1.Clinical analysis of puerperal mode in 490 multiparas

Objective To explore the puerperal mode of muliparas re-pregnancy, accumulate the clinical experience. Methods 490 multiparas' puerperal mode were analyzed as study group,and compared with 1600 prima-ry pregnancy at the same stage; compared the puerperal mode, the cesarean section rate and the rate of maerosoma. Results Re-pregnancy mtthiparas' vaginal delivery 385 examples, CS 105 examples,CS rate 21.4%, meanwhile the primary pregnancy's vaginal delivery 1200 examples,CS 400 examples,CS rate 25% ,no statistical difference(P>0.05). CS rate was directly proportional to the away time from last delivery. Conclusion CS rate had no remarkable difference between multiparas and primary pregnancy. Mainly CS indication is fetal distress, CS rate directly response to the time interval to last delivery. Macrosoma rate is high in muhiparas by well-fed. Multiparas had low CS rate with macrosoma.
2.Mophological changes after percutaneous transluminal angioplasty in experimental cerebral vasospasm

Objective Experimentally to investigate the changes in vasospastic canine basilar arteries. Methods Vasospasm of basilar arteries was induced by double-injecting autologous blood into the brain cisterns. 28 adult mongrel dogs of both genders served as the subjects, of them 20 involved in experimental group and accepted the PTA treatment and the other 8 doges were in negative control group without any treatment. PTA was performed by a single inflation of balloon for 10 seconds at a peak pressure of 2.0 atmospheres. Angiography was re-performed at 30 day, 90 day and 300 day following PTA, respectively. The histologic changes were then evaluated using light and electron microscopy. Results Caliber of basilar arteries of all canines decompressed obviously, from 1.3 mm to 0.8 mm. 3.Association of β-3-adrenergic Receptor Gene with Insulin Resistance in the Patients with Polycystic Ovary Syndrome

Objective To explore the association of the Arg64 polymorphism β-3-adrenergic receptor(β3-AR) gene with polycystic ovary syndrome(PCOS)and its characterization with obesity,insulin resistance(IR),blood pressure (BP), blood lipid spectrum. Methods The polymorphism of the β-3-AR gene was determined in 163 patients with PCOS and 100 controls. Body mass index(BMI), waist to hip circumference ratio(WHR), BP were examined. Blood glucose,blood lipid spectrum,insulin were also measured. Insulin resistance index was calculated. Results The frequency of Arg64 allele in PCOS women was significantly higher than that of controls(0.33 vs 0.13 respectively, P<0.05). Fasting insulin, insulin resistance index, WHR in Arg64 group were significantly higher than those of Trp64 group. The frequency of Arg64 allele was positively correlated with FINS, insulin resistance index, WHR. Conclusion The Arg64 mutation in the β-3-adrenergic receptor(β-3-AR) gene might contribute to development of PCOS and IR.
4.Effect of Tetramethylpyrazine on IL-8,IL-12 and IL-33 Levels in Serum and MMP-10 and NF-κB Protein Expression in Joint Synovial Tissues in Rats with Collagen-induced Arthritis

Objective To observe the effect of tetramethylpyrazine (TMP) on serum interleukin 8 (IL-8),interleukin 12 ( IL-12) and interleukin 33 ( IL-33) levels and MMP-10 and NF-κB protein expression in joint synovial tissues in rats with collagen-induced arthritis ( CIA) ,and further to investigate the mechanism for treating rheumatoid arthritis. Methods Rats were randomly divided into normal control group( n=10) ,model control group,dexamethasone group,low-dose of TMP and high-dose of TMP groups ( n=9 each) . The CIA model was established in all the groups except for the normal control group. Rats of dexamethasone group were give 2 mg.kg-1 of dexamethasone;Rats of low-dose and high-dose of TMP group were given 50,100 mg.kg-1 of TMP, respectively.The toe volume and arthritis index (AI) were used to evaluate joint inflammation. The levels of IL-8,IL-12 and IL-33 in the serum of rats were detected by ELISA. The expression levels of MMP-10 and NF-κB in joint synovial were detected by Western blotting. Results Compared with CIA group,the toe volume and AI were significantly reduced in high-dose of TMP groups and dexamethasone groups,and levels of IL-8,IL-12 and IL-33 and the expression levels of MMP-10 and NF-κB were decreased significantly (P<0.01). TMP (50 mg.kg-1) could obviously decrease the toe volume and the levels of IL-8 and IL-33. There was no statistical difference in other indices (P>0.05). Conclusion TMP at 100 mg.kg-1 showed obvious inhibition on CIA rats. The mechanism may be correlated to balancing the effect of proinflammatory factor and anti-inflammatory cytokines,improving immune function in rats,reducing joint synovial inflammation,and alleviating the destruction of the articular cartilage.
5.Clinical observation of early enteral nutrition support for post-laryngectomy patients.

OBJECTIVE:
To compare the postoperative nutritional status and the incidence of postoperative infection between post-laryngectomy patients with early eternal nutrition and those with normal liquid diet.
METHOD:
The observational group were given eternal nutrition while the control group were given normal liquid diet. Nutrition indicators (total protein, albumin, globulin, prealbumin, hemoglobin, body weight) and immune parameters (lymphocyte count) were measured at preoperative day 3 and postoperative day 3, 7 and 10. The incidence of complications and postoperative hospitalization days were compared and analyzed.
RESULT:
Compared with the control group, the total protein, albumin and globulin of observational group had no statistic significance at postoperative day 3, the total protein, albumin,globulin of observational group were higher (P<0. 05) at postoperative day 7 and 10: The body weight had no satisic significance at postoperative day 3 and 7, while observation group higher at postoperative day 10, while the hemoglobin of observational group had no statistic significance: The prealbumin and lymphocyte count had no statistic significance. The postoperative hospitalization days of the observational group was lower (P<0. 05) than the control group.
CONCLUSION
The early eternal nutrition for post-laryngectomy patients are conducive to improve of nutritional status and reduce the rate of post-operative complications.

6.Ultimobranchial fistula and cyst of thyroid:4 cases report and review of literature.

OBJECTIVE:
To investigate the clinical feature,diagnostic and therapeutic methods of ultimobranchial fistula and cyst of thyroid.
METHOD:
Four cases of ultimobranchial fistula and cyst of thyroid in our hospital were reported and the relevant literatures were reviewed.
RESULT:
The branchial fistula and cyst of four cases were adhered to thyroid,with unclear anatomical landmarks, the function of recurrent laryngeal nerve were bad before the operation in two cases. The nerve of four cases were retained, the function of thyriod and parathyroid gland were normal and no recurrence were found.
CONCLUSION
Ultimobranchial fistula and cyst of thyroid is rare and is easier to be misdiagnosed and wrong treated. Surgical excision is effective for it and recurrent laryngeal nerve should be protected when performing the operation.

7.Diagnosis and treatment of bilateral bronchial foreign body in children.

OBJECTIVE:
To discuss the clinical characteristics of bilateral bronchial foreign body in children, increase the curative rate and reduce the complications.
METHOD:
The clinical data of 35 cases with bilateral bronchial foreign body in children were retrospectively analysed, including surgery, key surgery points, and postoperative combined therapy.
RESULT:
The foreign bodies of all cases were removed under intravenous general and tropical anesthesia without complications.
CONCLUSION
The bilateral bronchial foreign body is a serious case, the timely and effective treatment can lower the mortality rate and postoperative complications. The children lack of oxygen for a long time before and in operation should be give comprehensive therapy, for example: sedation and hyperbaric oxygen.

9.Feasibility analysis and safety of laparoscopic pylorus-preserving pancreaticoduodenectomy

Objective To investigate the safety and feasibility of laparoscopic pylorus-preserving pancreaticoduodenectomy.MethodsThe data of 59 patients with laparoscopic pancreaticoduodenectomy in Shangjin hospital of West China hospital of Sichuan University from January 2014 to December 2015 were analyzed retrospectively.The general data,operative time,estimated blood loss,conversion rate,time to liquid diet,postoperative hospital stay and complications were analyzed.Results The laparoscopic pylorus-preserving pancreaticoduodenectomy were successfully performed in 56 cases,the successful rate was 94.9%,the other 3 patients were converted to laparotomy for superior mesenteric vein/portal vein involvement.The operative time was 255~510 minutes,with mean operative time (384±145) minutes,the intraoperatve blood soss was 50~800 mL,with mean estimated blood loss (148±28)mL,the time to liquid diet was 1.0~4.0 days,with average time (3.2±1.1)days,the postoperative hospital stay was 5.0~53.0 days,with average time (10.3±2.6)days.After surgery,16 cases suffered from complication,the rate was 27.1%.The most common complication was pancreatic fistula which occurred in 13 cases including 12 cases of grade A and 1 case of grade pancreatic fistula.No grade C pancreatic fistula occurred in this series.Six patients suffered from delayed gastric emptying.One patient suffered from bile leakage.Conclusion Laparoscopic pylorus-preserving pancreaticoduodenectomy was safe and feasible.Pylorus-preserving does not increase the incidence of delayed gastric emptying.

Objective To study the experiences of severe acute pancreatitis (SAP) treated by combmation of traditional Chinese and western medicine.Methods Two hundred and forty patients with SAP were divided into A and B groups randomly,140 patients of group B were treated by combination of traditional Chinese and western medicine;100 patients of group A were treated by western medicine.Results After treatment in group B, the serum and urine amylase, and c-reactive protein,signiflcantly decreased.The complication rate of group A and group B was 55.00 % and 12.86 %(P
