ObjectiveTo explore the effect of home visit based on empowerment education on psychotic symptoms, self-management ability and well-being of schizophrenia patients in home rehabilitation. MethodsA total of 87 cases of schizophrenia patients who met the diagnostic criteria of International Classification of Diseases, tenth edition (ICD-10) and were recovering at home in Caiba town, Yibin city from January to July 2021 were selected by random sampling method as research subjects. They were divided into experimental group (n=43) and control group (n=44) according to the random number table method. The two groups received routine family visits, and the experimental group received family visits based on empowerment education. The intervention cycle of the two groups was 6 months. Before and after the intervention, the severity of psychotic symptoms, self-management ability and well-being of the patients in the two groups were assessed with the Brief Psychiatric Rating Scale (BPRS), the Schizophrenia Self-Management Instrument Scale (SSMIS) and the Index of Well-Being Scale (IWB). ResultsAfter intervention, the intra group comparison showed that the BPRS score in the experimental group was lower than that before the intervention (t=4.550, P<0.01), the SSMIS scores in both groups were higher than those before intervention (t=-17.107, -6.367, P<0.01), and the IWB score in the experimental group was higher than that before intervention (t=-9.239, P<0.01). The comparison between groups showed that the BPRS score of the experimental group was lower than that of the control group, and the SSMIS and IWB scores were higher than those of the control group (t=-3.899, 10.564, 9.690, P<0.01). ConclusionThe implementation of family visit based on empowerment education may help to improve the psychotic symptoms of home rehabilitation schizophrenia, and improve their self-management ability and well-being.
4.Visual recovery and complications after vitrectomy combined with phacoemulsification in PDR patients

AIM: To observe the visual recovery and complications in patients with proliferative diabetic retinopathy(PDR)after vitrectomy combined with phacoemulsification.
METHODS: According to different surgical methods, 95 cases(95 eyes)with PDR were divided into the observation group(50 cases)and the control group(45 cases). The observation group was treated with vitrectomy and phacoemulsification, while the control group was treated with vitrectomy and lensectomy. The visual recovery, changes of intraocular pressure before and after surgery and the complications were compared between the two groups.
RESULTS: The visual acuity of the two groups was significantly improved after surgery(P<0.05), and the improvement in observation group was significantly greater than that in the control group(P<0.05). The intraocular pressure of the two groups was increased significantly at 1, 3 and 6mo after surgery(P<0.05), without significant difference between the two groups(P>0.05). The incidence of iris neovascularization and capsule opacification in the observation group(2%, 6%)were significantly lower than those in the control group(18%, 27%; P<0.05).
CONCLUSION: Vitrectomy combined with phacoemulsification can obviously improve the visual acuity of patients with PDR, and the incidence of complications is low. Although it will lead to an increase in intraocular pressure, it can be improved after symptomatic treatment.
5.Correlation analysis of thromboelastogram and routine coagulation test in the detection of cerebrovascular diseases

【Objective】 To explore the changes of parameters of thromboelastogram (TEG) detection in patients with cerebral infarction and cerebral hemorrhage, and their correlation with routine coagulation indicators, and compare the two methods in detecting coagulation function and status in patients with cerebrovascular disease. 【Methods】 The detection data of 34 cases of cerebral infarction and 102 cases of cerebral hemorrhage, who underwent TEG and routine coagulation test in parallel, in Department of Neurosurgery of our hospital from January 1, 2018 to March 26, 2019 were collected. Fifty healthy subjects, who underwent TEG and routine coagulation tests in the same period, served as the control. The results of TEG and routine coagulation test of the 3 groups were compared and analyzed. 【Results】 Compared with cerebral hemorrhage (group) and the control, R value of cerebral infarction group decreased significantly, while the Angle angle, MA distance, CI value, Fib and D-D levels increased significantly(P<0.05); the K value, Fib, TT, and D-D of cerebral hemorrhage group increased significantly while Angle angle and MA distance decreased significantly compared with other two groups(P<0.05). The R value, K time (min), TT (s), D-D (mg/L), Angle angle (°), MA distance, CI value, and Fib (mmg/L) of cerebral infarction and cerebral hemorrhage group were 5.06±1.71 vs 6.95±4.34, 1.19±0.68 vs 3.43±1.46, 13.89±1.05 vs 18.63±1.25, 1.93±0.25 vs 2.63±0.45, 69.34±10.37 vs 56.54±10.48, 68.34±9.37 vs 54.35±7.48, 2.20±0.31 vs 0.78±0.41, and 380±70.02 vs 354±71.56 (P<0.05). In cerebral infarction group, the R value, APTT, Angle angle, MA, and CI values were positively correlated with Fib (P<0.05); while K time was negatively correlated with Fib ; MA distance with PT/TT; CI value with PT/APTT(P<0.05). In the cerebral hemorrhage group, the R value was positively correlated with PT/APTT; K time with TT; Angle with Fib (P<0.05); R value/K time were negatively correlated with Fib; Angle with TT; CI value with PT (P<0.05). 【Conclusion】 For patients with cerebrovascular disease, there is no correlation between TEG indicators and the D-D level of routine coagulation tests; the Angle angle of patients with cerebrovascular disease is positively correlated with the elevated Fib; and MA distance is more useful for diagnosing cerebral infarction than cerebral hemorrhage. TEG is complementary to routine coagulation test, and can be used as a laboratory index for assessing the severity of cerebrovascular disease.
6.The preliminary outcomes of percutaneously looped thread transection in the surgical treatment of carpal tunnel syn-drome

Objective To study the preliminary effects of percutaneously looped thread transection on the surgical treatment of carpal tunnel syndrome (CTS). Methods A total of 72 cases (103 sides) were treated from January 2012 to Decem?ber 2014 (27 males and 45 females, aged 22-94 years, with an average of 51.3 years). Among all the cases, 21 cases (30 sides) were treated with open decompression and 51 cases (73 sides) were treated with percutaneously looped thread transection. We did the incision in the ulnaris hypothenar pattern, revealed and cut off the transverse carpal ligament to decompress the carpal tunnel in the open surgery. Percutaneously looped thread transection was under the guidance of ultrasound, threading from the deep and shallow transverse carpal ligament, respectively, forming loops to cut off and decompressed the carpal tunnel. We observed the me?dian nerve aspect ratio (the ratio of length to width of the uncinatum median nerve on transverse section) and swelling ratio (the ra?tio of area of median nerve on transverse section of pisiform and distal radius) under ultrasound before, during and 3 months after operation. Telephone follow?up was conducted in postoperative 3 months according to the Boston Carpal Tunnel Questionnaire (BCTQ) to evaluate the symptoms of carpal tunnel syndrome and wrist joint function. Results There were 11 cases lost in the fol?low?up, and 61 cases were followed up for 3 to 27 months, with an average of 11.6 months. The preoperative BCTQ scores of open decompression group and looped thread transection group were 28.5±4.6 and 29.4±5.3, respectively. There was no statistically sig?nificant difference between two groups (t=1.34, P=0.528). The postoperative 3 months BCTQ scores were 16.3±5.7 and 15.7±4.9. There was no statistically significant difference between two groups (t=1.12, P=0.674). The median nerve aspect ratios measured under ultrasound of open decompression group before and 3 months after operation were 3.8 ± 0.7 and 2.6 ± 0.4, respectively. The swelling ratios were 2.3±0.4 and 1.2±0.3. The difference of preoperative and postoperative changes was statistically significant (P<0.05). The median nerve aspect ratios measured under ultrasound of looped thread transection group before and 3 months after op?eration were 3.9±0.6 and 2.7±0.5, respectively. The swelling ratios were 2.1±0.3 and 1.4±0.4. The difference of preoperative and postoperative changes was statistically significant (P<0.05). There were no infection, poor healing, blood vessel and nerve damage after operation in both two groups. Conclusion Percutaneously looped thread transaction under ultrasound for the surgical treat?ment of CTS has less trauma and rapid recovery. It can improve the symptoms of median nerve stimulation, restore the morphology and function of the median nerve and reach the same effects as the open decompression surgery.
7.Clinical analysis of severe craniocerebral injuries complicated by upper cervical vertebral injuries

Objective To study clinical features of severe craniocerebral injuries complicated by upper cervical vertebral injuries so as to have a deeper understanding and higher vigilantness. Methods Twenty-nine cases of severe craniocerebral injuries complicated by upper cervical vertebral injuries were clinically analyzed. Results Injury causes included traffic accident in 26 cases, fall in one, rolling down in one and mine collapse in one. Treatment results showed that 16 cases survived and 13 died. As for Glasgow Outcome Scale (GOS) score, two cases were ranked at grade Ⅴ, seven at grade Ⅳ, six at grade Ⅲ, one at grade Ⅱ and 13 at grade Ⅰ. Conclusions The severe craniocerebral injuries complicated by upper cervical vertebral injuries are critical and complex mainly because the severe craniocerebral injuries can be paid more attention and diagnosed and treated early while the upper cervical vertebral injuries are easy to be neglected, as gives rise to missed injuries, lack of timely management and high mortality. Therefore, various severe complications are liable for deaths.
8.The distinguishing reaction of nitrate and nitrite in Dantong Tablets

AIM: We establish a suitable distinguishing reaction of nitrate and nitrite in Chinese traditional medicine containing reduction agents. METHODS: In different concentration solution of sulfate acid,we study on the distinguishing reaction,by paralleling samples with negative samples and positive samples. RESULTS:The nitrite and sample containing nitrite react with ferrous sulfate reagent from brown in 50%-5% solution of sulfate acid;The nitrate and sample containing nitrate react with ferrous sulfate reagent from brown in ≥25% solution of sulfate acid.The negative sample and sample without nitrite and nitrate does not react with ferrous sulfate reagent in 50%-5% solution of sulfate acid. CONCLUSION: The thin sulfate acid-ferrous sulfate reagent-nitrate ditinguishing reaction can be used to distinguish nitrate and nitrite in Chinese traditional medicine containing reduction agents.

10.Association of ABO genotype with acute rejection in renal transplantation

Objective To investigate a possible association of donor-recipient compatibility for ABO blood group alleles with acute rejection (AR) in renal transplantation. Methods A study comprising 87 pairs of donor and recipient was performed. The ABO genotype A1, A2, O1, O2, and B alleles of renal transplanted recipients and their respective donors were assessed by PCR amplification with sequence-specific primers (PCR-SSP). Accordingly, recipients were divided into donor-recipient ABO genotype matched and mismatched groups. Results The PCR-SSP based types of all cases showed total concordance with their serologically assigned ABO groups. Fifty pairs (57. 5%) were matched for ABO genotype among the 87 pairs of donor and recipient while 37 (42. 5%) were mismatched, including 1 allele mismatch in 31 pairs (83.8%), 2 alleles mismatches in 6 pairs (16. 2%).The incidence of AR was 12.0% (6 cases) and 29. 7% (11 cases) for ABO genotype matched and mismatched transplant patients, respectively ( P < 0.05). After high dose methylprednisolone (MP)treatment, all cases exepienced reversion of AR except a A2O1 recipient receiving kidney from a A1O1enced 4 AR episodes within 3-10 months, and the period of AR was gradually shortened. After high dose MP was administered empirically, even though short-term improvement of renal function was observed, the serum creatinine continued to increase progressively with decreased efficacy of high dose MP. One year after operation the serum creatinine rose to 441 μmol/L. Conclusions Simultaneous definition of the ABO genotype and HLA is highly feasible. The A2 patient is suitable for receiving kidneys from blood group O donors. DNA mismatch for ABO genotype of renal transplant recipients and their respective donors is an independent risk factor for AR. Genotyping of ABO blood group is conducive to prevent AR.
