1.Vascularization of methl-methacrylate modified polymerized 2-hydroxyethyl methacryate hydrogel orbital implant

BACKGROUND: The application of polymerized 2-hydroxyethyl methacryate (PHEMA) in many organs of living body shows good biocompatibility.OBJECTIVE: To observe the biocompatibility and fibrovascular condition of methl-methacrylate (MMA) modified PHEMA hydrogel orbital implant.DESIGN: Single sample observation.SETTING: Department of Ophthalmology, the General Hospital of Chinese PLA; Institute of Polymer Science,Department of Chemical Engineering, Tinghua university.MATERIALS: Twenty-five New Zealand rabbits of either gender and clean grade, weighing 2.0 to 2.5 kg, were provided by the Animal Experimental Center, General Hospital of Chinese PLA. The animals were observed at postoperative 2, 4,8, 12 and 24 weeks, 5 at each time point. The experiment was permitted by ethics committee. MMA modified PHEMA hydrogel orbital implant was provided by Institute of Polymer Research, Department of Chemical Engineering, Tinghua University. The experiment was permitted by ethics committee.METHODS: This experiment was carried out in the laboratory of Department of Ophthalmology, the General Hospital of Chinese PLA from March to October 2003. The right eye served as operation eye. When the rabbits were anesthetized,their eyelids were open with eyelid retractor. Bulbar conjunctiva was open along the edge of limbus of cornea for isolating fascia. Oculorotatory muscle was isolated and pre-set suture line was made, then muscle was cut off. When optical nerve was cut off, eyeball was completely excised. A MMA modified PHEMA hydrogel orbital implant with the diameter of 14 mm was implanted into the fossa orbitalis of rabbits. Single photon emission computed tomography (SPECT) was performed at postoperative 2, 4, 8, 12 and 24 weeks. The hydrogel orbital implant was taken out at postoperative 2, 4, 8,12 and 24 weeks for light microscope, immunohistochemical and electron microscope examinations.MAIN OUTCOME MEASURES: Vascularization of hydrogel orbital implant was observed in vivo by isotope tracing observation at different time points. Vascularization of hydrogel orbital implant was observed ex vivo by light microscope,immunohistochemical and electron microscope observations.RESULTS: All 25 New Zealand rabbits were involved in the final analysis. ①SPECT examination showed that isotope increased around the implants at week 2, and extended gradually at weeks 4 and 8. The radiotracer accumulated evenly in the implants from surrounding to the center at week 12. ②Light microscopic examination of the prostheses revealed fiber vessels grew from the surrounding to the center. At week 2,fibrovascular tissue grew in the hole of implant. Long or round cells with deeply dyed nucleuses were found in the hole, being fibroblasts. The vascular structure in the hole of the implant was clear, and vascular wall and blood cells were found. The inflammatory cells were mainly neutrophils and lymphocytes, and occasionally large macrophages. ③Scanning electron microscope showed that fibrovascular tissue grew in the hole of the implant, and was dense and intercrossed at week 12. ④ Conjunctiva split appeared postoperatively in 1 of 25 cases, and healed well after two times of operation. Conjunctival congestion lessened and conjunctiva healed within postoperative 7 days in the remaining 24 animals.CONCLUSION: MMA modified PHEMA hydrogel orbital implant has good histocompatibility and fast vascularization process, and is easy to be operated with fewer complications. So, it is a safe and practicable new type orbital implant.
2.Study progress of physiologic definition of bronchopulmonary dysplasia in preterm infants

Bronchopulmonary dysplasia (BPD)characterized as the abnormal development of premature lung and trachea influenced by perinatal factors is the most common respiratory disease in very premature infants,and is also one of the diseases confirmedly related to poor quality of life.With the advancement of neonatal research and the im-provement in clinical care,more very premature infants with BPD survived to discharge.BPD is becoming the disease which both neonatologists and pediatricians should pay much attention to.The definition of BPD in recent years is in controversy.In this paper,the physiologic definition of BPD was comprehensively reviewed as a recently more accepted definition.
3.Role of endoplasmic reticulum stress in AstragalosideⅣ-induced cardioprotection in H9c2 cardiac cells

Aim To explore the role of endoplasmic reticulum stress(ERS) in Astragaloside Ⅳ-induced cardioprotection in H9c2 cardiac cells, and to explore the potential mitochondrial mechanism.Methods Conventional culture was performed of rat heart tissue-derived H9c2 cells.Experiment was randomly divided into the control group, the ERS inducer 2-Deoxy-D-Glucose(2-DG) group, Astragaloside Ⅳ and 2-DG combination group, Astragaloside Ⅳ group.Confocal microscopy was used to observe the changes of TMRE fluorescence intensity so as to confirm the influence of ERS on the mitochondrial potential, and further speculate on the opening of the mitochondrial permeability transition pore(mPTP).Western blot analysis was applied to detect the expressions of ERS proteins GRP 78, GRP 94 and IRE1.Transmission electron microscopy was used to observe the ultrastructure of the cells.Results Different doses of 2-DG could mimic the mPTP opener atractyloside to induce the mPTP opening with the peak at 100 μmol·L-1;Astragaloside Ⅳ significantly reduced 2-DG-induced mPTP opening, the expression of GPR 78, GRP 94 and IRE1 and reduced injury of mitochondria and endoplasmic reticulum.Conclusions Endoplasmic reticulum stress could be induced by 2-DG.Astragaloside ⅳ-induced mitochondrial cardioprotection involves inhibition of the ERS through GRP 78, GRP 94 and IRE1 by prevention of the mPTP opening.
4.Long term outcomes of infants with moderate to severe laryngomalacia

Objective To determine the long term outcomes of laryngomalacia infants with anomalies and to determine the clinical practice guideline for these infants.Methods The charts of infants with moderate to severe laryngomalacia,who were admitted to our hospital between January 2013 and December 2015,were retrospectively reviewed.These infants were divided into two groups,anomaly(A) group(n=37) and non-anomaly (NA) group(n=19).Results Fifty-six cases were enrolled.Infants in A group were older at symptom relief than those in NA group[(10.00±3.56) months vs.(7.89±3.03) months,P<0.05],and the weight percentiles of infants in A group were lower at 3,6 and 12 months than those in NA group(P<0.05).There was no statistically significant difference between the two groups on the weights percentiles in infants at 24 months after diagnosis.Five of 37 cases in A group and 3 of 19 cases in NA group had supraglottoplasty.One infant in A group had tracheotomy.Conclusion Both breathing difficulty and development retardations of infants with moderate or severe laryngomalacia could gradually improved with age.There is not enough evidence to support the aggressive supraglottoplasty for infants with anomalies and laryngomalacia.
5.Role of systolic blood pressure gradient of limb test in diagnosing neonatal aorta anomaly

Objective To determine the effectiveness of systolic blood pressure gradient between the right arm and the right leg(SBPG) tests in the diagnosis of neonatal aorta anomaly(AoA).Methods The SBPG of infants suspected of having critical congenital heart diseases were collected prospectively,who were admitted to Guangdong General Hospital from January 2013 to December 2015.The results of SBPG test were compared with those of echocardiography or cardiac computed tomography (golden standard).The rates of true positive,true negative,false positive,false negative were calculated under cutoff values of 5 mmHg(1 mmHg =0.133 kPa),10 mmHg,15 mmHg and 20 mmHg,respectively.Receiver operating characteristic (ROC) curves was used to compare tests of different cutoff and the areas under the ROC curve were also calculated.Results Among 664 enrolled infants,67 cases were confirmed by golden standard test.The systolic blood pressure in the right arm,the legs and SBPG in AoA group and non-AoA group were (88.0±20.4) mmHgvs.(73.4±9.3) mmHg (P<0.01),(66.1 ±10.1) mmHg vs.(69.0 ±9.7) mmHg(P>0.05) and (22.6±17.8) mmHgvs.(2.3 ±4.8) mmHg(P <0.01),respectively.In these patients,31 cases(46.3%),31 cases(46.3%),27 cases(40.3%) and 21 cases(31.3%) were diagnosed of AoA,and 36 cases(53.7%),36 cases (53.7%),40 cases(59.7%),and 46 cases (68.7%) were missed by SBPG tests of 5 mmHg,10 mmHg,15 mmHg and 20 mmHg,respectively (P < 0.01).The rates of true negative among those groups were 94.1%,99.5%,99.7% and 100.0%,and the areas under ROC curve were 0.656,0.722,0.695 and 0.657,respectively (P < 0.01).Conclusions Almost half of AoA infants could be screened out by SBPG test.The cutoff of 10 mmHg could probably be used to screen potential AoA infants,with higher true positive rate and lower false positive rate.
6.Factors affecting cataract among the elderly population

Objective:
To investigate the factors affecting cataract among the elderly, so as to provide insights into cataract control.
Methods:
Based on the major public health monitoring project of Zhejiang Province, residents at ages of 60 years and older were selected from 7 districts (counties) using a multi-stage stratified cluster random sampling method, and were followed up every other year from 2014 to 2020. Demographics, lifestyle, dietary habits and cataract incidence were collected, and factors affecting the incidence of cataract were identified using a multivariable Cox proportional hazard regression model.
Results:
Totally 9 642 residents were investigated, with a mean age of (68.89±7.39) years, and including 4 635 males (48.07%). There were 828 incident cataract cases, with an incidence rate of 20.946/1 000 person-years. Multivariable Cox proportional hazard regression analysis showed that women (HR=1.695, 95%CI: 1.377-2.088), age of 65 years and older (HR=1.707-5.044, 95%CI: 1.400-7.327), overweight/obesity (HR=1.313, 95%CI: 1.131-1.524), educational level (primary school, HR=1.642, 95%CI: 1.400, 1.926; junior high school, HR=1.553, 95%CI: 1.148-2.102), annual family income (10 000 to 50 000 Yuan, HR=1.353, 95%CI: 1.155-1.585; 50 000 to 100 000 Yuan, HR=0.663, 95%CI: 0.500-0.881; 100 000 to 150 000 Yuan, HR=0.340, 95%CI: 0.204-0.565), smoking (HR=0.649, 95%CI: 0.494-0.853), frequency of vegetable intake of >3 days/week (HR=0.693, 95%CI: 0.527-0.912), frequency of fruit intake of >3 days/ week (HR=0.833, 95%CI: 0.694-0.899), frequency of egg intake of >3 days/week (HR=0.579, 95%CI: 0.450-0.745), frequency of soy products intake of >3 days/week (HR=0.706, 95%CI: 0.588-0.849), frequency of dairy products intake of >3 days/week (HR=1.510, 95%CI: 1.199-1.901) and frequency of nut intake of >3 days/week (HR=1.733, 95%CI: 1.162-2.586) were statistically associated with the development of cataract among the elderly.
Conclusion
Gender, age, body mass index, educational level, income, smoking, and frequency of vegetables, fruits, eggs, soy products, dairy products and nuts intake are associated with the development of cataracts.
7.Isolation of human umbilical cord mesenchymal stem cells and differentiation into adipocytes and osteblasts

BACKGROUND:Culture condition,isolation method and efficiency are different in reported human umbilical cord-derived mesenchymal stem cells,which lack of unified identification standards.Therefore,it is necessary to establish a high-efficiency and economical culture system for human umbilical cord-derived mesenchymal stem calls(hUCMSCs).OBJECTIVE:To isolate hUCMSCs and induced differentiate into adipocytes and osteblasts.METHODS:The hUCMSCs were isolated form human umbilical cord by tissue adherence and digested with collagenase.The morphology,proliferation and immunophenotype of the 3rd passage cells were analyzed,and then cells were induced to osteogenic and adipogenic differentiation in vitro.RESULTS AND CONCLUSION:The hUCMSCs isolated from human umbilical cord by tissue adherence and digested with collagenase could be cultured and proliferated in vitro.Flow cytometry analysis revealed that the hUCMSCs were positive for CD29 CD44,CD59,CD105,but were negative for CD40,CD86 and HLA-DR.These calls could be induced to differentiate into adipocytes and osteblasts under proper inducing conditions.The hUCMSCs retained the appearance and phenotype even after being expanded more than 40 passages in vitro.This confirmed that the existence of MSCs in human umbilical cord and they had the capacity of differentiating into adipocytes and osteblasts.
8.Effect of mint juice on nitrite in pickled cabbage

ObjectiveTo explore the effect of mint juice on the nitrite content in pickled cabbage, and to determine the best concentration of mint juice through comprehensive sensory evaluation. MethodsThe control variates method was used to conduct this single factor experiment. The pickled cabbage processing was divided into five groups, one control group CK, and four experimental groups, Lp100, Lp80, Lp60, Lp40, respectively. The data were analyzed by using test of normality, homogeneity of variance test, ANOVA and multiple comparisons. ResultsThe content of nitrite in the experimental group with mint juice was much lower than that in the control group. During each day, as the concentration of mint juice increased, the removal efficiency also increased and the nitrite content in pickled cabbage decreased, too. When the concentration of mint juice was 100%, the nitrite content in pickled cabbage remained the lowest during 1-5 days of fermentation. On the fifth day of fermentation, the removal efficiency of nitrite in pickled cabbage by mint juice with 100% concentration reached the highest (49.55%). Adding 60% mint juice to pickled cabbage showed the highest score of 92 in sensory evaluation. ConclusionMint juice can reduce the nitrite content in pickled cabbage and make pickled cabbage a better flavor. Result indicates that 60% mint juice is the best concentration for a comprehensive effect.
9.Correlation of 18F-NaF micro-PET/CT and score with histopathology in collagen-induced arthritis rats

Objective To explore the correlation between 18F-NaF micro-PET/CT,μCT score and histopathology of collagen-induced arthritis (CIA) rats.Methods Seventy healthy female SD rats were randomized into CIA group (n=50) and control group (n = 20).Micro-PET/CT imaging was performed 10 days after successful modeling.Minimum standardized uptake value (SUVmin),maximum standardized uptake value (SUVmax),mean standardized uptake value (SUVmean) and μCT score were measured,and the severity of rheumatoid arthritis was assessed with histopathology.The correlations between SUVmin,SUVmax,SUV μCT score and histopathology of CIA were analyzed.Results SUVmin,SUVmax,SUVmean,μCT score of CIA all showed positive correlations with histopathology (r= 0.920,0.943,0.923,0.768,all P<0.001).Conclusion 18F-NaF micro-PET/CT is a quantitative imaging method for evaluating bone erosion and bone malformation in rat rheumatoid arthritis models of CIA,which is superior to μCT examination.
10.Effect of different anesthetic and postoperative analgesic methods on early postoperative quality of recovery in patients undergoing radical mastectomy: efficacy of serratus plane block

Objective To evaluate the effect of serratus plane block (SPB) combined with general anesthesia and SPB with patient-controlled nerve analgesia (PCNA) after surgery on the early postoperative quality of recovery in the patients undergoing radical mastectomy.Methods Sixty American Society of Anesthesiologists physical status Ⅰ or Ⅱ female patients,aged 18-64 yr,weighing 45-70 kg,scheduled for elective unilateral modified radical mastectomy under general anesthesia,were assigned into 3 groups (n=20 each) using a computer software:SPB and general anesthesia plus patient-controlled intravenous analgesia (PCIA) group (SG+PCIA group),SPB and general anesthesia plus SPB with PCNA group (SG+PCNA group),and general anesthesia plus PCIA group (G+PCIA group).Ultrasound-guided ipsilateral SPB was performed before anesthesia induction.Analgesia was maintained with propofol-remifentanilsevoflurane,rocuronium or vecuronium was intermittently injected to maintain muscle relaxation.PCIA solution contained tramadol 800 mg and flurbiprofen axetil 100 mg in 54 ml of normal saline,and the PCA pump was set up to deliver a 2 ml bolus dose,with a 15 min lockout interval and background infusion at a rate of 0.5 ml/h after a loading dose of 5 ml.The location of the indwelling catheter was confirmed again using ultrasound at the end of surgery,PCNA solution contained 1% ropivacaine 500 mg in 250 ml of normal saline,and the PCA pump was set up to deliver a 5 ml bolus dose,with a 45 min lockout interval and background infusion at a rate of 5 ml/h after a loading dose of 5 ml.The PCA pumps were used until 48 h after surgery.The automatic key was pressed when pain scores (numerical rating scale [NRS] scores) ≥ 4 at rest or during activity (at 45° ipsilateral upper extremity-up tilt).Quality of Recovery-40 (QoR-40) score was used to assess the early postoperative quality of recovery at 24 and 48 h after surgery.NRS scores at rest or during activity were recorded at 6,8,12,24 and 48 h after surgery.The total pressing times of PCA and occurrence of adverse reactions such as respiratory depression,pruritus,infection at the puncture site or pneumothorax were also recorded.Results Compared with group G+PCIA,the postoperative QoR40 scores were significantly increased,NRS scores at rest or during activity were decreased at each time point after surgery,and the total pressing times of PCA were reduced in SG+PCIA and SG+PCNA groups,the incidence of nausea and vomiting was significantly decreased in group SG+PCIA,and the incidence of nausea and vomiting and dizziness was significantly decreased in group SG+PCNA (P<0.05 or 0.01).Compared with group SG+PCIA,the postoperative QoR-40 scores were significantly increased at 24 h after surgery,and NRS scores at rest or during activity were decreased after surgery in group SG+PCNA (P< 0.05 or 0.01).Conclusion SPB combined with general anesthesia and SPB with PCNA after surgery can raise the early postoperative quality of recovery in the patients undergoing modified radical mastectomy.
