1.The Role of SMS201-995 in the Prevention and Treatment of Pulmonary Edema Caused by Acute Pancreatitis in Rats

Pulmonary edema is associated with pancreatic enzymes released into the blood stream during acute pancreatitis.Acute hemorrhagic pancreatitis in rats was established by injecting 5%sodium taurocholate into the pancreatic duct.Before and at the time of induction,SMS201-995 2?g/kg was injected subcutaneously,and followed by 3 h continuous I.V.infusion of NS solution containing SMS201-995 5?g?kg-1?h-1 at a rate of 6ml?kg-1?h-1.The results of this experiment showed that SMS201-995 could:(1) decrease the serum levels of amylase and lipase effectively;(2)reduce lung index and lung extravascular water volume siginficantly;(3)ameliorate the pathologic changs of lung and pancreas evidently.These results also suggested that SMS201-995 could both prevent and treat acute pancreatitis and pulmonary edema induced by it in rats.Therefore,it is theoretically sound that evidence can be proviede by the results obtained in this experiment for the application of SMS201-995,a long acting analogue of somatostatin,to treat clinical acute pancreatitis by its inhibitory effect towards pancreatic exocrine secretion.
2.Intracranial aneurysm detection with 3T magnetic resonance angiography.

INTRODUCTIONThe new 3 Tesla (T) magnetic resonance (MR) scanners yield improved signal-to-noise ratio and spatial resolution with superior background suppression compared to lower field strength systems. This is advantageous for MR angiograms. The purpose of our study was to compare unenhanced three-dimensional time-of-flight magnetic resonance angiography (3D TOF MRA) at 3T with catheter digital subtraction angiography (DSA) in detecting unruptured intracranial aneurysms.
MATERIALS AND METHODSOut of 1375 consecutive patients who underwent unenhanced 3D TOF MRA at 3T, 15 patients with unruptured intracranial aneurysms were retrospectively identified. Nine of these 15 patients had DSA as the reference standard for comparison. Aneurysm size, location and morphology were independently assessed on both MRA and DSA by 2 radiologists.
RESULTSSeventeen aneurysms ranging in size from 1 mm to 24 mm were identified in 15 patients on MRA. DSA confirmed the aneurysms in 9 patients with good anatomical correlation compared with the MRA findings.
CONCLUSIONS3D TOF MRA at 3T has good correlation with DSA and aneurysms as small as 1 mm in size can be detected. This can be a promising, non-invasive method for aneurysm surveillance.

3.Application of reverse abdominal advancement flap in repairing soft tissue defect of chest wall after mastectomy

Objective:To investigate the application effect of reverse abdominal wall advancement flap in repairing chest wall soft tissue defect after breast tumor resection.Methods:From October 2020 to April 2021, the Department of Plastic Surgery and Breast Surgery of Hainan Cancer Hospital cooperated to repair the chest wall wounds of 4 female patients with unilateral giant breast tumors after primary lesion resection. Patients aged 40-63 years old, with an average of 51.5 years old. The size of the tumor estimated by physical examination was 7 cm × 6 cm-15 cm × 20 cm. The flaps were closely monitored after surgery, and complications were recorded. Local recurrence was followed-up.Results:The wound size of 4 patients after mastectomy was 16 cm×14 cm-20 cm×18 cm. Abdomen separation range reached anterior axillary line laterally and contralateral clavicle midline medially. Inferiorly, 1 case reached umbilical level, 1 case reached 2 cm below the umbilicus, and 2 cases reached 3 cm below the umbilicus. Three cases were diagnosed as breast lobulated tumors, and 1 case invasive lobular carcinoma. Among the 4 cases, 3 flaps survived completely and healed by first intention, while another flap healed under blister scab. There was no necrosis, infection, hematoma, seroma, or vascular crisis of the flap. All patients were satisfied. Three patients received radiotherapy and one received radiotherapy combined with oral chemotherapy. All patients were followed up for 3-6 months. No local recurrence was discovered. The abdomen was tighter than before operation, but no stiffness was found. There was no abdominal pain, abdominal wall bulging or abdominal hernia.Conclusions:Reverse abdominal wall advancedment flap was used to repair soft tissue defect of the chest wall after breast tumor resection. The operation was simple and fast, with no need for additional donor site incision. Quick recovery allowed shorter interval between surgery and next scheduled treatments.
