Conclusion The important measures to control risky drinking behavior in terms of health promotion and education include reducing the number of drinkers in the family,and preventing from undesirable drinking environment among friends and harmful drinking culture.These measures are crucial to prevent early onset of alcohol drinking among teenagers and to reduce risky drinking among middle aged and aged population.
