1.Case study on the education rehabilitation of mainstreaming children with autism

BACKGROUND: Autism is a kind of severe growth and development disorder that occurs in the infant period. It is basically characterized by social communication disorder, language development disorder, narrow area of interest, and stereotyped and repeated behavior to different extents, and is increasing in the worldwide. Education training is still taken as the major for the rehabilitation therapy of autism.OBJECTIVE: Case analysis is performed to provide elicitation for mainstreaming education training of children with autism and proper suggestions for education worker and children patients' parents.DESIGN: Case analysis.SETTING: Department of Rehabilitation, Nanjing Technical College of Special Education; Nanjing Shangxinhe Primary School.PARTICIPANTS: One male child patient, born in June 1998, was diagnosed as autism by Shenyang Children's Hospital. They met the diagnostic criteria of infantile autism of the 4th edition of diagnostic and statistical manual of mental disorders (1994, USA).METHODS: Comprehensive intervention was performed in the child with autism for 5 months. Education measures consist of: ① to well accept class learning. ② showing merits in class. ③ to well apply learning contents in life. ④ to establish friendship. ⑤ to establish good behaviors and eliminate abnormal behaviors. ⑥ to pay more attention in class.⑦ to train the perceptivity in class. ⑧ to choose proper examination methods. ⑨ to establish pleasant and confluent plate time. The procedures were as follows: we had one telephone interview to the teachers and parents of child patient every day, one face-to-face communication with the teachers, parents and classmates of child patient every week. Education strategy was regulated in time aiming to the concrete condition of child patient.Wechsler Intelligence scale was used to measure the intelligence quotient (IQ) at the beginning and end of the experiment. The changes in behavioral performance in class and playtime of patient were summed up.MAIN OUTCOME MEASURES: ① The measurement results of Wechsler IQ of patient at the end of the experiment. ② Changes in the behavioral performance of patient in class. ③ Changes in the behavioral performance of child patient in playtime.RESULTS: ① Measurement results of Wechsler IQ of patients at the end of the experiment were as follows: verbal IQ 80 points, performance IQ 110 points, full scale IQ 95 points. There was 30-point discrepancy between performance IQ and verbal IQ, and 9 points between the highest and lowest scores. The imbalance of intelligence development had turned better.② Changes in behavioral performance in class: The child patient was basically the same as other children. He could attend a class silently, followed the teacher to learn, did not disturb class, and show his brilliance with proper style. ③ Changes in the behaviors in playtime: made friends; was interested and participated in the games of the classmates, entered the classroom when he heard the ring without the help from the teacher.CONCLUSION: After 5-month rehabilitation training, the mainstreaming status of patient with autism is significantly improved as compared with before intervention. Application of various rehabilitation-training methods can improve the education effect of patient with autism.
2.Relationship of vitamin D in children with sepsis/severe sepsis and outcomes in PICU

Objective To determine the vitamin D status in children with sepsis/severe sepsis in pediatric intensive care unit (PICU) in order to explor the association between vitamin D status and clinical outcomes,in turn to provide evidence for optimizing nutrition support.Methods It was a prospective,observational,multi-center study,carried out in patients with sepsis/severe sepsis from March 1,2013,to March 30,2014,in the PICUs of three tertiary-care children's hospitals.Total serum 25-hydroxy vitamin D [25 (OH) D] was measured by using an enzyme-linked immunosorbent assay at admission.The association of vitamin D status at admission with length of PICU length of stay,total hospital stay,in-hospital mortality,28-days mortality and costs were analyzed.Results A total of 194 patients includng 117 boys (60.3%)and 77 girls (39.7%) were enrolled.There were 96 patients with sepsis and 98 with severe sepsis.The mortality on discharge and 28 days were 6.7% and 24.2% respectively.The median vitamin D level was 9.79 ng/mL (5.32,18.46) at admission.Of them 77.8% (151/194) had vitamin D deficiency and 50.5% (98/194) had severe vitamin D deficiency.Patients with severe vitamin D deficiency,had higher mortality on discharge (P =0.011).Vitamin D status had no significant correlations with 28 days mortality,length of PICU stay,total hospital stay and costs.Conclusions More than three-quarters (77.8%) of children with sepsis/severe sepsis in PICUs had Vitamin D deficiency.Patients with severe vitamin D deficiency at admission had higher risk of mortality at discharge.
3.Study on construction of core competence index system of nursing postgraduates in TCM colleges and universities

Objective To construct the core competence index system of nursing postgraduates in TCM colleges and universities.Methods The Delphi method was used to investigate 30 nursing experts and consult about core competence that nursing postgraduates of TCM colleges and universities should have.Results The effective recovery rates of questionnaire were respectively 85.7% and 86.7%.The authority coefficients were 0.86 and 0.84,and the consistency coefficients were 0.278 and 0.253 with statistical significance.Index system for core competence with 4 first-grade indexes and 20 second-grade indexes was formed.The full score ratio of first-class indexes was 0.96-1.00,coefficients variation was 0.10-0.13; for the second-class,the full score rate was 0.62-1.00,coefficient variation was 0.08-0.19.Conclusions For formation items of core competence index system of nursing postgraduates in TCM colleges and universities,the opinions of experts are centralized,which can provide reference for training program of nursing postgraduates in TCM colleges and universities.
4.Therapeutic Effect of Patent Doctus Arteriosus Closer for Low Body Weight Children

Objective To discusses the efficacy and safety of patent ductus arteriosus sealing for low body weight children. Methods From January 2008 to June 2012 in our hospital,32 cases (male14 and female 18) of PDA babies< 8 kg weight were given transcatheter closure treatment. The enrolled PDA babies aged 6 month-18 months, average (11 ±5.5 months),weighted (7.2 ±0.6) kg,aortic imaging showed the narrowest diameter of PDA was 1 ~ 8 mm (4.6 ±1.7) mm,descending aorta imaging was performed immediately and 30 min after transcatheter closure treatment. Echocardiography and X-ray radiography examination were performed 1 d,1,3, 6 months after treatment. Results 30 cases got successful patent ductus arteriosus sealing,all the successful patients were treated with domestic closer,and 1 case was complicated with the descending aorta stenosis and ventricular septal defect, and was transferred to surgerical treatment. 1 case of PDA gave up patent ductus arteriosus sealing as descending aorta stenosis caused by the closer. 2 cases were found a trace-residual shunt immediately after patent ductus arteriosus sealing. No serious complications were found in all patients during the follow-up period. Conclusions Application of domestic closer in treatment of PDA infants < 8 kg is a safe, effective and simple intervention treatment method with little trauma and fast recovery.
5.Comparative Study on the Effect of Implanted Stent or Selected Operation in Patients with Acute Occlusion of Right Coronary Artery

Objective To study the effect of implanted stent or selected operation in patients with acute occlusion of right coronary artery during the emergency coronary arteriongraphy. Methods Forty-three patients with acute occlusion of right coronary artery who underwent the emergency coronary arteriongraphy were divided into two groups: the implanted stent group ( n = 23) during the emergency coronary arteriongraphy and the selected operation group (n =20) during the emergency coronary arteriongraphy . Then we observed the general data, the coronary artery pathological changes, preoperative and postoperative thrombus, the blood flow, remained stenosis and the prognosis. Results The implanted stent group was similar to the control group in general condition, but implanted stent group was significantly different from the control group in the near, intermediate and distal coronary artery pathological changes ( <0.05) . The control group was significantly different from the implanted stent group in the thrombus and slowly blood flow ( <0.05) . After three months, we reviewed coronary arteriongraphy and selected operation, The implanted stent group was significantly different from the control in the remained stenosis and slowly blood flow (<0.05) . But the implanted stent group was similar to the control groug in the thrombus and new proceeding cardiovascular events. Conclusion When thrombus appears in acute occlusion of right coronary artery, we can implant stent during the emergency coronary arteriongraphy if the thrombus is little, or we can select operation after PTCA on the contrary.
6.Regulative effect of complement factor H and adrenomedullin on HBZY-1 rat mesangial cells cultured under high glucose concentration

The renal protective effect of complement factor H(CFH)and adrenomedullin (AM) on rat mesengial ceils (HBZY-1) cultured under high glucose levels was investigated. Results indicated that AM and CFH inhibited the expressions of transforming growth factor-β1 and extracellular matrix in HBZY-1 cells, suggesting that CFH and AM in combination seem to show some renal protective effects on diabetic nephropathy.

8.Clinical analysis of electrocardiogram after transcatheter closure of perimembranous ventricular ;septal defects

Objective To explore if any rules in electrocardiogram changes after transcatheter closure of perimembranous ventricular septal defects ( PMVSD ) . Methods We included all the 358 patients who have accepted transcatheter closure of PMVSD in our hospital between July 2006 to October 2014 and the electrocardiogram (ECG) done in hospital and during follow up in 1,3, 6 and 12 months after operation were all reviewed. Results No changes were found in heart rates and electrical axis during follow-up as compared to preclosure ECG. PR interval was shorter, the QRS duration and QT interval were longer than preclosure. Incidence rate of arrhythmia was 38. 0% ( 136/358 ) and incidence rate of serious arrhythmias ( including Ⅱ° or Ⅲ° atrioventricular block and complete left bundle branch block) was 5. 0%(18/358). Among the 180 patients who had ECG done in all follow up between the first 12 months post closure, the rates of new developed arrhythmias was 12. 8% ( 23/180 ) and severe arrhythmia was 0. 6%(1/180) during follow-up. Conclusions Incidence rate of serious arrhythmias after transcatheter closure of PMVSD is low and most patients have good clinical outcome.
9.Effects of Bosentan in Treatment of Severe Pulmonary Hypertension Related to Congenital Heart Disease

Objective To observe the clinical efficacy of bosentan in treatment of severe pulmonary hypertension related to congenital heart disease (CHD-PAH) .Methods 5 patients with severe CHD-PAH patients received bosentan therapy, then pulmonary artery pressure, pulmonary vascular resistance (PVR), 6min walk test,right ventricular end-systolic diameter (RVSD) changes were observed and statistically analyzed after six months medication. Results Pulmonary arterial systolic pressure (sPAP) was significanfly decreased from (96±11) mmHg to (86±10) mmHg, <0.01.pulmonary arterial diastolic pressure (dPAP) was significanfly decreased from (56±10) mmHg to (46±9) mmHg ( <0.01),pulmonary arterial mean pressure (mPAP) was significanfly decreased from (73 ±11) mmHg to (59 ±10) mmHg ( <0.05), pulmonary vascular resistance was significanfly decreased from (17.8±1.9) Wood to (13.1±1.7) Wood (<0.01) . 6min walk test was improved from (136±40) m to (198±55) m, <0.01.right ventricular end-systolic diameter significanfly decreased from (40±5) mm to (36±6) mm after 6 months therapy ( <0.05) . Conclusion Bosentan can decrease pulmonary arterial systolic pressure, improve exercise tolerance, improve right ventricular function in patients with severe CHD-PAH.

Objective:The 11?-hydroxysteroid dehydrogenase(11?-HSD) plays an important role in tumor biological behavior,and the 11?-HSD inhibitor Glycyrrhetinic acid(GA) may have an anticancer effect,although its mechanism remains unkown.This study aimed to observe the expressions of 11?-HSD 1 and 2 in colorectal cancer and the effects of glucocorticoid and GA on colorectal cancer cells.Methods: The mRNA expressions of 11?-HSD1 and 2 in the human colorectal cancer cell line HCT-8 were detected by reverse transcription polymerase chainreaction(RT-PCR),and the protein expressions of 11?-HSD1 and 2 were detected by Western blot.The inhibition of the growth of the HCT-8 cells was determined by MTT assay after treated with glucocorticoid,cosubstrate,GA only or their combination.Results: The expression of 11?-HSD2 mRNA and proteins but not that of 11?-HSD1 mRNA and proteins was expressed in the HCT-8 cells.The rate of cell growth inhibition was(40.87 ? 1.47)% after a 12-hour treatment with cortisol,and it was(5.79 ? 0.20)% in the cortisol + NAD group,with statistically significant difference(P
