1.Emphasis on prevention and treatment of high blood pressure
Chinese Journal of Practical Internal Medicine 2001;0(09):-
Hypertension is a highly prevalent cardiovascular risk factor worldwide.Although the treatment of hypertension has been shown to prevent cardiovascular diseases and to extend and enhance life, hypertension remains inadequately managed, especially in developing country, such as China.Hypertension is one of the major causes of current global disease burden and is as prevalent in China as in the developed world.Blood pressure-induced cardiovascular risk rises continuously across the whole blood pressure range.During the past decades, much evidence has become available to support a systolic blood pressure threshold of 140 mmHg for even 'low-risk' patients, and,in high-risk patients there is evidence for lower thresholds.Lifestyle modification is recommended for all individuals and should be taken as the cornerstone of anti-hypertensive schedule.Monotherapy is inadequate for the majority of patients and more than two-thirds patients should receive combined anti-hypertension treatment, with fixed-dose combination being an ideal choice. The situation of blood pressure controlling could be improved if the government is involved in the anti-hypertension program.

Objective To clarify the influence on component and pharmacological action of Astragalus polysaccharides (APS) as complementary therapeutic agents prepared by different extraction and purification techniques. Methods Components of APS prepared by different extraction and purification techniques were analyzed, and these APS were used for synergy and attenuation of chemotherapy, radiotherapy treatment with H22 liver cancer and Lewis lung cancer of tumor-bearing mice, and also used for the regulation of immune function to immunosuppression mice. Results Experimental data were analyzed by means of statistical method to get pharmaco-result: A3 (extracted by microwave assistance and purified by membrane separation) > A4 (extracted by refluxing and purified by membrane separation) > A1 (extracted by refluxing and no purification)≈ A2 (extracted by microwave assistance and no purification). There were no significant differences on pharmacodynamic action between A1 and A2. However, compared with A1 and A2,it was worth noting that A3 and A4 exhibited good pharmacodynamic action. Then A3-in and A4-in, the samples in dialyzer after dialysis, were separated and purified to get homogeneous APS, which were the principal constituents of APS in dialyzer, with the molecular weight (Mw) of 7669 and 14 142 determined by HPGPC, respectively. The average Mw of APS outside of the dialyzer, A3-out was 3102 and A4-out 3256, which were the main compositions of A3 and A4, accounted for 79.63% and 53.92%, respectively. Conclusion APS with Mw about 5000 Da exhibit better antitumor effect and immunological activity. Refluxing, microwave assistance extractions, and membrane enrichment techniques bring different cases on Mw distribution, components and pharmacodynamic action, and obviously exhibit relationship among component, Mw distribution, and pharmacological action.

In this paper, the antigens of both Vibrio cholerae on 55 strains of El Tor and on 28 strains obtained with genetical method were studied by ELISA method using 12 strains of McAb against Vibrio cholerae (VCOMcAb). The results indicated that the 55 strains of Vibrio cholerae El Tor were expressed 11(A-K) of the recognized antigenic determinants, and fell into XⅧ(Ⅰ-XⅧ) distinct McAb reactivity types or groups, and the 28 varients of Vibrio cholerae from genetical method were expressed 9 (A-Ⅰ) of the recognized antigenic determinants, and fell into Ⅹ (Ⅰ-Ⅹ) distinct McAb reactivity types or groups. From those VCOMcAbs, the “F3” could recognize all 55 strains of Vibrio cholerae El Tor, indicating it has specificity; other 11 strains of VCOMcAb have a variety of reactivity to antigens on the 55 strains of Vibrio cholerae El Tor in both Inaba and Ogawa and their percentages of reactivity are different. This shows that these strains may have subtypes or subgroups.

Objective:To investigate the assessment and occurrence of first-occured venous thromboembolism(VTE) among hospitalized patients. Methods:The clinical data of 6 532 surgical patients in Shenzhen Hospital, Peking University who were admitted from May 1, 2021 to June 30, 2021 were collected and analyzed retrospectively. The demographic data, Caprini score at admission and the incidence of VTE during hospitalization were analyzed by two independent sample t test and chi square test. Results:The Caprini score at admission of 6 532 patients was 1.81 ± 1.71. The number of cases in high, medium and low risks was 363 (5.6%), 1 189 (18.2%), 4 980 (76.2%), respectively. There was significant difference in VTE risk assessment scores and grades in different gender ( t=5.31, χ 2=48.31), length of stay ( F=195.21, χ 2=548.52) and hypertension ( t=17.07, χ 2=280.89), diabetes ( t=12.14, χ 2=51.18), smoking ( F=31.71, χ 2=53.23) and drinking ( F=18.78, χ 2=30.07) ( P<0.05). Forty-four(0.7%) patients got hospital-acquired VTE totally, among which, 24 cases (6.6%) were in high-risk, 14 cases (1.2%) were in medium-risk and 6 cases (0.1%) were in low-risk. What′s more, the top five VTE risky departments based on the assessment were not completely consistent with the top five departments with the highest incidence of VTE. Conclusions:The hospitalized patients are at high risk of VTE. The risk factors of diabetes, hypertension, smoking, drinking and other related factors should be included in the evaluation model. Meanwhile, the VTE risk assessment of in-patients should be emphasized and prophylactic treatments should be taken to reduce the incidence of VTE.

OBJECTIVE:To establish HP LC ch aracteristic ch romatogram of different medicinal parts of Cirsium japonicum , and to compare the difference of chemical components in different medicinal parts of C. japonicum according to chemical identification method ,and to provide reference for quality control and evaluation of C. japonicum . METHODS :Medicinal material (overground part ),leaves, flower, main stem and lateral stem of C. japonicum were determined by HPLC. According to the TCM Chromatographic Fingerprint Similarity Evaluation System (2012A edition ),the chromatograms were matched to generate the HPLC characteristic chromatogram of each medicinal part. The differences of common characteristic peak area were analyzed according to variance analysis of single factor. The chromatographic peaks were identified by comparison of reference substance. Meanwhile, the chemical pattern recognition was performed to research the different medicinal parts of C. japonicum according to principal component analysis (PCA)and cluster analysis. RESULTS :HPLC characteristic chromatograms of medicinal material , leaves, flower, main stem and lateral stem from C. japonicum were established respectively ,and 15 common peaks were confirmed for medicinal material ,leaves and flower of C. japonicum ;11 common peaks were confirmed in chromatograms of main stem and lateral stem from C. japonicum (absence of No. 7,9,12,13 peak). The contents of chemical components were different greatly among different medicinal parts. No. 1,2,3,10,11 peaks were identified as neochlorogenic acid ,chlorogenic acid , cryptochlorogenic acid ,linarin and pectolinarin. Results of PCA and cluster analysis showed that chemical pattern recognition and clustering of the flower and stem of C. japonicum were distinct and can be clustered into one category respectively. However ,the leaves distribution of C. japonicum was relatively scattered ,so it was difficult to cluster . CONCLUSIONS :Established HPLC characteristic chromatogram-chemical pattern recognition can reflect the differences of different medicinal parts of C. japonicum integrally, comprehensively and truly , which has vital significance for origin indentification , quality control and overall evaluation of C. japonicum .

OBJECTIVE:To establish an overall quality evaluation model for Agrimonia pilosa based on extract and characteristic spectrum, and to provide evidence for comprehensive quality evaluation of the medicinal material and screening of high-quality provenance. METHODS :Referring to different extraction method and solvent condition stated in 2015 edition of Chinese Pharmacopoeia (part Ⅳ),using the content and total peak area of HPLC characteristic chromatogram of extract as indexes, and the extraction technology was optimized by weighted comprehensive score. HPLC characteristic spectrum of 15 batches of A. pilosa was established ,and similarity evaluation and characteristic peak identification were performed. SPSS 25.0 software was used to conduct single factor analysis and Pearson correlation analysis for the extract content and total peak area of A. pilosa from different origins. The quality of medicinal materials from different origins were compared. Entropy weight TOPSIS method was adopted to evaluate comprehensive quality of A. pilosa using the extract content and total peak area of 15 batches of A. pilosa . RESULTS :The extraction technology of A. pilosa extract, which was extracted with hot dip plating using 50% ethanol as solvent ,was optimized. The similarity of 15 batches of A. pilosa was higher than 0.92,and 4 characteristic components were identified(ellagic acid ,quercetin, apigenin, kaempferol). There were significant differences in average extract content and total peak area of characteristic chromatogram of A. pilosa from different origins (P<0.05 or P<0.01),and there was a certain positive correlation between them (r=0.86,P<0.01). Results of entropy weight TOPSIS evaluation showed that the average Ci values of A. pilosa in Anhui ,Zhejiang,Sichuan,Henan and Jiangsu provinces were 0.689,0.351,0.218,0.308 and 0.361 respectively. CONCLUSIONS:The quality of A. pilosa from Anhui was the best ,that from Zhejiang and Jiangsu was better ,that from Henan was the second ,and that from Sichuan was poor. Established extraction technology and characteristic spectrum determination method of A. pilosa are stable and feasible. The entropy weight TOPSIS model is objective and quantifiable for comprehensive quality evaluation of A. pilosa ,and can effectively evaluate its quality.
