1.Allogeneicversus heterogeneous bone marrow mesenchymal stem cells for laryngeal cartilage repair

BACKGROUND:Because chondrocytes have no regeneration ability, to select suitable seed cels is the primary problem to repair cartilage defects.
OBJECTIVE:To investigate the effect of alogeneicversus heterologous bone marrow mesenchymal stem cels (BMSCs) in repairing laryngeal cartilage defects after chondrogenic induction.
METHODS:BMSCs from human and rabbits were isolated and cultured. Passage 3 cels were cultured in chondrogenic induction medium containing transforming transforming growth factor beta 1 and bone morphogenetic protein, and then were dropped onto a poly(lactic-co-glycolic acid) (PLGA) scaffold. Thirty New Zealand rabbits were randomly assigned into three groups: blank control group, human BMSCs group, rabbit BMSCs group. Animal models of laryngeal cartilage defects were made in the three groups. After modeling, saline-soaked PLGA scaffold, PLAG scaffold with human BMSCs or with rabbit BMSCs were implanted respectively into the rabbits in the normal blank, human BMSCs and rabbit BMSCs groups. The expression of type II colagen in the larynx and its surrounding tissues was detected by immunohistochemistry at 4 and 8 weeks postoperatively.
RESULTS AND CONCLUSION: The animals in each group breathed normaly with no presence of wheezing, and their eating and activity were good. Moreover, there was no purulency or infection in the three groups. At 4 and 8 weeks after operation, the positive rates of type II colagen in the two BMSCs groups were significantly higher than that in the blank control group (P < 0.05). There was no significant difference between two BMSCs groups (P > 0.05). These results show that both alogeneic and heterologous BMSCs have good therapeutic effects on the repair of laryngeal cartilage defects in rabbits.
2.Treatment of children with femoral subtrochanteric fractures by the external fixation combined with single hip plaster.

OBJECTIVETo explore curative effects of external fixation combined with single hip plaster in treating children with femoral subtrochanteric fracture.
METHODSForm March 2009 to July 2016, 15 children with femoral subtrochanteric fracture were treated with external fixation combined with single hip plaster, including 9 males and 6 females with a mean age of 8.5 years old ranging from 5 to 14 years old. According to fracture classification of Seinsheimer, 3 cases were type IIA, 4 cases were type IIB, 3 cases were type IIC, 2 cases were type IIIA, 1 case was type IIIB, 1 case was type IV, 1 case was type V. Complications and radiographs were retrospectively reviewed. Postoperative function of hips were evaluated according to Sanders criteria.
RESULTSAll children were followed up from 16 to 48 months with an average of 32 months. No early closure of epiphysis, bone nonunion and breakage of screw occurred. According to the Sanders score standard of hip function, the result was excellent in 14 cases, good in 1 case. There were no hip inversion, limb shortening, excessive growth and other malformations.
CONCLUSIONSExternal fixation combined with single hip plaster for the treatment of children is a safe and effective fixation, which provide a new choice of femoral subtrochanteric fracture.
3.Screening results of neonatal diseases and exploration of management mode in Yichang in 2017-2019

Objective To retrospectively analyze the results of neonatal diseases screening in Yichang from 2017 to 2019, understand the incidence and recall of the diseases, and explore the management mode of neonatal disease screening suitable for this region. Methods The subjects were newborns who were delivered in Yichang midwifery institutions from 2017 to 2019 and were screened for neonatal diseases. Heel blood of the newborns was collected for the screening of neonatal diseases, including congenital hypothyroidism (CH), phenylketonuria (PKU), G6PD deficiency, congenital adrenal hyperplasia (CAH) and thalassemia. Those newborns with positive initial screening were recalled for reexamination and confirmation. The recall rates of different diseases were compared by Chi-square test. Results There were 85 891 live births in Yichang area from 2017 to 2019, and 84 063 cases were screened for neonatal diseases, with a screening rate of 97.87%. A total of 6 043 cases were positive in the initial screening, of which 5,047 cases were recalled, with a recall rate of 83.52%. The recall rates of the traditional two diseases (CH and PKU) and the new three diseases (CAH, G6PD deficiency and thalassemia) were significantly different ( χ2= 197.93, P<0.01). A total of 501 cases were diagnosed. The incidence rate of CH was 1/1,911, the incidence rate of PKU was 1/12 009, the incidence rate of CAH was 1/28 021, the incidence rate of G6PD deficiency was 1/1 121, and the incidence rate of thalassemia was 1/226. Conclusions The neonatal disease screening rate increased year by year in Yichang, but the recall rate of suspicious positive initial screenings decreased. It is necessary to explore a more suitable management mode for the five neonatal disease screenings in this region, improve the recall rate of children with positive screening, reduce the incidence of disabled children, and improve the quality of the birth population.
4. Epidemiological analysis of 1494 cases of pneumoconiosis in a city

Objective:
To analyze the prevalence of pneumoconiosis in Yichang City from 2006 to 2016, and to provide a scientific basis for the administrative department to formulate prevention and control strategies for pneumoconiosis.
Methods:
From 2006 to 2016, through the China Disease Control Information System Occupational Disease and Occupational Health Inspection System, relevant data were obtained, and the types of pneumoconiosis, the disease area and the dust-collecting age in Yichang City were statistically analyzed.
Results:
There were 1494 cases of pneumoconiosis in Yichang City, which were mainly distributed in 1 076 cases (72.02%) of coal workers’ pneumoconiosis and 382 cases (25.57%) of silicosis, which occurred in Yiling District, Changyang County, Yidu City and Zigui County. The average age of dust collection was 13.78 years, and the age of onset was mainly concentrated in 5~9 years (35.27%) and 10~19 years (30.32%) . 5.Dynamic evaluation of blood glucose fluctuations in type 2 diabetic and normal subjects

BACKGROUND: Dynamic glucose monitoring can reflect the fluctuation of blood glucose by successive examination at the interval of 5 minutes,which contributes to the overall and objective evaluation of glucose metabolism.OBJECTIVE: To investigate the tendency and characteritics of blood glucose fluctuation in type 2 diabetic and normal subjects. DESIGN: A case-controlled retrospective experiment based on patients and healthy controls.SETTING: Endocrine Metabolism Ward and Clinic of Sixth People's Hospital Affiliated to Shanghai Jiaotong University.PARTICIPANTS: Totally 47 patients were newly diagnosed as having type 2 diabetes mellitus at Endocrine Metabolism Ward and Clinic, Sixth People's Hospital Affiliated to Shanghai Jiaotong University, between December 2002 and July 2004. They had not received intervention (diet, exercise or medicine) before. Those with acute complications such as ketoacidosis and hyperosmolar coma were excluded. Meanwhile, 43 healthy controls were selected from those coming for routine health examinations, and they had no family history of diabetes, and impaired fasting glucose and impaired glucose tolerance were excluded. Their lipid and liver and kidney indexes as well as blood pressure were within normal range. They all signed informed consent.METHODS: Totally 43 normals and 47 patients with type 2 diabetes mellitus were subjected to dynamic glucose monitoring for consecutive (71±10) hours, and (846±122) results of glucose level were obtained.MAIN OUTCOME MEASURES: The average glucose level, diurnal maximum and minimum, fluctuation amplitude, the average preprandial and postprandial glucose values and the time percentage of diurnal glycemic fluctuations.RESULTS: According to the objective of this study, all subjects went through the experiment without loss. ① The fluctuation tendency of blood glucose in two groups: The average glucose, diurnal maximum and maximum fluctuation amplitude in type 2 diabetes mellitus group were [(12.2±2.6) mmol/L], [(18.5±2.7) mmol/L] and [(11.2±2.8) mmol/L,respectively], which were significantly higher than those in control group [(5.3±0.5), (7.8±1.4) and (3.9±1.6) mmol/L](t=16.68, 23.04, 15.14;P < 0.001). ② The percentage of diurnal glycemic fluctuations time: The time percentage of glycemic fluctuations within 2.8-7.8 mmol/L was 99% (87%-100%) in control group, 95% (28%-100%) and 62% (6%-100%) when blood glucose.was increased to > 7.8 mmol/L or > 11.1 mmol/L in type 2 diabetes mellitus group.CONCLUSION: Blood glucose in type 2 diabetes mellitus has been proved to fluctuate at a relatively higher level than that of normals, with greater fluctuation amplitude.Dynamic glucose monitoring is benefical to evaluating the tendency and characteritics of blood glucose fluctuations in type 2 diabetic and normal subjects.
6.Effects of hyperoxia solution on pulmonary surfactant and histological changes in rabbits with oleic acid-induced acute lung injury

Objective To observe the effects of hyperoxia solution on pulmonary surfactant and histology of rabbits with oleic acid-induced acute lung injury.Methods Thirty healthy rabbits weighing 2.0 - 2.5 kg were randomly divided into three groups with 10 rabbits each:control group,hyperoxia solution treatment group,and saline treatment group.Blood samples were taken for blood-gas analysis before and at 30,60 and 120 min after oleic acid or normal saline administration.Two hours later,the animals were killed and pathologic changes of lung tissue were observed microscopically.The expression of SP-A was investigated using the immunohistochemical method and image analysis system.Results PaO2 was significantly higher in hyperoxia solution treatment group than in saline treatment group two hours after treatment (P < 0.001 ),but PaCO2 was significantly lower than that in saline treatment group (P <0.001).Optical microscopy showed that lung tissue damage was milder in hyperoxia solution treatment group than in saline treatment group.The content of surfactant A in hyperoxia solution treatment group was significantly greater than that in saline treatment group (P < 0.001 ).Conclusion Hyperoxia solution can lessen lung tissue injury and damage to pulmonary surfactant.
7.Protective effects of total saponins of Panax japonicas on HepG2 cell apoptosis induced with palmitic acid.

This study aimed to investigate the molecular mechanism and protective effect of total saponins of Panax japonicas (TSPJ) on HepG2 cells apoptosis induced by palmitic acid (PA).The HepG2 cells were cultured , and divided into five groups: the control group, the model group, the high-dose group (50 mg·L⁻¹), the middle-dose group (25 mg·L⁻¹) and the low-dose group (12.5 mg·L⁻¹).The cells of the five groups were cultured continuously for 24 hours. The cell viability was measured with MTT. HepG2 cells apoptosis was detected by Hoechest staining and Annexin V-FITC/PI staining. The protein expressions of BCL-2, CHOP and TLR4 were measured with western blotting and flow cytometry analysis. The mRNA expressions of TNF-α, IL-1β, BCL-2, CHOP and GAPDH were measured with RT-PCR. The results suggested that compared with the control group, the number of HepG2 cells of the model group were reduced significantly (<0.01), while the number of apoptotic HepG2 cells were increased. Compared with the model group, the number of HepG2 cells of the high-dose group and the middle-dose group were increased significantly (<0.01), whereas the number of apoptotic HepG2 cells were reduced. Compared with the control group, TNF-α, IL-1β and CHOP mRNA expressions and CHOP and TLR4 protein expressions in the model group were significantly up-regulated (<0.01), while BCL-2 protein and mRNA expressions in the model group were significantly decreased (<0.01). Compared with the model group, TNF-α, IL-1β and CHOP mRNA expressions and CHOP and TLR4 protein expressions in the high-dose group were significantly decreased (<0.01), while BCL-2 protein and mRNA expressions in the high-dose group were significantly up-regulated (<0.01).In conclusion, TSPJ can reduce inflammation and apoptosis induced by palmitic acid, with a certain protective effect on liver cells.

8.Mechanism of Chaenomelis Fructus in treatment of rheumatoid arthritis based on network pharmacology and experimental verification.

The material basis and mechanism of Chaenomelis Fructus in the treatment of rheumatoid arthritis(RA) were explored by network pharmacology, and the potential anti-RA targets of Chaenomelis Fructus were verified by molecular docking and animal experiments. The active components and targets of Chaenomelis Fructus were searched against the Traditional Chinese Medicine System Pharmacology Database and Analysis Platform. GeneCards, DisGeNET, and OMIM were used to obtain RA-related targets. The common targets shared by Chaenomelis Fructus and RA were considered as the potential targets of Chaenomelis Fructus in the treatment of RA. Cytoscape 3.9.0 was employed to establish a "traditional Chinese medicine-active component-common target-disease" network. The protein-protein interaction(PPI) network was established by STRING, and the core genes were visualized by RStudio 4.1.0. DAVID was used for Gene Ontology(GO) annotation and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment to predict and visualize the involved signaling pathways. Molecular docking was carried out with the active components screened out as ligands and RA core genes as the targets. Finally, the prediction results were verified by animal experiments. Four main active components of Chaenomelis Fructus were obtained, which corresponded to 137 targets. Chaenomelis Fructus and RA shared 37 common targets. GO annotation yielded 239 terms(P<0.05), and KEGG pathway enrichment analysis screened out 94 signaling pathways(P<0.05), mainly involving interleukin-17(IL-17), tumor necrosis factor, Toll-like receptor, and nuclear factor-kappa B(NF-κB) signaling pathways. Molecular docking results showed that the main active components of Chaenomelis Fructus bound well with the core targets of RA. The results of animal experiments proved that Chaenomelis Fructus can alleviate joint swelling in the mice with RA. The results of ELISA showed that Chaenomelis Fructus lowered the levels of interleukin-6(IL-6) and interleukin-1β(IL-1β). Western blot showed that Chaenomelis Fructus down-regulated the protein level of vascular endothelial growth factor A(VEGFA). Chaenomelis Fructus exerts anti-inflammatory effect and reduces pannus formation by regulating the core targets such as VEGFA, IL-1β, and IL6 in the treatment of RA. The findings of this study provide new ideas for the future treatment of RA with Chaenomelis Fructus.

9.Effect of saponins extracted from Panax japonicus on inhibiting cardiomyocyte apoptosis by AMPK/Sirt1/NF-κB signaling pathway in aging rats.

To investigate the effects of saponins extracted from Panax japonicus(SPJ) on cardiomyocyte apoptosis in natural aging rats and explore its underlying mechanisms. SD male rats were randomly divided into four groups: young control group, natural aging group, SPJ low dose group and SPJ high dose group, with 10 rats in each group. The rats in natural aging group, SPJ low and high dose groups were respectively treated with normal saline, SPJ 10 and 60 mg•kg-1•d-1 from the beginning of 18 month-old, 6 days per week for 6 months till 24 month-old. Then the animals were sacrificed. Their myocardial morphology changes were observed by using haematoxylin-eoin(HE) staining; cardiomyocyte apoptosis was tested by using Tunel assays; and the protein expression levels of Bcl-2, Bax, IL-1β, TNF-α, AMPK, p-AMPK, Sirt1, and Ac-NF-κB p65 in myocardial tissues of rats were detected by Western blot. The results showed that SPJ could effectively improve the arrangement disorder of myocardial fibers, reduce the infiltration of inflammatory cells and inhibit cardiomyocyte apoptosis in natural aging rats. At the same time, SPJ could significantly inhibit the protein expression of Bax, IL-1β, TNF-α and Ac-NF-κB p65, and increase the expression of Bcl-2, Bcl-2/Bax, p-AMPK/AMPK and Sirt1 in the heart tissues of natural aging rats. SPJ can effectively inhibit cardiomyocyte apoptosis in natural aging rats, and its mechanisms may be related with the regulation of inflammatory reaction by AMPK/Sirt1/NF-κB signaling pathway.
10.Effect of saponins extracted from Panax japonicas on inhibiting myocardial fibrosis by TGF-β1/Smad3 signaling pathway in aging rats.

To investigate the amelioration effect of saponins extracted from Panax japonicas (SPJ) on myocardial fibrosis in natural aging rats and its mechanisms, male SD rats aged 18 months were randomly divided into 3 groups (aging model group, low-dose SPJ group and high-dose SPJ group), with 10 rats in each group. SPJ groups were given SPJ at different doses (10, 60 mg·kg⁻¹·d⁻¹) consecutively for 6 months, meanwhile, aging model group was treated with the equal volume of saline for 6 months until 24 months old. Another 10 rats aged 6 month were used as young control group. The changes of myocardial morphological were observed by haematoxylin-eosin (HE) staining. Masson staining was used to observe the changes of collagen deposition in rat hearts. RT-PCR was used to detect the mRNA expression levels of myofibroblast marker α-SMA, collagen-related protein COL1α2, COL3α1 and matrix metalloproteinase MMP2, MMP9. Western blot was used to test the changes of the protein expressions of TGF-β1, p-Smad3, IL-1β and TNF-α in heart tissues. SPJ can effectively improve the arrangement of myocardial fibers, decrease inflammatory infiltration and reduce collagen deposition in aging rats. SPJ can effectively down-regulate the mRNA expression levels of COL1α2, COL3α1, α-SMA, MMP9, MMP2 and inhibit the protein expressions of TGF-β1, p-Smad3, TNF-α, IL-1β in the natural aging heart tissues. SPJ can effectively alleviate myocardial fibrosis in natural aging rats, and its mechanisms was related to the inhibition of the protein expressions of TGF-β1, p-Smad3 and the reduction of myocardial inflammation in rat hearts.
