1.Analysis of the Utilization of Lipid-regulating Agents from 74 Hospitals in 6 Areas of China in 2013

OBJECTIVE:To provide reference for drug procurement and supply and rational use of lipid-regulating agents. METHODS:The epidemiological investigation was carried out among 159 506 cases from 74 hospitals in Beijing,Chengdu, Guangzhou,Hangzhou,Shanghai and Tianjin in 2013. The utilization of lipid-regulating agents was analyzed statistically in re-spects of purchase value,DDDs,DDC,actual average daily dose and sort ratio. RESULTS:The prevalence rate of hyperlipidemia was relatively high,accounting for 29.56% and showing a tendency of regional distribution and young age in all regions. The pa-tients with hypertension,diabetes and coronary heart disease had a higher incidence to suffer from hyperlipidemia. The use of atorv-astatin was in the first place,but it also had a higher DDC;while rosuvastatin hasd the advantage over aorvastatin in drug market. Simvastatin had a lower DDC and was more suitable for the patients with low income. The doses of lipid-regulating agents in other regions were lower than DDD except for those in Beijing and Tianjin. CONCLUSIONS:Statins dominate the lipid-regulating agents market. But new lipid-regulating agents and drug combination provide a new choice for clinical treatment.
2.The EC301-CR test of acalculia for patients with cerebral infarction

Objective To investigate errors and the mechanism of acalculia in patients with left or right hemisphere cerebral infarction.Methods Fifty-six patients with single hemisphere cerebral infarction and 56 normal adults who were matched in age,sex and years of education were tested with EC301-CR.The patients were divided into a left hemisphere cerebral infarction group(n =34)and a right hemisphere cerebral infarction group(n =22).Results The scores on 30 out of 32 EC301-CR items were significantly lower among the patients than in the normal control group.Scores on 14 EC301-CR items were significantly lower in the left hemisphere group than in the right hemisphere group.In the left hemisphere group the item scores of aphasia patients were significantly lower than those of no-aphasia patients except on digit identity.Conclusion Mathematical processing and calculation were impaired in patients with single hemisphere cerebral infarction.Calculation was significantly worse among left hemisphere patients compared with right hemisphere cerebral infarction.There was a highly significant correlation between acalculia and aphasia.
3.Clinical features of the elderly patients over 80 years with cute cerebral infarction

Objective To investigate the risk factors,severity and infarct site features and clinical characteristics of the elderly patients over 80 years with cute cerebral infarction.Methods One hundred and sixty-two patients with acute cerebral infarction in Red Cross Hospital of Guangzhou,The Forth Affiliated Hospital of Medical College of Jinan University from January 2012 to May 2015 were enrolled and randomly divided into the elderly patients (≥ 80 years old) and the middle aged patients (< 60 years old).The risk factors,national institutes of health stroke scale (NIHSS) scores and Oxfordshire community stroke project (OCSP) criteria were compared between the two groups.Results Coronary artery disease,atrial fibrillation and NIHSS in the elderly patients (25% (22/88),13.6% (12/88),7.74 ± 4.986) were significantly higher than those of the middle aged group (12.2% (9/47),4.1% (3/74),5.04± 4.305),and the differences were significant (x2 =4.281,4.393,t =-3.649;P< 0.05 or P< 0.001).The logistic regression analysis finally showed that smoking,hyperlipemia,NIHSS scores and gender(male) were the independent risk factors(OR=3.851,3.609,1.100 and 2.670;P<0.05).There were more LACI patients in the elderly group than he middle aged group ((40.9%,36/88) vs.(60.8%,45/74),x2 =6.369,P < 0.05).Conclusion Compare to the middle aged patients,occurrence of the elderly patients with acute cerebral infarction is more severe,and the clinical features and risk factors have its particularity.Secondary prevention strategy should be emphasized on the control of different risk factors based on the patients' age.
4.Development of grip strength and its relationship with body composition in preschoolers

Objective:
To study the development of grip strength and its relationship with body weight, height and body composition in preschool children.
Methods:
Data was collected from 900 children aged 3-6 years in 4 preschools in Beijing during Sept. to Oct. of 2020. A questionnaire survey (birth weight, dietary and exercise), height, weight, bioelectrical impedance analysis of body composition and dominant hand grip strength tests were conducted. Partial correlational analysis, multivariate linear regression were used for association between grip strength and body composition.
Results:
The grip strength gradually increased with age, from (5.2±1.7) to (9.3±2.5)kg in boys ( F=57.93, P <0.01), and from (4.3±1.6) to (7.9±2.5)kg in girls ( F=48.77, P <0.01). Grip strength in boys were significant higher than that of girls ( t=6.42, P <0.01). The grip strength increased with weight status in boys ( F=5.35, P <0.01), with highest in obese group, followed by overweight and normal weight group. After adjusted for height, weight and birth weight, as well as maternal education, food consumption during the past week, grip strength in boys was positively correlated with fat free mass, upper limb muscle mass ( r=0.11, 0.09, P <0.05), and negatively correlated with body fat mass ( r=-0.18, P <0.05). Grip strength in girls was negatively correlated with body fat mass, and positively correlated with fat free mass ( r= -0.09, 0.09, P <0.05). Grip strength varied significantly by age, fat free mass and upper muscle mass in boys ( F=66.75, P < 0.01), and by age and fat free mass in girls ( F=150.81, P <0.01).
Conclusion
The grip strength can partially reflect changes in fat free mass for boys and girls, it also reflect changes in upper limb muscle mass for boys. Increases in fat free mass helps grip strength improvement.
5.Ischemia-free liver transplantation improves the prognosis of recipients using functionally marginal liver grafts

Background/Aims:
The shortage of donor liver hinders the development of liver transplantation. This study aimed to clarify the poor outcomes of functionally marginal liver grafts (FMLs) and provide evidence for the improvement of ischemia-free liver transplantation (IFLT) after FML transplantation.
Methods:
Propensity score matching was used to control for confounding factors. The outcomes of the control group and FML group were compared to demonstrate the negative impact of FMLs on liver transplantation patients. We compared the clinical improvements of the different surgical types. To elucidate the underlying mechanism, we conducted bioinformatic analysis based on transcriptome and single-cell profiles.
Results:
FMLs had a significantly greater hazard ratio (HR: 1.969, P=0.018) than did other marginal livers. A worse 90-day survival (Mortality: 12.3% vs. 5.0%, P=0.007) was observed in patients who underwent FML transplantation. Patients who received FMLs had a significant improvement in overall survival after IFLT (Mortality: 10.4% vs 31.3%, P=0.006). Pyroptosis and inflammation were inhibited in patients who underwent IFLT. The infiltration of natural killer cells was lower in liver grafts from these patients. Bulk transcriptome profiles revealed a positive relationship between IL-32 and Caspase 1 (R=0.73, P=0.01) and between IL-32 and Gasdermin D (R=0.84, P=0.0012).
Conclusions
FML is a more important negative prognostic parameter than other marginal liver parameters. IFLT might ameliorate liver injury in FMLs by inhibiting the infiltration of NK cells, consequently leading to the abortion of IL-32, which drives pyroptosis in monocytes and macrophages.
6.Drug-target protein interaction prediction based on AdaBoost algorithm.

The drug-target protein interaction prediction can be used for the discovery of new drug effects. Recent studies often focus on the prediction of an independent matrix filling algorithm, which apply a single algorithm to predict the drug-target protein interaction. The single-model matrix-filling algorithms have low accuracy, so it is difficult to obtain satisfactory results in the prediction of drug-target protein interaction. AdaBoost algorithm is a strong multiple classifier combination framework, which is proved by the past researches in classification applications. The drug-target interaction prediction is a matrix filling problem. Therefore, we need to adjust the matrix filling problem to a classification problem before predicting the interaction among drug-target protein. We make full use of the AdaBoost algorithm framework to integrate several weak classifiers to improve performance and make accurate prediction of drug-target protein interaction. Experimental results based on the metric datasets show that our algorithm outperforms the other state-of-the-art approaches and classical methods in accuracy. Our algorithm can overcome the limitations of the single algorithm based on machine learning method, exploit the hidden factors better and improve the accuracy of prediction effectively.
7.Design of Portable Spirometer Based on Internet of Things of Medicine.

A kind of portable device for detecting common lung function parameters is mentioned in this paper. Using the singlechip microcomputer as the master control block to collect and process the data from high-accuracy gas pressure sensor, through the way of parametric calibration and linear interpolation to test and calculate the Forced Vital Capacity (FVC), Peak Expiratory Flow (PEF), Forced Expiratory Volume in one second (FEV1), and FEV1/FVC. Meanwhile, the detected parameters can be uploaded to the intelligent mobile terminal through the wireless transmission module. The device is able to show expiratory volume-time curve and the final parameters clearly, the error of measurement is less than 5%. In addition, that device is small and convenient, not only is good for clinical application, but also can be used for family in a house.

8.Study on Preparation and Pharmacokinetics of Puerarin Microemulsion Based on Phase Ⅰ Metabolic Regulation

OBJECTIVE: To prepare puerarin microemulsion with phase Ⅰ metabolic regulation (R-PR-ME) and to study pharmacokinetic characteristics of rats in vivo. METHODS: R-PR-ME and Puerarin microemulsion without metabolic regulation (NR-PR-ME) were prepared by Shah method. Pseudo-ternary phase diagram was used to optimize microemulsion formula using drug loading amount as index. The particle size and PDI of microemulsion were characterized by using a laser particle size analyzer. Rats were used as animal models, and HPLC method was used to determine the blood concentration of puerarin before and 5, 10, 15, 20, 30, 45, 60, 90, 120, 180, 240, 360, 480, 600 min after intragastric administration of R-PR-ME, NR-PR-ME and puerarin suspension (PR-SP) at puerarin dosage of 120 mg/kg. The pharmacokinetic parameters were calculated by using DAS 2.0 software. SPSS 19.0 software was used for statistical analysis. The relative bioavailability of R-PR-ME was calculated with NR-PR-ME as reference preparation. RESULTS: The formula of R-PR-ME included that oleoyl polyoxyl-6 glyserides (oil phase)-polysorbate 20 (emulsifier)-glycerides (co-emulsifier) mass ratio of 2 ∶ 4 ∶ 4; drug-loading amount of 67.50 mg/g, particle size was (22.59±0.53) nm (n=3) and PDI was 0.182±0.017 (n=3). The formula of NR-PR-ME included that soybean oil (oil phase)-polysorbate 80 (emulsifier)- glycerol (co-emulsifier) mass ratio of 1 ∶ 4.5 ∶ 4.5, drug-loading amount of 61.32 mg/g, particle size of (15.45±1.06) nm(n=3) and PDI of 0.156±0.012 (n=3). Pharmacokinetic parameters of R-PR-ME, NR-PR-ME and PR-SP included that AUC0-600 min were (134.187±37.152), (65.145±18.762) and (49.623±12.143) μg·min/mL; cmax were (1.316±0.306), (1.082±0.294) and (0.425±0.106) μg/mL; MRT were (155.068±33.204), (100.264±27.683), (60.524±14.086) min; t1/2β were (365.880±101.250), (283.280±80.940), (80.063±21.189) min (n=6), respectively. Compared with PR-SP, AUC0-600 min, cmax, MRT and t1/2β of R-PR-ME and NR-PR-ME were increased significantly (P<0.05 or P<0.01). Compared with NR-PR-ME, AUC0-600 min, MRT and t1/2β of R-PR-ME were more higher (P<0.05). The relative bioavailability of of R-PR-ME was 205.98%. CONCLUSIONS: R-PR-ME is prepared successfully with high drug-loading amount, and can significantly increase the bioavailability of puerarin in rats, compared with PR-SP and NR-PR-ME.
9.Analysis of epidemic characteristics of hand-foot-mouth disease Virus using circular distribution method

Objective To understand the epidemic characteristics of different virus types of hand, foot and mouth disease (HFMD) in Zhaotong City, and provide guidance and recommendations for the prevention and control of HFMD, and to analyze seasonal characteristics of different virus types of HFMD in Zhaotong City. Methods The epidemiological characteristics of different virus types of HFMD in Zhaotong City from 2014 to 2017 were analyzed using the concentration and circular distribution methods. Results The main pathogens detected were EV71, Cox A16 and other enteroviruses, which were 216, 182, and 294, respectively, accounting for 57.45%, 73.44%, and 67.11%. M was 0.86, indicating that EV71 had strong seasonality. The Rayleigh test showed statistically significant differences (Z = 99.53, P <0.001). ā = 157 °, the peak day of onset was May 10, similar to untyped (May 16), the peak period was April 21-June 1, and the epidemic period was April 1-June 21. Conclusion According to the incubation period of hand-foot-mouth disease and the period of time during which the vaccine develops protective effects, vaccination of hand-foot-mouth disease vaccine at the peak period has a good guiding significance for the timeliness and pertinence of vaccination.
