1.Simultaneous Determination of 4 Components in Compound Metronidazole Clindamycin Cream by HPLC

OBJECTIVE:To establish the method for simultaneous determination of the content of metronidazole,clindamycin hydrochloride,spironolactone and vitamin B6 in Compound metronidazole clindamycin cream.METHODS:HPLC method was adopted.The determination was performed on Diamonsil C18 column with mobile phase consisted of acetonitrile-[0.68% dipotassium hydrogen phosphate solution (containing 1% triethylamine)] (gradient elution) at the flow rate of 1.0 mL/min.The detection wavelength was set at 210 nm,and column temperature was 25 ℃.The sample size was 20 μL.RESULTS:The linear ranges of metronidazole,clindamycin hydrochloride,spironolactone and vitamin B8 were respectively 20.0-120.0 i.tg/mL(r=0.999 9),10.0-60.0μg/mL(r=0.999 1),4.0-24.0 μg/mL(r=0.999 4),10.0-60.0 μg/mL (r=0.999 4).The quantitative limits were respectively 13.06,7.36,2.43,6.42 μg/mL.The detection limits were 4.18,1.82,0.76,1.79 μg/mL.RSDs of the precision,stability,repeatability tests were all less than 2%.The recovery rates were respectively 98.97%-100.91% (RSD=0.79%,n=9),99.02%-101.80% (RSD=1.03%,n=9),98.18%-101.05% (RSD=1.01%,n=9) and 98.75%-101.36% (RSD=0.87%,n=9).CONCLUSIONS:The method is simple,rapid,accurate and can be used for the determination of metronidazole,clindamycin hydrochloride,spironolactone and vitamin B6 in Compound clindamycin metronidazole cream.

3.Stability of Dexamethasone Sodium Phosphate Eye Drops

OBJECTIVE: To study the stability of dexamethasone sodium phosphate(DSP) eye drops and predict its validity duration.METHODS: The content of DSP in eye drops was determined by HPLC and its validity duration was predi-cated using initial average rate method.RESULTS: The linear concentration range of DSP was 18~70 ?g?mL-1(r=0.999 9) with an average recovery rate of 99.74%(RSD=0.37%).The content of DSP in the eye drops decreased as its storing temperature increased and the storing time prolonged,but its pH was stable. The validity duration of the eye drops was computed to be 70 days at 25 ℃ and 438 days at 4 ℃.CONCLUSION: This eye drops were stable at 4 ℃.
4.Clinical research in prevention and cure effect of anisodamine gel on phlebitis caused by intra-venous infusion of KLT

Objective To investigate the prevention and cure effect of anisodamine gel on phlebitis caused by intravenous infusion of KLT. Methods 96 patients with intravenous infusion of KLT were divided into the experimental group and the control group, 48 patients in each group, and the skin at the top of transfusion puncture points of the patients of the experimental group was evenly embrocated with 2% anisodamine gel along the vein before 3 to 5 minutes of intravenous infusion of KLT, while the control group was without any treatment. After observing the situation of venous injury, the skin at the top of transfusion puncture points of the positive patients would be embrocated with 2% anisodamine gel. The incidence, degree of phlebitis and treatment effect were compared between the two groups. Results The incidence rate of phlebitis of the control group was 68.7%, of which grade II, III occupied 60.4%, while the incidence rateof phlebitis of the experimental group was 10.4%, in which there was no occurrence of grade II, III phlebitis. All the venous injury symptoms of 33 patients of the control group and 5 patients of the experi -mental group disappeared after treatment, and after statistical processing, the incidence rate of phlebitis in the experimental group and the control group showed significant differences. Conclusions The embrocating of 2% anisodamine gel can effectively avoid the occurrence of the phlebitis caused by intravenous infusion of KLT.
5.Research in prevention and treatment of venous injury caused by intravenous infusion of Delisheng with anisodamine gel

Objective To investigate the effect of prevention and treatment of venous injury caused by intravenous infusion of Delisheng with anisodamine gel.Methods 100 patients with intravenous infusion of Delisheng were randomly divided into the experimental group and the control group,50 patients in each group,and the skin at the top of transfusion puncture points of the patients of the experimental group were evenly embrocated with 2% anisodamine gel along the vein before 3 to 5 minutes of intravenous infusion of Delisheng,while the control group received no treatment.After observing the situation of venous injury,the skin at the top of transfusion puncture points of the positive patients was embrocated with 2% anisodamine gel,and the effect on venous injury was observed.Results The incidence rate of intravenous pain of the experimental group was 4%,there wasn't any occurrence of poor intravenous infusion or swelling of the infusion position.The incidence rate of poor intravenous infusion of the control group was 34% and phlebitis incidence rate was 42%,and intravenous pain incidence rate was 68%.All the venous injury symptoms of 39 patients of the control group and 2 patients of the experimental group disappeared after treatment.Conclusions The embrocating of 2% anisodamine gel can effectively prevent and treat the venous injury caused by intravenous infusion of Delisheng.
6.Crisis management in immediate response to emergent public health event in China

Crisis is an emergent event that threatens the national security, social regulation, civilian property and life, and needs urgent, decisive response in the condition of high instability. Crisis management should be prompt, active, honest, responsible and soluble, and should be carried out according to the model of preparation, reduction, response and recovery. SARS outbreak in China revealed constitutive defects in the immediate response for emergent public health event as well as crisis management. This paper provides some information for establishing the immediate response system for emergent public health event and crisis management.
7.Application of vulnerability appraising in constructing theoretical model for early warning of emergent public health event

Objective:To strengthen the research on theory and method of early warning for emergent public health event,so as to obtain timely information,correctly analyze the situation,and apply effective measures when the event happens.Methods: The definition of vulnerability of emergent public health event and the vulnerability factors were discussed.The indicator system and the evaluating method for public health vulnerability were systematically studied.The extenics theory and the theory of vulnerability evaluation were used for warning exercise.Results: We constructed an extenics model for early warning of emergent public health event;the model was used to evaluate the public health of a population.Conclusion: The theory of vulnerability evaluation is introduced in emergent public health event to construct an extenics model for early warning of emergent public health event;the model can be used to evaluate the public health of certain population.
8.Determination and analysis of amnesic shellfish poisoning by mass spectrometry

BACKGROUND: Amnesic shellfish poisoning(ASP) is caused by consumption of seafood contaminated by marine neurotoxins, mainly domoic acid (DA).OBJECTIVE: To ensure the safety of shellfish consumption by determining DA in shellfish by means of liquid chromatography(LC) combined with quadrupole time-of-flight mass spectrometry(Q-TOF MS).DESIGN: A self-controlled prospective study SETTING: The experiment was carried out in the Center for Animal, Plant and Food Inspection and Quarantine, Shanghai Exit-Entry Inspection and Quarantine Bureau.MATERIALS: MUS-1B, a nominal sample of homogenized mussel tissue (containing 36 ± 1μg DA per gram, Batch No. MUS-1B 0159) and the standard DA solution (100 mg/L, Batch No. DACS-1C 352) were purchased from National Research Council of Canada (Halifax, Canada). Positive musscl samples were collected from the waters during the occurrence of red tide in China East Sea, and blank mussel samples were obtained from local seafood market. LC system (Waters 2695) combined with Q-TOF MS (Qultima, Micromass) and ultravilet (UV) detection (Waters 2487 ) was employed for analysis of ASP toxins.METHODS: The shellfish tissue samples were extracted with methanol-water (1: 1, V/V) and cleansed with a SAX solid-phase extraction cartridge. Separation was carried out on a Zorbax Eclipse XDB-18 column (250×2.1 mm, 3.5μm) with gradient elution of CAN-H20 containing 0.05% formic acid at a flow rate of 0.20 mL per minute. In a positive electrospray ionization DA was ionized effectively, and collision-induced dissociation(CID) produced predominant characteristic ions[M + H] +allowing sensitive TOF MS detection.MAIN OUTCOME MEASURES: ① Limit of quantification(LOQ); ②Accuracy and precision of LC/Q-TOF MS method.RESULTS: The average recoveries of DA spiked to tissue homogenates through the complete cleanse procedure ranged from 87.8% to 94.6%, and the relative standard deviations(RSD) ranged from 8.4% to 11.9%. TheLOQ in the shellfish tissue was 0.1 μg/g.CONCLUSION: LC/Q-TOF MS suits well the purpose of detecting and identifying ASP toxins in shellfish with high sensitivity and specificity, and may provide reference values for monitoring the safety of consumption of shellfish.
9.Risk factors of neovascular glaucoma after pars plana vitrectomy for proliferative diabetic retinopathy

Objective:To analyze the risk factors of neovascular glaucoma (NVG) after 25G pars plana vitrectomy (PPV) in proliferative diabetic retinopathy (PDR).Methods:A retrospective study. From January 2017 to December 2018, 340 PDR patients (340 eyes) with vitreous hemorrhage (VH) who were first treated with PPV in Tianjin Medical University Eye Hospital were included in the study. Among them, 185 were male and 155 were female, with an average age of 55.79±10.82 years. The duration of diabetes was 13.01±7.70 years, the fasting blood glucose was 7.55±2.15 mmol/L. Nineteen patients combined coronary heart disease, and 20 patients combined cerebral infarction. All patients underwent best-corrected visual acuity (BCVA), intraocular pressure (IOP), non-contact fundus examination, and fundus color photographs. BCVA was measured using an international standard Snellen visual acuity chart, and the values were converted to logarithm of the minimum angle of resolution (logMAR) scores for data analysis. The baseline logMAR BCVA was 2.04±0.73, The baseline IOP was 15.45±2.93 mmHg (1 mmHg=0.133 kPa). The duration of VH was 2.98±1.46 months, ranged from 3 weeks to 6 months. Three hundred and forty eyes included 93 eyes of PDR Ⅳ stage (27.35%), 107 eyes of Ⅴ stage (31.47%), and 116 eyes of Ⅵ stage (34.12%), combined tractional retinal detachment (TRD) 83 eyes. All patients underwent 25G standard three channel vitrectomy through the pars plana of the ciliary body. Preoperative anti-VEGF injection was performed in 57 eyes, internal limiting membrane (ILM) peeling in 234 eyes, combined phacoemulsification cataract surgery in 262 eyes and 141 eyes intravitreal anti-VEGF injection at the end of surgery. The patients were followed up for at least 12 months, with an average follow-up time of 10.80±5.79 months. NVG was defined as the presence of neovascularization in the anterior chamber angle or iris with an IOP higher than 21 mmHg after vitrectomy. Kaplan-Meier method and Cox univariate and multivariate regression were used to analyze the relationship between baseline factors, ocular factors, surgical factors and the occurrence of NVG after surgery.Results:Among 340 eyes, 66 eyes (19.41%) developed NVG after vitrectomy during 12 months of observation, NVG occurred from 6 to 335 days after surgery, and the mean period between vitrectomy and developing NVG was 98.00±5.79 days. The incidence of NVG was 11.50%, 15.29% and 20.75%, respectively in the 3rd, 6th and 12th month after PPV. The result of univariate analysis with the Cox regression analysis showed that the development of NVG at 12 months after surgery and age, combined coronary heart disease or cerebral infarction, combined with cataract phacoemulsification, ILM peeling, preoperative anti-VEGF injection had effect on postoperative NVG ( P<0.05). Ocular factors such as PDR staging, combined TRD, preoperative logMAR BCVA, preoperative intraocular pressure, etc. had no effect on the occurrence of NVG after surgery ( P>0.05). Combined cataract phacoemulsification surgery, internal limiting membrane peeling, surgical factors such as intracavity injection of anti-VEGF drugs 3 days before surgery, had an impact on the occurrence of NVG after surgery ( P<0.05). The meaningful variables of the Cox univariate analysis were incorporated into the multivariate Cox proportional hazard model for analysis, and the influencing factors of NVG after surgery were gradually regressed. The results showed that age, coronary heart disease or cerebral infarction, combined with phacoemulsification of cataract, and internal limiting membrane removal during surgery were independent risk predictors of NVG after surgery ( P<0.05). Conclusions:Younger, coronary heart disease or cerebral infarction, combined with cataract phacoemulsification are the risk factors of NVG in PDR patients after PPV. The removal of internal limiting membrane can reduce the incidence of NVG.
10.Rapid and undamage determination of fruit of Eucalyptus globules labill by near infrared spectroscopy and clustering analysis

AIM: To provide a rapid method for identifying fruit of Eucalyptus globule labill. METHODS: A new method, using NIRS for rapid and nondestructive classfication of the fruit of Eucalyptus globules labill and its counterfeit drug, the fruit of Eucalyptus robusta Smith, was proposed and then cluster analysis was adopted. RESULTS: The result showed that two kinds of fruits had their own characteristic NIRS spectra. CONCLUSION: The method is quick, simple and reliable, and in combination of the fingerprint NIRS spectrum and the clustering analysis is easy to identify the adulteration.
