1.Effect of Standard Management for Hypertension in Community Population of Suzhou City

Objective: To evaluate the effect of standard management for hypertension in community population of Suzhou city. Methods: We conducted a prospective study of standard management for hypertension in community population of Suzhou city. The project included 3 groups: Standard group, the patients received standardized management,n=81 711, Control group, the patients received general management, n=38 944, and No-management group,n=1 644 249. The incidence rate of stroke and coronary artery disease (CAD) were compared among different groups according to 2010 chronic disease report of Suzhou city. Results: Compared with initial condition, the final average blood pressure drop in Standard group was (13.78/7.06) mmHg, which was better than that in Control group, X2=1852.91,P<0.01. The incidence rate of stroke in Standard group, Control group and No-management group were 1.72%, 2.41% and 3.59%, the incidence rate of CAD were 0.43%, 0.66% and 1.29% respectively. Conclusion: Standard management for hypertension was better than general management for preventing and controlling the incidence of stroke and CAD in community population.
2.Unicompartmental knee arthroplasty for single compartmental knee disease

[Objective] To analyze the cases performed by unicompartmental knee arthroplasty in order to assess the early outcome,and to discuss the indications and the key points of surgical technique.[Methods]Under the principle of "slight undercorrection",15 patients were performed by unicompartmental knee arthoplasty with the technique of limited medial release if necessary.The HSS score,the fomeral tibial angle and the maxima degree of flexion and extension measured preoperatively and in the latest follow-up were analyzed by SPSS 13.0 with the method of paired t test.Meanwhile the angle of the posterior slope of the tibial implants was measured.[Results]Postoperatively the HSS score,the fomeral tibial angle and the maxima degree of flexion and extension were improved significantly in the latest follow-up,and the average angle of the posterior slope of the tibial implants was 4.2 degrees.[Conclusion]Unicompartmental knee arthroplasty has a satisfing early outcome.And it is very important to carry out the indications strictly.With the surgical technique of limited medial release,the flexional contracture could also be improved significantly.
3.Treatment of bony defects in revision total hip arthroplasty with allograft grafting:midterm follow-up study

[Objective]To explore the intermediate result of the use of allograft grafting for bone defects in revision total hip arthroplasty.[Methods]A retrospective study was carried out on 22 patients(22 hips) with the use of allograft grafting for bone defects in revision of total hip arthroplasty from April 2000 to April 2005.There were 13 male and 9 female with mean age of 65.5 years.According to the AAOS system,the bone defects in acetabulum were classified into type I in 5 hips,type II 14 hips and type III 3 hips,and on the femoral side,they were classified into type I in 3 hips,type II 12 hips and type III 2 hips.The reconstruction of bone defects in acetabulum contained structural bone graft in 5 hips,impaction bone graft in 12 hips,and metal wire mesh with bone graft in 5 hips.On the femoral side,3 hips were dealed with structural bone graft,12 hips were done with impaction bone graft,and 2 hips were repaired by metal wire mesh with bone graft.Harris Hip Score System and radiography were used to evaluate the clinical efficiency.[Results]The mean follow-up time was 61 months(37~97 months).Harris score improved from 45 points(30~65) pre-revision to 86 points(62~96) at the final follow-up.Insion drainage occurred in one hip.One patient remain slight pain in hip.No prothesis loosening,radiolucent lines were observed during the follow-up.None need to second revision.[Conclusion]During the THA revision surgery,adopting suitable allografts to repair the different types of bone defects is feasible and the mid-term result is good.
4.A Small Anterior Incision Applied to Single Lumbar Segment Artificial Disc Replacement

Objectives To introduce a small incision of anterior extraperitoneal approach to the lumbar spine for single lumbar segment artificial disc replacement. Methods Nine patients suffered from single segment herniation of lumbar intervertebral disc underwent operation of artificial disc replacement. The approach of this operation was with a small incision through medial of left sheath of rectums and access to the retroperitoneal region through extraperitoneal appearance. The clinical and surgical data including the length of incision, length of operation, blood lose, effect and complication of operation were analyzed retrospectively in these 9 patients.Results The average length of incision,time of operation and blood lose was 6 7cm, 150 minutes and 335ml, respectively. The clinical evaluation scales (the 5-point Likert scales for pain, function, economic status, and medication usage) showed great improvement after operation of these patients(P=0 008). No serious complication related with the approach was found. Conclusions The a small anterior incision through peritoneum of left rectus abdominis sheath and access to the retroperitoneal region through extraperitoneal, which can expose the region of lumbar segment from L3 to S1 sufficiently, is a safe and mini-lesion approach for single lumbar segment artificial disc replacement.
5.Surgery of hip fracture in elderly patients aged equal or greater than 80 years old

Objective To explore the outcomes of surgery of hip fracture in elderly pafienta aged equal or greater than 80 years old.Method The data of totally 132 patients with hip fracture aged equal or greater than 80 years old were treated with surgery between January 2004 and December 2008 was analyzed retrospectively Results Of 132 cases,68 cases were femoral neck fracture,while 64 cases were intertroehanteric fracture.The average in.hospital time waft(17.2±4.6)d,and 2 cases died of cerebral embolism and respiratory dysfunction in perioperative period respectively.One hundred and eight cases were followed up for 6 months to 5 yeats,with an average of 3.2 years.According to Harris hip scores,the average score of femoral neck fracture at 1 year and at 2 years postoperatively were(77.6±10.2),(77.3±9.3)scores,respectively;while intertrochantefic fracture at 1 year and at 2 years postoperatively were(84.6±9.8),(83.9±10.1)scores,respectively.Conclusions Aged patients with hip fracture should receivesurgical treatment as early as possible.The reasonable perioperative treatment and operative method are the keys to gain good outcomes.
6.Evaluation of molluscicidal effect of screening snails in soil layer

Both the snail number and the detection rate of living snails by means of snail screening in soil layer were higher than those of snail survey on soil surface. It is indicated that snail screening in soil layer can make up the shortage of survey on soil surface and improve the efficiency of snail control.
7.Shoulder hemiarthroplasty in treatment of severe comminuted intraarticular fractures of the proximal humerus(31 cases report)

Objective To evaluate retrospectively the hemiarthroplasty in the treatment of severe comminuted intraarticular fractures of the proximal humerus.Method Thirty one patients suffered from severe comminuted intraarticular fractures of the proximal humerus were treated with shoulder hemiarthroplasty through Thompson approach from March 2000 to October 2004.The results were graded into the excellent,the good,the fair and the poor according to the scoring system-modification for hemiarthroplasty(SSMH) of Los Angeles California,USA(UCLA) basing on shoulder paining,functional status muscle strength and range of motion.Rusults The average operative time cost 65 minutes(ranged from 40 to 90 minutes),average amount of bleeding during operation were 280 ml(ranged from 200 to 350 ml).The patients exercised early postoperatively.After an average duration of 14.1 months of follow-up(ranging from 5 to 42 months),there were no infection and/or nerve damage occurred postoperatively,and the position of the prosthesis was confirmed to be excellent,moreover no lessening prosthesis,prosthesis dislocation,and/or prosthesis milieu fractures occurred radiographically.The outcomes of the treatment were excellent in 13,good in 14,fair in 3 and poor in 1 out of the 31 cases. The rate of both the excellent and the good were 87.10%.Conclusion Shoulder hemiarthroplasty is a preferable technique in the treatment of severe comminuted intraarticular fractures of the proximal humerus.
8.Experiment of Oncomelania hupensis snail reproduction and susceptibility in schistosomiasis non-endemic area in Southern Jiangsu

Objective To observe the survival and reproduction of Oncomelania hupensis snails and their susceptibility to schistosome in schistosomiasis non-endemic area of the Yangtze River estuary in Southern Jiangsu.Methods The soil and water from the Yangtze River estuary of Southern Jiangsu were used for the experiment.The snails reproduced in the same year were collected from the upper reaches of the Yangtze River and raised in the laboratory.The snail survival and reproduction rates and schistosome infection of the snails were observed.The soil collected from schistosomiasis endemic area was used in the control group.Results There was no significant difference between the experimental and control groups for the snail survival rates both in 6 and 12 months (X~2= 0.727 8,P > 0.05 and X~2 = 0.416 1,P > 0.05).Each female snail reproduced 67.69 eggs in average (95 % CI:24.026 0-110.097 4).The average hatchability rate of snail eggs was 83.60%,and there was no significant difference between the 2 groups (X~2= 9.131 8,P > 0.05).The schistosome infection rate of the second generation snails was 1.40% (5/356) 60 days after the infection in the laboratory.Conclusions The snails collected from the upper reaches of the Yangtze River marshland can survival and reproduce in the soil and water from schistosomiasis non-endemic area of the Yangtze River estuary of Southern Jiangsu,and the second generation of the snails can be infected with schistosome in the laboratory.
9.Effect of comprehensive care for chronic filariasis patients with lymphedema

Objective To evaluate the effect of chronic filariasis patients with lymphedema after comprehensive cared. Meth?ods A total of 386 chronic filariasis patients with lymphedema received the comprehensive care including soaking feet by TCM washing feet by single Chinese medicine or clear water preventing and eliminating invasive wound physical training raising the limb and wearing suitable shoes. The attack frequency of inflammation of lymphatic vessels the stage of lymphedema dis?ease and leg circumference were observed before and after the care. Results After the comprehensive care the attack rates of inflammation of lymphatic vessels decreased from 9.65%to 4.67% and there was a significant difference χ 2=7.34 P=0.006 7 . The proportion of the patients with high stage of lymphedema disease decreased significantly all P 0.01 and the average leg circumference decreased of 1.06 cm. The skin appearance improved significantly. Other signs such as pitting edema ulcer and skin folds also improved significantly all P 0.01 . Conclusion The comprehensive care for chronic filariasis patients with lymphedema has a remarkable effect and is worthy of further application.
10.Curative effects of unipedicular and bipedicular vertebroplasty in treating osteoporotic vertebral compression fractures in the elderly population

Objective To investigate the clinical effect of the treatment of osteoporotic vertebral compression fractures in the elderly populationthrough different surgical approaches.Methods 98 cases with a single-level osteoporotic vertebral compression fracture in the elderly population were randomly divided into two groups from February 201 1 to June 2013.48 patients were performed by percutaneous vertebroplasty (PVP)through unipedicular approach and 50 patients through bipedicular approachs.The clinical data of patients were prospectively analyzed and the clinical efficacy were compared between two groups using VAS (visual analogue scale method)and ODI (Oswestry disability index)in preoperative,postoperative 1 day and 1 year postoperatively .The data of age, gender,injury to the patients with operation time,postoperative follow-up time,operation time,bone cement injection,bone cement leakage and other complications were observed.Cobb angle,vertebral compression ration were observed by imaging data.Results All the cases were followed-up.There was no statistical difference in preoperative clinical data between the two groups (P >0.05).In unilateral group (48 cases),the data of operation time,bone cement injection,bone cement leakage,Cobb angle improve,vertebral compression ration improve were (34.87±5.91)min,(6.20±0.66)mL,1 6 cases(33%),(10.1 9±2.12)%,(13.23°±1.58°)and adjacent vertebral fractures was 10 cases (20.9%).VAS score was respectively improved (4.05 ± 0.12 ),(5.42 ± 0.12 ) in postoperative 1 day and 1 year than preoperative.VAS score was improved (1.40 ±0.1 1 )in postoperative 1 year than 1 day.ODI score was respectively improved (35.46 ± 1.89)%,(47.88 ±2.21 )% in postoperative 1 day and 1 year than preoperative.ODI score was improved (1 1.42±0.24)% in postoperative 1 year than 1 day.In bilateral group (50 cases).The data of operation time,bone cement injection,bone cement leakage,Cobb angle improve, vertebral compression ration improve were (41.66±6.90)min,(4.88±0.52)mL,9 cases(18.0%),(10.48±1.43)%,(13.04°±2.03°)and adjacent vertebral fractures was 6 cases(12.0%).VAS score was respectively improved (4.06±0.1 1),(5.30±0.10)in postoperative 1 day and 1 year than preoperative.VAS score was improved (1.34± 0.08)in postoperative 1 year than 1 day.ODI score was respectively improved (36.08±2.13)%,(47.54±1.97)%in postoperative 1 day and 1 year than preoperative.ODI score was improved (1 1.26 ± 0.54)% in postoperative 1 year than 1 day.There was no obvious clinical problems after occurred leakage in two groups.there was statistical difference in cement injection,bone cement leakage and postoperative adjacent vertebral fractures after operation between the two groups.there was no statistical difference in Cobb angle improve,vertebral compression ration improve,VAS score and ODI score between the two groups.Conclusion Both approaches are effective in the treatment of osteoporotic vertebral compression fractures in the elderly population ,but there is advantage of decrease the incidence of bone cement leakage and postoperative adjacent vertebral fractures through bilateral approach.
