Objective To investigate the effect of mechanical thrombectomy device Solitaire AB for the treatment of distal occlusion of the intracranial artery caused acute anterior circulation ischemic stroke. Methods The clinical data of 9 patients with arterial occlusion of distal anterior circulation treated by using the Solitaire AB thrombectomy device were analyzed retrospectively. Seven of them had M2 middle cerebral artery occlusion and 2 had A3 occlusion. Their clinical features, imaging data, treatment, and the results of 3-month clinical follow-up were analyzed. The effectiveness of treatment was evaluated through the recanalization rate after stent thrombectomy, the National Institutes of Health Stroke Scale (NIHSS)scores before and after treatment, the modified Rankin scale (mRS)scores, and the 3-month clinical follow-up results. The surgery-related complications and mortality of patients were used to evaluate the safety of the treatment. Results (1 )Of the 9 intracranial arterial occlusions,8 were recanalized successfully. The modified thrombolysis in cerebral infarction (mTICI)was 2b or grade 3. (2)The NIHSS score median 9. 5 (3. 0,15. 5)at discharge dropped significantly compared with 19. 0 (16. 0,22. 0)before procedure. (Z=2. 703,P=0. 007). No permanent complications related to operation occurred. Four of the recanalized patients had good prognosis (mRS 0-1)and 4 had disability (mRS 3-4 ). The mRS score of one non-recanalized patient was 4 at the 3 months follow-up. Conclusion The embolectomy effect is good whom using Solitaire AB device for acute occlusion occurred in the intracranial artery M2 or A3 segment. Its efficacy, safety, and indications still need to be further explored in a larger sample controlled trial.

Objective To study clinical features of severe craniocerebral injuries complicated by upper cervical vertebral injuries so as to have a deeper understanding and higher vigilantness. Methods Twenty-nine cases of severe craniocerebral injuries complicated by upper cervical vertebral injuries were clinically analyzed. Results Injury causes included traffic accident in 26 cases, fall in one, rolling down in one and mine collapse in one. Treatment results showed that 16 cases survived and 13 died. As for Glasgow Outcome Scale (GOS) score, two cases were ranked at grade Ⅴ, seven at grade Ⅳ, six at grade Ⅲ, one at grade Ⅱ and 13 at grade Ⅰ. Conclusions The severe craniocerebral injuries complicated by upper cervical vertebral injuries are critical and complex mainly because the severe craniocerebral injuries can be paid more attention and diagnosed and treated early while the upper cervical vertebral injuries are easy to be neglected, as gives rise to missed injuries, lack of timely management and high mortality. Therefore, various severe complications are liable for deaths.

OBJECTIVE:To study the effects of Salvionolic acids dialyzate on anaphylatoxin,C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-?(TNF-?)in peripheral blood of patients with maintenance hemodialysis(MHD)and to discuss whether the Salvionolic acids dialyzate could improve the blood-membrane biocompatibility.METHODS:Thirty-six patients undergoing MHD for at least half a year were randomly divided into 3 groups:control group(conventional hemodialysis,n=12),low-dose group(dialysate with salvionolic acids 40 mg?L-1,n=12)and high-dose group(dialysate with Salvionolic acids 80 mg?L-1,n=12).Dialyzer of the polysulfone membranes was firstly used in all patients.Patients' blood sampling was scheduled before dialysis,30 minutes after dialysis and at the end of dialysis(5 minutes before withdrawal of dialyzer)for measurement of serum levels of complement(C3a,C5a),CRP,IL-6 and TNF-? using enzyme linked immunosorbent assay(ELISA).RESULTS:As compared with before dialysis, the levels of C3a and C5a in both control group and low-dose group were evidently increased at 30 minutes, while the difference in high-dose group was nonsignificant. No significant differences of IL-6 and TNF-? were found between pre-dialysis and post-dialysis in three groups. As compared with before dialysis, the concentration of CRP in control and low-dose groups obviously increased but decreased obviously in high-dose group after dialysis.CONCLUSION:The production of C3a,C5a and CRP in the process of hemodialysis could be inhibited by a certain dose of Salvionolic acids dialyzate, which can improve the biocompatibility of the dialysis-membrane to some degree, but can't improve the levels of IL-6 and TNF-? after single-time dialysis.

OBJECTIVETo retrospectively explore the effects of damage control orthopaedics concept on coagulation and curative effects in unstable pelvic fractures and multiple fractures of limbs.
METHODSFrom March 2014 to December 2015, 40 patients with unstable pelvic fractures and limbs multiple fractures in treatment group included 22 males and 18 females with an average age of (39.00±4.12) years old were treated with the damage control orthopaedics concept, the ISS score was (25.36±10.81) on average;Other 40 patients with the same trauma in conventional group included 25 males and 15 females with an average age of (38.00±3.24) years old were treated with conventional method from January 2012 to January 2014 served as control, the average ISS score was 26.56±11.44. Matta criteria and Majeed function standard were used respectively to evaluate the fracture reduction and therapeutic effects postoperatively. Coagulation function on the 7th day postoperatively was compared between two groups.
RESULTSAll patients were followed up for 6 to 24 months. According to Matta criteria, the fracture reduction of the treatment group and the conventional group were (7.38±5.09) mm and (10.11±6.53) mm, respectively (<0.05). Majeed functional results of the treatment group and the conventional group were (86.12±6.84) points and (77.53±8.30) points, respectively (<0.05). On the 7th day after surgery, PT, APTT, TT of the treatment group were significantly higher than that of the control group;and Fib of the treatment group was also significantly lower than that of the conventional group(<0.05).
CONCLUSIONSThe concept of damage control orthopaedics could effectively improve coagulation function of the patients with unstable pelvic fractures and limbs multiple fractures, thus are beneficial to the functional recovery as well as improve the curative effect postoperatively.
